Abstract

Aim: This study sought to evaluate the accuracy of the Ages and Stages Questionnaires 3rd Edition (ASQ-3) in identifying developmental delay (DD) in children with congenital heart disease (CHD) born at term who underwent surgical repair. Methods: Participants had to complete ASQ-3 and Bayley Scales of Infant and Toddler Development 3rd Edition (BSID-III) at 12 and 24 months. A child was considered at risk of DD for a ASQ-3 domain when he scored below the cutoff (≤-1SD or ≤-2SD). A child had a DD in a BSID-III domain when the score was ≤-1SD. The validity for each ASQ-3 domain and for overall ASQ-3 was measured. Results: At 12 months (n = 64), overall ASQ-3 (≤-2SD) sensitivity was 88%, specificity 74%. At 24 months (n = 82), overall ASQ-3 (≤-2SD) sensitivity was 74%, specificity 88%. Conclusion: The results support the utility of the ASQ-3 for screening the overall risk of DD in children with CHD. Abstract

Purpose: This retrospective study aims to describe the gross motor development of children aged 4 to 24 months with congenital heart disease (CHD) enrolled in a systematic developmental follow-up program and to describe the frequency of physical therapy sessions they received between 4 and 8 months of age. Methods: Twenty-nine infants with CHD underwent motor evaluations using the AIMS at 4 months, and the Bayley-III at 12 and 24 months. Results: Based on AIMS, 79% of 4-month-old infants had a gross motor delay and required physical therapy. Among these, 56.5% received one to two physical therapy sessions, and 43.5% received three to six sessions. Infants who benefited from regular interventions tended to show a better improvement in motor scores from 12 to 24 months. Conclusion: This study highlights the importance of early motor screening in infants with CHD and suggests a potential benefit of early physical therapy in at-risk children. Abstract

The aim of this study is to explore participants' views and experiences of an eHealth phase 3 cardiac rehabilitation (CR) intervention: Physical Activity Towards Health (PATHway). Sixty participants took part in the PATHway intervention. Debriefs were conducted after the six-month intervention. All interviews were audio recorded and transcribed verbatim. Transcripts were analysed with Braun and Clarke's thematic analysis. Forty-four (71%) debriefs were conducted (n = 34 male, mean (SD) age 61 (10) years). Five key themes were identified: (1) Feedback on the components of the PATHway system, (2) Motivation, (3) Barriers to using PATHway, (4) Enablers to using PATHway, and (5) Post programme reflection. There were a number of subthemes within each theme, for example motivation explores participants motivation to take part in PATHway and participants motivation to sustain engagement with PATHway throughout the intervention period. Participant engagement with the components of the PATHway system was variable. Future research should focus on optimising participant familiarisation with eHealth systems and employ an iterative approach to development and evaluation. [ABSTRACT FROM AUTHOR]

138. What's Wrong with the Global Slavery Index?

Abstract

The Global Slavery Index (GSI), which has been produced by the Walk Free Foundation in 2013, 2014 and 2016, seeks to calculate the number of victims of human trafficking (or ‘modern slavery’) in each country and to assess and rank government responses. Using the latest iteration of the Index, this article examines each of the three elements (vulnerability measurement; prevalence measurement and response measurement), making some preliminary findings about the quality of the methodology and its application under each heading. It concludes with a consideration of two broader issues: (i) the conspicuous lack of critical engagement with the Index; and (ii) what the Index reveals about the changing face of anti-trafficking/anti-slavery work—most particularly, the growing involvement of metrics-focused strategic philanthropy in defining the ‘problem’ and directing responses.

Abstract

Purpose: Cardiovascular disease (CVD) is the leading cause of premature death and disability in Europe. Effective exercise-based cardiac rehabilitation (CR) can improve mortality and morbidity rates, yet uptake of community-based CR is low (1). PATHway (Physical Activity Towards Health) is a technology-enabled lifestyle behaviour change intervention designed to enhance patient self-management of CVD through adherence to physical activity and other health behaviours. This paper explains the co-design and user validation process that is being employed for the development of the PATHway platform. Methods: CVD patients from a) hospital-based CR and b) community-based CR across two sites (Dublin, Ireland; Leuven, Belgium) are invited to participate in the study. To facilitate an iterative process, three separate rounds of semi-structured interviews, a total of twelve focus groups (4 groups x 3 rounds) are planned between February and April 2016. In round one interviews, participants were exposed to the PATHway intervention and system designed by the research team, and feedback was elicited. Interviews were audio-recorded, transcribedand content analysed. Key recommendations regarding technical and intervention content were identified and are currently being used by the research team to improve the PATHway system. Completion of the interviews is due for April 2016 Results: Round one data indicated that i) tailoring intervention content for exercise prescription and health behaviour change was important; ii) further health information and monitoring through the system was valuable, and iii) the potential for patients to be connected with their healthcare professionals was appealing. Technical recommendations included amending the avatar display to reflect an average BMI and be ‘more human-like’. Additional behaviour change recommendations included stressing the importance of positive reinforcement even when a participant is not meeting set goals. Conclusions: An iterative co-design and user validation process can be a vital component of the development of complex behavioural interventions for CVD patients. 147. Editorial: The Problems and Prospects of Trafficking Prosecutions: Ending impunity and securing justice

Abstract

Having been guest editor of the very first issue of the Anti-Trafficking Review, it was with great pleasure that I accepted the invitation of the editorial board to oversee the production of its sixth issue. Back in 2012 I identified the emergence of the Anti-Trafficking Review, thefirst specialist journal on human trafficking, as a watershed moment, signalling the transformation of‘trafficking’ from a niche (perhaps even a fringe) academic sub-discipline into a legitimate, substantial and discrete area of study. The past four years since its launch have vindicated that assessment. New specialist journals on trafficking and its variants have been launched, and the range and depth of research being undertaken in this field has significantly expanded. While law, sociology and human rights continue to be the dominant lenses through which trafficking is studied, analysed and explained, there is no denying the expanding and enriching influence of other disciplines: from geography to anthropology; from health sciences to migration studies. These changes in research and writing around trafficking have brought tangible benefits: helping improve our understanding of what is happening and why, as well as strengthening the evidence base on which credible, effective responses can be built.

Abstract

Background Cardiovascular disease (CVD) is the leading cause of premature death and disability in Europe. Effective cardiac rehabilitation (CR) can improve mortality and morbidity rates, yet uptake of community-based CR is low (Department of Health and Children, 2010). PATHway (Physical Activity Towards Health) is a technology enabled behaviour change intervention designed to enable patients to self-manage their health. Aims From a patient’s perspective, this study aims to determine the key personal, social and physical factors that inhibit or promote a patient’s capacity to engage with a technology enabled CR programme. From a healthcare professional’s perspective, this study aims to explore a) opinions and preferences for the most appropriate content and most viable approach for delivery of the PATHway intervention, b) strategies for recruitment and retention of study participants, c) treatment adherence, and d) strategies to overcome barriers to the acceptability of the intervention in the proposed study. Methods 40 individual patient interviews and 21 individual healthcare professional interviews were conducted across two sites (Dublin, Ireland and Leuven, Belgium). The COM-B model of behaviour change (Michie et al. 2011) was used to develop interview scripts and provided a framework to analyse all interviews assessing 1) capability, 2) opportunity and 3) motivation to engage with PATHway. Thematic analysis was also used (Braun & Clarke, 2006). Results Capability: Some patients felt unsure of physical capability to engage with PATHway, while psychological capability could be summarized in terms of patients’ psychological readiness but also technological readiness to engage with the intervention. Opportunity: Participants felt the main obstacles were around time, space and equipment. Patients suggested a family-wide intervention may facilitate greater opportunity for engagement.
