It is almost 120 years since Alzheimer's disease (AD) was first reported, and the concept of managing some of the modifiable risk factors associated with the disease has been present from the outset. Intervening to manage risk factors as a way of tackling AD is not new, but optimizing brain health as a way of minimizing risk and maximizing the potential benefits of revolutionary new treatments for AD is becoming increasingly important. This book, the Handbook of Intervention and Alzheimer's Disease, presents 47 papers exploring factors which may either inspire or inform future treatment and clinical trials. While novel interventions such as anti-amyloid immunotherapy present great opportunities, they may also increase the risk of brain bleeds and edema, which in turn may lead to adverse clinical outcomes. Such adverse outcomes are demonstrably more likely to occur in persons with poor brain health, so improved management of the risk factors which make up the AD preventome will also minimize the risks associated with such novel therapies. The papers in this volume can therefore be thought of as offering insight into those factors that can optimize brain health or providing key insights into interventions which may achieve such outcomes. Together with its companion volume on prevention, the book provides a comprehensive overview of strategies for tackling Alzheimer's disease, and will be of interest to all those working in the field. As a result of severe wounds received in World War II, I have spent many months in military hospitals, including 20 months in an Army hospital immediately after the war. I continue to use the Military Health System, as do many of my colleagues in Congress, because I firmly believe the quality of health care delivered in military and veterans hospitals is second to none. The largest system of its type in the world, the U.S. military healthcare system is undergoing changes as dramatic as those experienced by the entire country. During Desert Storm, we saw new technologies, such as telemedicine, at work in the field. Since then, military medicine has contin­ ued to imprave and develop innovations that often focus on healthcare issues of concern to society as a whole. We already have seen technology transfer at work. Things we use in our everyday lives, from sunscreen to the Internet, have come to us directly from innovations developed by federal researchers. The private sector, working with the public agencies, has creatively adapted federal research. For example, the hemopump is used successfully by heart surgeons world­ wide to save heart patients. This device, developed by Richard Wampler, was based on satellite technology information that was declassified in the early 1980s. The chapters in this book focus on current federal sector efforts to shape health care and technology transfer. Many of the initiatives described involve some degree of partnering between the public and private sectors.

Cooper In Volume 23 we considered, in seven chapters, the basic armamentarium of the invertebrate immune system and its cells, as well as an analysis of antigens, setting the stage for the initiation of an immune response. We studied cell products, natural or induced, as revealed by nonspecific and specific responses following antigenic challenge such as the pro phenol oxidase system, the lytic responses, the Ig superfamily, and the place this family offers invertebrates and insect hemolymph proteins as candidates for membership. At this point, these various topics seemed to converge, almost to overlap, in some instances, presenting a challenge as to how to move from one subject to another. Chapter 1, in this volume offers the bridge to Volume 23 and its final Chapter 7. This Volume 24 contains contributions pertaining to cell activities and the environment. Chapters 1-4 refer specifically to interactions between cells and the integration of cell activities. The focus is on a functional immune system, with antigenic challenge as a subtopic. In Chapters 5-7, the environment is considered from several points of view and the main subtopic here is the result of the consequences of connections and missed signals. The internal and external environments are treated, revealing what may happen when normal immune responses are interfered with. All this is integrated by the consideration of the three great regulatory systems, the ever-present network that somehow acts as the monitor or control for all incoming and outgoing signals.

The ageing of the world's population is one of the major achievements of modern society. By 2050, an estimated 2 billion people will be aged 60 years or over. However, ageing poses major challenges and this is especially true for the field of ophthalmology, given that the major eye diseases – age-related macular degeneration, glaucoma, and cataract – predominantly affect the elderly. The challenges facing ophthalmology in an ageing society have not previously been addressed in a comprehensive way, although there are common denominators of the various eye diseases that affect the elderly. Helicobacter pylori has attracted widening interest from basic scientists and clinical investigators and the information on this organism is increasing exponentially. It is now accepted that H. HISTORICAL PERSPECTIVE This book has been published at the close of the twentieth century when the medical profession and the general community are increasingly concerned about the threat of multidrug-resistant tuberculosis (MDRTB)[1. 2]. However, at this epoch, it is enlightening to move back from our immediate concerns about MDRTB'hot spots'in Asia, South America, and the former Soviet Union [3], and to place our current predicament in an historical context. If the results of the global survey of antituberculosis drug resistance conducted by the World Health Organisation (WHO) and the International Union against Tuberculosis and Lung Disease (IUATLD) can be extrapolated, only 2. 2% of TB cases worldwide are due to multi drug­ resistant strains [3]. At the beginning of the 20th century, all TB cases were refractory to all available therapies. Great advances had been made during the 19th century in the understanding of the epidemiology and pathogenesis of TB, and in the diagnosis of the disease (reviewed in references 4-7).
