Abstract

Can the conscious patient in the midst of a medical emergency provide adequate informed consent for a clinical research protocol? Adequate consent is crucial to the ethical conduct of clinical trials, including those performed in emergency settings. We examine the problem of emergency informed consent. As an illustrative case, we discuss a pilot trial of prehospital thrombolytic therapy for myocardial infarction. Federal regulations for clinical research do not provide clear guidelines on emergency research in the conscious patient. Clinical investigators currently approach emergency consent in four ways: (1) avoid such research, (2) omit the consent process, (3) obtain deferred consent, or (4) obtain customary consent. We suggest a fifth alternative, two-step consent, which permits the conduct of emergency research while protecting the rights of the emergency research subjects. Such a process may serve as an alternative solution for future studies faced with the problem of informed consent in emergencies. [ABSTRACT FROM AUTHOR]

111. Effect of direct current countershock on atrial and ventricular electrophysiological properties and myocardial potassium efflux in the thoracotomised dog1.

Abstract

We reinvestigated the effect of direct current countershock (DCS) on atrial and ventricular electrophysiological properties and myocardial K+ release in the open-chested dog. This model allowed direct application of the electroshock paddles to the heart permitting reproducible energy input, simultaneous and comparative epicardial recordings for the measurement of conduction times (CT) and current threshold in different tissues, the determination of monophasic action potential duration (MAPD) with suction electrodes, and the measurement of coronary sinus K+ concentration [K+]cs. DCS produced changes in atrial and ventricular CT, in excitability, and in atrioventricular conduction (AVCT) even in the presence of cholinergic and (β-adrenergic receptor blockade. The type of electrophysiological response and its temporal relationship to [K+]cs after DCS suggest the DCS-induced release of myocardial K+ to play a role both in the antidysrhythmic effect of DCS and in the dysrhythmias following DCS. These effects were quite distinguishable both electrophysiologically and temporally from those of acetylcholine and catecholamines. More specifically, acetylcholine's effect on sinus rate, atrial APD, AVCT, and perhaps atrial excitability persisted for less than 10 s. The effect of catecholamines on automaticity and ectopic activity had a similar time course. The alterations in CT and excitability (often biphasic) attributable to the transient K+ elevation induced by DCS persisted for a half minute or longer. It seems DCS may have 2 antidysrhythmic phases: the first being immediate involving synchronisation of the non-refractory myocardium and the suppression of automaticity; the latter being delayed and dependent on transient local hyperkalemia. Abstract

[The following fragments were found among the papers of the late R. C. Childers. When his last illness overtook him, he was known to have been engaged for some time on a comprehensive handbook of the Pali language. Before, however, he had far proceeded with the elaboration of the greater work, he proposed to bring out an elementary grammar first, to meet the pressing want of intending students of that language. After his death, it was hoped that some other Pali scholar would be found willing to complete the work in either form from the materials collected by Mr. Childers: and with this view these were submitted to Prof. Pischel at Kiel. But as this gentleman has since expressed his intention of publishing a grammar on a plan of his own, it is thought that the following two fragments may, even as such, be of interest to Pali students. Each of them treats of the Sandhi rules, and is complete in itself. But while the former, intended for the shorter work, was left ready for the press, the latter, intended for the larger, had not yet received the benefit of the gifted author's revision.—R. 114. Art. IV.—Notes on the Sinhalese Language. No. II. Proofs of the Sanskritic Origin of Sinhalese.

Abstract

Since I published my first Note on the Sinhalese Language, I have heard that two eminent Orientalists dispute the truth of my conclusions, and still hold to the old view, originating with Rask, that Sinhalese belongs to the Dravidian group of languages. It is true that in my paper I dealt in any detail only with one grammatical form: but if, as I venture to think, I have proved beyond doubt that the Sinhalese neuter plural is formed by compounding the noun with a vocable val, which is the Sanskrit vana used in the sense of multitude, I have gone far to demonstrate the Sanskritic origin of Sinhalese. But in addition to this I have adduced a great number of Sinhalese nouns, all of which I have traced direct to their Sanskrit equivalents. And it gives me great pleasure to find that so eminent a scholar as Dr. Max Müller has recently expressed himself clearly and unmistakably in favour of my Aryan theory. I am, however, perfectly willing to make allowance for scholars who decline to accept my theory without evidence of a more extended character than I have yet given in its favour, and I propose to continue the publication of these Notes until I have overcome the scepticism of my opponents, and established on the widest grammatical basis the Sanskritic origin of the Sinhalese language. Instead of dealing in detail with a single grammatical form, I propose in the present Note to touch briefly upon several different points, and I may as well say at once that it is not part of my plan in these Notes to show that Sinhalese is not Dravidian. This has been already done by Mr. James D'Alwis, who, in his paper “On the Origin of the Sinhalese Language” (Journal Ceylon Br. E.A.S. 1867–70), has shown that, whether we compare the vocabulary or the grammar, we find absolutely no resemblance between Sinhalese and the Dravidian languages. To me this course appears superfluous, and my own plan will be to let alone the Dravidian languages, and show that Sinhalese is Sanskritic. 115. Art. III.—Notes on the Sinhalese Language No. 1.— On the Formation of the Plural of Neuter Nouns.

Abstract

The Sinhalese is one of the Aryan vernaculars of India, and is spoken by the descendants of a people who migrated from Magadha to Ceylon at a very remote period. The tradition recorded in Mahavansa is that Ceylon was colonised, by a prince of Lala, a district of Magadha, who landed in the island with seven hundred followers on the day of Gautama Buddha's death. Accepting this tradition, and comparing it with the tradition that Pali was a Magadha dialect, we should expect to find a close resemblance between Pali and Sinhalese. And such in fact is the case. With a few exceptions, Sinhalese follows Pali so closely that at first sight one might feel inclined to say that the former was derived from the latter. As a general rule, where Pali differs from the other Prakrits, the Sinhalese agrees with it; and this is the case not only with words but with grammatical forms. And there are several words not found in the other Prakrits or Sanskrit which are found in both Pali and Sinhalese. I have alluded to exceptions, and these deserve full consideration; but they are such as may be explained by the circumstance, to which the Buddhist traditions clearly point, that the language of Buddha's sermons was the dialect of one district of Magadha, and the language spoken by the colonisers of Ceylon that of another district. As an instance of these exceptions, I may mention the Sinhalese itiri “woman,” which clearly cannot have come to us through the Pali itthi, since the latter has lost the original r of the Sanskrit . Abstract

Khuddaka Páṭha, or “Lesser Readings,” is one of the books of the Buddhist Scriptures. It is the first of the fifteen divisions of Khuddaka Nikáya, and immediately precedes Dhammapada, The text which I have adopted is that of a manuscript written and collated for me by a Singhalese priest of great learning. I have compared with it the Burmese manuscript belonging to the collection presented by Sir A. Phayre to the India Office Library. Abstract

Using the ARGUS detector at the DORIS II e/sup +/e/sup -/ storage ring at DESY, we have obtained evidence for a new charmed resonance which decays into D/sup asteriskplus-or-minus/(2010)..pi../sup - +/. The observed mass and width are 2420 +- 6 MeV/c/sup 2/ and 70 +- 21 MeV/c/sup 2/, respectively. The fragmentation function is found to be hard, as expected for a state containing a leading charm quark produced by nonresonant e/sup +/e/sup -/ annihilation.

122. Left bundle branch block and right axis deviation: A report of 36 cases

Abstract

Right axis deviation (RAD) with left bundle branch block (LBBB) is a rare combination. From a database of 636,000 electrocardiograms we report a series of 36 patients with this disorder. The majority of subjects had dilated cardiomyopathy with biventricular enlargement. LBBB was fixed in 21 of 36 cases. It was freshly acquired, episodic, intermittent, or physiologic in 15 of 36. The RAD was episodic in 30 of 36; it was fixed and concurrent with LBBB in only 2 cases, and never episodically concurrent. Reported for the first time here were 4 of 36 cases in which the combination of LBBB and RAD was elicited with atrial premature impulses as a rare form of QRS aberration. In one case where the combination was intermittent, a clear relationship with freshly acquired intermitent posterior fascicular block was demonstrated. The possible relationship of the deviation with variable degrees of right ventricular overload is discussed.

Abstract

Using the ARGUS detector at thee+e− storage ring DORIS II at DESY, we have observed the decaysD+→K−K0π+π+ andDs+→K−K0π+π+. The branching ratios were determined to be BR(D+→K−K0π+π+)=(1.0±0.5±0.3)% and BR(Ds+→K−K0π+π+)/BR(Ds+→φπ+)=1.2±0.2±0.2. These decays are found to proceed mostly via the " decay channel, with BR(D+→ ")=2.6±0.8±0.7)% and BR(Ds+→ ")/BR(Ds+→φπ+)=1.5±0.4±0.4.

Abstract

The reactionγγ→π+π−π+π− has been studied with the ARGUS detector. The rate in the invariant mass region below 1.8 GeV/c2 is found to be largely due toρ0ρ0 production. A spin-parity analysis shows a dominance of the partial wave (JP,Jz)=(2+, 2) with a small admixture fromJP=0+. The contribution of negative parity states is consistent with zero. The large ratio of cross sectionsσ(γγ→ρ0ρ0)/σ(γγ→ρ+ρ−)≃4, and the dominance of theJP=2+ wave in the reactionγγ→ρ0ρ0 is a signature consistent with the production of an exotic (I=2) resonance.

Abstract: We report on a high statistics study of π0 and η production in continuum events and in direct decays of the Γ(1S) and Γ(2S) resonances. The measured production rates per event are $$\left\langle {n_{\pi ^0 } } \right\rangle$$ =3.22 0.07 0.31 (3.97 0.23 0.38) and 〈n η〉=0.19 0.04 0.04 (0.40 0.14 0.09) for continuum events (direct Γ(1S) decays).

Abstract

Two-photon production of the exclusive final statesp "+nπ (n=0, 1, 2, and 3) has been investigated using the ARGUS detector at thee+e− storage ring DORIS II at DESY. The reactionsγγ→p "π andγγ→p "π+π−π0 have been observed for the first time, as have theΔ++ and " baryons in the final statep "π+π−. No evidence was found forΔ++ " production. Topological cross sections for two-photon production ofp ",p "π0,p "π+π− andp "π+π−π0, as well as the crosssection forγγ→Δ++ "π+π−+c.c., have been measured. Upper limits are given for the cross section forγγ→Δ0 ",γγ→Δ++ " andγγ→Λ ".

Abstract

The production cross sections for the ?, S0, ?-, S0 (1385), ?0 (1530) and O- hyperons have been measured, both in the continuum and in direct ? decays. Baryon rates in direct ? decays are enhanced by a factor of 2.5 or more compared to the continuum. Such a large baryon enhancement cannot be explained by standard fragmentation models. The strangeness suppression for baryons and mesons turns out to be the same. A strong suppression of spin 3/2 states is observed.

Abstract

The production ofDS+ mesons inB meson decays, and inq " continuum events, has been studied with the ARGUS detector at thee+e− storage ring DORIS II. In addition to the measurement of inclusiveDS+ production in γ(4S)→B " decays, all eight two-body decay modesB→DS(*)D(*) have been measured with branching ratios between 1% and 3%. By comparing our inclusive and exclusive results to predictions of heavy quark effective theory, a value of (267±28) MeV × [2.7%/BR(Ds+→φπ+)]1/2 is obtained for the weak decay constant fDS(*), averaged overDS+ andDS*+ mesons.

Abstract

We have measured theR value in non-resonante+e- annihilation using the ARGUS detector at the storage ring DORIS II. At a centre-of-mass energy " GeV the ratio of the hadronic cross-section to the µ-pair cross section in lowest order QED has been determined to beR=3.46±0.03±0.13. In addition, we have measured the charged-particle multiplicities in non-resonant hadron production at " GeV just below theB " threshold and in ? (4S) resonance decays. For the average charged-particle multiplicities in continuum events and ?(4S)?B " decays we obtain cont=8.35±0.02±0.20 and ?(4s)=10.81±0.05±0.23.

Abstract: Using the ARGUS detector at thee + e − storage ring DORIS II, we have measured branching ratios for the Cabibbo-suppressed decaysD 0→π-π+,D 0→K − K +, andD 0→K s0 K -π+, observed the Cabibbo-allowed decayD 0→K s0 K s0 and obtained an upper limit on the branching ratio for $$D^0 \to K^0 \bar K^0$$ .

Abstract

Using the ARGUS detector at thee+e- storage ring DORIS II, we have investigated inclusive production of p±,K±,Ks0 and " in multihadron events at 9.98 GeV and in direct decays of the ?(1S) meson, i.e. from quark and gluon fragmentation. The most pronounced difference is the rate of baryon production. The Lund Monte Carlo program gives a reasonable qualitative description, although it cannot reproduce our data in detail.

Abstract

Using the ARGUS detector at DORIS II, we have determined the branching ratios for the decays τ−→K*−vτ and τ−→ρ−vτ to be (1.23 ±0.21−0.21+0.11)% and (21.5±0.4±1.9)%, respectively. These results are in agreement with theoretical expectations based on the standard model.

Abstract

Using the ARGUS detector at the DORIS II e+e−storage ring at DESY, we have obtained evidence for a new charmed-strange meson which decays into D*+K0. Its mass is observed to be 2535.9±0.6±2.0 MeV/c2and its width to be less than 4.6 MeV/c2at the 90% confidence level. No structure is observed at this mass in the D+K0invariant mass spectrum, which suggests that an unnatural spin-parity is preferred.

Abstract

We report on the first observation of Δ(1232)++and Δ(1232)++¯baryons in e+e−annihilation at energies around 10 GeV, using the ARGUS detector at DORIS II. The sum of the rates of Δ++and Δ++¯per hadronic event in the continuum is measured to be 0.040±0.008±0.006, and the rate in direct ϒ(1S) decays is 0.124±0.016±0.015. The momentum spectrum of Δ++baryons in direct ϒ(1S) decays has been measured.

Abstract

The production of charged kaon pairs in two-photon interactions has been studied with the ARGUS detector and the topological cross section has been measured. The ??-widths and interference parameters have been determined for the tensor mesonsf2 (1270),a2 (1318) andf'2 (1525). The helicity structure assumed for the continuum contribution has a significant effect on the result. Upper limits have been obtained for the ??-widths of the glueball candidate statesf2 (1720) andX (2230).

140. Prospective evaluation of a microprocessor-assisted cardiac rhythm algorithm

Abstract

A new microprocessor-assisted cardiac rhythm algorithm (MAC-RHYTHM), based on median QRST subtraction from 10-second electrocardiographic (ECG) data, was prospectively tested on 10,761 ECGs acquired at one of three clinical centers. Atrial waves (P waves, atrial fibrillatory waves, and flutter waves) were detected from the median QRST-subtracted residual signals. Rhythm criteria were applied to the detected atrial waves and their temporal relation to QRS complexes for generating rhythm interpretations. Rhythm statements generated by MAC-RHYTHM were compared against the true rhythm of the ECGs as read by an experienced cardiologist. The results of prospective testing were compared with the results of an earlier retrospective testing using MAC-RHYTHM and a released commercial ECG analysis program on stored ECGs. The prospective results were very similar to the results of MAC-RHYTHM on retrospective data for all the rhythms examined (sinus rhythms, atrial fibrillation, atrial flutter, junctional rhythms, second degree atrioventricular blocks). For three of the abnormal rhythms, namely, atrial fibrillation, junctional rhythms, and second degree atrioventricular blocks, MAC-RHYTHM gave significantly higher sensitivity in both prospective (87.5%, 92.2%, and 80.8%, respectively) and retrospective (82.0%, 81.2%, and 79.6% respectively) testing than the released commercial ECG analysis program (65.0%, 39.6%, and 12.0% respectively). Similarly, for sinus rhythms, MAC-RHYTHM had significantly higher specificity (prospective, 91.0% and retrospective, 91.7%) than the released commercial program (86.5%). The specificity for the abnormal rhythms remained very high with MAC-RHYTHM (prospective, 99.4% to 99.7% and retrospective, 99.1% to 99.7%) compared to the released commercial program (99.0% to 99.9%). This prospective study also indicated that further work is needed to improve the detection of pacemaker spikes and the interpretation of paced rhythms in 10-second resting ECGs.

The decay " has been investigated using data accumulated by the ARGUS experiment, which operates at around 10 GeV centre-of-mass energies The decay " is a signature for flavor annihilation byW-exchange in weak decays. The observed correlations exclude the production of point-like diquark-antidiquark pairs as the dominant source of baryons. Information concerning angular momentum compensation follows from the observedcorrelation.

Abstract

Normal and atherosclerotic aortas from rats and man have been analyzed by neutron activation to determine changes in the content of Ca, Cd, Co, Cu, Mg, Mn, Se, V, and Zn with the development of degenerative changes. Samples of aortic tissue were weighed, dried at 110°C overnight, and irradiated for 30 min in a thermal neutron flux of 1.8X1012n/cm2-sec in a TRIGA Mark I reactor. Immediately after irradiation, the samples were wet-ashed in the presence of carriers, the various elements were then radiochemically separated, and the separated fractions were counted by multichannel gamma-ray spectrometry. Definite values, or firm upper limits, for the elements were calculated by computer. With increasing degree of atherosclerosis, the Ca content rose from <350 to 36 000±2000 ppm; Co from undetectable to 0.34 ppm; Mg from <110 to 880 ppm; Mn from 0.34 to 1.26 ppm; and Zn from 37 to 152 ppm. Significant changes could not be detected in Cd, Se, and V; changes in Cu were not consistent. These results are discussed in relation to the pathological changes and to zinc therapy in the treatment of atherosclerosis.

15 pp. (1988)., The production of η-mesons in τ-decays has been studied with the ARGUS detector at the DORIS II storage ring. τ+τ− events were selected by their characteristic 1-versus-3 charged particle topology. The π+π− invariant mass distribution was used to search for evidence of η production in τ-decays. No signal was observed, leading to upper limits at 95% CL of Br(τ−→ηπ−π0vτ)

Abstract

6 pp. (1987)., The final state K$^+K^− \pi^+\pi ^-$ has been studied in γγ interactions using the ARGUS detector at the e+e− storage ring DORIS II at DESY. Production of the vector meson pair K$^{∗0}$(892)K$^{∗0}$(892) is observed for the first time. The cross section for K$^+K^− \pi^+\pi^-$, K$^∗0K^−\pi^+$c.c. and K$^{∗0}$K$^{∗0}$ are all found to be of the order of a few nb. In the W$_{\gamma\gamma}$ range accessible, a mean upper limit of 0.5 n$_b$ at 95% CL is derived for φ$\rho^0$ production.
