Abstract

Pre-operative chemoradiotherapy (CRT) followed by surgical resection is still the standard treatment for locally advanced low rectal cancer. Nowadays new strategies are emerging to treat patients with a complete response to pre-operative treatment, rendering the optimal management still controversial and under debate. The primary aim of this study was to obtain a snapshot of tumor regression grade (TRG) distribution after standard CRT. Second, we aimed to identify a correlation between clinical tumor stage (cT) and TRG, and to define the accuracy of magnetic resonance imaging (MRI) in the restaging setting. Between January 2017 and June 2019, a cross sectional multicentric study was performed in 22 referral centers of colon-rectal surgery including all patients with cT3-4Nx/cTxN1-2 rectal cancer who underwent pre-operative CRT. Shapiro-Wilk test was used for continuous data. Categorical variables were compared with Chi-squared test or Fisher's exact test, where appropriate. Accuracy of restaging MRI in the identification of pathologic complete response (pCR) was determined evaluating the correspondence with the histopathological examination of surgical specimens. In the present study, 689 patients were enrolled. Complete tumor regression rate was 16.9%. The "watch and wait" strategy was applied in 4.3% of TRG4 patients. A clinical correlation between more advanced tumors and moderate to absent tumor regression was found (p = 0.03). Post-neoadjuvant MRI had low sensibility (55%) and high specificity (83%) with accuracy of 82.8% in identifying TRG4 and pCR. Our data provided a contemporary description of the effects of pre-operative CRT on a large pool of locally advanced low rectal cancer patients treated in different colon-rectal surgical centers.

Abstract

Background: The reasons behind low adherence to the Mediterranean diet (Med-diet) are still not entirely known. We aimed to evaluate the effect of biological (i.e., sex-related) and psycho-socio-cultural (i.e., gender-related) factors on Med-diet adherence. Methods: Baseline Med-diet adherence was measured using a self-administered questionnaire among adults with ischemic heart disease (IHD) from the EVA (Endocrine Vascular Disease Approach) study. A multivariable analysis was performed to estimate the effect of sex- and gender-related factors (i.e., identity, roles, relations, and institutionalized gender) on low adherence. Results: Among 366 participants (66 ± 11 years, 31% women), 81 (22%) adults with low adherence demonstrated higher rates of diabetes, no smoking habit, lower male BSRI (Bem Sex Role Inventory) (median (IQR) 4.8 (4.1 to 5.5) vs. 5.1 (4.5 to 5.6) and p = 0.048), and higher Perceived Stress Scale 10 items (PSS-10) (median (IQR) 19 (11 to 23) vs. 15 (11 to 20) and p = 0.07) scores than those with medium-high adherence. In the multivariable analysis, only active smoking (odds ratio, OR = 2.10, 95% confidence interval, CI 1.14 to 3.85 and p = 0.017), PPS-10 (OR = 1.04, 95% CI 1.00 to 1.08, and p = 0.038) and male BSRI scores (OR = 0.70, 95% CI 0.52 to 0.95, and p = 0.021) were independently associated with low adherence. Conclusions: Male personality traits and perceived stress (i.e., gender identity) were associated with low Med-diet adherence regardless of the sex, age, and comorbidities. Therefore, gender-sensitive interventions should be explored to improve adherence in IHD.

Abstract

Purpose: To evaluate the predictive value of $^{18}$F-FDG PET/CT semiquantitative parameters of the primary tumour and CA 19-9 levels assessed before treatment in patients with locally advanced pancreatic cancer (LAPC). Methods: Among one-hundred twenty patients with LAPC treated at our institution with initial chemotherapy followed by curative chemoradiotherapy (CRT) from July 2013 to January 2019, a secondary analysis with baseline $^{18}$F-FDG PET/CT was conducted in fifty-eight patients. Pre-treatment CA 19-9 level and the maximum standardized uptake value (SUVmax), metabolic tumour volume (MTV) and total lesion glycolysis (TLG) of primary tumour were measured. The receiving operating characteristics (ROC) analysis was performed to define the cut-off point of SUVmax, MTV, TLG and CA 19-9 values to use in prediction of early progression (EP), local progression (LP) and overall survival (OS). Areas under the curve (AUCs) were assessed for all variables. Post-test probability was calculated to evaluate the advantage for parameters combination. Results: For EP, CA 19-9 level > 698 U/mL resulted the best marker to identify patient at higher risk with OR of 5.96 (95% CI, 1.66-19.47; p = 0.005) and a Positive Predictive Value (PPV) of 61%. For LP, the most significant parameter was TLG (OR 9.75, 95% CI, 1.64-57.87, p = 0.012), with PPV of 83%. For OS, the most significant parameter was MTV (OR 3.12, 95% CI, 0.9-10.83, p = 0.07) with PPV of 88%. Adding consecutively each of the other parameters, PPV to identify patients at risk resulted further increased (>90%). Conclusions: Pre-treatment CA 19-9 level, as well as MTV and TLG values of primary tumour at baseline $^{18}$F-FDG PET/CT and their combination, may represent significant predictors of EP, LP and OS in LAPC patients.

Abstract

Acute chest pain (CP) is a potentially related both to acute coronary syndrome and to other morbidities; this means that 2%-10% patients with cardiogenic CP are improperly discharged from the Emergency Room (ER). In order to identify risk to develop cardiovascular (CV) events in patients admitted to ER with CP, we used Heart Risk Score (HRS).We included 165 patients referred to the ER for non-traumatic acute CP. We retrospectively analyzed clinical records from ER and Chest Pain Unit (CPU). We calculated HRS, then we analyzed HRS sensitivity and specificity, and correlated raw data of all variables with Spearman's analysis.Diagnosis of ischemic heart disease was made in 53.9% patients referring CP. The remaining patients were affected by other non-ischemic heart disease (35.5%), gastro-esophageal disease (32.3%), pleuro-pulmonary pathology (15.8%), musculoskeletal disorders (10.5%), and panic attacks (6.6%), respectively. Patients affected by coronaropathy had hypertension (80.9%), history of cardiopathy (61.8%), chronic smoking (49.4%), hypercholesterolemia (37.0%) , diabetes (33.7%) and obesity (24.7%). Low, medium and high HRS patients were 15.7%, 59.4% and 24.8%, respectively. Risk of CV events increased with the increase of the score. The negative predictive value (NPV) in low score was 92.3%. In high score, sensitivity and specificity were 94.7% and 82.7%, respectively. Finally, the following positive Spearman's correlations were found: HRS vs its risk variables, including individual risk variables, ischemic heart disease vs CV risk factors, history of ischemic cardiac disease vs risk factors, number of stenotic vessels vs risk factors (significance values: p0.05).HRS contains history of all risk factors for coronary artery disease and considers mild ECG and troponin alterations, giving the possibility to undertake the most appropriate path for the patient.Our work evidences relevance, reliability and ease of use of HRS in CV risk stratification in the emergency department, giving an important contribution in the evaluation of individuals who are likely to experience ischemic heart disease.

Abstract

The paper discusses the emerging forms of civil society in the NoLo area of Milan that have acquired political relevance by deploying a combination of leisure and activism. The heterogeneous set of initiatives and events that, using the NoLo label, animate the urban space have two distinct traits: firstly, they exert a subtle political action which is played out at a cultural level, in particular in their ability to draw on and influence common sense and taken-for-granted perceptions; and secondly, the chosen initiatives effectively influence local policy-making processes, in line with the neoliberal governance of the city and the authorities' promise to govern not just for the citizens but with them. The case study provides an empirical illustration of an emerging urban civil society, with specific attention on its functioning, how certain situated events were set up and unfolded, and two specific episodes of involvement in local policy-making. The proposed research findings - including the exclusionary/inclusive pattern that shapes citizens' involvement, the consensus-building strategy enacted by the studied civil society and the increasing political relevance of bottom-up urban initiatives - illustrate the meaning of governing through leisure activism in Milan. Abstract

By examining three policy-oriented research projects funded by the European Union (RAMSES, RESIN, SMR) the paper critically discusses operationalization efforts of urban resilience as they are designed and produced at the interface between science and policy. By analysing the documents and conducting interviews with the projects' coordinators and participants, three main research questions were addressed concerning how urban resilience is defined across the projects and the actors involved, the role of the tools produced by them in their difficult task to reconcile wider applicability and local specificity and finally how stakeholder engagement and co-creation were framed and implemented. Based on the evidence collected, the authors argue that conceptualizations of urban resilience within operationalization efforts being produced at the science policy-interface are still quite plural and open, that such openness is largely confirmed by the flexibility of the tools produced by the projects and that while becoming increasingly relevant stakeholder engagement and co-creation strategy are yet to be fully framed and theorized. Finally, they present further research pathways aimed at strengthening our knowledge of operationalization efforts, the role of practitioners and of urban resilience implementation in discrete scientific and political environments. Abstract

This study investigates the him pressure (BP) 24-hour pattern in representative samples of 2 industrialized countries, Italy and Japan, showing different cultures in salt intake. BP was monitored by means of a noninvasive ambulatory device whose readings were analyzed by means of chronobiometric procedures. The results show that the 24-hour BP pattern is not substantially different in Italian and Japanese subjects. In particular, the expected lower BP in the Italians was not detected despite their lower salt intake. Because the 24-hour mean BP value was seen not to be proportional to salt intake, the hypothesis is formulated that maintenance of the pressure regimen within a given range of variability is a principle of human physiology. To comply with this rule the Japanese people are supposed to have ethnically developed a certain resistance to dietary salt for which their cardiovascular apparatus is protected (phyletic escape to dietary sodium excess).
