102. Process-Oriented Design Methodologies Inspired by Tropical Plants.

Abstract

In light of the escalating climate crisis, there is a pressing need for a significant shift in how we design the built environment to effectively confront global challenges. Natural systems have inspired scientists, architects, and engineers for centuries; however, conventional biomimetic approaches often focus on superficial aspects, disregarding the underlying complexities. While this approach may lead to a more efficient outcome, it operates under the assumption that the organism functions exclusively within the confines of human knowledge, which are inherently limited by established epistemological and technological systems. This study advocates for a departure from conventional biomimetic approaches and asks the mechanisms of the biological system to inform the process of translation, as opposed to simply defining the outcome. By relinquishing control to material properties and dynamic processes of the biological analog, this study explores the generation of novel, bio-inspired dynamic formworks through non-linear fabrication processes. Specifically, it investigates the thermal properties of accessible building materials, enabling them to respond to environmental conditions without sophisticated technology or human intervention. By embracing chance and unpredictability, translated behaviors are granted the same influence as human intervention. Drawing inspiration from adaptive plant physiology, this research seeks to inspire innovative, climate-responsive methodological practices within broader architectural systems. Abstract

This exploratory study presents Greenland as a case of a peripheral destination that complicates and contradicts global definitions of sustainable tourism. Using empirical data that consists of 39 semi-structured interviews, the author employs an inductive approach to discuss the conceptualisation of sustainable tourism according to local stakeholders in Greenland. The key points of conflict surrounding sustainable tourism in Greenland are identified and discussed, with a focus on how local stakeholders contradict each other, and on how the debates prevalent at the local scale can inform tourism development in other peripheral places. The paper contributes to academic literature by offering a deeper understanding of how core-periphery dynamics can influence perceptions of and priorities for sustainable tourism in peripheral places. 104. The treatment of plantar warts using microwave—A review of 85 consecutive cases in the United States.

Abstract

Background: Plantar warts (verrucae plantaris) are a common source of pain for patients and are often refractory to treatment. Previous work has shown a high clearance rate of verrucae using a surface‐based microwave device (Swift®). Aims: To assess the efficacy, defined as the complete visible clearance of warts, in patients with verrucae plantaris receiving microwave treatment. Patients: We undertook a retrospective review and identified records of 85 patients who underwent a course of microwave treatment at a single US‐based podiatry centre. Efficacy was analyzed on the basis on intention‐to‐treat. Results: In patients who received ≥1 session there was a complete clearance rate of 60.0% (51/85) (intention‐to‐treat; 59 patients completed treatment, 26 lost to follow‐up) and 86.4% (51/59) per treatment completion; no significant differences in clearance rates of children and adults were observed (61.0% [25/41] vs. 59.1% [26/44]). There were 31 patients who received three sessions of microwave therapy with a clearance rate of 71.0% (22/31) as per intention‐to‐treat (27 patients completed treatment, 4 lost to follow‐up). An average of 2.3 sessions (SD: 1.1; range: 1–6) was required for the complete clearance of plantar warts. Complete clearance was also observed in some patients with recalcitrant warts following additional treatment sessions (42.9% [3/7]). A significant reduction in wart related pain was reported for all patients undergoing treatment. Some patients continued to report a reduced amount of pain post‐therapy compared with pretherapy. Abstract

Emergence and increase of terbinafine‐resistant dermatophytosis led to the identification of Trichophyton mentagrophytes internal transcriber space (ITS) genotype VIII in 2017, later renamed as Trichophyton indotineae and classified as a separate species in 2020. With its suspected origin in South Asia, this novel strain has emerged in Bahrain, Canada, Denmark, Finland, France, Germany, India, Iran, Japan, Russia, and Switzerland, with its spread attributed primarily to travel and migration. Diagnosis using routine mycology laboratory techniques is unable to distinguish T. indotineae from T. mentagrophytes and T. interdigitale; specific identification requires genomic sequencing to identify unique, specific markers. One speculated reason for this recent outbreak is the unrestricted use of topical steroid creams and antifungal agents. Patients with extensive tinea corporis and cruris due to T. indotineae present with inflammatory red plaques in multiple body sites. The majority of these infections prove to be resistant to conventional antifungals, including allylamines and azoles (itraconazole and fluconazole), thus emphasizing the need for antifungal susceptibility testing before treatment initiation and for reassessing in nonresponsive patients. Molecular studies have identified several point mutations in the ERG1 (terbinafine resistance) and ERG11 (azole resistance) genes, which need to be analyzed further. Use of relatively new agents, such as voriconazole and luliconazole, as well as device modalities and combination therapy, could be investigated for recalcitrant T. indotineae infections. Abstract

The three most commonly used methods for diagnosing non-dermatophyte mold (NDM) onychomycosis are culture, polymerase chain reaction (PCR), and histopathology. Toenail samples from 512 patients (1 sample/patient) with suspected onychomycosis were examined using all three diagnostic tests. A statistically significant association was found between PCR and histopathology results, as well as between fungal culture and histopathology results. All PCR-positive and culture-positive dermatophyte samples were confirmed by histopathology. However, 15/116 (12.9%) of culture-positive NDM samples had negative histopathology results, while all PCR-positive NDM samples were confirmed by histopathology. The overall rate of dermatophyte detection was higher using PCR compared to culture (38.9% vs. 11.7%); the lower rate of NDM detection by PCR (11.7% vs. 38.9%) could be attributed to the restriction of the assay design to seven pre-selected targets. When repeat sampling in the clinic is not possible, a combination of NDM detection by PCR and positive histopathology of hyphae may be a proxy for NDM infection, particularly where the NDM occurs without a concomitant dermatophyte. There was a high degree of correlation between negative PCR and negative histopathology. A negative PCR result with negative histopathology findings may be a reliable proxy for the diagnosis of non-fungal dystrophy. [ABSTRACT FROM AUTHOR]

Abstract

In this paper, we use game theory to understand decisions to cooperate or to compete in the delivery of public health services. Health care is a quasi-public good that is often associated with altruistic behavior, yet it operates in an increasingly competitive environment. With mounting health care regulation and changes in privatization, altruistic arguments give way to more competitive rationales for market decisions. Profit and not-for-profit institutions must address widespread health care needs while balancing the needs of more lucrative markets against the needs of lesser ones. Recognizing the roles of cooperation and competition as motivators in the delivery of health care to the public is imperative. We explore two game theory models (Nash's Equilibrium and the Prisoner's Dilemma) and their related concepts of simultaneous interdependence and rationality to examine decision-making. Four hypothetical public health case studies are presented. We conclude that understanding game theory and the factors influencing decision-making allows potential competitors to make more efficient decisions, including decisions to cooperate or compete. As public health agencies move toward more collaborative models of service delivery, such understanding may help enhance efficient and effective service delivery. (Contains 6 tables.)

Abstract

In this report we document work aimed at improving the quality of the representation of land and rivers in multi-year, coupled atmosphere-land (UM-JULES) simulations over the British Isles. The approach taken was to use standalone (uncoupled) simulations of JULES to investigate the potential to improve the coupled system. The use of alternative soil ancillary information, generated by using a data assimilation framework and observations of soil moisture from the COSMOS-UK network to optimise the constants in a pedotransfer function, was found to result in improved simulations by JULES of river flow in a diverse sample of British rivers (as measured by standard statistics). The revised soil parameters tended to increase the variability of the simulated river on short timescales, and reduce variability on the annual timescale. In catchments with a large influence of slow baseflow the revised parameters tended to give poorer simulations, with too much variability on short timescales. A new representation of groundwater processes was implemented in JULES and applied, for the first time, across Great Britain. This was shown to allow an influence of groundwater on nearsurface hydrology and fluxes over large parts of the country. Modelled river flows were more realistic in many cases, though much of the improvement was due to differences in the representation of runoff generation rather than the introduction of groundwater. A more physically-complete parameterisation of river physics, using the local inertial equation, was applied in a range of catchments, again for the first time. Simulated river flows were improved in most cases. The final section of this report offers a perspective on how terrestrial hydrology and its impacts could be considered in the next generation of UK Climate Projections. Abstract

We investigated the temporal profile of multiple components of the serological response after asymptomatic or mildly symptomatic SARS-CoV-2 infection, in a cohort of 67 previously SARS-CoV-2 naive young adults, up to 8.5 months after infection. We found a significant decrease of spike IgG and neutralization antibody titers from early (11 to 56 days) to late (4 to 8.5 months) time points postinfection. Over the study period, S1-specific IgG levels declined significantly faster than that of the S2-specific IgG. Further, serum antibodies from PCR-confirmed participants cross-recognized S2, but not S1, of the betacoronaviruses HKU1 and OC43, suggesting a greater degree of cross-reactivity of S2 among betacoronaviruses. Antibody-Dependent Natural Killer cell Activation (ADNKA) was detected at the early time point but significantly decreased at the late time point. Induction of serum Antibody-Dependent Monocyte Phagocytosis (ADMP) was detected in all the infected participants, and its levels remained stable over time. Additionally, a reduced percentage of participants had detectable neutralizing activity against the Beta (50%), Gamma (61 to 67%), and Delta (90 to 94%) variants, both early and late postinfection, compared to the ancestral strain (100%). Antibody binding to S1 and RBD of Beta, Gamma, Delta (1.7 to 2.3-fold decrease), and Omicron (10 to 16-fold decrease) variants was also significantly reduced compared to the ancestral SARS-CoV-2 strain. Overall, we found variable temporal profiles of specific components and functionality of the serological response to SARS-CoV-2 in young adults, which is characterized by lasting, but decreased, neutralizing activity and antibody binding to S1, stable ADMP activity, and relatively stable S2-specific IgG levels.

140. What Do We Know about Out-of-School Youths? How Participatory Action Research Can Work for Young Refugees in Camps

Abstract

This paper reports on an initiative that took the strategy of youth consultation in programme planning one step further by putting a research project's design, data collection, analysis and presentation of findings in the hands of young women and men who have experienced education and discontinuity of education in a long-term refugee camp. The participatory action research (PAR) process is described and assessed with attention to how PAR may serve as a practical, credible and ethical methodology for research with refugee youths about refugee youths. This case study reflects that PAR can yield new insights for developing youth-focused initiatives and positive personal experiences for youth participants, including limited forms of empowerment. Ultimately, however, the structural inequalities imposed by refugee status require redress if the goal is the long-term empowerment of youths in camps.

Abstract

The development of chemotherapies for glioblastoma is hindered by their limited bioavailability and toxicity on normal brain function. To overcome these limitations, we investigated the structure‐dependent activity of heptamethine cyanine dyes (HMCD), a group of tumour‐specific and BBB permeable near‐infrared fluorescent dyes, in both commercial (U87MG) and patient‐derived GBM cell lines. HMCD analogues with strongly ionisable sulphonic acid groups were not taken up by patient‐derived GBM cells, but were taken up by the U87MG cell line. HMCD uptake relies on a combination of transporter uptake through organic anion‐transporting polypeptides (OATPs) and endocytosis into GBM cells. The uptake of HMCDs was not affected by p‐glycoprotein efflux in GBM cells. Finally, we demonstrate structure‐dependent cytotoxic activity at high concentrations (EC50: 1–100 μM), likely due to mitochondrial damage‐induced apoptosis. An in vivo orthotopic glioblastoma model highlights tumour‐specific accumulation of our lead HMCD, MHI‐148, for up to 7 days following a single intraperitoneal injection. These studies suggest that strongly ionisable groups like sulphonic acids hamper the cellular uptake of HMCDs in patient‐derived GBM cell lines, highlighting cell line‐specific differences in HMCD uptake. We envisage these findings will help in the design and structural modifications of HMCDs for drug‐delivery applications for glioblastoma. [ABSTRACT FROM AUTHOR]
