Abstract

Somatoform disorders occasionally surface in employees and others who, ordinarily unknowingly, are reacting to the stresses in their lives. Somatization is a process by which individuals communicate psychological distress through physical symptoms, thus essentially making themselves ill. However, by learning to recognize the signs of possible somatic problems the manager can apply various strategies to decrease an employee's apparent physical problems and enhance job performance.

Abstract

About 17% of humanity goes through an episode of major depression at some point in their lifetime. Despite the enormous societal costs of this incapacitating disorder, it is largely unknown how the likelihood of falling into a depressive episode can be assessed. Here, we show for a large group of healthy individuals and patients that the probability of an upcoming shift between a depressed and a normal state is related to elevated temporal autocorrelation, variance, and correlation between emotions in fluctuations of autorecorded emotions. These are indicators of the general phenomenon of critical slowing down, which is expected to occur when a system approaches a tipping point. Our results support the hypothesis that mood may have alternative stable states separated by tipping points, and suggest an approach for assessing the likelihood of transitions into and out of depression.

Abstract

The patient had been in an automobile accident and was recuperating from a brain concussion. He was scheduled for 24 h of evaluative procedures to rule out injuries which might require surgery. He had been on a back board for a possible spine injury, had a variety of X-rays, blood work, and an EKG, and had been given oxygen, medication by injection, and intravenous fluid and medication through a pump. A variety of health team members including nurses, physicians, and a surgeon had done assessments and taken a history to assist in the diagnostic process. "The best thing about the hospitalization was the nurses telling me what they are doing and why they are doing it! It was really reassuring to have the procedures explained and to know what is being done and why." "I didn't always know if they were going through the steps to help remind themselves what they were doing or to tell me what they were doing. In either case, it was helpful for me to have information on what was happening." "They answered my questions as best they could. When I asked the nurse adding an IV bag what was in it he said, "Potassium" and he explained that my level was 3.3 when the normal was 3.5 to 5.0. He said that since they didn't know my normal they were giving me potassium just in case I needed it. He knew just what he was talking about and the facts were reassuring." "When I asked him to explain what the injection was, he said it was "Tetanus." He said I could refuse it if I wanted to, that it would probably get sore. However, he said I did have open abrasions and if I had not had one in 10 years it would be a good idea. So understanding this reasoning, I was glad to get the shot." "Patient education is really important. When I was in the hospital 10 years ago I was not given all this information and it really helps! I really appreciate the different approach by the nurses. It is really reassuring."

Abstract

Zahlreiche chronische Erkrankungen in der Kindheit und im Erwachsenenalter haben ihren Ursprung in der perinatalen Entwicklungsphase und sind zudem möglicherweise transgenerational, epigenetisch beeinflusst. Daher tragen gerade prospektive Geburtskohorten maßgeblich zum Verständnis der Ätiologie von Erkrankungen bei und können veränderbare Risikofaktoren identifizieren. Die beiden populationsbasierten deutschen Geburtskohorten GINIplus und LISAplus verfolgen als Hauptziele den natürlichen Verlauf chronischer Erkrankungen im Kindesalter sowie intermediäre Phänotypen und Funktionseinschränkungen zu beschreiben, deren Determinanten zu analysieren und mögliche Effektmodifikationen durch genetische Polymorphismen zu identifizieren. Mitte der 1990er-Jahre wurden in vier Regionen Deutschlands 5991 (GINIplus) beziehungsweise 3097 (LISAplus) gesunde reife Neugeborene für ein Langzeit-Follow-up-Programm rekrutiert und bislang mit einer Response von etwa 55% über zehn Jahre hinweg beobachtet. Es wurden Wachstums- und Gewichtsentwicklung, Infektionskrankheiten und Allergien, seelische Gesundheit, Mundgesundheit, metabolische und inflammatorische Parameter teils im Längsschnitt auf potenzielle Risikofaktoren hin untersucht und der Einfluss genetischer Varianten analysiert. Ergebnisse dieser beiden Geburtskohorten haben den Wissensstand zum Verlauf häufiger Erkrankungen und intermediärer Phänotypen im Längsschnitt maßgeblich bereichert. Ergebnisse dieser Studien sind in zahlreiche internationale Projekte und Konsortien mit dem Ziel eingeflossen, deutsche Daten in den europäischen Kontext zu stellen und gemeinsam auszuwerten. Abstract

There is evidence that environmental factors are important for the development of eczema. Different mechanisms have been discussed in the literature, the best known of which is the hygiene hypothesis. However, epidemiological data give reason for questioning this hypothesis with regard to childhood eczema. We present results from two German birth cohort studies (LISAplus and GINIplus) concerning regional prevalence patterns of eczema and the association of eczema with day care center attendance and older siblings. Our findings are not in line with the hygiene hypothesis and question its validity with regard to eczema. It seems reasonable to assume that the effect of environmental factors is somehow disease-specific.

Abstract

Der Anstieg der Prävalenzen von Asthma, Heuschnupfen und allergischer Sensibilisierung in Westdeutschland während der Zeit der deutschen Teilung und der rasche Anstieg dieser Krankheitshäufigkeit bei ostdeutschen Kindern und Jugendlichen nach der Wiedervereinigung sind untrügliche Zeichen für die herausragende Rolle von lebensstilabhängigen und umweltbezogenen Determinanten der Entwicklung atopischer Erkrankungen. Dabei spielt die perinatale Entwicklungsphase, in der das Immunsystem geprägt wird, vermutlich eine besondere Rolle. Deswegen können potenzielle Determinanten epidemiologisch am besten mittels Geburtskohorten bzw. Schwangerschaftskohorten untersucht werden. Gegenstand dieses Übersichtsartikels ist es, das Design der zwei großen deutschen Geburtskohorten GINIplus und LISAplus zu beschreiben und ausgewählte Ergebnisse zu atopischen Erkrankungen darzustellen. Mit GINIplus und LISAplus wurden Mitte bis Ende der 90er Jahre 5.991 bzw. 3.097 gesunde, reife Neugeborene in München, Wesel, Leipzig und Bad Honnef rekrutiert. Nach 10 Jahren sind noch circa 55% der Kohorte im Langzeitbeobachtungsprogramm eingeschlossen. Im Vordergrund standen bislang Fragen des natürlichen Verlaufs atopischer Erkrankungen sowie lebensstil- und umweltabhängige Faktoren. Darüber hinaus wurden Gene für atopische Erkrankungen, intermediäre Phänotypen sowie für oxidativen Stress verantwortliche Gene bestimmt und analysiert, ob deren Genvarianten den Einfluss identifizierter Umweltfaktoren modifizieren können. Die bisherigen Ergebnisse der beiden Geburtskohorten GINIplus und LISAplus tragen maßgeblich zu den Kenntnissen über atopische Erkrankungen und deren Determinanten bei.

Abstract

Graphical models are omnipresent in the software engineering field, but most current graphical modeling languages do not scale with the increasing size and complexity of today’s systems. The navigation in the diagrams becomes a major problem especially if different aspects of the system are scattered over multiple, only loosely coupled diagrams. In this paper we present the hierarchical navigation capabilities of the Adora modeling tool. The user of this tool can freely control the level of detail in different parts of the model to reduce the size and complexity of the diagrams being displayed. Our fisheye visualization technique makes it possible to integrate all modeling aspects (structure, data, behavior, etc.) in one coherent model while keeping the size and complexity of the diagrams within reasonable limits.
