Two children with an acquired aphasia were observed during the recovery process. In the spontaneous speech, paraphasias belonging to different categories, such as neologisms, verbal paraphasias, and literal paraphasias, were found. Especially with regard to neologisms the time of investigation was very important. In addition, one child with a phonemic jargon aphasia and one child with a fluent aphasia and empty speech were observed. These observations implicate a modification of the current clinical picture of childhood aphasia. Some aspects are discussed in relation to adult aphasia.

Een spontane-taalanalyse bij afasie is een belangrijke toevoeging om de linguïstische en communicatieve mogelijkheden in kaart te brengen, een passend therapieplan op te stellen en de mate van vooruitgang te meten. In het onderhavige onderzoek wordt bij een groep van tien mensen met een milde afasie in de chronische fase onderzocht hoe stabiel de spontane taal is binnen een interview. Uit de spontane-taalinterviews zijn twee keer 300 woorden getranscribeerd en geanalyseerd volgens de Analyse voor Spontane Taal bij Afasie (ASTA; Boxum, Van der Scheer & Zwaga, 2010). Van veertien linguïstische variabelen is bekeken of er verschil is tussen de scores in de eerste en tweede 300 woorden. Ook is onderzocht of het gespreksonderwerp en de emotionele lading ervan invloed hebben op het taalgebruik. De scores op de linguïstische maten blijken stabiel te zijn gedurende een interview, met uitzondering van het aantal semantische parafasieën en het aantal seconden onverstaanbaar. Een verandering van het gespreksonderwerp blijkt geen significante invloed te hebben, maar de emotie die erbij beleefd wordt wel. Bij een negatieve emotie worden meer lexicale werkwoorden en minder zelfstandige naamwoorden gebruikt dan bij een positieve emotie. Toch lijkt een spontane-taalanalyse een stabiel en betrouwbaar beeld te geven van de taalproductie. Vervolgonderzoek kan uitwijzen of dit ook geldt voor andere fasen van afasie.

Background Aphasia is a severely disabling condition occurring in 20 to 25% of stroke patients. Most patients with aphasia due to stroke receive speech and language therapy. Methodologically sound randomised controlled trials investigating the effect of specific interventions for patients with aphasia following stroke are scarce. The currently available evidence suggests that intensive speech and language therapy is beneficial for restoration of communication, but the optimal timing of treatment is as yet unclear. In the Rotterdam Aphasia Therapy Study-3 we aim to test the hypothesis that patients with aphasia due to stroke benefit more from early intensive cognitive-linguistic therapy than from deferred regular language therapy. Methods/design In a single blinded, multicentre, randomised controlled trial, 150 patients with first ever aphasia due to stroke will be randomised within two weeks after stroke to either early intensive cognitive-linguistic therapy (Group A) or deferred regular therapy (Group B). Group A will start as soon as possible, at the latest two weeks after stroke, with a four week period of one hour a day treatment with cognitive-linguistic therapy. In Group B professional speech and language therapy is deferred for four weeks. After this period, patients will follow the conventional procedure of speech and language therapy. Participants will be tested with an extensive linguistic test battery at four weeks, three months and six months after inclusion. Primary outcome measure is the difference in score between the two treatment groups on the Amsterdam-Nijmegen Everyday Language Test, a measure of everyday verbal communication, four weeks after randomisation.
