103. Masculinities, humour and care for penile cancer: a qualitative study.

Abstract

AIM: To explore how men with penile cancer construct humour in relation to their diagnosis and treatment. BACKGROUND: Functionalist, relief and incongruity theories attempt to account for humour, but there is a dearth of empirical evidence in nursing care. This is particularly so in relation to a condition like penile cancer where some nurses think that humour in their interactions with patients would be inappropriate. DESIGN: The study employed a participative, mixed-qualitative-methods design. METHOD: Focus groups and patient-conducted interviews were both used during a one-day 'pilot workshop' in March 2011. The data were initially analysed using framework analysis. This paper explores the theme of humour in depth. FINDINGS: Humour helped participants make light of their condition, which meant that they could laugh about the consequences of treatment ('laughing about urination') and build rapport with health professionals ('humour with health professionals'). Nevertheless, the use of humour was less important than the treatment of their cancer ('humour discounted') and there was a fear that they would be subject to ridicule because of their condition ('fear of ridicule'). CONCLUSION: The findings suggest a combination of functionalist, relief and incongruity theories of humour; the emotions these men experience are contained (functionalist) and released (relief) through humorous interaction, and the potential for comedy lies in an incongruity between what is expected socially and the experiences of these men, for example, around expectations that men use urinals in public toilets. Nurses should continue to use humour to build rapport with patients, should they judge this to be appropriate, although they may want to avoid jokes about sexual and urinary functioning until after treatment.

107. Patients’ Experiences of penile Cancer

Abstract

Penile cancer is a rare but destructive disease in Western populations. In the United Kingdom, penile cancer accounts for <1% of all new cases of cancer and <1% of deaths due to cancer every year (see Table 1). [Table 1] Surgical removal of the cancer is the primary form of treatment. This involves surgical excision of the primary tumour and of involved inguinal lymph nodes. Chemotherapy and radiotherapy are rarely helpful, with their use restricted either to adjuvant use or for palliative treatment of extensive disease. In treating the primary tumour, the standard of care is to provide a surgical cure (ie, excising the tumour and a margin of normal penile tissue) while maintaining the function of the penis. The traditional view was that at least a 2-cm margin of normal tissue should be removed, but recent publications suggest that more conservative surgery may be safe [1], and [2]. The advantage of such organ-preserving surgery is intuitively advantageous to the patient in that penile function can be better preserved, but the evidence to support this view is limited at present.

138. Physiology of Sexual Function.

Abstract

The parasympathetic nervous system provides the primary pro-erectile innervation of the penis. Originating from the sacral nerve roots (S2-S4) the nerves provide vasodilating innervation to the cavernosal tissue. The parasympathetic nerves release a cocktail of pro-erectile neurotransmitters, of which the most important is nitric oxide (NO), which acts on the smooth muscle cell via a second messenger system involving cyclic GMP. The sympathetic innervation mediates detumescence and originates within the thoraco-lumbar cord (T11-L2) via release of noradrenaline. Our knowledge of the pharmacology of erection has recently provided us with a number of therapeutic approaches to the treatment of erectile dysfunction. The pudendal nerve represents the somatic innervation of the penis carrying both afferent impulses from the genitalia and motor fibres to the muscles of the pelvic floor. The erection itself is a vascular event during which the degree of erection depends upon the balance between the arterial inflow and venous outflow of the penis. Parasympathetic stimulation (with an accompanying reduction in sympathetic stimulation) results in smooth muscle relaxation in the penile arteries, relaxation of the cavernosal (trabecular) smooth muscle and closure of the venous outflow from the penis. Ejaculation has two phases, emission and ejection, the latter usually being accompanied by orgasm. Emission involves the sequential contraction of the epididymis, vas deferens, seminal vesicles and prostate, with ejaculatory fluid being "emitted" into the posterior urethra. Ejection is accompanied by tight closure of the bladder neck, with contraction of the prostatic musculature together with a sequence of variably coordinated contractions of the bulbocavernosus, ischiocavernosus and other pelvic floor muscles. The ejaculate is propelled into the anterior urethra and beyond. The foreskin or the prepuce is a specialized, junctional mucocutaneous tissue that marks the boundary between mucosa and skin. It may have a number of functions including one as a sensory erogenous area and there may be others, but little is actually known. [ABSTRACT FROM AUTHOR]
