Abstract: Severity of vascular damage of white matter may predict hemorrhagic transformation (HT). We assess the relationship between leukoaraiosis (LA) severity and the type of hemorrhagic transformation in elderly patients treated with thrombolysis. We retrospectively analyzed the clinical data and pretreatment magnetic resonance imaging (MRI) of 180 consecutive ischemic stroke patients aged over 75 years. LA severity was graded according to the Fazekas scale, and acute diffusion-weighted-imaging (DWI) lesion volumes were semi-automatically outlined. Predictors of hemorrhagic infarction (HI) and parenchymal hemorrhage (PH) were identified using logistic regression analysis and exact multinomial logistic analysis. HT occurred in 31 patients (17 %). Baseline National Institute of Health Stroke Score (NIHSS; p = 0.008), severe LA (p = 0.02), and diffusion lesion volume (p = 0.02) were predictors of HT in univariable logistic regression. Adjusted to lesion volume and baseline NIHSS score, exact multinomial logistic analysis showed that severe LA was the only independent predictor of parenchymal hemorrhage (p = 0.03). In elderly patients, LA severity better predicts parenchymal hemorrhage than infarct size.

Abstract: Background: Tuberculin skin test (TST) is widely used for the assessment of Bacillus Calmette–Guérin (BCG) vaccine efficacy and screening of latent TB infection (LTBI). Poor or no data are available on the reactivity of tuberculin in Kassala State. The aim of the present study was to assess the response to the BCG vaccine and to estimate the prevalence of LTBI and the annual rate annual risk of tuberculous infection (ARTI) among vaccinated school children using TST. Methods: School-based cross-sectional study was conducted in three localities of Kassala State during 2016–2018. A cluster random sampling method was used for the enrolment. Five tuberculin units of 0.1 mL were injected intradermally in the left forearm of 2568 school children aged 5–15 years. The test was performed after the assessment of child health, nutrition status, and BCG scar status. Tuberculin reaction size was interpreted after 48–72 h. The collected data were analyzed using SPSS (v 20). The classical method was used to estimate ARTI. Results: Overall, there was no reaction in 81.5% of children. Only 0.66% of children had induration 10 mm–28 mm, indicating the prevalence of latent TB with an annual risk of 0.1%. Tuberculin reactivity was statistically significant affected by child age, gender, geographical location, and nutrition status (P < 0.05), whereas BCG scar status had no effect (P > 0.05). Conclusion: The study documented a high proportion of tuberculin nonreactivity irrespective of BCG vaccination status and provides data on the prevalence of latent infection among studied groups. Further studies are needed to address the reasons of low and nonreactivity of tuberculin, and evaluation of the BCG vaccine is recommended.

Abstract: Background Peer training has been identified as a useful tool for delivering undergraduate training in basic life support (BLS) which is fundamental as an initial response in cases of emergency. This study aimed to (1) Evaluate the efficacy of peer-led model in basic life support training among medical students in their first three years of study, compared to professional-led training and (2) To assess the efficacy of the course program and students’ satisfaction of peer-led training. Methods A randomized controlled trial with blinded assessors was conducted on 72 medical students from the pre-clinical years (1st to 3rd years in Syria) at Syrian Private University. Students were randomly assigned to peer-led or to professional-led training group for one-day-course of basic life support skills. Sixty-four students who underwent checklist based assessment using objective structured clinical examination design (OSCE) (practical assessment of BLS skills) and answered BLS knowledge checkpoint-questionnaire were included in the analysis. Results There was no statistically significant difference between the two groups in delivering BLS skills to medical students in practical (P = 0.850) and BLS knowledge questionnaire outcomes (P = 0.900). Abstract: Rationale, aims and objectives In the global context of population ageing, understanding and monitoring intensive care use by the elderly is a strategic issue. National-level data are needed to overcome sampling biases that often limit epidemiologic studies of the critically ill elderly. The objective of this study was to describe intensive care use for hospitalized elderly patients using secondary data from the French national hospital discharge database. Method Structured assessment of the national database coverage and accuracy; cross-sectional analysis of hospitalizations including at least one admission in an intensive care unit (ICU) for patients aged ≥ 80 years from 1 January to 31 December 2009. Results In 2009, people aged ≥ 80 years accounted for 5.4% of the population but 15.3% of the 215 210 adult hospitalizations involving intensive care in France. In this elderly group, the mean age was 84.0 (± 3.56) years, and 51.6% were male. In-hospital mortality was 33.9%. The median time spent in the ICU was 3 [interquartile range (IQR), 2–8] days, the median time spent in hospital was 14 (IQR, 8–24) days and 9% of hospitalizations ended by the patient's death involved intensive care. A surgical procedure was included in 43% of hospitalizations. Medical and surgical diagnosis-related group hospitalizations were characterized by significant differences in volume, mortality, ICU days and costs. Conclusions There was marked clinical heterogeneity in the population of elderly patients hospitalized in the ICU. These data provide baseline information and prompt further studies comparing intensive care utilization across age groups, between countries and over time.

Abstract: Mulibrey (MUscle-LIver-BRain-EYe) nanism is a rare autosomal recessive disease characterized by growth failure, dysmorphic features and a wide range of abnormalities affecting multiple organ systems. This report is the first to present two cases of Mulibrey nanism affecting two siblings from Syria. Mulibrey nanism can be suspected clinically due to the distinctive features of the patients. The aim of this report is to document the presence of Mulibrey nanism in Syria and to familiarize physicians in and out of Syria with this rare disease and encourage them to develop high clinical suspicion if faced with patients with similar presentations.

Abstract: The two dimensional stagnation flows towards a shrinking sheet of Newtonian fluids has been solved numerically by using SOR Iterative Procedure. The similarity transformations have been used to reduce the highly nonlinear partial differential equations to ordinary differential equations. The results have been calculated on three different grid sizes to check the accuracy of the results. The problem relates to the flows towards a shrinking sheet when 0 α the flows towards a stretching sheet. The numerical results for Newtonian fluids are found in good agreement with those obtained previously.

Abstract: Background This study was conducted in Kassala Teaching Hospital, Kassala State, Sudan (January 2006–June 2008) to determine the rate of mycobacterium drug resistance to anti-tuberculous treatment and to explore the genotype of Mycobacterium tuberculosis resistant isolates using rpoB gene. Methods 53 isolates of mycobacterium isolated from pulmonary tuberculosis (PTB) patients from Kassala State were subjected to drug susceptibility testing (DST) to anti-tuberculous drugs; 10 M. tuberculosis complex (MTBC) resistant isolates were subjected to polymerase chain reaction (PCR), and commercially the amplified DNA was sequenced. Results DST detected resistance in 23/53 (43.39%) isolates, among which rifampicin had a high number of resistant isolates (13/23), followed by streptomycin (11/23), and multi-drug resistance was detected in 5 isolates. DNA sequence analysis of 10 MTBC-resistant isolates detected variations within and outside the rifampicin resistant determining region (RRDR). Variation within RRDR was detected at positions 512 (AGC/ATC, Ser/Ile), and 528 (CGC/CTC, Arg/Leu). Outside the RRDR region variations were detected at positions 498 (GTG/GGG, Val/gly), 488 (ACA/ACC, Thr/Thr), which is a silent mutation. Insertions were observed at positions 484, 496 (GTG/GTGA, CGG/CAGG, respectively). Deletion was observed at position 487 (ATC/_TC). Discussion and conclusion This study revealed that high resistance to rifampicin was associated with various point mutations in and out of the RRDR of the rpoB gene. Molecular methods are needed for early detection of TB disease and drug resistance.

Abstract: Transverse vaginal septum is a rare cause of genital outflow tract obstruction. It results from incomplete fusion/recanalization between the vaginal components of the mullerian ducts and the urogenital sinus. The septum varies in thickness and may be present anywhere along the vagina, although most are found in the upper and mid-vagina. A complete septum results in primary amenorrhoea and several short term and long term sequelae including endometriosis and infertility. We present a case of 12-year old girl who presented with intermittent, colicky abdominal pain. She had undergone low anorectoplasty at 18 months of age. She had not yet attained menarche. Secondary sexual characters were appropriately developed. The clinical examination revealed a small, blind ending lower vagina and a tender, bimanually palpable mobile mass in the suprapubic region. The ultrasound examination revealed fluid collection with internal echoes within the endometrial cavity extending into the upper vagina. The magnetic resonance imaging of pelvis confirmed the thickness (four cm) and location of the transverse vaginal septum (lower one third) along with presence of hematocolpos in upper 2/3rd of the vagina, hematometra and hematosalpinx. She underwent successful excision of the septum. Amnion was used for the epithelialisation of the denuded vaginal mucosa.

Abstract: The study aimed to assess the mosquito larval abundance in seasonal man-made larval habitats associated with rainfall in Kassala town, eastern Sudan during the short rainy season 2016, evaluate the physicochemical characteristics, and determine the relationships between such characteristics and immatures abundance. Immature stages were collected monthly in the selected larval habitats, identified into genera by morphological criteria and counted. Physical characteristics were estimated by visual inspection. Water sample was collected from each larval habitat and analyzed for chemical characteristics. A total of 5020 mosquito larvae and pupae were collected and identified into two genera, Anopheles and Culex . All the investigated larval habitats were found exposed to direct sunlight with still water. The monthly mean values of measured chemical characteristics did not vary significantly except for the following parameters: pH, calcium and magnesium. Occurrence of Anopheline immatures was only associated, positively, with nitrates (r = 0.53; p < 0.05) while the presence of Culex immatures was associated, negatively with pH value (r = -0.56; p < 0.05) and positively, with sulphate (r = 0.56; p < 0.05). Correlations found between certain chemical parameters and larval abundance, perhaps, confirms the influence of these parameters on the abundance of two mosquito genera in their breeding habitats.

115. Molecular identification of Mycobacterium tuberculosis causing Pulmonary Tuberculosis in Sudan

Abstract: Tuberculosis (TB) is a major public health problem in Sudan. However, the strains responsible for the epidemic of TB have been poorly characterized. The aim of this study was to characterize the causative agent of TB isolates circulating in Kassala and Gezira States during 2011-2012.Ninety two Mycobacterium tuberculosis complex (MTBC) strains were isolated from sputum samples of pulmonary TB patients, attending the Teaching Hospitals in Kassala and Gezira states. Molecular identification was performed using region of difference 9 (RD9) deletion and spoligotyping methods. Spoligotype patterns were compared with those listed in the international SITVIT2 spoligotyping database.The results determined the circulation of Euro-American, Central Asian, and Indo-Oceanic Lineages. They were clustered in the following clades: Manu2 (64.8%), CAS (22.7%), EAI (4.5), LAM2 (2.3%), H3 (1.1%), T (1.1%), T3-ETH (1.1%), T5-RUS1 (1.1%). Comparison with the international multi-marker database SITVIT2, 76.1% of isolates matched the database, while 23.9% of the isolates were not represented in SITVIT2 (orphans). SIT54 (Manu2) was the most common strain circulating in the two states.The study showed that a large variety of Mycobacterium tuberculosis (MTB) genotypes were circulating in the two states. Further studies on identification and molecular characterization of mycobacterial are needed to understand the biodiversity, rate of transmission and the associated risk factors of MTB in Sudan.

The complexes were characterized and elucidated using FT-IR, UV-Vis, a fluorescence spectrophotometer and thermo gravimetric analysis (TGA). TGA data showed that the stoichiometry of complexes was 1:2 metal/ligand except for Ag(I) and Mn(II) where the ratio was 1:1. The metal complexes showed varied antibacterial, fungicidal and nematicidal activities. The silver and zinc complexes showed highest activity against Bacillus subtilis and Bacillus licheniformis respectively. Fusarium oxysporum was highly susceptible to nickel and copper complexes whereas Macrophomina phaseolina was completely inert to the complexes. The silver and cadmium complexes were effective against the root-knot nematode Meloidogyne javanica.

Abstract: To use the delivery site according to the birth weight as a marker of changes in the referral practices after regionalisation of perinatal care.Analysis of the distribution of low birth weight infants according to the level of care in Rhone-Alpes from 1998 to 2000 and analysis of the birth rate heterogeneity according to the delivery site characteristics.The distribution of infantsor=1500 g remained constant at all levels (60% at level 3). That of infants 1500-2000 g born at level 3 dropped in 2000 but raised at levels 1 and 2. For both weight categories, the lower birth rates corresponded to the private, the lower-flow, and the more distant from neonatal intensive care units facilities. For infantsor=1500 g, the level 3 birth rate was four times the level 2 (P=0.0006) and five times the level 1 (P0.0001) rates. For infants 1500-2000 g, level 3 birth rate was twice the level 2 (P=0.0096) and 3.6 times the level 1 (P0.0001) rates. Birth rates were always significantly higher in university than in private facilities.Supervising level 3 is insufficient to show the effect of regionalisation. A more accurate analysis of intermediate-risk referral determinants is needed to reach a more demand/supply adequacy.

Abstract: In cystic fibrosis (CF), there is an imbalance in the oxidant/antioxidant system, leading to oxidative damage. The aim of this study was to assess antioxidant-scavenger deficiencies and lipid peroxide variations in three clinical situations (stable status, acute exacerbation, and after intravenous antibiotic treatment). The objective was also to correlate oxidative stress with age, nutritional status, and respiratory function. The study included prospectively 312 consecutive patients and 53 controls. Antioxidants (vitamin A, vitamin E, carotenoids, and glutathione) and oxidative markers (malondialdehyde and lipid peroxides) were measured in plasma. Regression analyses were performed. Antioxidant levels were lower in CF patients than in controls. These levels decreased during acute exacerbation and increased after antibiotic treatment. Carotenoid levels were not modified by infection or age. Only vitamin A and carotenoid levels were positively correlated to body mass index. Antioxidant levels were correlated to forced expiratory volume at 1 sec. Lipid peroxidation markers were lower in patients than in controls. Their levels decreased during infection, and increased after antibiotic treatment. Impaired lung function was correlated with elevated malondialdehyde levels. In conclusion, this study demonstrates antioxidant deficiency in a very large cohort of CF patients. Carotenoid and vitamin E deficiencies occur early in the course of the disease. Antioxidants decrease with bronchial infection. By contrast, nutritional disorders did not modify antioxidant levels during acute exacerbations. Thus, pulmonary disorders rather than nutritional disorders seem to be essential in the imbalance of the oxidant/antioxidant system. Results concerning glutathione and oxidative-marker levels highlighted the fact that their plasma values do not reflect oxidative stress in the respiratory tract.

Abstract: This work describes a RP-HPLC method for the determination and interaction studies of cefpirome with ACE-inhibitors (captopril, enalapril and lisinopril) in various buffers. The separation and interaction of cefpirome with ACE-inhibitors was achieved on a Purospher Star, C18 (5 μm, 250×4.6 mm) column. Mobile phase consisted of methanol: water (80:20, v/v, pH 3.3); however, for the separation of lisinopril, it was modified to methanol-water (40:60, v/v, pH 3.3) and pumped at a flow rate of 1 mL min(-1). In all cases, UV detection was performed at 225 nm. Interactions were carried out in physiological pH i.e., pH 1 (simulated gastric juice), 4 (simulated full stomach), 7.4 (blood pH) and 9 (simulated GI), drug contents were analyzed by reverse phase high performance liquid chromatography. Method was found linear in the concentration range of 1.0-50.0 μg mL(-1) with correlation coefficient (r(2)) of 0.999. Precision (RSD%) was less than 2.0%, indicating good precision of the method and accuracy was 98.0-100.0%. Furthermore, cefpirome-ACE-inhibitors' complexes were also synthesized and results were elucidated on the basis of FT-IR, and (1)H NMR. The interaction results show that these interactions are pH dependent and for the co-administration of cefpirome and ACE-inhibitors, a proper interval should be given.

Abstract: Background Patients with 2000 FIGO low-risk gestational trophoblastic neoplasia are commonly treated with single-agent chemotherapy. Methotrexate is widely used in this indication in Europe. Analysis of relapse after treatment and identification of factors associated with relapse would help understand their potential impacts on 2000 FIGO score evolution and chemotherapy management of gestational trophoblastic neoplasia patients. Objective This retrospective study analyzes the predictive factors of relapse in low-risk gestational trophoblastic neoplasia patients whose hormone chorionic gonadotropin (hCG) normalized with methotrexate alone. Study Design Between 1999 and 2014, 993 patients with gestational trophoblastic neoplasia were identified in the French Trophoblastic Disease Reference Center database, of which 465 were low-risk patients whose hCG normalized with methotrexate alone. Using univariate and multivariate analysis we identified significant predictive factors for relapse after methotrexate. The Kaplan-Meier method was used to plot the outcome of patients. Results The 5-year recurrence rate of low-risk gestational trophoblastic neoplasia patients whose hCG normalized with methotrexate alone was 5.7% (confidence interval [IC], 3.86–8.46). Univariate analysis identified an antecedent pregnancy resulting in a delivery (HR = 5.96; 95% CI, 1.40–25.4, P = .016), a number of methotrexate courses superior to 5 courses (5–8 courses vs 1–4: HR = 6.19; 95% CI, 1.43–26.8, P = .015; 9 courses and more vs 1–4: HR = 6.80; 95% CI, 1.32–35.1, P = .022), and hCG normalization delay centered to the mean as predictive factors of recurrence (HR = 1.27; 95% CI, 1.09–1.49, P = .003). Multivariate analysis confirmed the type of antecedent pregnancy and the number of methotrexate courses as independent predictive factors of recurrence. A low-risk gestational trophoblastic neoplasia arising after a normal delivery had an 8.66 times higher relapse risk than that of a postmole gestational trophoblastic neoplasia (95% CI, 1.98–37.9], P = .0042). A patient who received 5–8 courses of methotrexate had a 6.7 times higher relapse risk than a patient who received 1–4 courses (95% CI, 1.54–29.2, P = .011). A patient who received 9 courses or more had an 8.1 times higher relapse risk than a patient who received 1–4 courses of methotrexate (95% CI, 1.54–42.6, P = .014). Conclusion Low-risk gestational trophoblastic neoplasia following a delivery and patients who need more than 4 courses of methotrexate to normalization are at a higher risk of relapse than other low-risk patients. Allotting a higher score to the "antecedent pregnancy" FIGO item should be considered for postdelivery gestational trophoblastic neoplasia. Further analysis of the need for consolidation courses is warranted.

Abstract: Background: Although transplantation is known to impair glucose tolerance, evolution of pre-existing diabetes after lung transplantation (LT) in cystic fibrosis (CF) has never been described. Objectives: We aimed to assess the outcome of CF-related diabetes (CFRD) after LT, with the hypothesis that suppressing chronic inflammatory foci may improve glucose tolerance in some patients. Methods: In a retrospective study of 29 CF diabetic patients treated with insulin and undergoing LT, CFRD control was assessed 3 months before LT and 1 (n = 27) and 2 (n = 18) years after LT by measuring insulin dosage, fasting blood glucose and glycosylated hemoglobin (HbA1c) levels. Patients with HbA1c ≤7% and an insulin dose ≤1 UI/kg/day were defined as having controlled CFRD (group A). Other patients were assigned to group B. Results: Before LT, 19 (65.5%) patients were in group A. At 2 years, 6 of 10 (60%) patients who were in group B prior to LT had moved into group A, which then comprised 77.8% of all patients. Insulin could have been stopped in 5 patients. Uncontrolled CFRD before LT (OR = 16) and a long delay between the diagnosis of CFRD and LT (OR = 1.3) were significant predictors of uncontrolled CFRD at 1 year. Conclusions: LT does not seem to worsen CFRD in some patients, suggesting that in some cases, glucose tolerance may be improved by the suppression of chronic pulmonary infection.

Abstract: Aims Our aim was to examine intellectual development in children with congenital hemiplegia from early childhood to adolescence. Method Full-scale IQ (FIQ), Verbal IQ (VIQ), and Performance IQ (PIQ) scores were measured in 32 participants (19 males, 13 females) with congenital hemiplegia at mean ages of 4 years 6 months (SD 7mo; 31 participants), 7 years (SD 6mo; 23 participants), and 14 years (SD 1y 5mo; 26 participants). Results The FIQ and VIQ scores did not change with age, but the PIQ declined significantly (0.7 points per year; p=0.004). The estimated mean (95% confidence intervals) scores in males born at term with right-sided lesions without epilepsy were FIQ 106.5 (95.29–117.74), VIQ 105.9 (95.57–116.24), and PIQ 103.7 (93.19–114.31). Those means were negatively associated with preterm birth. PIQ was negatively associated with epilepsy. VIQ increased more quickly in males and in children with right-sided lesions. Interpretation The results confirm previous findings of FIQ stability, PIQ decline, the impact of epilepsy, and the status of females with left-sided lesions, and also reveal the effect of gestational age at birth. They underline the importance of management focused on nonverbal functions and further the debate about the early lateralization of language, the ‘crowding effect’, and the difference in brain plasticity between males and females.

Abstract: In Africa, HIV and feeding practices deeply affect child mortality. To prevent mother-to-child transmission, the World Health Organization recommends exclusive breastfeeding for 6 months and replacement feeding when acceptable, feasible, affordable, and sustainable. Determining the proportion and number of children saved with exclusive breastfeeding and replacement feeding is essential to design and implement crucial nationwide policies. Using data on 31 sub-Saharan countries and a decision tree for risk assessment, the authors estimated the number of children's lives potentially saved according to 6 scenarios that combine exclusive breastfeeding for 6 months or replacement feeding with 3 promotion strategies. Among all HIV-negative children born to HIV-positive mothers who die in sub-Saharan Africa per year, 52 315 (9.6%) would be saved yearly with exclusive breastfeeding versus 21 638 (4.0%) with replacement feeding. Promotion support would double these numbers (110 625 vs 45 330; ie, 20.3% vs 8.3%), and with additional prenatal group education, 132 633 versus 54 192 lives would be saved (24.3% vs 9.9%). Abstract: The present authors investigated whether cystic fibrosis is linked to a defect in fatty acids and assessed the impact of the main patients' characteristics on the levels of several fatty acids, mostly during respiratory exacerbation and after antibiotic therapy. Fatty acid phospholipid and cholesteryl ester levels were measured in stable-state patients and controls. No differences were found concerning either the fractions of palmitic and oleic acids or the cholesteryl esters of alpha-linolenic and arachidonic acids. However, phospholipids of alpha-linolenic and arachidonic acids, as well as cholesteryl esters and phospholipids of stearic and linoleic acids, were lower in patients than in controls, but fractions of dihomo-gamma-linolenic, docosatetraenoic, docosapentaenoic, palmitoleic and eicosatrienoic acids were higher. Fatty acid levels, oxidative stress markers, nutrients, body mass index and forced expiratory volume in one second (FEV(1)) were measured in patients before and after antibiotic courses for bronchial exacerbation. After adjustments, palmitic, stearic, alpha-linolenic, linoleic, arachidonic, palmitoleic and oleic acids generally decreased during exacerbation but almost all increased after antibiotic courses. Nearly all fractions increased along with FEV(1) and a positive relationship linked fatty acids to lipid hydroperoxides. There was no general drop in fatty acids. Patients' fatty acid profiles depended on the pulmonary function and the inflammation state.

131. Simultaneous Administration Compared With a 24-Hour Mifepristone–Misoprostol Interval in Second-Trimester Abortion.

Abstract: OBJECTIVE: To compare outcomes with simultaneous administration of mifepristone and misoprostol with a regimen in which the drugs are administered at a 24-hour interval for second-trimester abortion. METHODS: In this placebo-controlled, double-blind trial, participants were randomized to receive mifepristone either 24 hours before or at the same time as misoprostol. Participants were hospitalized to receive 400 micrograms buccal misoprostol at 3-hour intervals up to 48 hours or until uterine expulsion. The primary outcome was the proportion of women who experienced uterine expulsion within 24 hours after the first misoprostol dose and this required 504 women to examine our hypothesis that this rate would be 85% in the 24-hour interval arm compared with 70% in the simultaneous arm. Secondary outcomes included total abortion time from mifepristone and misoprostol. RESULTS: From February 2013 to April 2014, 509 women were enrolled. Women in the 24-hour interval arm were more likely to abort within 24 hours (94.4% compared with 85.0%, relative risk 1.11, 95% confidence interval [CI] 1.05-1.18). At 48 hours, the rate was similar in the two arms (96.8% [24-hour interval] and 95.7% [simultaneous], relative risk 1.01, 95% CI 0.97-1.04). Median misoprostol dosing time was shorter in the 24-hour interval arm (7.7 compared with 13 hours; P<.001) and consistent with the median misoprostol doses required (three compared with five; P<.001). Median time from mifepristone to uterine expulsion was longer in the 24-hour interval arm (32.3 compared with 13 hours; P<.001). Both regimens had high acceptability rates and reported similar side effects and pain scores. CONCLUSION: Administering mifepristone and misoprostol simultaneously results in lower expulsion rates within 24 hours of taking misoprostol, longer median misoprostol treatment times, and requires more misoprostol doses. Abstract: Editor, A lthough open-globe injuries are one of the major reasons for visual loss in children, there are little published data analysing the prognostic factors in such injuries according to internationally standardized terminology (Kuhn et al. 2002). This review was undertaken to identify the potential prognostic factors that relate to final visual outcome after penetrating ocular injury in children. The study included 74 patients younger than 14 years admitted to the Edouard Herriot Hospital, Lyon, France, between January 1998 and December 2002. Patients were followed for a median of 22 months (range 18–30 months). Annual incidence was 1.5 per 100 000 inhabitants. Males were affected in 76% of cases. The average age at admission was 7 years. The highest proportion of injuries (61%) occurred at home and sharp objects were the most frequent causative agents (66%). The overall outcome showed 45% of patients attained visual acuity (VA) of 20/40 or better. For statistical analysis, the final clinical outcome was defined as poor if final VA was less than 20/100. The most common preoperative factors were studied using univariate analysis (Table 1). Two variables were predictive of poor visual outcome: (1) initial lens damage (p < 0.001), and (2) intravitreal haemorrhage (p < 0.001).

Abstract: A computer can read and write multiple languages and today?s computers are capable of understanding various human languages. A computer can be given instructions through various input methods but OCR (Optical Character Recognition) and handwritten character recognition are the input methods in which a scanned page containing text is converted into written or editable text. The change in language text available on scanned page demands different algorithm to recognize text because every language and script pose varying number of challenges to recognize text. The Latin language recognition pose less difficulties compared to Arabic script and languages that use Arabic script for writing and OCR systems for these Latin languages are near to perfection. Very little work has been done on regional languages of Pakistan. In this paper the Sindhi glyphs are identified and the number of characters and connected components are identified for this regional language of Pakistan. A graphical user interface has been created to perform identification task for glyphs and characters of Sindhi language. The glyphs of characters are successfully identified from scanned page and this information can be used to recognize characters. The language glyph identification can be used to apply suitable algorithm to identify language as well as to achieve a higher recognition rate.

Casivirimab and imdevimab was provided free of charge for this trial by Regeneron. Background : Aspirin has been proposed as a treatment for COVID-19 on the basis of its anti-thrombotic properties. We aimed to evaluate the efficacy and safety of aspirin in patients admitted to hospital with COVID-19. Methods : In this randomised, controlled, open-label, platform trial, several possible treatments were compared with usual care in patients hospitalised with COVID-19. The trial took place at 177 hospitals in the UK, two hospitals in Indonesia, and two hospitals in Nepal. Eligible and consenting adults were randomly allocated in a 1:1 ratio to either usual standard of care plus 150 mg aspirin once per day until discharge or usual standard of care alone using web-based simple (unstratified) randomisation with allocation concealment. The primary outcome was 28 day mortality. All analyses were done by intention to treat. The trial is registered with ISRCTN (50189673) and ClinicalTrials.gov (NCT04381936). Findings : Between Nov 1, 2020, and March 21, 2021, 14 892 (66%) of 22 560 patients enrolled into the RECOVERY trial were eligible to be randomly allocated to aspirin. 7351 patients were randomly allocated (1:1) to receive aspirin and 7541 patients to receive usual care alone. Overall, 1222 (17%) of 7351 patients allocated to aspirin and 1299 (17%) of 7541 patients allocated to usual care died within 28 days (rate ratio 0·96, 95% CI 0·89-1·04; p=0·35). Consistent results were seen in all prespecified subgroups of patients. Patients allocated to aspirin had a slightly shorter duration of hospitalisation (median 8 days, IQR 5 to >28, vs 9 days, IQR 5 to >28) and a higher proportion were discharged from hospital alive within 28 days (75% vs 74%; rate ratio 1·06, 95% CI 1·02-1·10; p=0·0062). Among patients not on invasive mechanical ventilation at baseline, there was no significant difference in the proportion meeting the composite endpoint of invasive mechanical ventilation or death (21% vs 22%; risk ratio 0·96, 95% CI 0·90-1·03; p=0·23). Aspirin use was associated with a reduction in thrombotic events (4·6% vs 5·3%; absolute reduction 0·6%, SE 0·4%) and an increase in major bleeding events (1·6% vs 1·0%; absolute increase 0·6%, SE 0·2%). Interpretation : In patients hospitalised with COVID-19, aspirin was not associated with reductions in 28 day mortality or in the risk of progressing to invasive mechanical ventilation or death, but was associated with a small increase in the rate of being discharged alive within 28 days. Abstract: Background: Prediabetes is a metabolic condition characterized by blood sugar levels higher than normal but below the diabetic range. The onset of microvascular complications may set in this phase, hence intervening during the prediabetic stage becomes crucial in halting the progression to irreversible diabetes. We compared blood sugar levels between lifestyle modification and metformin medication and identified which option is better for managing prediabetes. Methods: A quasi-experimental study was done from January to October 2023 recruiting 150 prediabetic adults coming to Sikander Abad clinic through convenience sampling including 18 to 65 years of age while obese adults, those who had hypoglycemia within the past 3 months, and GDM women were excluded. Patients were divided into two groups and given a choice to choose pharmacotherapy (Metformin) or non-pharmacological measures (lifestyle modification). 137. Mutational Analysis of EZH2 in the Germinal Centre Type of Diffuse Large B Cell Lymphoma

Abstract: OBJECTIVE: To determine the Frequency of Germinal Center B (GCB) type Diffuse Large B Cell Lymphoma (DLBCL) in Karachi, Pakistan, and to screen the DLBCL patients for EZH2 mutation. METHODOLOGY: A cross-sectional study was conducted on DLBCL patients, and IHC analysis, PCR, and sequencing of the EZH2 gene were used. This study was conducted from August 2016 to July 2020. Surgical specimens received in the histopathology section of DDRRL centrally and from different collection units throughout Sindh having been diagnosed with DLBCL were collected. Three markers (CD10, BCL6, and MUM1) were used for subtyping. DNA was manually extracted from the FFPE Blocks of DLBCL cases using phenol: chloroform: isopropanol alcohol. Polymerase chain reaction (PCR) was performed, and samples were sent from Dow University to Korea (Macrogen) for sequencing. The amplified product was 189bp. Primers were used for EZH2 to flank the region containing the tyrosine domain on exon 16 and exon 18. RESULTS: Out of 116 cases, 60.3% and 39.7% were male and female, respectively. The mean age was 49.73±16.38 years. The mean survival duration of cases diagnosed with GCB was 8.21±7.39 months. Polymorphism of EZH2 mutation (Tyr 701 Asn) was found to be positive in GCB-DLBCL for three samples (16.6%). CONCLUSION: In the current study, Sanger sequencing was used to detect any mutation in the coding region of EZH2, and a unique mutation was reported (Tyr701Asn).

Abstract: Background: Injury is a significant issue of public health, a major global burden and first aid training programs incorporated as part of school curriculum will have a considerable impact on public health, and this will in turn reduce out of hospital cardiac arrests. The study aimed to determine the immediate First aid knowledge and long term retention of its knowledge (after three months) of the students (13-18 years), participating in a volunteer program in Karachi. Methods: This study was conducted in a tertiary care hospital, delivering First aid training to 143 participants by Basic life support (BLS) trained staff in a tertiary care hospital. The design consisted of a pre-test, post-test and follows up assessments, carried out via questionnaire and hands-on assessment. SPSS was used for statistical analysis. ANOVA and Friedman test was applied to evaluate the dependent variable’s improvement in knowledge between pre, post and follow up tests. Results: From 143 participants, 41.5 %, 63.8 % and 61 % satisfactory results were seen in the pre-test, post-test and follow up assessments, respectively. In Pre-test, males scored 18.75 % and the females scored 8.42%, whereas, in the post-test, the scores were 75.7% and 64.58% respectively. Similarly, in follow up evaluations results obtained were 62.1% and 45.8% respectively. Hands-on scores were 88.4% in males and 95.8 % in females. Conclusion: The level of first aid knowledge was unquestionably less satisfactory in the pre-test evaluations compared to post-test and follows up assessments. Abstract: OBJECTIVE: This study evaluates the frequency and relationship between Thrombocytopenia and CRP levels in Covid-19 patients. METHODOLOGY: The cross-sectional retrospective study was conducted at Dr. Ishrat ul Ebad Khan Institute of Blood Diseases from October 2021 to September 2022. Data was collected from existing data from the blood bank and hematology laboratory. C reactive protein (CRP) and complete blood count (CBC) levels were observed in Covid-19 patients. Both genders, all age groups, COVID-19 patients and patients having Thrombocytopenia and raised levels of CRP were included in this study. At the same time, other bone marrow transplant patients and other infectious diseases such as Hepatitis B, Hepatitis C, Dengue, and Malaria were excluded from this study. Statistical analysis was done by using the SPSS 20.0 version. RESULTS: Out of 844 patients, the mean age was 50 years, and the male proportion was high at 70% (n=590). Abstract: Objective: To evaluate the relevance of thrombocytopenia to other disease states in patients reporting at a haematology laboratory. Method: The cross-sectional, retrospective study was conducted at the Dow Diagnostic Research and Reference Laboratory of the Dr Ishrat ul Ebad Khan Institute of Blood Diseases, Down University of Health Sciences, Karachi, and comprised data from the blood bank and haematology laboratory related to patients who visited during October 2021to August 2022. Data was analysed using SPSS 26. Results: Of the 1,249 patient with mean age 38.5±19.7 years (range: 0-89 years), 79(6.3%) were aged 40. Most of the patients were males 793(63.5%), and 604(48.4%) had Grade 1 thrombocytopenia. Among the females, Grade 1 thrombocytopenia was found in 246(53.9%) and Grade 2 in 140(30.7%) patients compared to 358(45.1%) and 222(28%) in males, respectively (p0.05). Abstract: Dyspnea is a disorder that occurs because of shortness of breath and may lead to multiple complications. Intoxicated dyspnea patients are those who are addicted to alcohol, as well as have breathing problems. Mostly, problems faced by dyspnea patients are inhalation, when they take alcohol with severe concentration. For intoxicated dyspnea patients, there is a continuous need for alcohol breath testing along with breath indicators, since, an individual is prone to alcohol addiction, and faces problems of breathing such as asthma, hypertension, etc. The severe outbreak of dyspnea in these individuals may be deadly. To handle this condition, an alcoholic meter along with an incentive spirometer is designed that helps in the assessment of breathing problems by measuring the alcohol concentration and inhaled air. Several parameters have opted for both inhalation detection and alcohol concentration which include the severity of health problems encompassing asthma, hypertension for the detection of inhaled air, and normal, moderate, severe alcohol concentration. The working of the proposed device is validated through the analysis of these parameters. The results are obtained on the graph as well as on display, which shows a variation in the range among every individual with respect to selected parameters either in incentive spirometer (inhalation detection) or alcoholic meter, and these ranges are also displayed in a numeric form on LCD. The idea is to design an economical and portable device for intoxicated dyspnea patients for emergency conditions which is a combination of an alcoholic meter with an incentive spirometer (IS) for the measurement of alcohol concentration along with breathing problems by making use of sensors. This unique idea of combining an alcoholic meter with an incentive spirometer will aid intoxicated dyspnea patients in case of emergency.

Abstract: Background:A serious worldwide medical and public health concern occurs with the dengue virus, which is also an emerging health threat. A victim can be vulnerable and acquire dengue because of his age, genetics, nutritional status, viral strain and secondary infections. Our objective was to identify the ABO blood group system's distribution and relationship in dengue fever (DF) patients.Methodology:The current study was conducted at Dr. Ishrat ul Ebad Institute of Blood Diseases Department between September 2019 and September 2020. The research work was viewed and accepted by the ethics and research committees. Case records of patients with blood systems and their relation among dengue nonstructural protein 1(NS1)antigen /dengue immunoglobulin M (IgM) and immunoglobulin G (lgG) were included in this study.Results:Among 204 cases, 82 were blood group O positive, 59 were B positive, 38 were A positive, 16 were AB positive, 4 were O negative, and 2 were blood groupA negative. The association of the ABO blood group with dengue was statistically significant (p=0.046). Blood group O+ has the highest number of cases, 66(42.9%), than controls, 16(32%).Conclusion:This study overall consummates that blood group O is linked with a greater possibility of acquiring DF in comparison with different other blood group types. There is a significant relationship between blood group and DF observed.

Abstract: Objective: To evaluate the outcomes, clinical and radiological application of Bone Morphogenetic Protein-2 alone versus Bone Morphogenetic Protein-2 with autograft in long bone non-unions., Methods: A prospective review of patients was done with fracture non-union admitted to Aga Khan University Hospital, Karachi, from January 2016 to January 2019. The patients were divided into two groups; those exposed to Bone Morphogenetic Protein-2 alone in group 1, and those exposed to Bone Morphogenetic Protein-2 plus autologous graft in group 2., Results: Background characteristics of both the groups were analysed. Patients were followed up at 6, 12 and 24 weeks through their medical records. The primary outcome was postoperative union at 6, 12 and 24. Union was defined by having the clinical union as well as the radiological union at the same time of assessment. Of the 80 patients enrolled, 13(16.25%) were excluded, and 5(6.25%) were lost to follow-up. The final sample had 62(77.5%) patients; 35(56.5%) in group 1, and 27(43.5%) in group 2. Union at 6 weeks was observed in 13(21%) patients; 8(62%) in group 1, and 5(38%) in group 2. Union at 12 weeks was observed in 38(61%) patients; 20(53%) in group 1, and 18(47%) in group 2., Conclusions: Results showed that using the adjuvant treatment alone was not worse than using it along with bone autologous graft.

Abstract: Background: Tuberculin skin test (TST) is widely used for the assessment of Bacillus Calmette-Guérin (BCG) vaccine efficacy and screening of latent TB infection (LTBI). Poor or no data are available on the reactivity of tuberculin in Kassala State. The aim of the present study was to assess the response to the BCG vaccine and to estimate the prevalence of LTBI and the annual rate annual risk of tuberculous infection (ARTI) among vaccinated school children using TST., Methods: School-based cross-sectional study was conducted in three localities of Kassala State during 2016-2018. A cluster random sampling method was used for the enrolment. Five tuberculin units of 0.1 mL were injected intradermally in the left forearm of 2568 school children aged 5-15 years. The test was performed after the assessment of child health, nutrition status, and BCG scar status. Tuberculin reaction size was interpreted after 48-72 h. The collected data were analyzed using SPSS (v 20). The classical method was used to estimate ARTI., Results: Overall, there was no reaction in 81.5% of children. Only 0.66% of children had induration 10 mm-28 mm, indicating the prevalence of latent TB with an annual risk of 0.1%. Tuberculin reactivity was statistically significant affected by child age, gender, geographical location, and nutrition status (P < 0.05), whereas BCG scar status had no effect (P > 0.05)., Conclusion: The study documented a high proportion of tuberculin nonreactivity irrespective of BCG vaccination status and provides data on the prevalence of latent infection among studied groups. Further studies are needed to address the reasons of low and nonreactivity of tuberculin, and evaluation of the BCG vaccine is recommended., Competing Interests: None

145. Molecular identification of Mycobacterium tuberculosis causing Pulmonary Tuberculosis in Sudan.

Abstract: Background: Tuberculosis (TB) is a major public health problem in Sudan. However, the strains responsible for the epidemic of TB have been poorly characterized. The aim of this study was to characterize the causative agent of TB isolates circulating in Kassala and Gezira States during 2011-2012., Method: Ninety two Mycobacterium tuberculosis complex (MTBC) strains were isolated from sputum samples of pulmonary TB patients, attending the Teaching Hospitals in Kassala and Gezira states. Molecular identification was performed using region of difference 9 (RD9) deletion and spoligotyping methods. Spoligotype patterns were compared with those listed in the international SITVIT2 spoligotyping database., Results: The results determined the circulation of Euro-American, Central Asian, and Indo-Oceanic Lineages. They were clustered in the following clades: Manu2 (64.8%), CAS (22.7%), EAI (4.5), LAM2 (2.3%), H3 (1.1%), T (1.1%), T3-ETH (1.1%), T5-RUS1 (1.1%). Comparison with the international multi-marker database SITVIT2, 76.1% of isolates matched the database, while 23.9% of the isolates were not represented in SITVIT2 (orphans). SIT54 (Manu2) was the most common strain circulating in the two states., Conclusion: The study showed that a large variety of Mycobacterium tuberculosis (MTB) genotypes were circulating in the two states. Further studies on identification and molecular characterization of mycobacterial are needed to understand the biodiversity, rate of transmission and the associated risk factors of MTB in Sudan.
