111. JTabWb: A Java Framework for Implementing Terminating Sequent and Tableau Calculi

Abstract

JTabWb is a Java framework for developing provers based on sequent or tableau calculi. It provides a generic engine which searches for proof of a given goal driven by a user-defined prover. The user is required to define the components of a prover by implementing suitable Java interfaces. In this paper we describe the structure of the framework and the role of its components through a running example. To show the generality of the framework we review some of the provers implemented in JTabWb. Finally, to corroborate the fact that the framework can be used to generate efficient provers, we compare the performances of one of the implemented provers with the state-of-the-art provers for Intuitionistic Propositional Logic.

Abstract

Background: We report a prospective, nationwide cohort evaluating the safety and effectiveness of CT-P13. Methods: A structured database was used to record serious adverse events (SAEs), clinical remission/response, inflammatory biomarkers (CRP and calprotectin), and endoscopic findings. Results: Eight hundred ten patients with inflammatory bowel disease (IBD) (452 Crohn's disease [CD]) were enrolled. Four hundred fifty-nine patients were naïve to anti-TNFα (group A), 196 had a previous exposure (group B), and the remaining 155 were switched to CT-P13 (group C). All patients were included in the safety evaluation with a mean follow-up of 345 ± 215 days and a total number of 6501 infusions. One hundred fifty-four SAEs were reported (19%), leading to cessation of the biosimilar in 103 subjects (12.7%). Infusion reactions were 71, leading to cessation of the biosimilar in 53 subjects (6.5%), being significantly more frequent in patients pre-exposed to anti-TNFα (P = 0.017). The efficacy of therapy was calculated in 754 IBD patients, with a mean follow-up of 329 ± 202 days. Forty-eight patients had a primary failure (6.4%), and 188 (25.6%) lost response during follow-up. Six hundred twenty-eight (364 CD) and 360 IBD patients (222 CD) completed the follow-up at 6 and 12 months, respectively. At 12 months, patients without loss of response were 71%, 64%. and 82% in groups A, B, and C, respectively (log rank P = 0.01). Clinical/endoscopic scores and inflammatory biomarkers dropped significantly in CD and UC patients (P = 0.01 and P < 0.0001) compared with baseline. Conclusions: In this large prospective cohort, no further signals of difference in safety and effectiveness of CT-P13 in IBD has been observed.

In this paper, we present a preliminary result on the generation of Hilbert proofs for classical propositional logic. This is part of our ongoing research on the comparison of proof-search in different proof-systems. Exploiting the notion of evaluation function, we define a fully deterministic terminating decision procedure that returns either a derivation in the Hilbert calculus of the given goal or a counter model witnessing its unprovability.

Abstract

Il rischio biologico (RB) in ambito sanitario è universalmente riconosciuto per tutti gli operatori sanitari (OS) esposti al contatto con materiale biologico o strumenti medici potenzialmente contaminati da agenti biologici. In Italia, le esposizioni a RB negli OS sono molto frequenti e rappresentano la prima causa di infortunio (40% di tutti gli infortuni denunciati). MATERIALI E METODI: Un’indagine descrittiva retrospettiva degli incidenti a RB è stata condotta presso l’A.O.U.P. “P. Giaccone” di Palermo analizzando le schede di segnalazione degli incidenti pervenute presso l’U.O. di Prevenzione e Sorveglianza delle Infezioni Ospedaliere negli anni 2005-2016. RISULTATI: Sono stati registrati 765 incidenti a RB (range 43-81/anno, media 65) che hanno coinvolto infermieri (35.1%), medici (9.6%), tecnici (8.2%) con una frequenza maggiore nell’area chirurgica (42.1%). Le cause più frequenti sono risultate: prelievo ematico (14.2%), somministrazione di terapie iniettive (11.4%), gestione dei cateteri (10.6%). La modalità di esposizione più frequente è stata la puntura (75.7%), seguita dalla contaminazione muco cutanea (15.8%) e dal taglio (8.1%). Tali risultati sono stati messi in relazione con l’uso dei dispositivi di protezione individuale (DPI) tra gli OS (64.9% per il camice, 62.7% per i guanti, 18.7% per il filtrante facciale con un trend d’uso crescente negli anni). È stato valutato, inoltre, il volume di attività ospedaliere come i ricoveri ordinari e le prestazioni erogate in relazione alla quantità di personale impiegato. La copertura vaccinale osservata tra gli OS è stata: 79.6% per anti epatite B, 13.0% per antitubercolare, 15,0% per antitetanica e 2.5% per antitifica, con trend crescente negli ultimi anni. Non è stata registrata nessuna sieroconversione. CONCLUSIONI: Negli anni si è registrato un trend negativo (-9.2%) di accadimento d’incidenti a RB in relazione alla diminuzione degli OS in servizio (-12.2%), all’aumento delle prestazioni sanitarie erogate (+39.8% dal 2010) e alla diminuzione dei ricoveri ordinari (–39.1%). I fattori determinanti la riduzione degli incidenti potrebbero essere la continua sensibilizzazione nei reparti alle buone prassi di lavoro, alla pratica delle vaccinazioni, all’uso di adeguati DPI e all’incremento dei programmi di formazione sulla sicurezza aziendale e sulla prevenzione del RB.

125. Effect of tumour necrosis factor antagonists on avoiding surgery in stricturing Crohn's disease: a tertiary centre real-life experience

Abstract

BackgroundStenosis is the most common complication of Crohn’s disease (CD), often requiring surgical resection. Long-term outcome of patients receiving anti-TNF therapy for such disease complication is poorly known.MethodsEnrolled were 51 CD patients followed-up in a tertiary IBD Centre between July 2006 and November 2015. All of them had stricturing CD (ileal = 49 and colonic = 2), diagnosed by colonoscopy and/or MRI enterography. Thirty-one subjects (61%) were given adalimumab and 20 (39%) infliximab. The primary outcome was the rate of surgery for stricturing CD. Statistical analysis included descriptive analysis, Wilcoxon test for differences in median values, logistic regression with univariate analysis for risk factors, and Kaplan–Meyer curves and Cox analysis for estimation of efficacy of anti-TNFs in avoiding surgical resection. All differences were considered statistically significant for p < 0.05.ResultsIn the study, 51 CD patients (27 males; median age at diagnosis 31.9 yr [range 12–61]) were analysed. Median duration of disease at the beginning of anti-TNF therapy was 4.4 yr (range 0.3–37). The location of CD was ileal in 22 subjects, colonic in one, and ileocolonic in 28 patients. After a median follow-up period of 35.8 months (range 3–105), 20/51 patients (39%) underwent abdominal surgery. Amongst patients not undergoing surgery, half of them (25/51) continued anti-TNF therapy at last follow-up and 6/51 (11.7%) stopped anti-TNF treatment (1 for secondary loss of response, 1 for infusion reaction, 2 for psoriasis, 1 for pregnancy, and 1 for recurrent tonsillitis). Median values of the Harvey–Bradshaw Index (HBI) and the Simple Endoscopic Score for Crohn’s Disease (SES-CD) were reduced significantly from baseline (p = 0.01 for both), whereas the median value of the Lémann score did not change significantly (4.9 vs 4.2, p = 0.28). Based on univariate analysis, only penetrating behaviour at baseline was more likely to be associated with the risk of surgery (OR 5.09; CI 95% 1.45 to 17.82; p = 0.01). Survival curve analysis and Cox regression model showed that patients treated with infliximab were more likely to avoid surgery compared with those treated with adalimumab (HR 3.13, CI 95% 1.63–12.67, p = 0.0038 and OR 6.54; CI 95% 0.66–3.09; p = 0.0026, respectively). The use of infliximab and longer duration of anti-TNF therapy were protective from surgery (OR 1.05; CI 95% 0.01–0.08; p = 0.0017).ConclusionHalf of CD patients starting anti-TNF therapy for stricturing disease avoided surgery after a median follow-up of 3 years. Penetrating behaviour was associated with an increased risk of surgery. Patients treated with infliximab remained free of surgery longer than patients treated with adalimumab did.

128. Prevalence of Bowel Damage Assessed by Cross-Sectional Imaging in Early Crohn's Disease and its Impact on Disease Outcome

Abstract

Background and Aims: Bowel damage in Crohn's disease [CD] is defined as the presence of intestinal strictures, fistulas or abscesses. Early disease may represent a window of opportunity for timely intervention. We evaluated disease activity and severity by the Lemann Index [LI] and the Magnetic Resonance Index of Activity [MaRIA] score, and their prognostic value in early CD. Methods: All consecutive patients diagnosed with CD in two referral centres, assessed by magnetic resonance imaging or computerized tomography, were prospectively included. Disease activity and bowel damage in early CD, the correlation between the LI and the MaRIA score, and the value of cross-sectional imaging findings in predicting disease progression were assessed. Statistical analyses employed time-to-event methods. Results: We included 142 consecutive CD patients. Median time from diagnosis to baseline imaging was 0.3 years median follow-up time was 4.9 years. At diagnosis, 39.4% of CD patients had bowel damage. Biological agents are widely used in rheumatology, dermatology and inflammatory bowel disease. Evidence about their efficacy and safety has been strengthened for all those therapeutic indications over the last decade. Biosimilar agents are monoclonal antibodies similar to previously approved biologics. In the European Union, they have been approved for all the indications in the management of immune-mediated inflammatory diseases (IMIDs), although data only in rheumatoid arthritis and ankylosing spondylitis are currently available. Direct evidence on efficacy, safety, and immunogenicity of biosimilars is mandatory in psoriasis, psoriatic arthritis, and inflammatory bowel disease, as well as in children. Based on the current evidence in the literature, we present the joint official position of the Italian Societies of Rheumatology, Dermatology and Inflammatory Bowel Disease on the use of biosimilars in IMIDs.

Abstract

OBJECTIVES: The long-term safety of exposure to anti-tumor necrosis factor (anti-TNF alpha) drugs during pregnancy has received little attention. We aimed to compare the relative risk of severe infections in children of mothers with inflammatory bowel disease (IBD) who were exposed to anti-TNF alpha drugs in utero with that of children who were not exposed to the drugs. METHODS: Retrospective multicenter cohort study. Exposed cohort: children from mothers with IBD receiving anti-TNF alpha medication (with or without thiopurines) at any time during pregnancy or during the 3 months before conception. Non-exposed cohort: children from mothers with IBD not treated with anti-TNF alpha agents or thiopurines at any time during pregnancy or the 3 months before conception. The cumulative incidence of severe infections after birth was estimated using Kaplan-Meier curves, which were compared using the log-rank test. Cox-regression analysis was performed to identify potential predictive factors for severe infections in the offspring. RESULTS: The study population comprised 841 children, of whom 388 (46%) had been exposed to anti-TNF alpha agents. Median follow-up after delivery was 47 months in the exposed group and 68 months in the non-exposed group. Both univariate and multivariate analysis showed the incidence rate of severe infections to be similar in non-exposed and exposed children (1.6% vs. 2.8% per person-year, hazard ratio 1.2 (95% confidence interval 0.8-1.8)). In the multivariate analysis, preterm delivery was the only variable associated with a higher risk of severe infection (2.5% (1.5-4.3)). CONCLUSIONS: In utero exposure to anti-TNF alpha drugs does not seem to be associated with increased short-term or long-term risk of severe infections in children.
