Abstract

Ethiopia is ninth among the world high tuberculosis (TB) burden countries, pastoralists being the most affected population. However, there is no published report whether the behavior related to TB are different between pastoralist and the sedentary communities. Therefore, the main aim of this study is to assess the pastoralist community knowledge, attitude and perceived stigma towards tuberculosis and their health care seeking behavior in comparison to the neighboring sedentary communities and this may help to plan TB control interventions specifically for the pastoralist communities.A community-based cross-sectional survey was carried out from September 2014 to January 2015, among 337 individuals from pastoralist and 247 from the sedentary community of Kereyu district. Data were collected using structured questionnaires. Three focus group discussions were used to collect qualitative data, one with men and the other with women in the pastoralist and one with men in the sedentary groups. Abstract

Conflict over natural resources is a widespread phenomenon in the global south. Trends in consumption, demographics, environmental degradation, and socio-political dynamics are exerting significant pressure on the availability and accessibility of natural resources. In many countries, the governance of land for commercial agricultural investment leads to conflict. Such conflicts are complex, and the drivers extend beyond resource scarcity to issues of access and competition. This paper is based on the results of field research on investments in large-scale Jatropha plantations for biofuel production in Ethiopia. A case study of a large-scale Jatropha plantation in eastern-central Ethiopia is used to reconstruct the history of a short-lived, large-scale biofuel development in Ethiopia, and to examine the processes and the reasons for its failure. Between 2006 and 2009, more than 400,000 ha of land were acquired for industrial biofuel development. Given the country's dependence on imported fossil fuels and the global rush for alternative energy sources, the Ethiopian government, not surprisingly, gave emphasis to investment in biofuel development, mainly through Jatropha plantation. However, after costly experimentation, the "miracle crop" did not deliver what had been promised. Instead, it dispossessed and displaced thousands of marginalized local smallholder agropastoralists, created conflict over water and land resources, and kept thousands of hectares of land out of production. In the end, the rhetoric of "Jatropha revolution" and promises of local and national development were never materialized; the government's ambitious biofuel strategy also failed to achieve its objectives. The main argument of this paper is that the failure of large-scale biofuel companies to fulfill their ambitious promises in Ethiopia is part of the general failure in the governance of large-scale commercial agriculture and misleading views toward, and unrealistic promises of, Jatropha investments. These negatively affected the livelihood of the local agropastoralists and their interactions with the companies, which caused instabilities and conflicts in the area. It is hoped that the findings of this research will contribute to knowledge production and policies on the governance of large-scale commercial agriculture.

111. Community-based prevalence of undiagnosed mycobacterial diseases in the Afar Region, north-east Ethiopia

Abstract

Background: Information on the community-based prevalence of tuberculosis (TB) in different settings is vital for planning, execution and evaluation of strategies to control the disease. Objective: To assess community-based prevalence of undetected active pulmonary TB (PTB) in pastoralists of the Amibara District. Methods: Between March and April 2010, a community-based cross-sectional survey of undiagnosed active PTB was conducted in the pastoralists of the Amibara District of the Afar Region, north-east Ethiopia. The study participants were interviewed for symptoms suggestive of PTB using a structured questionnaire. Sputum samples were collected and processed for smear microscopy and culture. Mycobacterium genus typing was performed using a multiplex polymerase chain reaction (PCR). Results: Out of 222 individuals who had symptoms suggestive of PTB, 4 (1.8%) were found positive by smear microscopy, while mycobacterial growth was observed on 62 (27.9%) samples. Mycobacterium genus typing was carried out for 42 of these 62 samples; 39 (92.9%) gave a positive signal for the genus Mycobacterium. Of these, 23 (59%) isolates proved to be members of the Mycobacterium tuberculosis (Mtb) complex, while the remaining 16 (41.0%) were found to be members of non-tuberculous Mycobacteria (NTM) species. Conclusion: Sputum culture is highly sensitive, and it is the gold standard for the bacteriological diagnosis of PTB, while smear microscopy is less sensitive to detect acid fast bacilli (AFB) in stained sputum smears. The findings of the present study warrant the strengthening of culture facility services in the study area. The study also provides important preliminary information on the status of NTM infection in the pastoral setting. Nevertheless, further investigations into the species identification of the NTM infections would be useful in the study area.

Abstract

AimsTo determine outcomes in atrial fibrillation patients undergoing percutaneous left atrial appendage occlusion (LAAO) based on the underlying stroke risk (defined by the CHA2DS2-VASc score).Methods and resultsData were extracted from the National Inpatient Sample for calendar years 2016-20. Left atrial appendage occlusion implantations were identified on the basis of the International Classification of Diseases, 10th Revision, Clinical Modification code of 02L73DK. The study sample was stratified on the basis of the CHA2DS2-VASc score into three groups (scores of 3, 4, and ≥5). The outcomes assessed in our study included complications and resource utilization. A total of 73 795 LAAO device implantations were studied. Approximately 63% of LAAO device implantations occurred in patients with CHA2DS2-VASc scores of 4 and ≥5. The crude prevalence of pericardial effusion requiring intervention was higher with increased CHA2DS2-VASc score (1.4% in patients with a score of ≥5 vs. 1.1% in patients with a score of 4 vs. 0.8% in patients with a score of 3, P < 0.01). In the multivariable model adjusted for potential confounders, CHA2DS2-VASc scores of 4 and ≥5 were found to be independently associated with overall complications [adjusted odds ratio (aOR) 1.26, 95% confidence interval (CI) 1.18-1.35, and aOR 1.88, 95% CI 1.73-2.04, respectively] and prolonged length of stay (aOR 1.18, 95% CI 1.11-1.25, and aOR 1.54, 95% CI 1.44-1.66, respectively).ConclusionA higher CHA2DS2-VASc score was associated with an increased risk of peri-procedural complications and resource utilization after LAAO. These findings highlight the importance of patient selection for the LAAO procedure and need validation in future studies.

Abstract

Predicting the probability of non-performing loans for individuals has a vital and beneficial role for banks to decrease credit risk and make the right decisions before giving the loan. The trend to make these decisions are based on credit study and in accordance with generally accepted standards, loan payment history, and demographic data of the clients. In this work, we evaluate how different Machine learning models such as Random Forest, Decision tree, KNN, SVM, and XGBoost perform on the dataset provided by a private bank in Ethiopia. Further, motivated by this evaluation we explore different feature selection methods to state the important features for the bank. Our findings show that XGBoost achieves the highest F1 score on the KMeans SMOTE over-sampled data. Abstract

Tuberculosis (TB) is among the leading causes of morbidity and mortality. The causative agent, Mycobacterium tuberculosis (Mtb), has evolved virulent factors for entry, survival, multiplication and immune evasion. Rv2031 (also called alpha crystallin, hspX, 16-kDa antigen), one of the most immunogenic latency antigens, is believed to play a key role in long-term viability of Mtb. Here, we report the dynamics of pro-inflammatory (IFN-γ, TNF-α) and anti-inflammatory (IL-10) cytokines against Rv2031 using whole blood assay in human cohorts in a TB endemic setting. Cytokine responses to ESAT-6-CFP-10 were also measured for comparison. Blood samples were collected from smear positive pulmonary TB patients and their contacts at baseline, 6 and 12 months, and from community controls at entry. At baseline, 54.4% of controls and 73.2% of contacts were QFT-GIT test positive. Abstract

This study investigates the spatial distribution of emergency alarm call events to identify spatial covariates associated with the events and discern hotspot regions for the events. The study is motivated by the problem of developing optimal dispatching strategies for prehospital resources such as ambulances. To achieve our goals, we model the spatially varying call occurrence risk as an intensity function of an inhomogeneous spatial Poisson process that we assume is a log-linear function of some underlying spatial covariates. The spatial covariates used in this study are related to road network coverage, population density, and the socio-economic status of the population in Skellefte{\aa}, Sweden. A new heuristic algorithm has been developed to select an optimal estimate of the kernel bandwidth in order to obtain the non-parametric intensity estimate of the events and to generate other covariates. Since we consider a large number of spatial covariates as well as their products, and since some of them may be strongly correlated, lasso-like elastic-net regularisation has been used in the log-likelihood intensity modeling to perform variable selection and reduce variance inflation from overfitting and bias from underfitting. As a result of the variable selection, the fitted model structure contains individual covariates of both road network and demographic types. We discovered that hotspot regions of calls have been observed along dense parts of the road network. Evaluation of the model also suggests that the estimated model is stable and can be used to generate a reliable intensity estimate over the region, which can be used as an input in the problem of designing prehospital resource dispatching strategies.

Abstract

Phytoplankton succession in open lakes depends on the availability of nutrients, temperature, light intensity and transparency. Phytoplankton communities usually undergo a fairly predictable annual cycle, but some species may grow exponentially forming the blooms. Accordingly, the seasonal diversity and dynamics of phytoplankton and their correlation with environmental factors were investigated in Lake Chamo, from December, 2013 to November, 2014. Samples were taken from four different sites of the lake; each of these sampling sites had their own unique characteristics. Seasonal plankton samples were collected using nylobolt plankton net (No. 25). pH, water temperature, specific conductivity, saturation, total dissolved solids, salinity and dissolved oxygen were measured in the field. The results showed that there is significant difference in the mean value of pH across the different seasons at the first sampling site, season II having significantly higher value than season I and IV. Water temperature also significantly varied among sampling sites and seasons, site 3 in spring 2014 having significantly higher value (30.4 ± 0.21 °C) than site 2 during summer 2014 (26.0 ± 0.12 °C). Cyanophyta, Chlorophyta and Bacillariophyta accounted 96% of the total phytoplankton abundance during the study period. Pediastrum, Scendesmus and Closterium species were the predominant Chlorophyta, during the rainy seasons. The average Cyanophyta count was positively and non-significantly correlated with water temperature and turbidity of the lake. Overall, the four seasons exhibit different phytoplankton species composition across different sites. The diversity was high during winter season in all sampling sites.

147. Tuberculosis in Goats and Sheep in Afar Pastoral Region of Ethiopia and Isolation of Mycobacterium tuberculosis from Goat

Abstract

A cross sectional study was conducted on 2231 small ruminants in four districts of the Afar Pastoral Region of Ethiopia to investigate the epidemiology of tuberculosis in goats and sheep using comparative intradermal tuberculin skin test, postmortem examination, mycobacteriological culture and molecular typing methods. The overall animal prevalence of TB in small ruminants was 0.5% (95% CI: 0.2%–0.7%) at ≥4 mm and 3.8% (95% CI: 3%–4.7%) at cutoff ≥2 mm. The herd prevalence was 20% (95% CI: 12–28%) and 47% (95% CI: 37–56%) at ≥4 mm and ≥2 mm cut-off points, respectively. The overall animal prevalence of Mycobacterium avium complex infection was 2.8% (95% CI: 2.1–3.5%) and 6.8% (95% CI: 5.8–7.9%) at ≥4 mm and ≥2 mm cut-off points, respectively. Mycobacteriological culture and molecular characterization of isolates from tissue lesions of tuberculin reactor goats resulted in isolation of Mycobacterium tuberculosis (SIT149) and non-tuberculosis mycobacteria as causative agents of tuberculosis and tuberculosis-like diseases in goats, respectively. The isolation of Mycobacterium tuberculosis in goat suggests a potential transmission of the causative agent from human and warrants further investigation in the role of small ruminants in epidemiology of human tuberculosis in the region.
