The United Nations Convention on the Rights of the Child (CRC) may be a leading example of psychological jurisprudence. By its emphasis on promotion of the "child's sense of dignity and worth," the CRC offers a framework for comprehensive child policy in a manner that is consistent with the promotion of mental health and prevention of mental health problems.

Background: Although research suggests that witnessed violence is linked to adverse mental health outcomes among adolescents, little is known about its prevalence or its significance in predicting psychiatric symptoms beyond the contribution of co-occurring risk factors. The purpose of this study was to identify the national prevalence of witnessed parental and community violence and to examine these life stressors as independent risk factors for posttraumatic stress disorder (PTSD) and major depressive episode (MDE) among adolescents. A secondary aim was to determine which characteristics of witnessed violence were associated with mental health outcomes., Method: Participants were 3,614 adolescents recruited from a 2005 US national household probability sample who completed structured telephone interviews assessing witnessed violence and Diagnostic and Statistical Manual of Mental Disorders-IV (DSM-IV) criteria for PTSD and MDE., Results: National prevalence of witnessed parental violence and witnessed community violence was estimated to be 9% and 38%, respectively. Both forms of witnessed violence predicted PTSD and MDE beyond variance accounted for by age, gender, race/ethnicity, income, and other traumatic event history. Perceptions of threat, repeated violence exposure, location of the violence, and relationship to the victim were associated with psychiatric diagnoses., Conclusions: Findings suggest that witnessed violence represents a significant public health burden with implications for psychological assessment and prevention efforts.

This paper reports findings of an integrative review of the literature on spirituality in AA breast cancer survivors, isolates key spiritual themes, and recommends future research. Inclusion criteria are 1994 to 2004 research studies that included AA breast cancer survivors 18 years old and older. Content analysis was used to isolate spiritual themes and spiritual domains/dimensions. Seven studies resulted that used qualitative, quantitative, and mixed methods. Themes identified were spirituality provided the strength to cope; the need to care for others and receive care; beliefs that God is the healer and in control; God assists in decision-making; and closeness to God. Spiritual domains were beliefs, functions, and social support. Future research should explore the domains/dimensions and meanings of spirituality experienced by diverse groups ofAA breast cancer survivors. Culturally appropriate, evidence-based nursing care should include spiritually based interventions that acknowledge the significance of God.

The authors' purpose in the current study was to apply P. J. Lang's (1968, 1971, 1985) theory of multiple systems of emotional response to the study of subjective sleepiness. A total of 274 participants completed the Stanford Sleepiness Scale (SSS), the Profile of Mood States (POMS), three sleepiness-related Visual Analogue Scales (VAS), and the Epworth Sleepiness Scale (ESS). The authors completed correlations and factor analysis of the 4 sleepiness measures. The SSS, POMS, and VAS measures were better correlated with each other than with the ESS. Similarly, the SSS, POMS, and VAS measures loaded highly onto one factor, whereas the ESS loaded highly onto a separate factor. These results indicated that the ESS measured a different aspect of subjective sleepiness than the SSS, POMS, or VAS. According to Lang's emotional responses theory, the ESS assessed a behavioral component of sleepiness, and the SSS, POMS, and VAS measures assessed an internal state related to sleepiness.

Lower extremity lesions are the primary cause of hospitalization for people with diabetes, resulting in enormous personal and financial costs. This study used a survey designed to describe the characteristics and foot-care behaviors of people with diabetes who attended a clinical nurse specialist managed foot-care clinic. Forty-eight patients who received care at the participating foot-care clinic completed a 21-item multiple-choice questionnaire designed to determine the presence of foot pathology and foot-care behaviors. Most of the patients were between 65 and 74 years of age, had concurrent illnesses, and had four or more primary care visits per year. Although 69% had existing foot pathology, only 44% reported inspecting their feet daily and only 54% reported that their primary care provider examined their feet on each visit. Twenty-five percent reported going barefoot sometimes and eight percent would either treat a foot lesion themselves or wait for it to get better.

The realist approach that has dominated American jurisprudence has long had especially great acceptance in regard to children's issues. Ironically, however, decision making on such topics has seemed to be particularly unlikely to be informed by careful assessments of social reality. Symbolism has prevailed over pragmatism. Psycholegal research on children's issues has also often been misdirected. Application of the Convention on the Rights of the Child may point the way to more psychologically minded children's law.

Whether we like it or not, genetic testing will almost certainly become routine medical practice within the next 25 years. Integrated circuit chips already exist that can perform 400 genetic tests simultaneously, thus greatly reducing the costs. At least one company is already working on a prototype for a handheld genetic tester that would allow primary care physicians to perform hundreds or thousands of genetic tests on a simple blood smear in just a few minutes. "Genetic report cards" for children are not very far off at all. The use of such widespread testing poses a variety of ethical dilemmas. One problem that has not been appreciated sufficiently, however, is the question of how to interpret the test results. Because of the ways the genes implicated in diseases are discovered and marketed, quantitative analysis of the tests can be extremely misleading. The difficulty is that we simply do not have sufficient information about variance in genetic and other factors in the general population to make accurate projections of a patient's risk, given the presence of a gene. This uncertainty is obscured, however, when we provide the patient with a numerical analysis of risk because it is well established that people tend to overestimate the information content of numerical projections. This situation is made far worse by the fact that we do not have enough adequately trained genetic counselors to handle the load that will soon be placed on them (and studies have shown that physicians are generally very poorly prepared to act as accurate sources of information on complex genetic issues). For these reasons, I argue that access to genetic testing should be treated the same way as access to new medical procedures and medications--namely, withheld from the general public until proven safe and effective in large-scale trials. This is certain to be an unpopular policy, but it seems the only way to prevent a great deal of abuse of genetic tests.

This study expands on the normative data available for the newly revised Beck Depression Inventory-II. Data from both an outpatient, counseling-center sample (n = 152: 106 women, 46 men) and a classroom sample (n = 152: 79 women, 65 men) of college students are presented, including sex differences on the inventory. Means and standard deviations of individual items and total scores are reported, along with statistical tests of differences between groups. Comparisons of these data with norms reported by the authors of the inventory (Beck, Steer, & Brown, 1996) are offered. The lack of sex differences found in comparison of total scores for both samples is presented. Implications for the use of the inventory with college populations are discussed.

An integration of the Machiavellianism (MACH) and psychopathy constructs based on a dimensional view of personality and personality disorders and a recognition of B. Karpman's (1941, 1948) conceptual distinction between primary and secondary psychopathy is presented. Positive associations between MACH and both primary and secondary psychopathy were found. It is concluded that the Mach-IV is a global measure of psychopathy in noninstitutionalized populations (i.e., one that assesses but confounds both primary and secondary psychopathy) and that the primary differences between MACH and psychopathy are not traceable to substantive theoretical issues but to the different professional affiliations they are associated with: personality and social psychology and clinical psychology, respectively.

The information technology environment is here to stay. Computers are the devices being used to manage the extension of care out of hospitals and into the community. Nurses are the focal point of clinical documentation because they access patient data most often. Nurses know what information is needed and seek ways to collect that information. Nurses can dictate the type of data collected and how that data is presented to allow the most appropriate, well-informed decisions possible. Nurses have the knowledge to make decisions, and with information management skills, we will have the means to communicate that knowledge to others.

The present study investigated potential sex differences in preferences for spicy, hot or unusual foods and in food aversions or neophobia. This study also reexamined the issue of a gender difference in preference for sweet foods. Questionnaires concerning past and current food use and preferences as well as food and condiment use in one actual meal were completed by 148 people between 17 and 32 years of age. Their responses clearly support the prediction that men tend to have a stronger preference than women for spicy, hot foods. The results also support the prediction that men are more likely than women to seek unusual and new foods. Both sexes showed the same degree of preference for condiment use.

Previous research has identified Emotional Separation and Personal Control as two sets of home leaving indicators that are associated, respectively with relatively troubled and relatively untroubled parent-adolescent separation. As expected, among late adolescent males, parental divorce was highly related to endorsement of Emotional Separation as an important composite indicator of having left home. For females, being a firstborn was associated with endorsing Personal Control as an important composite indicator of home-leaving. Findings were interpreted as supporting previous research which has indicated that males experience more adjustment problems after parental divorce than females.
