Actinic cheilitis (AC) is a potentially malignant disorder of the lips. Because of its heterogeneous clinical aspect, it is difficult to indicate representative biopsy area. The purpose of this study was to evaluate the usefulness of tissue autofluorescence in AC diagnosis. The system was composed of a 405-nm light-emitting diode, sent to the sample by a dichroic, that allows the fluorescence signal to reach a camera directly plugged in the system. Fifty-seven patients with clinical diagnosis of AC and 45 normal volunteers were selected. According to clinical and fluorescence features, one or more areas were selected for biopsies in the AC group and epithelial dysplasia (ED) grades were established. The autofluorescence images were processed by a clustering algorithm for AC automated diagnosis. The tissue autofluorescence image revealed a heterogeneous pattern of loss and increase of fluorescence in patients with AC. ED was found in 93% of the cases, and most of the areas graded as moderate or severe ED were chosen with the aid of autofluorescence. The processed autofluorescence images from AC patients showed a higher number of spots in an irregular pattern. Tissue autofluorescence image system is a useful technique in association with clinical examination for AC diagnosis.

Purpose To investigate the front of tumor invasion as a prognostic factor in association with E-cadherin and β-catenin immunostaining in patients with tongue and floor of mouth carcinoma (TFSCC) at clinical stages I and II. Material and methods Records from 156 patients were analyzed and submitted to bivariate (X 2 ) and survival (Kaplan–Meier) analysis. Results Caucasian (61.4%) males (75%) aged between 41 and 60 years (50.6%), who smoke (83.8%) and drink (74.8%), with tumors of the tongue (69.2%), at clinical stage II (66.4%) and pathological stage II (28.2%) were the most affected. We identified 76 cases (48.7%) of recurrence, especially in cervical lymph nodes (53.9%). One hundred and fifteen of 156 patients (73.7%) were alive. Moderately differentiated tumors (78.8%), with pattern of invasion showing small islands (79.4%), with perineural invasion of small nerves (60.9%) and strong lymphocytic infiltrate (49.4%) were predominant. The intermediate score was the most frequent (56.4%) at histologic risk assessment (HRA). Immunohistochemical analysis was performed on 131 patients. Both E-cadherin and β-catenin showed most frequently concomitant membrane and cytoplasmic expression (71.0% and 53.4%) with a diffuse distribution (61.5% and 82.1%) respectively. Tumors involving both the tongue and floor of mouth occurred only in men ( p = 0.003). Decreased E-cadherin expression was also associated with death ( p = 0.009). There were worse overall survival (OS) and disease free survival for poorly differentiated tumors in advanced pathological stage, with a high-risk AHR and low E-cadherin expression. Conclusions Demographic profile of patients with TFSCC in initial clinical stage does not differ from the classic profile for individuals with SCC of mouth. The presence of recurrence was associated with histopathological parameters of aggressiveness. There was an association between cytoplasmic expression of E-cadherin with death and worse OS rates. Even though the study population was composed by initial stage tumors, an aggressive behavior was observed.

O estudo foi do tipo longitudinal, pois os dados foram coletados em três momentos: o primeiro momento com entrevista a respeito de dados sociodemográficos, exame clínico de cárie e doenças bucais – mucosite, xerostomia, candidíase oral, gengivite e afta, foi realizado antes da quimioterapia; o segundo momento aproximadamente um mês e o terceiro momento aproximadamente dois meses após a quimioterapia, com exame de manifestações bucais. Um total de 51 indivíduos foi avaliado no primeiro momento e devido a perdas por óbitos, transferência para outro hospital ou radioterapia, 42 foram avaliados no terceiro momento. Os resultados mostraram uma baixa prevalência de cárie que se associou a pior higiene bucal (p - 0,016) e menor escolaridade da mãe (p - 0,021). Foram observadas algumas doenças bucais antes do início da quimioterapia, também ocorreram complicações bucais após a quimioterapia destacando-se a mucosite que foi a mais prevalente, sendo que outras manifestações como xerostomia, afta, candidíase apareceram em menor número. Foram observadas manifestações orais que necessitaram de atenção clínica, pois afetaram atividades humanas básicas, como a comunicação e a alimentação. É fundamental, portanto a participação do cirurgião-dentista na equipe multidisciplinar de tratamento oncológico, para que ele possa avaliar a condição bucal dos indivíduos antes e durante o tratamento, podendo dessa forma elaborar um plano de tratamento adequado, atuando ativamente na prevenção, diagnóstico precoce e tratamento das complicações orais, evitando que elas desenvolvam um curso clínico mais grave, podendo afetar negativamente o tratamento dos indivíduos. The aim of this study was to assess and monitor child and adolescent cancer patients with the first cancer diagnosis receiving chemotherapy in a hospital called child state hospital Nossa Senhora da Glória – Vitória – ES, from April 2013 to April 2014. It was a longitudinal study and the data were collected at three times: the first time by an interview concerning demographic data, a clinical examination of dental caries and oral diseases - mucositis, xerostomia, oral candidiasis, gingivitis and thrush, performed before chemotherapy; the second and third moments were performed in about a month and two months after chemotherapy, respectively, with examination of oral manifestations. A total of 51 subjects were evaluated at the first time and due to losses by death and transfer to another hospital or radiotherapy, 42 were evaluated in the third period. The results showed a low prevalence of caries associated with worse oral hygiene (p - 0.016) and low maternal education (p - 0.021). Some oral diseases before the start of chemotherapy were observed and oral complications also occurred after chemotherapy, highlighting mucositis as the most prevalent disease. Other manifestations such as dry mouth, aphthous stomatitis and candidiasis were observed in a lower frequency. Oral manifestations affecting basic human activities such as communication and feeding, requiring medical attention, were observed. Therefore, the participation of dentists is essential in the multidisciplinary team of cancer treatment to evaluate the oral health status of individuals before and during the treatment. It is also important in order to develop a proper treatment plan, to work actively in prevention, early diagnosis and treatment of oral complications, preventing patients from developing a more severe clinical course that would adversely affect their treatment.

A primeira proposta de artigo teve como objetivo conhecer as médias dos escores geral ou por dimensões encontrados nos estudos realizados os quais utilizaram o Pediatric Quality of Life Inventory 3.0 Cancer Module. Aplicou-se a estratégia de busca nas bases Scopus, Web of Science, Bireme, PsycoInfo e a EBSCO host. Encontrou-se 210 resumos e títulos, excluiu-se 189 por duplicidade e por não atender aos critérios de inclusão. Analisou-se e elegeu-se 21 para compor o estudo. Todos os artigos publicados na língua inglesa, em sua maioria na América (47,6%) e no período de 2011 a 2013 (61,9%), utilizaram-se do questionário para a coleta de dados tanto para criança e adolescentes quanto para seus pais na maioria dos artigos (61,9%). A variação dos escores por dimensões e geral provavelmente esteve relacionada com a seleção dos grupos de comparação pois, pode-se perceber a diversidade dos critérios de inclusão e das variáveis para a análise em cada estudo. Concluiu-se que as médias dos escores por dimensões e geral não alcançaram valores abaixo de 30 e os maiores escores por dimensões estão acima de 80. Sugere-se que a dimensão ansiedade frente ao tratamento, no relato das crianças, pode ter obtido os maiores escores dentro de cada estudo. A segunda proposta de artigo objetivou-se avaliar a qualidade de vida de crianças e adolescentes com câncer em tratamento ou em acompanhamento e verificar associações com as variáveis socioeconômicas e clínicas. Realizou-se um estudo transversal com 98 crianças e adolescentes com diagnóstico de câncer de uma unidade de referência em oncologia pediátrica no estado do Espírito Santo - Brasil. Aplicou-se o questionário PedsQLTM e coletou-se informações sociodemográficas e clínicas. A amostra caracterizou-se com predominância do sexo masculino, de faixa etária de 08 a 12 anos, de classe econômica C e com diagnóstico de leucemia. Houve associação entre a faixa etária nas médias escores de 5 das 8 dimensões da avaliação da qualidade de vida. Não houveram diferenças estatisticamente significante em nenhuma das dimensões nem no escore total da qualidade de vida com as variáveis sexo da criança e adolescente, tipo de tumor, classe econômica e a escolaridade da mãe. Concluiu-se que faixa etária, o status do tratamento e frequentar a escola são fatores que modificam a avaliação qualidade de vida no relato das crianças e adolescentes, aumentando ou diminuindo as dificuldades frente ao tratamento e a doença. This is a study on assessing the quality of life of children and adolescents with cancer who aimed to produce two proposed article. The first article proposal aimed at knowing the mean overall scores or dimensions found in studies which used the Pediatric Quality of Life Inventory 3.0 Cancer Module. Applied the strategy for searching databases Scopus, Web of Science, BIREME, PsycoInfo and EBSCO host. Met 210 titles and abstracts, 189 were excluded for duplication and for not meeting the inclusion criteria. Analyzed and 21 elected to compose the study. All articles published in English, mostly in America (47.6%) and in the period 2011-2013 (61.9%), we used the questionnaire to collect data for both children and adolescents and for their parents in most articles (61.9%). The range of scores for overall dimensions and probably was related to the selection of comparison groups because one can see the diversity of the inclusion criteria and variables for analysis in each study. It was concluded that the mean scores for dimensions and overall did not reach values below 30 and the highest scores for dimensions are above 80. It is suggested that the dimension anxiety during treatment, on account of children, may have achieved the greatest scores within each study. The second article proposal aimed at evaluating the quality of life of children and adolescents with cancer treatment or follow-up and investigate associations with socioeconomic and clinical variables. We conducted a cross-sectional study with 98 children and adolescents diagnosed with cancer a reference unit in pediatric oncology in the state of Espirito Santo - Brazil. Applied the PedsQLTM questionnaire and collected the sociodemographic and clinical information. The sample was characterized predominantly male, age range 08-12 years, from economy class C and diagnosed with leukemia. Was no association between the age of 5 in mean scores of the eight dimensions of quality of life assessment. There were no statistically significant differences in any of the dimensions or total score of quality of life with the sex of the child and adolescent, tumor type, economic class and maternal education. It was concluded that age, the status of treatment and attending school are factors that modify the quality of life assessment in children and adolescents reporting, increasing or decreasing the difficulties to the treatment and the disease.
