La ciudad de Benarés se alza sobre la orilla oeste del Ganges saludando al Sol todas las mañanas desde los ghats. Unas construcciones escalonadas con carácter independiente que aportan la identidad de esta ciudad. Escalones que se mantienen vivos durante todas las horas del día, aunque algunos estén en contacto directo con la muerte. Son espacios donde tienen lugar una superposición de rituales, actividades desde lo más cotidianas a lo más trascendentales. Gracias al ritual se genera un sentimiento de comunidad y el espacio se vuelve atemporal. Entender el origen es fundamental para la comprensión de esta cuestión, un origen que se remonta a 3500 años. La sacralidad de esta ciudad y el respeto de la cultura hindú hacia su propia cultura, patrimonio e historia, hacer perdurar a Benarés en el tiempo. Como dirían los hindúes, no se trata de vivir en el tiempo, sino de creer que no existe nada fuera de ese tiempo. Vivir inmersos en la realidad de nuestro tiempo nos hace desestimar la eternidad. Gracias a un viaje de expedición, se toma conciencia de este fenómeno en persona, viviendo el ritual y participando en él, experimentando en directo como convive con la arquitectura. El escalón se convierte en edificio, se prolonga, se quiebra, se extruye, se habita… Es una arquitectura única tanto de esta ciudad como del mundo entero. Es indisoluble pensar en ghats sin el ritual, una capa más de la arquitectura y lo que da lugar a la consolidación de estos espacios en el tiempo.

Purpose The objective is to present a case of well-leg compartment syndrome in the Galdakao-modified supine Valdivia position. Results The case of a 32-year-old male, obese (105 Kg) and a former smoker is presented. The patient was positioned in the Galdakao-modified supine Valdivia position, with lower limbs bandaged, to perform a right percutaneous nephrolithotomy. In the immediate postoperative period, significant pain was reported in the left lower limb. The limb appeared oedematous and cyanotic, although pedis pulses were preserved. Doppler ultrasound ruled out venous thrombosis. Suspecting compartment syndrome, the patient underwent a complete decompression fasciotomy of the four left leg compartments. After the surgery, values of creatine phosphokinase reached 80.000 UI/L and serum creatinine levels were 1.53 mg/dL. The patient was taken to the intensive care unit. Six months after the episode, the patient still needs rehabilitation care. The compartment syndrome is a rare complication in lithotomy position, but never described in the Galdakao-modified supine Valdivia position before, with the lower limbs in moderate flexion, and with the ipsilateral lower limb in a slightly inferior position with respect to the other. It may lead to skin necrosis, permanent neuromuscular dysfunction, myoglobinuric renal failure, amputation and even death. Therefore, this complication must be suspected and early decompression of the compartment must be performed. Risk factors include obesity, peripheral vascular disease (advanced age, hypertension, hyperlipidemia and diabetes mellitus), height, hypothermia, acidemia, BMI, male sex, combined general-spinal anesthesia, prolonged surgery time, systemic hypotension, ASA (American Society of Anesthesiologists) class, lack of operative experience, vasoconstricting drugs, important bleeding during the surgery and increased muscle bulk. Conclusion Compartment syndrome is a potentially life-threatening complication that may occur in the Galdakao-modified supine Valdivia position. It should be suspected in cases with risk factors and compatible clinical symptoms and signs, and treated rapidly to avoid further complications.

This chapter explores the testicular involvement of lymphoma. Testicular lymphoma may either represent secondary involvement by systemic disease or primary malignancy. Regarding primary testicular lymphoma (PTL), it is a rare form of extranodal lymphoma and the most frequent malignant testicular neoplasm in men over the age of 60 years. The diffuse large B-cell lymphoma (DLBCL) accounts for the majority of cases. The morphologic manifestation of PTL on imaging may be in the form of a localized mass or a diffuse enlargement of the testis. On ultrasonography, PTL usually appears as a hypoechoic area with hypervascularity. MRI and positron emission tomography with computed tomography (PET/CT) are useful diagnostic tools. The latter is crucial in staging and follow-up of these patients. The treatment of PTL is based on orchiectomy, chemotherapy, and radiotherapy. The prognosis is poor and PTL exhibits a propensity to relapse in the central nervous system (CNS) and in the opposite testis. Secondary involvement of the testis by non-Hodgkin lymphoma (NHL) is more frequent than PTL. Patients may develop the relapsed or refractory disease in the testis in the context of disseminated lymphomas due to the existence of the blood-testis barrier. This chapter discusses the treatment of secondary involvement by lymphoma.

La Innovación educativa busca estrategias, recursos y metodologías del modelo constructivista, visibilizando la forma en que los estudiantes aprenden y la gestión de aprendizaje autónomo. Objetivo: determinar la medida en que el Portafolio influye significativamente en el aprendizaje autónomo de los estudiantes de la Escuela de Arquitectura (ESAQ) de la Universidad Nacional Jorge Basadre Grohmann (UNJBG); material y métodos: aplicada de nivel explicativo; diseño cuasi experimental; la muestra estuvo compuesta por 30 estudiantes de la asignatura Planeamiento Urbano II. Se validó el instrumento utilizado, el cual consistía en un pre y post test con escala de Likert, para medir el nivel de aprendizaje autónomo, basados en sus cuatro niveles: Cognitivo, Metacognitivo, Criterial y Motivacional; y un cuestionario de preguntas abiertas que recogió las experiencias personales sobre el uso de la estrategia Portafolio. Resultados: hubo un incremento significativo en el aprendizaje autónomo en los estudiantes de la ESAQ - UNJBG 2019, después de aplicar el Portafolio verificado; con un p-valor=0,00 una significancia al 0,05% y nivel de confianza al 95%. Conclusión: los resultados pueden reforzarse al señalar que, reflexionar sobre su propio aprendizaje les motiva a mejorar Constantemente.

RESUMEN: Este Trabajo de Fin de Grado pretende mostrar un análisis detallado de la importancia de preservar la integridad de los datos de carácter personal. El desarrollo de los medios tecnológicos y de la economía de datos ha producido los últimos años un abuso en la utilización y tratamiento de los datos personales de los ciudadanos. Algunas grandes compañías tecnológicas, con el pretexto de obtener cuantiosos beneficios han vulnerado la integridad de los datos personales de sus usuarios: transfiriendo su información a terceros sin consentimiento o utilizando los datos para otros fines distintos a los que se consintió. Para evidenciar esta situación, se ha consultado tanto la jurisprudencia del Tribunal de Justicia de la Unión Europea como la normativa para analizar los supuestos en los que se ha debatido respecto la protección de los datos personales. Actualmente, los datos de carácter personal tienen una especial relevancia en el ámbito tecnológico debido a los grandes beneficios económicos que implica su tratamiento y posterior análisis. A lo largo de los años, de forma paralela al desarrollo de las tecnologías, se han evidenciado continuos abusos a la integridad de los datos personales de los ciudadanos por lo que las autoridades, concretamente la Unión Europea, ha respondido de forma contundente a estas situaciones por medio de distintas normas. ABSTRACT: This Final Degree Project shows a detailed analysis of the importance of preserving the integrity of personal data. The development of technological means and data economy has produced in recent years an abuse of citizens' personal data. Some technology companies, under the pretext of obtaining substantial profits, have violated the integrity of their users' personal data: transferring their information to third parties without consent or using the data for purposes other than those to which they consented. To highlight this situation, the case law of the Court of Justice of the European Union and regulations have been consulted to analyze the cases in which the protection of personal data has been discussed. Currently, personal data are of special relevance in the technological field due to the great economic benefits of their processing and subsequent analysis. Over the years, parallel to the development of technologies, continuous abuses of the integrity of citizens' personal data have become evident, and the authorities, specifically the European Union, have responded forcefully to these situations by means of different regulations and strategies. Objetivo: Determinar el impacto de la enfermería de práctica avanzada (EPA) en cuanto al progreso y evolución de la enfermedad en pacientes con enfermedad renal crónica en estadios tempranos de la Comunidad de Madrid. Metodología: Proyecto de investigación cuasiexperimental de tipo pre-post, analítico, prospectivo y longitudinal, con un grupo control en el Hospital Universitario Infanta Leonor y un grupo experimental en el Hospital General Universitario Gregorio Marañón de Madrid. La recogida de datos se realizará antes, durante y después de la intervención de enfermería. Beneficios: Reconocimiento de la profesión de enfermería, mejorar la práctica clínica, calidad de vida de los pacientes y reducción de costes sanitarios. Abstract: Objective: To determine whether the implementation of the figure of the advanced practice nurse (APN) has positive effects in terms of the progress and evolution of kidney disease in patients with chronic kidney disease in early stages of the disease of the Community of Madrid. Trabajo fin de grado. Defendido en junio de 2022. Introducción: el miedo a la muerte está relacionado con el miedo a lo desconocido, pérdida de uno mismo, sufrimiento y dolor. El problema existe cuando el miedo se convierte en patológico. Objetivo: evaluar las variables que influyen en el miedo a morir de las personas mayores, en una residencia de la tercera edad, situada en un pueblo de la Comunidad de Madrid en el año 2022. Material y métodos: se realizará un estudio de investigación cuantitativo, observacional, analítico de casos y controles, longitudinal, retrospectivo, asignando a los participantes de manera voluntaria. Beneficios: ayudar a las enfermeras a adaptarse a las diferentes necesidades que deben ser cubiertas en las personas mayores. Abstract: Introduction: fear of death is related to fear of the unknown, loss of self, suffering and pain. The problem exists when the fear becomes pathological. Objective: to evaluate the variables that influence the fear of death in elderly people, in a nursing home, located in a town in the Community of Madrid in the year 2022. Material and methods: a quantitative, observational, analytical case-control, longitudinal, retrospective, retrospective, quantitative research study will be carried out, assigning participants on a voluntary basis. Background: The purpose of this study was to describe temporal trends in the use of antiretroviral therapy (ART) among people living with HIV (PLWHIV) from the cohort of the Spanish HIV/AIDS research network (CoRIS), 2004-2020. Methods: We described the yearly evolution of the proportion of patients receiving ART and the most frequently prescribed antiretroviral drugs among newly recruited treatment-naïve patients and among all patients with active follow-up. Results: Of 15,539 patients included, 14,618 (94.1%) started ART during their follow-up. Regarding initial regimens, the use of 2NRTI plus 1NNRTI (which were the most frequently prescribed until 2014) and 2NRTI plus 1bPI decreased after 2014, being gradually replaced by INI-based triple therapies. Since 2019, other regimens started to be prescribed, mainly dual therapies. TDF/FTC/EFV was the single-tablet regimen (STR) most frequently prescribed as initial ART until 2012, decreasing thereafter as TDF/FTC/RPV, TDF/FTC/EVG/COBI, and ABC/3TC/DTG became available. TAF/FTC/BIC accounted for 53.6% of initial prescriptions in 2020, followed by DTG/3TC (24%). The percentage of patients on ART increased from 45.7% in 2004 to 98.2% in 2020. Among all patients receiving ART, regimens based on 2NRTI plus 1INI increased from 0.1% in 2007 to 53.3% in 2020. During 2007-2015, most patients were receiving TDF/FTC/EFV, which was replaced after 2017 by ABC/3TC/DTG. In 2020, 13.0% of patients were receiving dual therapies. Conclusions: Robust real-world data on ART use in PLWHIV over more than 15 years show historical trends in prescriptions with an unprecedented visualization of the contemporary treatment patterns. En este trabajo de fin de grado se ha realizado un estudio de la fabricaci´on y aplicaci ´on de estructuras fot´onicas en 3D en las escalas nanom´etrica y microm´etrica. Concretamente, el estudio se bas´o en el concepto de cristales fot´onicos y en gu´ıas de onda ´opticas microestructuradas como objetivo tecnol´ogico. El proyecto comienza definiendo el marco te´orico en el que est´an basadas estas estructuras de inter´es y sus interacciones con factores externos, como puede ser la luz. Para ello, se ha recurrido a conceptos te´oricos de estructuras peri´odicas que definen estos objetos fot´onicos y se ha realizado un repaso de definiciones y ecuaciones relacionadas tanto con el ´ambito del Electromagnetismo como con el de la ´ Optica. Adem´as, se ha estudiado en detalle la t´ecnica experimental desarrollada para la fabricaci´on de un patr´on peri´odico con nanoporos huecos en el material mediante la irradiaci´on l´aser de pulsos de femtosegundos. Durante este trabajo, el material utilizado fue el cristal de YAG. (“yttrium aluminium garnet”). Para lograr esto fue necesario el uso de diversos dispositivos tecnol´ogicos como un l´aser de femtosegundos, un “pulse-picker”, un sistema de nanoposicionamento 3D controlado por ordenador, un microscopio ´optico, un microscopio de electrones, una m´aquina de pulido ´optico, infraestructuras de grabado qu´ımico h´umedo y adem´as, herramientas de simulaci´on num´ericas (software comercial del BandSOLVE). Una vez estudiada la t´ecnica de nanolitograf´ıa para la obtenci´on de nanoporos, se procedi´o a la elaboraci´on de experimentos con la ayuda de la estudiante de tesis doctoral PhD, Franzette Paz Buclatin. El objetivo era llevar a cabo un estudio sobre las caracter´ısticas de esta t´ecnica de litograf´ıa basada en una t´ecnica descubierta por el tutor Dr. R´odenas. Por tanto, se realiz´o un an´alisis para estudiar la dependencia de la longitud de poros y su forma y tama˜no transversal submicrom´etricos en funci´on de diferentes par´ametros del proceso de fabricaci´on como: el ritmo de repetici´on del l´aser, energ´ıa de pulso o velocidad de escritura del l´aser. Para alcanzar este objetivo se hizo uso del programa de procesamiento de im´agenes ImageJ. En este ´ultimo caso fue necesario recurrir a servicios adicionales como el SEGAI para la utilizaci´on del microscopio electr´onico de barrido. Adem´as, durante este proyecto, tambi´en se utiliz´o el software BandSOLVE RSOFT. La aplicaci´on de este software permiti´o poner en pr´actica los conocimientos te´oricos sobre estructuras peri´odicas en un sencillo marco de simulaci´on num´erica. De esta manera, fue posible realizar una optimizaci´on de una red hexagonal con el objetivo de obtener las caracter´ısticas m´as adecuadas para el dise˜no y estudio de una gu´ıa de ondas fot´onica, desde el rango del UV hasta el IR en el espectro electromagn´etico. El rango electromagn´etico de inter´es fue limitado al rango de transparencia del YAG, desde aproximadamente 250 nm (UV) hasta 5000 nm (mid-IR), para lo cual se tuvo siempre en cuenta la dispersi´on del ´ındice de refracci´on del cristal. In this final degree project, a study of the fabrication and implementation of 3D photonic structures at nanometric and micrometric scales was carried out. In particular, the study was based around the concept of photonic crystals and microstructured optical waveguides, as technology goal. It starts defining the theoretical framework in which these structures of interest are based on and their interactions with external factors, such as light. To do this, it was necessary to go to theoretical concepts of periodic structures which define these photonic objects and a review of the Electromagnetism and Optics definitions and equations has been done. Furthermore, the experimental technique for fabricating a periodic pattern with hollow nanopores in the material by means of femtosecond-pulse laser irradiation, has been studied. During this project, the material which was used was YAG crystal (yttrium aluminium garnet). To achieve this, it was necessary the use of different technological devices such as a femtosecond-pulse laser, a pulse-picker, a computer controlled 3D nanopositioning system, an optical microscope, an electron microscope, optical polishing machine, wet-chemical etching infrastructure, and also numerical simulation tools (commercial BandSOLVE software). Once the nanolithography technique used for obtaining nanopores was studied, the elaboration of experiments was carried out, also with the assistance of PhD student, Franzette Paz Buclatin. The goal was to study the characteristics of this lithography technique, which is based on a technique discovered by the supervisor Dr. R´odenas. Therefore, an analysis was developed in order to study the dependence between the pore length, and also its sub-micron cross-sectional shape and size, as a function of some fabrication parameters such as: pulse repetition rate of the laser, pulse energy, or laser writing speed. To achieve this objective, the use of the image processing software ImageJ was needed. For this last task, it was necessary additional support from SEGAI facilities with the purpose of using the scanning electron microscope (SEM). Also, through this project, the BandSOLVE RSOFT software was used. The implementation of this software allowed to put into practice the theoretical knowledge about periodic structures in an easy to use numerical simulation framework. Doing this, it was possible to make an optimization process of an hexagonal lattice with the purpose of obtaining the most suitable characteristics for designing and studying a photonic waveguide, from the UV to the IR range in the electromagnetic spectrum. The EM range of interest was limited to the transparency range of YAG, from around 250 nm (UV) to 5000 nm (mid-IR), for which the dispersion of the index of refraction of the crystal was always taken into account.
