119. Costs of Bordetella pertussis in tertiary hospitals of Argentina

Abstract

The National Immunization Commission and the National Program for the Control of Vaccine-Preventable Diseases (Programa Nacional de Control de Enfermedades Inmunoprevenibles, ProNaCEI) updated the immunization policy in relation to Bordetella pertussis (BP) in 2009 in order to improve the control of this disease in accordance with international recommendations. To evaluate the financial impact of this new immunization policy, we must first know the cost on the health system of having a hospitalized or outpatient child infected with BP. The objective of this study was to describe the profile of costs of hospitalized or outpatient children with laboratory-confirmed BP infection in three hospitals of Argentina. This was a prospective study of the cost of BP in the period between December 2010 and March 2012. Results. The total cost for the entire cohort was 1 170 663.32 ARS (236 497.64 USD); direct medical costs were 1 124 052.31 ARS (227 081.27 USD); indirect costs and out-of-pocket expenses were 46 611 ARS (9416.6 USD). From this data, it is possible to conclude that the total average cost per patient was 10 546.52 ARS (95% CI: 9009-13 840) (2130.60 USD, 95% CI: 1820-2795), the direct medical cost per patient was 10 126.6 ARS (95% CI: 8607-13 171) (2045.77 USD, 95% CI: 1738-2660), and the indirect plus out-of-pocket costs (transportation and extras) were 419.92 ARS (95% CI: 344.7-565.3) (84 USD, 95% CI: 69-115). Conclusion. The cost of a hospitalized child with confirmed BP is 10 546.52 ARS (95% CI: 9009-13 840) (2130.60 USD, 95% CI: 1820-2795). Direct non-medical costs and overhead costs account for 4% of the total cost, amounting to 419.91 ARS per family (84 USD, 95% CI: 69-115), approximately an 8% of an average salary. © 2013 Sociedad Argentina de Pediatría.

Abstract

Introduction: In 2012, PCV13 was introduced into the National Immunization Program in Argentina, 2+1 schedule for children <2 years. Coverage rates for 1st and 3rd doses were 69% and 41.0% in 2012, 98% and 86% in 2013; 99% and 89% in 2014, respectively. The aims of this study were to evaluate impact of PCV13 on Consolidated Pneumonia (CP) and Pneumococcal Pneumonia (PP) burden, and to describe epidemiological-clinical pattern of PP during the three-year period following vaccine introduction. Methods: Hospital-based study at 10 pediatric surveillance units in Argentina. CP and PP discharge rates per 10,000 hospital discharges were compared between the pre-vaccination period 2007–2011 (preVp), the year of intervention (2012) and the post-vaccination period 2013–2014 (postVp). Results: Significant reduction in CP and PP discharge rates was observed in patients <5 years [% reduction (95%CI)]: 10.2% (6.3; 14.0) in 2012 and 24.8% (21.3; 28.2) in postVp for CP discharge rate; 59.5% (48.0; 68.5) in 2012 and 68.8% (58.3; 76.6) in postVp for PP discharge rate. Significant changes were also observed in children ≥5 years, mainly in PP discharge rate. A total of 297 PP cases were studied; 59.3% male; 31.3% <2 years; 42.9% had received PCV13 in 2012 and 84.5% in posVp. Case fatality rate was 3.4%. PCV13 serotypes decreased from 83.0% (39/47) in 2012 to 64.2% (52/81) in postVp, p = 0.039. Conclusions: After PCV13 introduction, significant reduction in CP and PP discharge rates was observed in hospitalized children <5 years. 121. Recent changes in the epidemiology of Neisseria meningitidisserogroup W across the world, current vaccination policy choices and possible future strategies

Abstract

ABSTRACTInvasive meningococcal disease (IMD) is a serious disease that is fatal in 5–15% and disabling in 12–20% of cases. The dynamic and unpredictable epidemiology is a particular challenge of IMD prevention. Although vaccination against meningococcal serogroups A (MenA), MenC and, more recently, MenB, are proving successful, other serogroups are emerging as major IMD causes. Recently, surges in MenW incidence occurred in South America, Europe, Australia and parts of sub-Saharan Africa, with hypervirulent strains being associated with severe IMD and higher fatality rates. This review describes global trends in MenW-IMD epidemiology over the last 5–10 years, with emphasis on the response of national/regional health authorities to increased MenW prevalence in impacted areas. Several countries (Argentina, Australia, Chile, the Netherlands and UK) have implemented reactive vaccination campaigns to reduce MenW-IMD, using MenACWY conjugate vaccines. Future vaccination programs should consider the evolving epidemiology of MenW-IMD and the most impacted age groups.

Abstract

Las infecciones invasivas por Streptococcus pneumoniae (Spn) producen mortalidad elevada en países en desarrollo, con tasas entre 4 y 100 veces mayores que las de Estados Unidos o Canadá. Es el primer agente causal de neumonía en la infancia y de meningitis fuera de los brotes epidémicos por Neisseria meningitidis. La OPS, a través del grupo SIREVA, dedicado al desarrollo de vacunas en Latinoamérica, organizó un programa de vigilancia de infecciones invasivas por Spn en seis países: Argentina, Brasil, Chile, Colombia, México y Uruguay, iniciado en 1993 y que continúa actualmente. En Argentina participan en la actualidad más de 20 centros hospitalarios distribuidos en todas las áreas geográficas del país, actuando como Centro Nacional de referencia para la serotipificación y determinación de la resistencia a los antibióticos el Instituto ANLIS "Dr. Carlos G. Malbrán". Objetivos: 1) Determinar los serotipos predominantes, su resistencia a los antibióticos y los cambios temporales en infecciones invasivas por Spn de niños menores de 5 años de edad; 2) Obtener información confiable para la formulación de una vacuna conjugada adecuada para la región. Metodología: Diseño prospectivo observacional. Se incluyeron todos los niños menores de 6 años de edad internados por infecciones invasivas (neumonía, meningitis, sepsis o bacteriemia), en los que se aisló Spn de un sitio previamente estéril. Se realizó control externo de confiabilidad de los resultados, inicialmente en el laboratorio de referencia de Canadá y actualmente en el Instituto Adolfo Lutz de San Pablo. El análisis estadístico se realizó con EPIINFO 6 y con el programa WHONET de OMS. Resultados: Se destacan los siguientes hallazgos: Se aisló Spn en 1 390 muestras clínicas. La edad media (N = 1 175) fue de 19,5 meses, con 74,4% - 2 años; fueron neumonías 60,5%; meningitis 26,6%, sepsis 8,2%. Se halló sensibilidad disminuida a penicilina en 32,1% (414/1 288), con resistencia alta 16,1% e intermedia 16%. La resistencia a los antibióticos betalactámicos se incrementó en el período 1993-8. Los serotipos prevalentes fueron: 14 (32,5%), 6A/6B, 9V, 23F, 19F, 18C, 4, 5, 1. Los serotipos 14 y 6AB prevalecieron en menores de 2 años mientras que el 5 y el 1 prevalecieron en mayores de 2 años (p < 0,001). El serotipo 14, la neumonía y la edad menor de 2 años fueron los factores de riesgo para resistencia a penicilina. La representatividad para las vacunas conjugadas 7-valente, 9-valente y 11-valente fue de 53,3%, 77,4% y 82,6% respectivamente, mayor para la 7-valente en los menores de 2 años (61,2%) y en los menores de 2 años con neumonía (71,2%). Más del 90% de los serotipos aislados con algún grado de resistencia estuvieron contenidos en las tres vacunas analizadas. Conclusiones: Un programa nacional de vigilancia de Spn invasivo fue desarrollado en Argentina y otros países latinoamericanos. Se identificaron por primera vez los serotipos predominantes en infecciones invasivas y se comprobó el incremento significativo de la resistencia a penicilina y otros antibióticos, similar a lo informado en casi todos los países del mundo. Abstract

Background: Childhood pneumonia is a major cause of childhood illness and the second leading cause of child death globally. Understanding the costs associated with the management of childhood pneumonia is essential for resource allocation and priority setting for child health.Methods: We conducted a systematic review to identify studies reporting data on the cost of management of pneumonia in children younger than 5 years old. We collected unpublished cost data on non-severe, severe and very severe pneumonia through collaboration with an international working group. We extracted data on cost per episode, duration of hospital stay and unit cost of interventions for the management of pneumonia. The mean (95% confidence interval, CI) and median (interquartile range, IQR) treatment costs were estimated and reported where appropriate.Results: We identified 24 published studies eligible for inclusion and supplemented these with data from 10 unpublished studies. The 34 studies included in the cost analysis contained data on more than 95 000 children with pneumonia from both low- and-middle income countries (LMIC) and high-income countries (HIC) covering all 6 WHO regions. The total cost (per episode) for management of severe pneumonia was US$ 4.3 (95% CI 1.5-8.7), US$ 51.7 (95% CI 17.4-91.0) and US$ 242.7 (95% CI 153.6-341.4)-559.4 (95% CI 268.9-886.3) in community, out-patient facilities and different levels of hospital in-patient settings in LMIC. Direct medical cost for severe pneumonia in hospital inpatient settings was estimated to be 26.6%-115.8% of patients' monthly household income in LMIC. The mean direct non-medical cost and indirect cost for severe pneumonia management accounted for 0.5-31% of weekly household income. The mean length of stay (LOS) in hospital for children with severe pneumonia was 5.8 (IQR 5.3-6.4) and 7.7 (IQR 5.5-9.9) days in LMIC and HIC respectively for these children.Conclusion: This is the most comprehensive review to date of cost data from studies on the management of childhood pneumonia and these data should be helpful for health services planning and priority setting by national programmes and international agencies. Abstract

The refinement of tightly regulated prokaryotic expression systems that permit functional expression of toxic recombinant proteins is a continually evolving process. Unfortunately, the current best promoter options are either tightly repressed and produce little protein, or produce substantial protein but lack the necessary repression to avoid mutations stimulated by leaky expression in the absence ofinducer. In this report, we present three novel prokaryotic expression constructs that are tightly regulated by L-rhamnose and D-glucose. These expression vectors utilize the Escherichia coli rhaT promoter and corresponding regulatory genes to provide titratable, high-level protein yield without compromising clone integrity. Together, these components may enable the stable cloning and functional expression of otherwise toxic proteins.

Abstract

Background: RNA polymerase III (POLR3)-related leukodystrophy is a rare, neurodegenerative disorder characterized by hypomyelination, hypodontia, and hypogonadotropic hypogonadism. Despite the challenges of caring for a child with POLR3-related leukodystrophy, few studies have examined parents' disease burden. We sought to investigate quality of life and stress levels amongst parents of children with POLR3-related leukodystrophy. Methods: 43 parents of 32 children completed questionnaires on demographics, stress, quality of life, coping mechanisms, and experience of injustice. Detailed clinical data was collected from all patients. Results: Mothers (t [27] = –8.66, P <.001) and fathers (t[16] = –4.47, P <.001) had lower quality of life scores compared to the normative population, yet 80% of parents' stress scores fell within the normal stress range. Parents' experience of injustice scores were high (>60). Correlations were found between and within parents' scores. Years since disease onset and certain life circumstances correlated to mothers' quality of life scores; however, no correlation was found between modifiable factors and fathers' quality of life scores. Helpful coping mechanisms included those that allowed parents to be involved in their child's life. Conclusions: This is the first study to assess stress and quality of life in this population. Abstract

Lassa fever is a serious epidemic viral disease in West Africa affecting an estimated 2 million people annually with about 5000–10,000 deaths, although supporting data is sparse. Lassa fever significantly affects neonates, children, and pregnant women, however, comprehensive data on its impact in these populations are lacking. We reviewed the available literature on Lassa fever to assess its prevalence and impact in these populations and implications for vaccine development. Clinical features in children were similar to those observed in adults, with complications such as bleeding. Altered mental status, anasarca (swollen baby syndrome), bleeding, and poor urine output were risk factors for death. The case fatality rate (CFR) in 16 paediatric studies ranged from 6 % to 63 % and was 66.7 % and 75.0 % in two neonatal studies. In a systematic review of studies on pregnant women the CFR was 33.73 %. The adverse foetal outcomes included miscarriage, stillbirth, and intrauterine death associated with maternal death. Since Lassa fever significantly affects neonates, children, and pregnant women, developing a safe and effective, single-dose vaccine for these high-risk populations is vital. Currently, there are four clinical trials assessing Lassa virus vaccines. Only one of these trials is enrolling children aged ≥18 months, and exclude pregnant and breast-feeding women. It is essential that pregnant and breast-feeding women and young children are included in clinical trials that incorporate robust safety surveillance and risk mitigation measures. In our review, potential approaches to address the specific gaps in the areas of diagnosis, management, and prevention of Lassa fever in these specific populations, such as disease surveillance systems and vaccine development, were identified. A comprehensive strategy with investment focused on addressing specific knowledge gaps will be essential in protecting the health of these specific populations in Lassa virus endemic regions.
