101. P.83 Augmentation du volume de l’estomac proximal après insertion de ballons intra-gastriques

Abstract

Introduction Le ballon intra-gastrique, rempli avec de l’air ou du serum physiologique, est propose dans la prise en charge de l’obesite avec une efficacite variable selon les sujets. Des modifications des proprietes mecaniques de la paroi gastrique, liees a la presence du ballon, pourraient expliquer certains echecs du traitement. L’objectif etait d’evaluer l’evolution du volume gastrique et les modifications de la compliance de la paroi de l’estomac apres insertion, pendant 5 mois, de ballons intra-gastriques dans l’estomac de porcs miniatures. Materiels et Methodes Dix porcs miniatures adultes (35,4 ± 2,15 kg) de race Gottingen ont ete repartis en 2 groupes : 5 porcs porteurs d’un ballon, 5 porcs dans le groupe controle. Les ballons ont ete inseres sous controle endoscopique et durant une anesthesie generale. Les ballons ont ete remplis avec 350 ml d’air a 20 mmHg. Les volumes gastriques ont ete mesures par TEMP avant mise en place des ballons, 2 semaines apres l’insertion puis toutes les trois semaines jusqu’au retrait des ballons. Les acquisitions tomographiques ont ete realisees apres ingestion d’un repas marque au 99mTc, auquel etait incorpore de la cholestyramine, dont les proprietes d’adhesion au mucus gastrique permettent de surligner la muqueuse gastrique. Les reconstructions tridimensionnelles et les mesures de volume de l’estomac ont ete realisees a l’aide du logiciel Osirix. La compliance de la paroi gastrique a ete mesuree au decours immediat du retrait des ballons a l’aide d’un barostat a regulation electronique. Resultats Le volume du ballon, mesure par scanner, etait egal a 342 ± 8,7 cm3, sans modification au cours de la periode experimentale. Deux semaines apres l’insertion, le volume gastrique etait significativement augmente (2 047 ± 256,8 cm3 versus 1 674 ± 318,7 cm3 avant insertion, p Conclusion L’insertion d’un ballon intra-gastrique pendant plusieurs mois, dans des conditions semblables a celles utilisees en pratique clinique chez l’homme, est suivie d’une augmentation significative du volume de l’estomac proximal, sans modification de l’estomac distal. L’origine de cette augmentation de volume de l’estomac proximal semble liee a une augmentation de la compliance de la paroi gastrique. Abstract

Introduction La coloscopie virtuelle (CTC) possede une precision diagnostique acceptable pour la detection des polypes et neoplasies coliques comparee a la videocoloscopie en particulier pour les polypes ≥ 6 mm. Son utilisation en strategie de depistage en population recommandee aux USA est encore en Europe au stade preliminaire et necessite, des etudes medico-economiques prealables avant a sa diffusion. L’objectif De l’etude est a partir d’une modelisation du depistage en population des neoplasies colorectales (NRC) par CTC, de comparer le rapport cout-efficacite lorsqu’une taille seuil du polype est ou non requise pour realiser consecutivement une polypectomie et d’identifier les facteurs de variation de ce rapport. Materiels et Methodes Les rapports cout-efficacite relatif (ICER) pour un depistage par CTC avec polypectomie (adhesion 90 %) quelle que soit la taille (strategie PL) ou uniquement pour la presence de polypes ≥ 6 mm (strategie TS) ont ete compares a partir d’un modele de Markov. Dans le modele de base, 3 CTC etaient proposees a 50, 60 et 70 ans (participation 55 %, sensibilite et specificite de la CTC de 77 et 95 % pour les polypes ≥ 6 mm), avec une duree de suivi de 30 ans et une prevalence des adenomas avances (AA) de 6,9 % ou 3,8 % chez l’homme ou la femme. L’ICER etait compare a l’absence de depistage et determine en euros par annee de vie gagnee (€/avs). Resultats Pour la strategie PL et TS, l’ICER etait respectivement de 12 042 et 2 765 €/avs, avec une diminution globale dans la cohorte (participant ou non a la CTC) de 37,9 et 36,5 % de l’incidence des NRC et de 47,2 vs 46,5 % de la mortalite par NRC. L’ICER de ces deux strategies etait respectivement de 12 512 et 2 516 €/avs lorsque le suivi des sujets etait de 40 ans jusqu’a l’âge de 90 ans. L’ICER des strategies chutait si seulement 2 CTC etaient realisees a 50 et 70 ans (respectivement 9 985 et 2 357 €/avs) ou a 50 et 60 ans (respectivement 7 629 et 917 €/avs). Ces ICER dependaient moins de la participation (respectivement 11 889 et 2 719 €/avs si 40 % et 12 209 et 2 825 €/avs si 70 %) ou de la precision de la CTC (12 948 et 3 166 €/avs pour une sensibilite et specificite de 73 et 88 %) que du sex ratio des sujets (respectivement + 27 et + 49 % pour la strategie PL et TS chez la femme / homme) ou de la prevalence des AA avec des ICER pour la strategie PL et TS de 13 388 et 3 291 €/avs - 9 687 et 1 857 €/avs ou 6 158 et 506 €/avs pour une prevalence de 4,5 % - 8,6 et 9,6 %. Conclusion La coloscopie virtuelle de depistage en population presente un rapport cout-efficacite comparable au depistage par la recherche de saignement occulte dans les selles (3 000 a 6 000 €/avs). Ce rapport cout-efficacite est peu sensible aux taux de participation mais depend de la duree de suivi, de la prevalence des adenomes avances et du sex ratio de la cohorte modelisee. Ce rapport est optimal pour 2 CTC de depistage a 50 et 60 ou 50 et 70 ans avec polypectomie pour les polypes ≥ 6 mm.

Abstract

Pseudoaneurysms and bleeding pseudocysts are rare but life-threatening complications of chronic pancreatitis. This report summarizes our experience in 8 cases collected from among 250 patients admitted for chronic pancreatitis. We describe the radiological findings of nine vascular lesions and especially emphasize the contribution of recent radiological imaging, ultrasound and computed tomography scanning in establishing the diagnosis of vascular complication. We also report a case of successful embolization of a splenic pseudoaneurysm that ruptured into the colon.

104. Le coloscanner a l’eau : une technique performante pour le diagnostic des cancers coliques

Abstract

Objectifs Evaluer les performances du coloscanner a l’eau pour le diagnostic des cancers coliques. Materiels et methodes Dans une etude multicentrique, 194 patients d’âge moyen 68 +/-14 ans, suspects de cancer colique ont ete inclus prospectivement. Tous les patients ont eu un coloscanner a l’eau. La reference a ete la coloscopie et/ou la chirurgie. Soixante-et-onze patients avaient une tumeur maligne (68 adenocarcinomes, 2 lymphomes, 1 tumeur neuro-endocrine). Les indices habituels de performance ont ete calcules. Resultats Les 71 tumeurs ont ete diagnostiquees par le coloscanner a l’eau. Les 6 erreurs ont ete 6 faux positifs. La sensibilite et la specificite du coloscanner a l’eau pour le diagnostic de cancer colique etaient donc respectivement de 100 et 95,1 %. La valeur predictive positive et negative etaient de 92,2 et 100 %. A noter que 6 diagnostics ont ete faits par le coloscanner alors que la coloscopie etait incomplete et negative jusqu’au moment de son interruption. Conclusion Le coloscanner a l’eau est un examen simple de realisation, rapide d’apprentissage et dont les tres bonnes performances diagnostiques en font une technique valide pour le diagnostic de cancer colique.

Abstract

Although an increasing number and variety of complications of endoscopic injection sclerotherapy (EIS) of bleeding esophageal varices have been reported, infectious complications are rare. A case of brain abscess following EIS is reported which was characterised by an unusual clinical presentation in that there was no fever or leucocytosis. This presentation could have led to the abscesses being mistaken for brain metastases. The outcome of antimicrobial therapy was favorable which provided further evidence in support of the diagnosis.

Abstract

We report a case of splenic pseudoaneurysm rupture into the colon in chronic pancreatitis. At the time of bleeding, pancreatitis was undiagnosed and the colonic lesion was misinterpreted at colonoscopy. The lesion was also misinterpreted at laparotomy, because it simulated intra-abdominal carcinomatosis. The diagnosis was made by CT examination which showed an eccentric enhancing region within a hypodense mass in the region of the splenic hilum. Definitive control of bleeding was achieved by splenic arterial embolization. Splenic infarction and an intrasplenic abscess complicated the embolization, but they did not require surgical intervention. Healing of the colonic wall and of the colonic mucosa was observed at colonoscopy 3 weeks after the embolization.

Abstract

SummaryBackground Whether aminosalicylates or thiopurines reduce the risk of colorectal cancer (CRC) in inflammatory bowel (IBD) disease is controversial. Aim To assess simultaneously the chemopreventive effect of aminosalicylates or thiopurines in a case–control study nested in the CESAME observational cohort that enrolled consecutive patients with IBD between May 2004 and June 2005. Patients were followed up to December 2007. Methods Study population comprised 144 case patients who developed CRC from the diagnosis of IBD (65 and 79 cases diagnosed, respectively, before and from 2004, starting year of the prospective observational period of CESAME) and 286 controls matched for gender, age, IBD subtype and year of diagnosis, and cumulative extent of colitis. Exposure to aminosalicylates or thiopurines was defined by an exposure to the treatment during the year of the diagnosis of cancer. The propensity of receiving 5-ASA and thiopurines was quantified by a composite score taking into account patient and IBD characteristics. The role of aminosalicylates or thiopurines was assessed by multivariate analysis. Propensity scores and the history of primary sclerosing cholangitis were entered into the multivariate model for adjustment. Results By multivariate analysis adjusted for propensity, a significant protective effect of exposure to drugs during the year of cancer was found for aminosalicylates (OR = 0.587, 95% CI: 0.367–0.937, P = 0.0257), but not for thiopurines (OR = 0.762, 95% CI: 0.432–1.343, P = 0.3468). Conclusion In a case–control study nested in the CESAME cohort, a significant decrease in the risk of colorectal cancer in IBD was associated with exposure to aminosalicylates, not to thiopurines.
