105. 18-month hydration of a low-pH cement for geological disposal of radioactive waste: The Cebama reference cement

Abstract

Low-pH cements are candidate materials for use in the construction of geological disposal facilities for the long-term management of nuclear waste. Since these facilities will operate over long time scales, the changes in mineralogy and microstructure require evaluation as a function of time. As a first step towards this understanding, the hydration of a standardised low-pH cement paste, known as the Cebama reference cement, was investigated over an 18-month period. Characterisation was performed at 28 days of curing, at 20 °C and 40 °C, and novel synchrotron radiation X-ray diffraction experiments were performed, in-situ, from 90 min to 18 months of curing. Concurrent solid state 29Si and 27Al MAS NMR data were acquired for parallel samples to quantify the extent of cement hydration and the composition and mean chain length of the predominant calcium aluminosilicate hydrate (C-(A)-S-H) reaction product. After 18 months, cement clinker phases were still present, highlighting the slow hydration kinetics of this low-pH cement. The data presented provide a benchmark for ongoing and future studies of low-pH cements in geological disposal environments, over extended time scales.

Abstract

IMPORTANCE: Patients with hypertrophic cardiomyopathy (HCM) are prone to body weight increase and obesity. Whether this predisposes these individuals to long-term adverse outcomes is still unresolved. OBJECTIVE: To describe the association of body mass index (BMI, calculated as weight in kilograms divided by height in meters squared) with long-term outcomes in patients with HCM in terms of overall disease progression, heart failure symptoms, and arrhythmias. DESIGN, SETTING, AND PARTICIPANTS: In this cohort study, retrospective data were analyzed from the ongoing prospective Sarcomeric Human Cardiomyopathy Registry, an international database created by 8 high-volume HCM centers that includes more than 6000 patients who have been observed longitudinally for decades. Records from database inception up to the first quarter of 2018 were analyzed. Patients were divided into 3 groups according to BMI class (normal weight group, 30). Patients with 1 or more follow-up visits were included in the analysis. Data were analyzed from April to October 2018. EXPOSURES: Association of baseline BMI with outcome was assessed. MAIN OUTCOME AND MEASURES: Outcome was measured against overall and cardiovascular mortality, a heart failure outcome (ejection fraction less than 35%, New York Heart Association class III/IV symptoms, cardiac transplant, or assist device implantation), a ventricular arrhythmic outcome (sudden cardiac death, resuscitated cardiac arrest, or appropriate implantable cardioverter-defibrillator therapy), and an overall composite outcome (first occurrence of any component of the ventricular arrhythmic or heart failure composite end point, all-cause mortality, atrial fibrillation, or stroke). RESULTS: Of the 3282 included patients, 2019 (61.5%) were male, and the mean (SD) age at diagnosis was 47 (15) years. These patients were observed for a median (interquartile range) of 6.8 (3.3-13.3) years. There were 962 patients in the normal weight group (29.3%), 1280 patients in the preobesity group (39.0%), and 1040 patients in the obesity group (31.7%). Abstract

Background: Intravenous thrombolytic therapy (IVT) with tissue plasminogen activator for acute ischemic stroke is underutilized in many parts of the world. Randomized trials to test the effectiveness of thrombolysis implementation strategies are limited. Methods and Results: This study aimed to test the effectiveness of a multicomponent, multidisciplinary tissue plasminogen activator implementation package in increasing the proportion of thrombolyzed cases while maintaining accepted benchmarks for low rates of intracranial hemorrhage and high rates of functional outcomes at 3 months. A cluster randomized controlled trial of 20 hospitals in the early stages of thrombolysis implementation across 3 Australian states was undertaken. Monitoring of IVT rates during the baseline period allowed hospitals (the unit of randomization) to be grouped into 3 baseline IVT strata—very low rates (0% to ≤4.0%); low rates (>4.0% to ≤10.0%); and moderate rates (>10.0%). Hospitals were randomized to an implementation package (experimental group) or usual care (control group) using a 1:1 ratio. The 16-month intervention was based on behavioral theory and analysis of the steps, roles, and barriers to rapid assessment for thrombolysis eligibility and involved comprehensive strategies addressing individual and system-level change. The primary outcome was the difference in tissue plasminogen activator proportions between the 2 groups postintervention. The absolute difference in postintervention IVT rates between intervention and control hospitals adjusted for baseline IVT rate and stratum was not significant (primary outcome rate difference=1.1% (95% CI −1.5% to 3.7%; P=0.38). Rates of intracranial hemorrhage remained below international benchmarks. Conclusions: The implementation package resulted in no significant change in tissue plasminogen activator implementation, suggesting that ongoing support is needed to sustain initial modifications in behavior. Background: Pathogenic variants in MYBPC3, encoding cardiac MyBP-C (myosin binding protein C), are the most common cause of familial hypertrophic cardiomyopathy. A large number of unique MYBPC3 variants and relatively small genotyped hypertrophic cardiomyopathy cohorts have precluded detailed genotype-phenotype correlations. 110. Crowdsourcing in health and medical research: a systematic review

Abstract

BACKGROUND: Crowdsourcing is used increasingly in health and medical research. Crowdsourcing is the process of aggregating crowd wisdom to solve a problem. The purpose of this systematic review is to summarize quantitative evidence on crowdsourcing to improve health. METHODS: We followed Cochrane systematic review guidance and systematically searched seven databases up to September 4th 2019. Studies were included if they reported on crowdsourcing and related to health or medicine. Studies were excluded if recruitment was the only use of crowdsourcing. We determined the level of evidence associated with review findings using the GRADE approach. RESULTS: We screened 3508 citations, accessed 362 articles, and included 188 studies. Ninety-six studies examined effectiveness, 127 examined feasibility, and 37 examined cost. The most common purposes were to evaluate surgical skills (17 studies), to create sexual health messages (seven studies), and to provide layperson cardio-pulmonary resuscitation (CPR) out-of-hospital (six studies). Seventeen observational studies used crowdsourcing to evaluate surgical skills, finding that crowdsourcing evaluation was as effective as expert evaluation (low quality). Four studies used a challenge contest to solicit human immunodeficiency virus (HIV) testing promotion materials and increase HIV testing rates (moderate quality), and two of the four studies found this approach saved money. Three studies suggested that an interactive technology system increased rates of layperson initiated CPR out-of-hospital (moderate quality). However, studies analyzing crowdsourcing to evaluate surgical skills and layperson-initiated CPR were only from high-income countries. Five studies examined crowdsourcing to inform artificial intelligence projects, most often related to annotation of medical data. Abstract

Poppy (Papaver somniferum) L. is cultivated commercially in several countries legally to produce alkaloids (i.e. morphine, codeine, thebaine, narcotine, papaverine and oripavine) for pharmaceutical industries. The demand for these alkaloids is increasing and Global climate change is influencing poppy cultivated areas. Turkiye is responsible for poppy cultivation, breeding new cultivars and adaptation trials. Therefore, the present investigation was undertaken during two years to examine the most cultivated 16 cultivars and 3 new lines for capsule morphology properties, their alkaloid values and capsule yield in Ankara conditions which is not in the poppy cultivation area in Turkiye. The experiment was arranged in the randomized complete block design with four replications. The plot size of 2.4 m² (2 m × 1.2 m) and 4 rows with spacing 30 cm between rows and 10 cm within plants were applied. The capsule parameters and alkaloid content were measured in 10 dry capsules after harvesting. To measure the content of alkaloids (%) the capsules were ground into powder and HPLC-MS/MS analysis after the solvent extraction of poppy straw on dry matter was conducted. Genotypes showed variation in stigmatic rays and the number of stigmatic rays was higher in the first year. The maximum stigmatic ray value was seen in cv. Ofis-95 with 13.27 in the first year. The maximum value for capsule length and diameter was obtained from cv. Ofis-NP (51.75 mm) and cv. Ofis-95 (45.00 mm) in the mean of two years. The maximum capsule yield was obtained from cv. Ofis-95 for both years and the values were 155.90 and 160.30 kg da-1. The maximum morphine percentage was recorded in cv. Ofis-1 with 1.63 and 1.24% respectively. The percentage of noscapine, codeine, thebaine was noted as maximum values in Ofis-1 among genotypes. The results favoured the cultivation of these genotypes in Ankara which is not one of the poppy cultivation area and further agronomic studies should be undertaken to develop production technology of poppy in these environmental conditions. Abstract

All UK H&I laboratories and transplant units operate under a single national kidney offering policy, but there have been variations in approach regarding when to undertake the pre‐transplant crossmatch test. In order to minimize cold ischaemia times for deceased donor kidney transplantation we sought to find ways to be able to report a crossmatch result as early as possible in the donation process. A panel of experts in transplant surgery, nephrology, specialist nursing in organ donation and H&I (all relevant UK laboratories represented) assessed evidence and opinion concerning five factors that relate to the effectiveness of the crossmatch process, as follows: when the result should be ready for reporting; what level of donor HLA typing is needed; crossmatch sample type and availability; fairness and equity; risks and patient safety. Guidelines aimed at improving practice based on these issues are presented, and we expect that following these will allow H&I laboratories to contribute to reducing CIT in deceased donor kidney transplantation.
