Abstract

Aim of this retrospective study was to evaluate the most appropriate timing for the implant placement in oral cancer patients treated with radiation therapy.We collected data for 17 subjects (10 females, 7 males; mean age: 51 ± 19) with malignant oral tumors who had been treated with ablative surgery and radiation therapy of head and neck. The radiotherapy was set in accordance with the NCCN guidelines. Every patient received dental implant rehabilitation between 2014 and 2016. A total of 84 titanium implants were placed, at a minimum interval of 12 months, in irradiated residual bone. Every patient underwent a minimum follow-up of 12 months.Implant loss was dependent on the position and location of the implants (P = 0.05-0.1). Moreover, implant survival was dependent on the total dose of radiation and, mostly, on the timing of implant insertion after the end of the radiation therapy. This result was highly statistically significant (P0.01). Whether the implant was loaded is another highly significant (P0.01) factor determining survival. We observed significantly better outcomes when the implant was placed at least after 14 months and not loaded until at least 6 months after placement.Although the retrospective design of this study could be affected by selection and information biases, we conclude that a delayed implant placement after the end of the radiation therapy and a delayed loading protocol will give the best chance of implant osseointegration, stability and, ultimately, effective dental rehabilitation.

Abstract

Alveolar ridge augmentation procedures allow restoring jaw defects due to teeth extractions, periodontal diseases, trauma, or outcomes from a previous surgery. This case report describes a patient suffering from Fibrous Dysplasia of the right upper maxilla surgically reconstructed by fibula free flap. In 2003, four dental implants were placed in the 1.2, 1.3, 1.5, and 1.6 areas. Twelve years later, the onset of peri-implantitis led to the failure of osseointegration with consequent thinning of the fibula flap. To avoid the risk of fracture and to restore the bone volumes necessary for a new implant-prosthetic rehabilitation, we used heterologous biomaterials in combination with a non-reabsorbable membrane, according to the Guided Bone Regeneration (GBR) technique. GBR was performed using the Equimatrix® natural bone mineral matrix, Cytoplast™ Ti-150, a non-reabsorbable titanium-reinforced membrane, and four fastening screws to pin the membrane. After six months, the membrane was removed and two Zimmer® implants 3.7×13 mm were placed in the 1.1 and 1.2 areas. A fixed implant-supported prosthesis with a custom-milled titanium bar screwed to the implants was made. Computed tomography (CT) six months after GBR showed a good bone regeneration of 1.5 cm mesiodistal (MD), 1.8 cm buccopalatal (BP), and 2.8 cm in height. The main difficulty of this clinical case concerns the low predictability of success of GBR on a maxillary reconstructed area with a free fibula flap: there is no previous evidence in the literature. Clinical and radiographic exams nowadays show that there is no macroscopic bone reabsorption; however, further research is needed to obtain more information.

Abstract

Transthyretin (TTR) gene has a causal role in a hereditary form of amyloidosis (ATTRm) and is potentially involved in the risk of wild-type transthyretin amyloidosis (ATTRwt). To understand the genetics of ATTRm and ATTRwt, we conducted a phenome-wide association study of TTR gene in 361,194 participants of European descent testing coding and non-coding variants. Among the 382 clinically relevant phenotypes tested, TTR non-coding variants were associated with 26 phenotypic traits after multiple testing correction. These included signs related to both ATTRm and ATTRwt such as chronic ischaemic heart disease (rs140226130, p = 2.00 × 10−6), heart failure (rs73956431, p = 2.74 × 10−6), atrial fibrillation (rs10163755, p = 4.63 × 10−6), dysphagia (rs2949506, p = 3.95 × 10−6), intestine diseases (rs970866, p = 7.14 × 10−6) and anxiety (rs554521234, p = 8.85 × 10−6). Consistent results were observed for TTR disease-causing mutation Val122Ile (rs76992529) with respect to carpal tunnel syndrome (p = 6.41 × 10−6) and mononeuropathies of upper limbs (p = 1.22 × 10−5). Sex differences were also observed in line with ATTRm and ATTRwt epidemiology. Additionally, we explored possible modifier genes related to TTR function, observing convergent associations of RBP4 variants with the clinical phenotypes associated with TTR locus. In conclusion, we provide novel insights regarding the molecular basis of ATTRm and ATTRwt based on large-scale cohort, expanding our understanding of the phenotypic spectrum associated with TTR gene variation.

Abstract

Background There are no Italian data regarding the strategies for preventing neonatal group B streptococcal (GBS) infection. We conducted a national survey in order to explore obstetrical, neonatal and microbiological practices for the GBS prevention. Methods Three distinct questionnaires were sent to obstetricians, neonatologists and microbiologists. Questionnaires included data on prenatal GBS screening, maternal risk factors, intrapartum antibiotic prophylaxis, microbiological information concerning specimen processing and GBS antimicrobial susceptibility. Results All respondent obstetrical units used the culture-based screening approach to identify women who should receive intrapartum antibiotic prophylaxis, and more than half of the microbiological laboratories (58%) reported using specimen processing consistent with CDC guidelines. Most neonatal units (89 out of 107, 82%) reported using protocols for preventing GBS early-onset sepsis consistent with CDC guidelines. Conclusions The screening-based strategy is largely prevalent in Italy, and most protocols for preventing GBS early-onset sepsis are consistent with CDC guidelines. However, we found discrepancies in practices among centers that may reflect the lack of Italian guidelines issued by public health organizations.

In the case of collapses due to earthquakes in which there is evidence of a rocking-block behavior of the structures, the so-called “rocking-block” model can be successfully employed to understand the building behaviour. Following the seminal work by Housner in 1963, the problem of the motion of an oscillating parallelepiped-shaped rigid body has been widely investigated. The main issue addressed in this and other earlier studies is the performance of a structure in the framework of the rigid block dynamics, during an earthquake, assuming the building to be a block. The same situation can be recognized in very slender structures, like wind tower turbines, subjected to wind actions and in masonry walls loaded by seismic or in general out of plane actions. The problem can be treated analytically solving the equations of motion under the assumption of slender block geometry and rocking without sliding. Peak rocking response and overturning criteria for different waveforms can be evaluated. This paper presents a discussion on the reliability of this model, showing that in some cases a simplified approach involving the knowledge of a limited number of mechanical parameters can give information about the collapse of the building. Results of a numerical analysis are provided in order to assess the trustworthiness of the approach.

Abstract

Hybrid devices employing organic semiconductors interfaced with an aqueous solution represent a new frontier in bioelectronics and energy applications. Understanding of the energetics and photoinduced processes occurring at the organic/water interface is fundamental for further progress. Here, we investigate the interfacial electronic structure of poly-3-hexylthiophene (P3HT) sandwiched between an indium tin oxide (ITO) electrode and a liquid water electrolyte. The aqueous solution is found to polarize the polymer outermost layers, which together with the polymer p-(photo) doping by dissolved oxygen localizes photogenerated electrons at the P3HT/water interface, while holes can be transferred to the ITO electrode. Under illumination, the polymer/water interface is negatively charged, attracting positive ions from the electrolyte solution and perturbing the ion distribution in the aqueous solution. The observed mechanism is of general character and could underlie the behavior of a variety of devices characterized by an organic/water interface, such as prosthetic devices for artificial vision and organic-based systems for photoelectrochemical applications.

Abstract

IntroductionThe major unmet need in multiple sclerosis (MS) is for neuroprotective therapies that can slow (or ideally stop) the rate of disease progression. The UK MS Society Clinical Trials Network (CTN) was initiated in 2007 with the purpose of developing a national, efficient, multiarm trial of repurposed drugs. Key underpinning work was commissioned by the CTN to inform the design, outcome selection and drug choice including animal models and a systematic review. This identified seven leading oral agents for repurposing as neuroprotective therapies in secondary progressive MS (SPMS). The purpose of the Multiple Sclerosis-Secondary Progressive Multi-Arm Randomisation Trial (MS-SMART) will be to evaluate the neuroprotective efficacy of three of these drugs, selected with distinct mechanistic actions and previous evidence of likely efficacy, against a common placebo arm. The interventions chosen were: amiloride (acid-sensing ion channel antagonist); fluoxetine (selective serotonin reuptake inhibitor) and riluzole (glutamate antagonist).Methods and analysisPatients with progressing SPMS will be randomised 1:1:1:1 to amiloride, fluoxetine, riluzole or matched placebo and followed for 96 weeks. The primary outcome will be the percentage brain volume change (PBVC) between baseline and 96 weeks, derived from structural MR brain imaging data using the Structural Image Evaluation, using Normalisation, of Atrophy method. With a sample size of 90 per arm, this will give 90% power to detect a 40% reduction in PBVC in any active arm compared with placebo and 80% power to detect a 35% reduction (analysing by analysis of covariance and with adjustment for multiple comparisons of three 1.67% two-sided tests), giving a 5% overall two-sided significance level. MS-SMART is not powered to detect differences between the three active treatment arms. Allowing for a 20% dropout rate, 110 patients per arm will be randomised. The study will take place at Neuroscience centres in England and Scotland.Ethics and disseminationMS-SMART was approved by the Scotland A Research Ethics Committee on 13 January 2013 (REC reference: 13/SS/0007). Abstract

Structural inhomogeneity on a micrometer-scale across a CH3NH3PbI3 single crystal is responsible for a local modulation of the optical band gap, which is also highly sensitive to humidity., Here we identify structural inhomogeneity on a micrometer scale across the surface of a CH3NH3PbI3 perovskite single crystal. At the crystal edge a local distortion of the crystal lattice is responsible for a widening of the optical bandgap and faster photo-carrier recombination. These effects are inherently present at the edge of the crystal, and further enhanced upon water intercalation, as a preliminary step in the hydration of the perovskite material.

Abstract

OBJECTIVE: Achalasia is a very rare disease characterized by the lack of motor coordination in various phases of contraction-relaxation of esophageal muscles. The aim of the study is to determine the correlation between esophageal achalasia and oral diseases. PATIENTS AND METHODS: Thirteen esophageal achalasia patients were assessed for the presence of any potential oral symptoms through a questionnaire. 5 volunteered to be included in the study. The oral health status of these 5 patients was assessed by a clinical oral examination. The oral health status of the included cases was compared to the oral health status data from a published observational study on healthy individuals belonging to the same (Italian) population. RESULTS: Our results show that the number of periodontal pockets with Probing Pocket Dept > 4 mm is significantly increased, compared to the average of the Italian population; the Bleeding On Probing (BOP) is not related to the Plaque Control Record (PCR); so we can say that the value of periodontal index is not correlated with the value of oral hygiene index. CONCLUSIONS: Considering the limitations of a research based on small numbers, it nevertheless seems advisable to assert that there is a correlation between esophageal achalasia and periodontal disease. Abstract

This paper presents an approach to a constitutive model for anisotropic quasi-brittle materials, developed in the framework of rate independent softening plasticity, involving a yield criterion in which an anisotropic friction and cohesion tensors are involved. It turns out to be useful for materials characterized by ultimate behaviour which varies according to the direction, such as composite materials, anisotropic rocks, textiles, masonry. The intrinsic structure of a wide range of structural, geological and industrial materials, such as composites, masonry, wood, textiles and several types of rocks and clays, is the major cause of their anisotropic behaviour. The material response to the same stress state is strongly linked to the sampling orientation with respect to the principal stress axes. The position and the orientation of the clay particles, for example, define a sort of material geometry on which the stress geometry depends. Similar considerations can be applied to a Representative Volume Element in order to describe the macroscopical behaviour of an equivalent anisotropic orientation of the fibers in a composite, the disposition of the bed and head joints in a masonry wall medium resulting from an homogenization technique. Moreover, anisotropic friction plays an important role in the formation of wrinkles and folds in textiles. The intrinsic structure of a wide range of structural, geological and industrial materials, such as composites, masonry, wood, textiles and several types of rocks and clays, is the major cause of their anisotropic behavior. The material response to the same stress state is strongly linked to the sampling orientation with respect to the principal stress axes. The position and the orientation of microfibers, for example, define a sort of material geometry on which the stress geometry depends. Some of the research efforts made in recent years to achieve a better understanding of constitutive behaviour of quasi-brittle materials have been developed in the framework of plasticity. The theoretical and numerical problems about the variational formulation of non-associated plastic flow rule has been developed although the complexity of the problem welcomes more studies in the future. Most contributions towards establishing a rational failure theory of quasi brittle materials exhibiting a strong anisotropic behaviour are phenomenological: experimental results are interpreted on the basis of a criterion which is assumed to be suitable for the observed mode of failure. The yield condition, based on the generalization of the Mohr-Coulomb criterion, involves six material parameters. A geometrical representation of the failure domain is provided 3 in the case of plane stress. Experimental tests arranged for the identification of the criterion and performed on composite samples are considered and discussed in this paper. It is shown that the agreement between the theory and experiments is reasonably good.

Abstract

Dye-sensitized photocatalytic H2 generation has been investigated using a metal-free phenothiazine-based donor-acceptor sensitizer (PTZ-GLU) in combination with coadsorbents. The coadsorption of the PTZ-GLU dye, functionalized with a glucose end-group, in combination with a glucose-based coadsorbent, afforded improved photocatalytic activity compared to the absence of coadsorbents, to the use of a conventional (chenodeoxycholic acid) coadsorbent, or by replacing the dye glucose functionality with an alkyl chain. The results suggest the strategic role of directional intermolecular dye-coadsorbent interactions on the semiconductor surface, as confirmed by first principles computational modeling, which likely suppressed detrimental recombination processes.

Abstract

The Balkan Peninsula is known to represent a complex cultural mosaic and it is a strategic area because it represents a gateway into Europe from the Near East . This research seeks to evaluate the variability of both uniparental markers (mtDNA and non-recombining region of the Y chromosome) to dissect the genetic makeup of Serbians. The whole sample pertains to 257 Serbians (87 from the central region and 170 from the southern area) who have been analyzed for both uniparental genetic markers. The results showed that the extant inhabitants of the Balkan Peninsula have a homogeneous genetic background, despite their linguistic and cultural differences. The obtained data were compared with those of neighboring populations to detect possible relationships among groups. On the whole, the genetic variability of the Balkan populations seems to be due to an admixture process of European and Asian lineages in different proportions whose contributions constitute the current maternal and paternal genetic landscape.
