Abstract

The aim of this study was to attain long-lasting alpha-sarcoglycan gene expression in limb-girdle muscular dystrophy, type 2D (LGMD2D) subjects mediated by adeno-associated virus (AAV) gene transfer under control of a muscle specific promoter (tMCK).rAAV1.tMCK.hSGCA (3.25 × 10¹¹ vector genomes) was delivered to the extensor digitorum brevis muscle of 3 subjects with documented SGCA mutations via a double-blind, randomized, placebo controlled trial. Control sides received saline. The blind was not broken until the study was completed at 6 months and all results were reported to the oversight committee.Persistent alpha-sarcoglycan gene expression was achieved for 6 months in 2 of 3 LGMD2D subjects. Markers for muscle fiber transduction other than alpha-sarcoglycan included expression of major histocompatibility complex I, increase in muscle fiber size, and restoration of the full sarcoglycan complex. Mononuclear inflammatory cells recruited to the site of gene transfer appeared to undergo programmed cell death, demonstrated by terminal deoxynucleotide transferase-mediated deoxyuridine triphosphate nick-end labeling and caspase-3 staining. A patient failing gene transfer demonstrated an early rise in neutralizing antibody titers and T-cell immunity to AAV, validated by enzyme-linked immunospot on the second day after gene injection. This was in clear distinction to other participants with satisfactory gene expression.The findings of this gene replacement study in LGMD2D subjects have important implications not previously demonstrated in muscular dystrophy. Long-term, sustainable gene expression of alpha-sarcoglycan was observed following gene transfer mediated by AAV. The merit of a muscle-specific tMCK promoter, not previously used in a clinical trial, was evident, and the potential for reversal of disease was displayed.

Abstract

"MicroRNAs (miRNAs) are small molecules of noncoding RNA that range between ~19-22 nucleotides in length. In recent years, scientists have observed that these small RNA molecules exist in the extracellular environment within eukaryotic organisms. Furthermore, these microRNA molecules are known to associate with a family of proteins named Argonaute proteins. These microRNA/Argonaute protein complexes are the core of a larger assembly of proteins that compose the RNA Induced Silencing Complex (RISC). The RISC has exhibited the ability to inhibit the translation of, or cleave, its target messenger RNA (mRNA), the latter of which being exclusive only to Argonaute 2 protein (Ago2). It has also been observed in the literature that these Argonaute/miRNA complexes often target genomic regions associated with various cancers in humans and, currently, more than 2000 miRNAs have been discovered and published in the literature. Current methods for analyzing miRNA expression involve total RNA extraction using methods such as ethanol precipitation. However, total RNA extraction does not take into consideration that the major functional component of post-transcriptional inhibition is indeed that Argonaute protein/miRNA complex and not the miRNA alone. In this study, we investigate a novel method to capture and detect the active Ago2/miRNA (miRNP) complex and quantitate associated miRNAs by utilizing an antibody against Ago2 and subsequent application of real-time PCR to successfully capture the active miRNP complex and quantitate the associated miRNAs."--Abstract from author supplied metadata.

Abstract

Diagnostic medical sonography is a leading and favored imaging technique in pediatrics. However, diagnostic medical sonography education does not include a standardized specific curriculum in general pediatric sonography. This mixed-methods study aims to (1) measure pediatric sonographers’ self-assessed knowledge, skill, and competence immediately after graduation or cross-training from radiologic technology as compared with their current levels; (2) gain insights from these sonographers of potential topics/techniques necessary to improve students’ general pediatric sonography preparation; and (3) determine their opinions about a specialized registry examination for pediatric sonography. Twenty-two sonographers completed the self-assessment survey, and 11 sonographers participated in focus groups. Experienced general pediatric sonographers reported a significant increase in self-assessed knowledge and skill. The focus groups showed five consistent themes. Sonographers agreed that development of a general pediatric sonography course concentrating on learning general pediatric-related disease processes and protocols would enhance knowledge, skill, and competence immediately after graduation or cross-training; however, opinions were mixed about adding a general pediatric sonography registry examination. 142. Limited value of plain radiographs in infant torticollis

Abstract

OBJECTIVE File purpose of this work was to assess the frequency of clinically relevant findings from plain films of infants evaluated for torticollis. PATIENTS AND METHODS. After institutional review board approval, radiology records were searched for infants 0 to 12 months of age who underwent plain film study for torticollis or "head tilt." Infants evaluated for trauma or Down syndrome were excluded. All of the studies were reviewed, demographic data was recorded, and any additional imaging studies were examined. RESULTS. A total of 502 patients (189 girls and 313 boys) were identified with an average age of 0.37 [+ or -] 0.2 years. Head tilt was to the left in two thirds of patients. Ten patients had abnormal findings reported. Six of these proved normal on subsequent studies (3 suspected occipital-C1 fusions, 2 suspected cervical fusions, and 1 suspected hemivertebra). Four patients had true bony vertebral abnormalities including absent left C7 pedicle, multiple fusion anomalies from C4 to T2, C3 hemivertebra and thoracic spine anomalies, and C4 hypoplasia. This last patient bad abnormal kyphosis on physical examination and demonstrated instability with dynamic testing. Twenty-five additional patients with normal plain films underwent spine computed tomography or magnetic resonance imaging; all were normal. CONCLUSIONS. The true-positive yield of plain films in nontraumatic infant torticollis was low (4 of 502). There were more false-positive than true-positive results. A common rationale for imaging is to exclude craniocervical or other unstable abnormalities that might contraindicate physical therapy, seen in only 1 of the 502 cases. Abstract

Vomiting is a commonly reported symptom in infants less than three months of age. There are a multitude of pathologies to consider, both within and outside the gastrointestinal tract. In addition to conducting a thorough history and physical examination, a clinician formulates a reasonable differential diagnosis by consideration of two main factors: the infant's age and the characterization of the vomit as bilious or nonbilious. In this endeavor, the clinician is able to determine if an imaging study is needed and, if so, the urgency of the request. A review of the appropriate imaging evaluation of vomiting infants in the newborn to three-month-old age group is provided by organizing the discussion around the following three clinical scenarios: bilious vomiting, intermittent nonbilious vomiting since birth, and new-onset bilious vomiting. The ACR Appropriateness Criteria are evidence-based guidelines for specific clinical conditions that are reviewed every three years by a multidisciplinary expert panel. 1 he guideline development and review include an extensive analysis of current medical literature from peer-reviewed journals and the application of a well-established consensus methodology (modified Delphi) to rate the appropriateness of imaging and treatment procedures by the panel.
