Objective: Socio-demographic factors have been said to affect the perception of midline diastema. Aesthetic importance of maxillary midline diastema varies in individuals and there is no common consensus on the registration of diameter of maxillary midline diastema. The aim of the study was therefore to determine the acceptable width of diastema among a group of Nigerian subjects and the effect of their socio-demographic factors on the perception of midline diastema. Method: Subjects aged 19-45 yrs were recruited from the 6 geopolitical zone of Nigeria. A structured questionnaire was administered to participants. The questionnaire sought for information such as age, gender, profession, presence of midline diastema and created diastema. The questionnaire included a coloured smile photograph with digitally altered varying sizes of diastema (2mm - 6mm). Subject rated the different coloured photographs using the rating scale (very attractive =1, attractive =2, accepted =3, unattractive =4, and very unattractive =5). Data were analyzed with the Statistical Package for Social Sciences (SPSS) version 16.0 (SPPS Inc, Chicago IL). Result: A total of 374 participants aged 19 – 45 years was involved in this study. Majority (52.1%) were females and mean age was 37.07+/-0.63 years. Only 30.2% had midline diastema out of which 12% was artificially created. Majority (86.7%) of the created diastema was done by the dentist. Age (p=0.024) and level of education (p=0.017) had a significant relationship with perception of midline diastema. Older subjects and those with lower educational level considered midline diastema a sign of beauty. Majority of the participants perceived the 2-4mm midline diastema attractive and 5-7mm midline diastema unattractive. Conclusion: It can be concluded from this study that a relationship exists between age and perception of midline diastema. The acceptable width of the midline diastema in this study is 2mm-4mm.

Diastema closure is a frequently requested, technique-sensitive cosmetic procedure. There are many techniques and materials that can be employed to close diastemas. While direct composite techniques can be economical and successful, they do present challenges in achieving satisfactory clinical and esthetic results. Traditional porcelain veneer placement may offer an excellent esthetic result, but typically requires the removal of tooth structure; as such, this is an irreversible procedure. The present article reports the case of a maxillary midline diastema closure in a healthy dentition by means of sectional porcelain veneers simply cemented onto the natural teeth and without tooth preparation. A step-by-step procedure is proposed for illustrating the proper implementation of an additive-only and noninvasive indirect technique that yields a satisfactory clinical and esthetic outcome for clinician and patient. Objective: Pathological tooth migration (PTM) is an esthetic and functional problem that may be associated with multiple etiological factors. The purpose of this cross-sectional epidemiological study is to determine prevalence of PTM among periodontitis patients and investigate the association of various contributing factors such as tongue thrusting, missing posteriors, trauma from occlusion, class II malocclusion, and aberrant frenum. Materials and Methods: A cross-sectional study was conducted with a total of 445 participants (age of 25–65 years) with chronic periodontitis. Results: Comparison of categorical values was done using Chi-square test and continuous variables were done using independent-sample t-test. Statistically significant results were seen in patients with tongue thrusting (P < 0.002), missing posteriors (P < 0.032), trauma from occlusion (P < 0.007). The results are not statistically significant with abnormal frenum, class II malocclusion. Conclusion: We conclude that tongue thrusting, missing posteriors, and trauma from occlusion are associated with PTM in periodontitis patients. Though class II malocclussion and aberrant frenum are seen in patients with pathological migration, significant association is not present.

106. Jagiellonian University Medical College Researcher Provides New Data on Diastema (Retrospective Clinical Analysis of the Effectiveness of Skeletally Anchored Appliances for Maxillary Expansion).
- Abstract
A researcher from Jagiellonian University Medical College has conducted a retrospective clinical analysis on the effectiveness of skeletally anchored appliances for maxillary expansion in treating diastema, a condition characterized by gaps between teeth. The study evaluated the efficacy of using distractors supported by temporary orthodontic implants (TOIs) in patients of different ages and genders. The results showed that the procedure was effective in adolescents regardless of sex, but had limited effectiveness in adult males. This research provides valuable insights into alternative treatments for diastema. [Extracted from the article]

Objective: This study aimed to use eye tracking to analyse people's visual perceptions of smiles with a diastema between the maxillary central and lateral incisors, based on different categories of observers.Setting and Sample Populations: An image of a diastema between the maxillary central and lateral incisors was shown to dental students and laypeople.Materials and Methods: Our null hypothesis was that there would be no difference in the visual perception of the different graduations of diastemas between the two groups. The diastemas were edited in Photoshop® to gradually increase the distance between the maxillary central and lateral incisors from no diastema to 1.5 mm and 3.5 mm diastemas. These diastemas were evaluated by using a TheEyeTribe© tracker in conjunction with OGAMA© 5.0 software. A total of 37 dental students and 33 laypeople looked at eight randomly arranged images of smiles. The data were analyzed using color coded maps, scanpaths, and the Kruskal-Wallis test with a level of significance of 5%.Results: Laypeople tended to observe the eyes, focusing more on the right than the left, and spent less time focusing on the diastema region than did the dental students. There were differences for the same magnitude of diastema within participant groups. As the width of the diastema increased, both students and laypeople focused more on the diastema region. Conclusions: The two groups of evaluators presented different perceptions of smiles in cases of diastema between the maxillary central and lateral incisors when evaluated using eye-tracking. To evaluate the stability of maxillary interincisor diastema closure and the relationship between space relapse and interincisor diastema width, overjet, overbite, angulations between adjacent maxillary anterior teeth and presence of intermaxillary osseous cleft after orthodontic treatment with extractions. Twenty-four individuals with a maxillary interincisor diastema pretreatment, treated with maxillary first premolar extractions were evaluated. Dental casts and panoramic radiographs taken at pretreatment (T1), posttreatment (T2), and posttreatment follow-up (T3) were assessed. Periapical radiographs at T1 and T2 were also evaluated. Diastema relapse was assumed when T3-T2 interincisor space change was greater than zero. Diastema relapse was considered clinically significant when it was at least 0.50 mm. Data were analyzed using repeated-measures analysis of variance followed by post hoc Tukey tests or Friedman followed by Wilcoxon tests. T-test or Mann-Whitney U-test, Pearson correlation coefficient, and multiple linear regression analyses were also performed. No statistically significant relapse of maxillary interincisor diastemas was found. The percentage of clinically significant relapse of the maxillary interincisor diastemas was 27.78%. Specifically, for the interincisor midline diastema, it was 8.33%. Maxillary interincisor diastema closure showed no statistically significant relapse after orthodontic treatment with premolar extractions. Clinically significant stability for maxillary interincisor diastema closure was 72.22% and, specifically, for interincisor midline diastema closure, it was 91.67%. [ABSTRACT FROM AUTHOR]

This paper aims to report a clinical case of closure of inter-incisive diastema by the technique of direct restoration with composite resin. A female patient attended the Integrated Adult Clinic complaining about the aesthetics of her smile due to the yellow color and a space between the upper incisor teeth. In the clinical examination, the presence of mesial diastema between teeth 11 and 21 was noted. A treatment plan was made, in which the patient was suggested to close the diastema by means of direct restorations with composite resin. After the patient's approval, the color of the teeth was selected with the aid of the Vita Scale and it was proposed to perform a whitening procedure before the restorative treatment. After bleaching and after waiting 21 days for color stabilization, the restorative stage began, which consisted of the acid attack followed by the application of the adhesive system. The composite resin was placed in 1mm increments with the aid of polyester tape in order to reestablish the palatal wall and interproximal contact. Subsequently, the resin was placed up to the buccal surface, respecting the dental anatomy. One week after the restoration, finishing and polishing was carried out with fine-grained cone-shaped diamond tips, sanding discs, felt disc and diamond paste. The satisfactory result after the closure of the diastema by the direct technique in composite resin proved to be a viable treatment option, since it restored the function and aesthetics of the patient's smile. Background: Implantology or implant dentistry is growing fast during last four decades. Facing the growing demand of implant treatment, there are extreme challenges to clinicians and researchers. First is peri-implantitis with remarkable prevalence. Though investigators have revealed that the etiology of the peri-implant infection is similar to periodontitis, clinically there is no effective treatment. Second, implantation in patients with severe systemic conditions, i.e., severe diabetes, lupus, osteoporosis, organ transplant, and cancer with intensive radiotherapy and/or chemotherapy, is another challenge to implant treatment for lack of scientific research data. Animal models are crucial to help investigators reveal the mechanisms underlying these disorders. Murine models are used most commonly. Rats are the better subject in dental implant research, due to mice could not provide clinical compatible and macro-level measurable data for implant osseointegration and peri-implantitis in oral cavity for lacking enough cancellous bone to support an implant more than 1 mm in length. Objective: Our aim of this research is to find a clinical comparable rat dental implant model. Methods: Six male Sprague-Dawley rats with body weight more than 500 g were used in the experiment. Each rat received two implants. One implant was placed at maxillary diastema in each side. Seven weeks after the implantation, only one implant successfully osseointegrated without movement and inflammation. Implant success and failure rate is analyzed by using Clopper-Pearson's exact method at 95% confidence interval. Results: The present data indicate that the true success rate of implantation in maxillary natural diastema in rat is less than 38.4% at a confident level of 95%. Meanwhile, Micro-CT indicates maxillary first molar position will be a promising site for implantation. Conclusion: Maxillary nature diastema may not be an appropriate site for implantation research for its low successful rate, but maxillary first molar position could be a candidate for implantation research. Further researches are required to illustrate the details. [ABSTRACT FROM AUTHOR]

Diastema is a latin term of greek origin incorporated into Terminologia Anatomica under the code A05.1.03.078. Odontologically, it is an interdental space located between teeth adjacent to the dental arch, its presence being normal in the period of mixed dentition. Dictionaries of the Spanish, Greek and Latin languages, of medical and etymological terms were consulted, in search of the initial occurrences of the term Diastema and its subsequent uses in history. The search reported that Diastema has been used to indicate a distance or interval, referring to places, times and even between musical tones. In the area of medical sciences, it is a term that has indicated the separation between the fingers or the virtual space created by insufflation of the lungs, being its closest approach to morphology in the veterinary area to designate the space that separates the incisors and molars tooth, especially in rodents and ruminants. To be consistent with the current guidelines of the International Federative Program of Anatomical Terminology and the terminological agreements of the Ibero-Latin American Symposia of Anatomical, Histological and Embryological Terminology, we suggest the replacement of the Diastema period with Spatium interdentale, as an advance in accuracy and description of the structure incorporated into Terminologia Anatomica. An abnormal upper labial frenum is capable of retracting the gingival margin, creating a diastema, limiting lip movement, and in cases of a high smile line, affecting esthetics also. When an abnormal frenum is present, frenectomy is advised. This technique has been modified in many ways. But in most of the techniques the zone of attached gingiva and esthetics are not considered. This case report highlights a new technique of frenectomy that results in good esthetics, excellent color match, gain in attached gingiva, and healing by primary intention at the site of the thick, extensive abnormal frenum. Introduction: Class III malocclusions present a challenge for clinical management despite having a strong genetic base, these present a multifactorial etiology and must be early diagnosed to achieve success in their treatment and depends on an adequate diagnosis, timely intervention and the collaboration of patients. Clinical case: a 12-year-old female, white patient with a health history who visits the Orthodontic Clinic due to facial deformity and occlusal alterations in which anamnesis appears to be a precursor for maintaining a milk-bottle habit and lingual thrust, having bilateral molar ratio of mesioclusion, diastemas, negative highlight of -0.5 mm at level 11, exceeding 0 mm and discordant with bony bases. A therapeutic modality that combined with a fixed device (synergy system) was applied with the use of a chin rest for the correction of skeletal abnormalities and occlusion. Conclusions: with the treatment, it was possible to reestablish functional and aesthetic dental occlusion as well as a harmonic profile.

Multiple sclerosis (MS) is a chronic, autoimmune inflammatory disorder of the central nervous system (CNS) that affects myelinated axons, destroying the myelin and damaging axons to varying degrees. The course of MS is highly varied and unpredictable. Metals used during orthodontic treatment can negatively affect imaging techniques used to diagnose and monitor the progression of MS, while medications used to treat MS can negatively affect orthodontic tooth movement. The present case report highlights some of the challenges encountered during orthodontic treatment of a patient with MS and how to overcome them. The patient was a 20-year-old woman with complaints of diastema and spacing in the upper arch. Although closing the spaces was challenging due to some of the MS medications, she was treated successfully, without complications, within 20 months using closing loops.

Objectives To obtain views of orthodontists in the United Kingdom on frenectomy in terms of its indications and timing and a recommended retention regimen after correction of median diastema. Materials and Methods A 14-item online questionnaire was sent to orthodontic specialists for completion. The questionnaire covered demographics and orthodontists' experience and views on frenectomy. Results Three hundred and fifty-three orthodontists with various background and experience responded to the survey. Three-quarters of respondents routinely performed a blanche test to aid diagnosis of the abnormal frenum; however, only 15% carried out radiographic investigation. Three-quarters of the orthodontists would consider frenectomy as a part of orthodontic treatment, and variation existed among the clinicians in terms of its timing. Frenectomy without orthodontic treatment was not preferred. There was much variation in the retention regimen after diastema closure regardless of frenectomy. Conclusions Complete consensus among the orthodontists was not obtained; however, some agreement was found regarding the development of a logical diagnosis and treatment approach. High-quality studies are required to produce national protocols or UK guidelines.

- Abstract
Introduction: This study aimed to compare the characteristics of pleasant and unpleasant smiles from the perception of laypeople., Methods: Two-hundred posed smile photographs were obtained from adult participants with no anomaly, restoration, or severe crowding and spacing in anterior teeth. Photographs were shown to 26 judges to give each photograph a score for attractiveness. Upper and lower quartiles for the most and least rated smiles were determined, and variables including gingival display, smile arc, midline deviation, buccal corridor, smile width, tooth rotation or malposition, diastema, upper and lower vermilion show, and tooth form were measured. Independent-sample t test and Pearson chi-square analysis were used to determine the differences between the pleasant and unpleasant groups for quantitative and qualitative variables, respectively. The linear regression model revealed variables with the most significant impact on the mean score., Results: All quantitative and qualitative variables except tooth form significantly differed between pleasant and unpleasant smile groups (P <0.05). The consonant smile arc was associated with an increase of 12.59% in mean scores compared with the inconsonant smile arc. Each tooth reported with malposition was correlated with a decrease of 9.37% in the scores. In the same way, each 1-mm increase in midline diastema and occlusal cant coincided with a drop of 8.73% and 3.59% in scores, respectively., Conclusions: The results of this study suggested that smile arc, tooth malposition, midline diastema, and occlusal plane canting had the most impact on smile esthetics and should be given priority by orthodontists in the treatment plan of choice., (Copyright © 2023 American Association of Orthodontists. Caudal fin symmetry characterizes teleosts and likely contributes to their evolutionary success. However, the coordinated development and patterning of skeletal elements establishing external symmetry remains incompletely understood. We explore the spatiotemporal emergence of caudal skeletal elements in zebrafish to consider evolutionary and developmental origins of caudal fin symmetry.Transgenic reporters and skeletal staining reveal that the hypural diastema-defining gap between hypurals 2 and 3 forms early and separates progenitors of two plates of connective tissue. Two sets of central principal rays (CPRs) synchronously, sequentially, and symmetrically emerge around the diastema. The two dorsal- and ventral-most rays (peripheral principal rays, PPRs) arise independently and earlier than adjacent CPRs. Muscle and tendon markers reveal that different muscles attach to CPR and PPR sets.We propose that caudal fin symmetry originates from a central organizer that establishes the hypural diastema and bidirectionally patterns surrounding tissue into two plates of connective tissue and two mirrored sets of CPRs. Further, two peripheral organizers unidirectionally specify PPRs, forming a symmetric "composite" fin derived from three fields. Distinct CPR and PPR ontogenies may represent developmental modules conferring ray identities, muscle connections, and biomechanical properties. Our model contextualizes mechanistic studies of teleost fin morphological variation.

136. Research from MBBS Provide New Insights into Diastema (Prevalance of Maxillary Midline Diastema According To Gender in A Population of Hyderabad).
- Abstract
A study conducted in Hyderabad examined the prevalence of Maxillary Midline Diastema, a condition characterized by a gap between the upper front teeth. The study included 2546 participants aged 18 to 50 years, with 1952 males and 594 females. The results showed that 33% of the participants had midline diastema, with a higher incidence in males compared to females. The study suggests that early detection of diastema is important for timely treatment, especially considering its impact on facial aesthetics and social activities. This research provides valuable insights into the prevalence and awareness of diastema, paving the way for further studies. [Extracted from the article]

- Abstract
A recent study conducted at Saraswati Dental College and Hospital in Uttar Pradesh, India, focused on the orthodontic management of midline diastema caused by mesiodens, which are supernumerary teeth found in the maxillary anterior region. The study emphasized the importance of early diagnosis and intervention to avoid complications such as malocclusion, unesthetic appearance, food lodgment, and cyst formation. The researchers presented three cases in which extraction of the supernumerary teeth followed by fixed orthodontic treatment successfully corrected the malocclusion and improved facial esthetics.

Introduction: Pituitary pars intermedia dysfunction (PPID) and dental disorders are of major concern in horses older than 15 years. Although PPID in geriatric horses and dental disorders in all age groups are well described, a connection between this endocrine disease and pathological changes in equine dental structures has not yet been investigated. In humans, periodontitis is considered to be a complication of systemic diseases like diabetes mellitus type 2, obesity and various conditions leading to an impaired immune response. In PPID, cross links to insulin and immune dysregulations are proven. The aim of this study was to compare histological findings of the gingiva and the sub gingival periodontal ligament of PPID affected horses with control horses. Methods: In a case-control morphometric descriptive study, 145 dental locations of 10 PPID affected horses (27.3 ± 2.06 years) were compared with 147 dental locations of 10 controls (21.4 ± 4.12 years). Histological parameters were leukocyte infiltration, keratinization of gingival epithelium, blood vessel supply of the periodontium and structure of cementum. Results: The distribution and localization of gingival leukocyte infiltrations (LI) in PPID affected horses was more often multifocal to coalescing (p = 0.002) and reached into deeper parts of the periodontium, sometimes down to the sub gingival periodontal ligament (PDL). Aged animals of both groups showed higher prevalence (PPID: OR 1.66; controls: OR 1.15) for severe leukocyte infiltration in the PDL. PPID was not significantly associated with increased LI. The cementum bordering the soft tissue in interdental locations showed four times more irregularities in PPID affected horses than in controls which predisposes for interdental food impaction and periodontal diseases. Discussion: In summary, multifocal to coalescing leukocytes and irregular cementum are seen more often in PPID than in controls - however our findings mainly reflect an association of older age with periodontal disease.

Diastemas in the anterior region are aesthetic problems. With the discovery of new techniques, more conservative, modern and long-lasting materials, reduction of clinical time and cost of treatments, the closure of diastemas has become one of the most types of aesthetic treatments recently. This being associated with orthodontic as a factor of aesthetic and functional improvements, in addition to predictable and faster results. Dentists mind this technique on a large scale. The aim of this review is to evaluate how scientific evidence on the closure of diastema through the use of direct composite resins, after orthodontic treatment, determines as well as advantages and this type of treatment.

ABSTRACT The presence of diastemas is one of the most common aesthetic complaints among patients. The use of composite resins is a good aesthetic and functional option to perform restorations and implement restorative techniques in a minimally invasive way. The resin layering technique was introduced in 1980, giving very good results in color selection, but being the most complex procedure and requiring great technical skills in addition to an increase in clinical time. Composite materials and restorative techniques that aid in the simplified use of clinical protocols are highly sought after by dentists to reduce chair time and minimize post-operative sensitivity. It is for these reasons, in addition to simplifying shade taking, that Tokuyama Dental developed Omnichroma resin which claims to have the ability to copy the color of the remaining tooth substrate shortly after light curing known as the “Chameleon Effect” using a single shade. The objective of this article is to describe in detail, through a clinical case report, the closure of diastemas with Tokuyama Dental's Omnichroma universal resin.

This case report presents a new technique for sectional veneer fabrication and diastema restoration with a prefabricated composite veneer. For the purpose of diastema restoration, a prefabricated sectional veneer provides the same benefits of a traditional ceramic sectional veneer (highly esthetic restoration with no need for tooth preparation) but involves a less technically demanding and time-consuming clinical procedure and a less delicate restoration with a reduced risk of accidental breakage and post-bonding crack formation. The technique presented in this case report bridges the gap between a direct and indirect technique for diastema restoration and introduces a new treatment option to close anterior spacing with a highly esthetic sectional veneer in a predictable and timely manner.
