The experience of math anxiety can have detrimental effects on students’ math performance, and researchers have in recent years tried to design interventions aiming at reducing math anxiety. This meta-analysis aimed to examine the effectiveness of math anxiety interventions in reducing math anxiety and improving math performance. The meta-analysis comprised 50 studies and included 75 effect sizes. On average, the effect sizes were moderate (g = -0.467) for reducing math anxiety and improving math performance (g = 0.502). Interventions that focused on Cognitive support or regulating Emotions were effective both in reducing math anxiety and improving math performance. In addition, longer interventions and interventions targeting students older than 12 had the biggest decrease in math anxiety. Study quality was not related to intervention outcomes. Mortality in Slovakia has long been characterized by significant regional differences. One of the important factors of these differences is the influence of the quality and scope of the provided health care. We can express quality itself indirectly through the concept of avoidable mortality. Within it, deaths from lung, colorectal and breast tumors have an important position. A detailed analysis of the level of mortality, spatial differences for these groups of causes of death in Slovakia has not yet been carried out. In addition, in the contribution, we also try to identify, through specialized mortality tables and the concept of years of life lost, how these deaths shorten life expectancy and what losses affect human resources. The obtained results confirmed the persistence of regional differences, where the worst situation is primarily in the north and south of Slovakia. Moreover, these differences do not change significantly over time. This indirectly signals not only the regionally differentiated quality of care provided, but also the problems associated with prevention and personal responsibility for health. A Systematic Literature Review on The Fishermen's Knowledge of Integrating Fishing and Tourism Activities [ABSTRACT FROM AUTHOR]

The formation of disinfection byproducts (DBPs) in finished drinking water is an ongoing challenge for public health agencies and water utilities. The Fourth Unregulated Contaminant Monitoring Rule data were used to assess the prevalence and drivers of haloacetic acids (HAAs)—a class of DBPs—in New York State's (NYS) public water systems, with a focus on total measured (HAA9), regulated (HAA5), brominated (HAA6Br), and unregulated (HAA4) HAAs. The concentrations of all HAA groups in NYS are found to be similar to those nationally, with HAA4 composing approximately 20% of HAA9. Concentrations of all HAA groups are lowest in groundwater and highest in surface waters across NYS systems. Higher total organic carbon (TOC) concentrations lead to elevated HAA9 and HAA5, while higher bromide concentrations favor more HAA4 and HAA6Br. HAA4 concentrations are well predicted with pre‐oxidation/disinfection types, HAA5, TOC, and bromide concentrations, with an adjusted R2 of 70%. [ABSTRACT FROM AUTHOR]

Patients in randomized controlled trials (RCTs) must be successfully randomized to reduce or eliminate bias. Because pre-treatment symptoms have prognostic significance in cancer patients, qualitative and quantitative tools were developed to assess similarity of baseline pre-treatment symptoms across different treatment arms of RCTs as one measure of randomization success. Clinician-reported symptom data from two colorectal cancer RCTs, CO.20 and CO.17, were used to demonstrate the utility of a qualitative visualization tool and quantitative machine learning K-means tool, which grouped patients into clusters using baseline symptoms. Qualitatively, reflection bar graphs (RBGs) visualized potential imbalances in baseline symptoms (i) across treatment arms and (ii) by corresponding patient clusters identified within each treatment arm. RBGs found that the treatment arms for both RCTs had similar symptom profiles, while the lack of significant differences in the proportions of patients in each cluster across treatment arms further confirmed successful randomization. This paper details the creation of visualization, machine-learning, and statistical tools to compare baseline symptoms across RCT treatment arms, demonstrating that the CO.20 and CO.17 trials were successfully randomized by baseline symptoms and are comparable. In this article, we review prevention of serious adverse clinical outcomes and treatment side effects in patients with neuromuscular disorders including myopathies and myasthenia gravis. While neither of these entities is preventable, their course can often be modified, and severe sequelae may be prevented, with the identification of risk factors and proactive attention toward treatment planning. The recognition of pain and the treatments used for it are vital for all practitioners. Many types of pain can be treated in a locoregional fashion, which has significant implications not just for any individual patient but for society as a whole. These treatments are most effective when performed in a minimally invasive, image-guided fashion. Interventional radiologists should play a central role in providing these lifestyle-limiting treatments. Background: Executive function will gradually decline with the increase of age, which will have a negative impact on the quality of life and general health. Exercise intervention can improve executive function and prevent its deterioration, but the evidence from randomized controlled trials is not consistent. Aim: To assess the effect of exercise intervention on executive function of healthy middle-aged and elderly people, and briefly describe its mechanism. Methods: A search was conducted using PubMed, Web of science and EBSCO. The searches were limited to English articles published from January 2010 to January 2022. The information is extracted from searched articles included or excluded based on certain criteria. Results: The search returned 2,746 records, of which 11 articles were included in the systematic review, and 8 articles were supplemented according to the references included and related reviews. The results show that different types of exercise intervention have positive effects on the executive function of the middle-aged and elderly people. The intervention prescriptions inmost studies are executed in medium to low frequency, medium intensity and medium duration, while only 11% of the studies were followed up. Conclusions: The intervention, which was executed twice a week with 30-60min single intervention time and over 12 weeks total duration, showed a good intervention effect. Exercise intervention is to improve executive function by affecting the activation of brain network and the synthesis of neurotransmitters. [ABSTRACT FROM AUTHOR]

Functional electrical stimulation (FES) is widely used in neurorehabilitation to improve patients' motion ability. It has been verified to promote neural remodeling and relearning, during which FES has to produce an accurate movement to obtain a good efficacy. Therefore, many studies have focused on the relationship between FES parameters and the generated movements. However, most of the relationships have been established in static contractions, which leads to an unsatisfactory result when applied to dynamic conditions. Therefore, this study proposed a FES control strategy based on the surface electromyography (sEMG) and kinematic information during dynamic contractions. The pulse width (PW) of FES was determined by a direct transfer function (DTF) with sEMG features and joint angles as the input. The DTF was established by combing the polynomial transfer functions of sEMG and joint torque and the polynomial transfer functions of joint torque and FES. Moreover, the PW of two FES channels was set based on the muscle synergy ratio obtained through sEMG. A total of six healthy right-handed subjects were recruited in this experiment to verify the validity of the strategy. The PW of FES applied to the left arm was evaluated based on the sEMG of the right extensor carpi radialis (ECR) and the right wrist angle. The coefficient of determination (R 2) and the normalized root mean square error (NRMSE) of FES-included and voluntary wrist angles and torques were used to verify the performance of the strategy. The result showed that this study achieved a high accuracy (R 2 = 0.965 and NRMSE = 0.047) of joint angle and a good accuracy (R 2 = 0.701 and NRMSE = 0.241) of joint torque reproduction during dynamic movements. Moreover, the DTF in real-time FES system also had a nice performance of joint angle fitting (R 2 = 0.940 and NRMSE = 0.071) and joint torque fitting (R 2 = 0.607 and NRMSE = 0.303). It is concluded that the proposed strategy is able to generate proper FES parameters based on sEMG and kinematic information for dynamic movement reproduction and can be used in a real-time FES system combined with bilateral movements for better rehabilitation. The aim of this systematic review is to examine the effects of different anesthetic drugs and protocols on hepatic functions. The liver is a very important organ since it is directly involved in biotransformation of the drugs used in anesthesia. Comprehensive understanding of anesthetic drugs and their effects on hepatic functions remains fundamental to a successful anesthesia. Understanding the connections that exist between anesthetic drugs and liver function remains essential for a safe anesthesia both during the surgery and in the post-operative period. Almost all anesthetic drugs depend on hepatic biotransformation into their metabolites that can be transported and excreted by the kidneys. If the liver enzymes are altered, the biotransformation of the drugs will be decreased and the recovery from the anesthesia prolonged. [ABSTRACT FROM AUTHOR]

Although histories of cognitive behaviour therapy have begun to appear, their use with people with psychosis diagnoses has received relatively little attention. In this article, we elucidate the conditions of possibility for the emergence of cognitive behaviour therapy for psychosis (CBTp) in England between 1982 and 2002. We present an analysis of policy documents, research publications and books, participant observation, and interviews with a group of leading researchers and senior policy actors. Informed by Derksen and Beaulieu's articulation of social technologies, we show how CBTp was developed and stabilised through the work of a variety of overlapping informal, academic, clinical, professional, and policy networks. The profession of clinical psychology played a key role in this development, successfully challenging the traditional 'division of labour' where psychologists focused on 'neurosis' and left 'psychosis' to psychiatry. Following Abbott's systems approach to professions, we identify a number of historical factors that created a jurisdictional vulnerability for psychiatry while strengthening the jurisdictional legitimacy of clinical psychology in providing psychological therapies to service users with psychosis diagnoses. The National Institute for Health and Clinical Excellence played a significant role in adjudicating jurisdictional legitimacy, and its 2002 schizophrenia guidelines, recommending the use of psychological therapies, marked a radical departure from the psychiatric consensus. Our analysis may be of wider interest in its focus on social technologies in a context of jurisdictional contestation. A meta-analysis is substantial for integrating the findings of all related studies into one consistent research to establish a knowledge bank of a common issue. Although several studies have examined the impact of the individual aspect of human capital on farmers' technical efficiency, the composite impact was not synthesized. Therefore, our analysis examines the key determinants driving systematic variations in technical efficiency estimates from 268 food-crop farming studies published recently in peer-reviewed journals. Our results have supported our hypothesis that human capital increases farming efficiency. The studies' location indicates the importance of farmer beliefs, trust in their institutions and land accumulation to increase food-crop farmer technical efficiency. Our findings contribute to the applied agricultural economics literature by theoretically systematizing literature on human capital in accordance with social and natural capitals in agricultural productivity and empirically validating the technical efficiency in food-crop farming studies toward developing agricultural sustainability in harmony with our biosphere. Aim: To assess the beneficial and adverse effects on the dental and periodontal issues of periodontal-orthodontic treatment of teeth with pathological tooth flaring, drifting, and elongation in patients with severe periodontitis. Materials and methods: Nine databases were searched in April 2020 for randomized/non-randomized clinical studies. After duplicate study selection, data extraction, and risk-of-bias assessment, random-effect meta-analyses of mean differences (MDs) and their 95% confidence intervals (CIs) were performed, followed by subgroup/meta-regression analyses. Results: A total of 30 randomized and non-randomized clinical studies including 914 patients (29.7% male; mean age 43.4 years) were identified. Orthodontic treatment of pathologically migrated teeth was associated with clinical attachment gain (-0.24 mm; seven studies), pocket probing depth reduction (-0.23 mm; seven studies), marginal bone gain (-0.36 mm; seven studies), and papilla height gain (-1.42 mm; two studies) without considerable adverse effects, while patient sex, gingival phenotype, baseline disease severity, interval between periodontal and orthodontic treatment, and orthodontic treatment duration affected the results. Greater marginal bone level gains were seen by additional circumferential fiberotomy (two studies; MD = -0.98 mm; 95% CI = -1.87 to -0.10 mm; p = .03), but the quality of evidence was low. Conclusions: Limited evidence of poor quality indicates that orthodontic treatment might be associated with small improvements of periodontal parameters, which do not seem to affect prognosis, but more research is needed. Background: Mindfulness meditation has become a common method for reducing stress, stress-related psychopathology and some physical symptoms. As mindfulness programs become ubiquitous, concerns have been raised about their unknown potential for harm. We estimate multiple indices of harm following Mindfulness-Based Stress Reduction (MBSR) on two primary outcomes: global psychological and physical symptoms. In secondary analyses, we estimate multiple indices of harm on anxiety and depressive symptoms, discomfort in interpersonal relations, paranoid ideation and psychoticism. Methods: Intent-to-treat analyses with multiple imputations for missing data were used on pre- and post-test data from a large, observational dataset (n = 2155) of community health clinic MBSR classes and from MBSR (n = 156) and waitlist control (n = 118) participants from three randomized controlled trials conducted contemporaneous to community classes in the same city by the same health clinic MBSR teachers. We estimate the change in symptoms, proportion of participants with increased symptoms, proportion of participants reporting greater than a 35% increase in symptoms, and for global psychological symptoms, clinically significant harm. Results: We find no evidence that MBSR leads to higher rates of harm relative to waitlist control on any primary or secondary outcome. On many indices of harm across multiple outcomes, community MBSR was significantly preventative of harm. Conclusions: Engagement in MBSR is not predictive of increased rates of harm relative to no treatment. Rather, MBSR may be protective against multiple indices of harm. Socio‐economic inequalities in amenable mortality rates are increasing across Europe, which is an affront to universal healthcare systems where the numbers of, and inequalities in, amenable deaths should be minimal and declining over time. However, the fundamental causes theory proposes that inequalities in health will be largest across preventable causes, where unequally distributed resources can be used to gain an advantage. Information on individual‐level inequalities that may better reflect the fundamental causes remains limited. We used the Scottish Longitudinal Study, with follow‐up to 2010 to examine trends in amenable mortality by a range of socio‐economic position measures. Large inequalities were found for all measures of socio‐economic position and were lowest for educational attainment, higher for social class and highest for social connection. To reduce inequalities, amenable mortality needs to be interpreted both as an indicator of healthcare quality and as a reflection of the unequal distribution of socio‐economic resources. Intractable, chronic abdominal pain from upper abdominal malignant and benign diseases is a significant challenge for healthcare providers and burden on the healthcare system. While opioid analgesics are commonly used to provide pain relief, the adverse effects of chronic opioid use cannot be overlooked. Celiac plexus neurolysis via chemical or thermal means represents an alternative minimally invasive approach to provide palliative pain relief and increase patients' quality of life. Through the use of computed tomography guidance, celiac plexus neurolysis can be performed by accurately targeting the celiac plexus, while minimizing risks to adjacent structures. Historically, celiac plexus neurolysis was performed via instillation of ethanol or phenol; however, within the past decade cryoablation has gained increasing use with potentially fewer side effects. The article considers the main sources of pollutionin the territory of the Republic of North Ossetia–Alania. A study of environmental geophysical factors in the city of Vladikavkaz was carried out at 126 points; indicators of noise pollution, electric fields and the level of gamma radiation were measured. A geoaccumulation index of heavy metals in soils and indices of carcinogenic and non-carcinogenic risks were calculated and corresponding maps were constructed. The obtained data supporting a high level of carcinogenic risk are consistent with a high level of cancer morbidity in the city, which indicates a close relationship between morbidity and the carcinogenic risk index. It has been determined that emissions from road transport are greater by an order of magnitude than stationary sources emissions, while there is a steady trend towards an increase in air pollution as a result of the increasing negative impact of motor vehicle emissions. It has been established that the most hazardous way for heavy metals to enter the human body from the soil is by inhalation. It has been determined that in areas where environmental pollution with heavy metals is higher, cancer morbidity is also higher. [ABSTRACT FROM AUTHOR]

ehaviour change is a key to addressing many health and healthcare problems and interventions have been designed to improve health outcomes. These behaviour change interventions have been evaluated in many ways, including randomised controlled trials, and over recent decades there has been considerable progress in the conduct and reporting these studies. This paper is a personal retrospection on the changes occurring that have resulted in our current improved methods and their potential for future advancement. There has been steady development of methods for conducting trials, including advances in statistical methods enabled by increase computing power and programmes, greater attention to the recruitment of participants and in the specification of outcomes. Trial reporting has improved, largely due to publication of guidelines for reporting interventions and trials, but until recently the reporting of behaviour change interventions has been quite limited. Developments in the specification of active ingredients of these interventions, the behaviour change techniques, has transformed our ability to report interventions in a manner that facilitates evidence synthesis and enables replication and implementation. However, further work using ontological approaches is needed to adequately represent the evidence contained in the mass of accumulated studies. Meanwhile, attention is gradually being paid to the comparator groups in trials leading to better reporting but with continuing challenges about how control groups are selected. These developments are important for the advancements of behavioural science – but also in consolidating the expertise needed to address global social, environmental and health challenges. Introduction: In recent years, a number of clinical trials have been published comparing transcutaneous electrical nerve stimulation (TENS) and traditional speech therapy treatment of voice and swallowing functions, but results have been conflicting. Objective: Assess the methodological quality of studies and determine whether TENS is an efficient therapeutic strategy for speech therapy treatment of healthy adults or those with dysphonia and/or dysphagia. Methods: The databases used were Medical Literature Analysis and Retrieval System Online (MedLine), Biblioteca Virtual em Saúde (BVS), Cochrane Library and Web of Science (ISI Web of Knowledge). The study was conducted between May 2018 and January 2019, in line with Cochrane Handbook guidelines, and included studies on the use of TENS in healthy adults or those with compromised voice and/or swallowing function. Results: After the search and extraction of studies, the following were identified: TENS + VOICE: 7 articles; TENS + SWALLOWING: 5 articles. The studies exhibited medium quality and are heterogeneous, making it difficult to determine their effectiveness and the parameters to be used in future research. There were no statistically significant differences between the use of TENS alone or associated with another therapeutic technique for voice. For swallowing function, one study proved better results in cases of associated techniques – TENS + traditional therapy. Discussion: Speech therapy should increase the number of studies published and improve their methodological quality, reassessing methodological criteria. Current clinical practice is not grounded in evidence‐based science. Clinical Message: the studies analyzed exhibited medium methodological quality;there are variations in the time, number and periodicity of the therapeutic sessions for TENS;there were no statistically significant differences between the use of TENS alone or associated with another therapeutic technique in voice;there were statistically significant differences between the use of TENS associated with traditional therapeutic in swallowing function. Whether gamification is an organized structure that contributes to student achievement, a simple pontification process or total nonsense is a matter of debate. In such, this study was conducted to provide a scientific answer while exhibiting the gamification effect on student achievement with the meta-analysis method, which is based on experimental research that investigates the effect of gamification on student achievement between 2010 and 2016. As a result of the completed meta-analysis, a common effect size value that was estimated by compiling experimental studies was found to be 0.557. This value shows that gamification has a moderately positive (Cohen, 1988) effect on student achievement. It is exhibited that there is no publication bias regarding determining the effect's validity. Calculated 0.557 Hedges' g value's omega square equivalent is.072. This value shows that gamification has added 7.2% positive value to academic achievement according to 45 experimental results involving 3,487 students from different countries. Background: Since the 1990s, there have been a lot of research on single-nucleotide polymorphism (SNP) and different diseases, including many studies on 5,10-methylenetetrahydrofolate reductase (MTHFR) polymorphism and essential hypertension (EH). Nevertheless, their conclusions were controversial. So far, six previous meta-analyses discussed the internal relationship between the MTHFR polymorphism and EH, respectively. However, they did not evaluate the credibility of the positive associations. To build on previous meta-analyses, we updated the literature by including previously included papers as well as nine new articles, improved the inclusion criteria by also considering the quality of the papers, and applied new statistical techniques to assess the observed associations. Objectives: This study aims to explore the degree of risk correlation between two MTHFR polymorphisms and EH. Methods: PubMed, EMBASE, the Cochrane Library, CNKI, and Wan Fang electronic databases were searched to identify relevant studies. We evaluated the relation between the MTHFR C677T (rs1801133) and A1298C (rs1801131) polymorphisms and EH by calculating the odds ratios (OR) as well as 95% confidence intervals (CI). Here we used subgroup analysis, sensitivity analysis, cumulative meta-analysis, assessment of publication bias, meta-regression meta, False-positive report probability (FPRP), Bayesian false discovery probability (BFDP), and Venice criterion. Results: Overall, harboring the variant of MTHFR C677T was associated with an increased risk of EH in the overall populations, East Asians, Southeast Asians, South Asians, Caucasians/Europeans, and Africans. After the sensitivity analysis, positive results were found only in the overall population (TT vs. CC: OR = 1.14, 95% CI: 1.00–1.30, P h = 0.032, I 2 = 39.8%; TT + TC vs. CC: OR = 1.15, 95% CI: 1.01–1.29, P h = 0.040, I 2 = 38.1%; T vs. C: OR = 1.14, 95% CI: 1.04–1.25, P h = 0.005, I 2 = 50.2%) and Asian population (TC vs. CC: OR = 1.14, 95% CI: 1.01–1.28, P h = 0.265, I 2 = 16.8%; TT + TC vs. CC: OR = 1.17, 95% CI: 1.04–1.30, P h = 0.105, I 2 = 32.9%; T vs. C: OR = 1.10, 95% CI: 1.02–1.19, P h = 0.018, I 2 = 48.6%). However, after further statistical assessment by FPRP, BFDP, and Venice criteria, the positive associations reported here could be deemed to be false-positives and present only weak evidence for a causal relationship. In addition, when we performed pooled analysis and sensitivity analysis on MTHFR A1298C ; all the results were negative. Conclusion: The positive relationships between MTHFR C677T and A1298C polymorphisms with the susceptibility to present with hypertension were not robust enough to withstand statistical interrogation by FPRP, BFDP, and Venice criteria. Evaluation of toxicity induced in laboratory animals is paramount to the screening step in the assessment of the safety profile of all compounds. 3-(2,5-dimethoxyphenyl)-1-(5-methyl furan-2-yl) prop-2-en-1-one (DMPF-1); synthetic chalcone derivative has been investigated due to its pharmacological properties, including its antinociceptive and anti-inflammatory. However, the safety profile of this compound is yet to be determined. The present study seeks to highlight the toxicity effect of this compound using acute and subacute toxicity studies in the mice model. A single highest dose (1000 mg/kg) and repeated dose (0.1, 0.5, 1, 5 and 10 mg/kg) of DMPF-1 supplementation were executed. All toxicity study performed was supported by behavioural and body weight changes, haematological, serum biochemical analysis, macroscopic and microscopic analysis of the vital organs. The present result simplifies that DMPF-1 compound supplementation for four weeks is non-toxic as it caused no significant alteration in mice body weight and behaviour. Besides, no significant changes in haematological and biochemical parameters were observed. Further evaluation of its safety profile was confirmed by the normal architecture of the tissues organs obtained. Collectively, this report showed that DMPF-1 was safe to be consumed. Objective: To assess the survival, failure, and complication rates of veneered and monolithic all‐ceramic implant‐supported single crowns (SCs). Methods: Literature search was conducted in Medline (PubMed), Embase, and Cochrane Central Register of Controlled Trials until September 2020 for randomized, prospective, and retrospective clinical trials with follow‐up time of at least 1 year, evaluating the outcome of veneered and/or monolithic all‐ceramic SCs supported by titanium dental implants. Survival and complication rates were analyzed using robust Poisson's regression models. Results: Forty‐nine RCTs and prospective studies reporting on 57 material cohorts were included. Meta‐analysis of the included studies indicated an estimated 3‐year survival rate of veneered‐reinforced glass‐ceramic implant‐supported SCs of 97.6% (95% CI: 87.0%–99.6%). The estimated 3‐year survival rates were 97.0% (95% CI: 94.0%–98.5%) for monolithic‐reinforced glass‐ceramic implant SCs, 96.9% (95% CI: 93.4%–98.6%) for veneered densely sintered alumina SCs, 96.3% (95% CI: 93.9%–97.7%) for veneered zirconia SCs, 96.1% (95% CI: 93.4%–97.8%) for monolithic zirconia SCs and only 36.3% (95% CI: 0.04%–87.7%) for resin‐matrix‐ceramic (RMC) SCs. With the exception of RMC SCs (p < 0.0001), the differences in survival rates between the materials did not reach statistical significance. Veneered SCs showed significantly (p = 0.017) higher annual ceramic chipping rates (1.65%) compared with monolithic SCs (0.39%). The location of the SCs, anterior vs. posterior, did not influence survival and chipping rates. Conclusions: With the exception of RMC SCs, veneered and monolithic implant‐supported ceramic SCs showed favorable short‐term survival and complication rates. Significantly higher rates for ceramic chipping, however, were reported for veneered compared with monolithic ceramic SCs. [ABSTRACT FROM AUTHOR]

This article analyses special issues, also referred to as thematic issues, monographic issues, or supplements, a mode of scientific journal publishing that has rarely been studied. It presents a bibliometric analysis of the production of journals in the field of communication in the period 2015–2019 and studies the impact of the publication of special issues in this field. The sample analysed includes 21,458 articles and reviews, 524 special issues, 418 publishers, and 94 journals. The study considers the presence of special issues in communication journals and the distribution and number of papers published in them and compares the impact of papers published in this mode with papers published in regular issues. The results reveal that 19% of articles and reviews published in the period studied appeared in special issues, which generally contain the same number of articles as regular issues. Moreover, 75% of journals achieved a higher average impact factor with articles published in special issues than they did with articles in regular issues. It is concluded that the publication of special issues appears to offer the potential benefits of attracting submissions, increasing profits, and improving impact, although it also has disadvantages. Background The reported initial strong treatment effects reported in early trials that are refuted in subsequent future studies assessing the same interventions have been attributed to novelty bias. The aim of this study was to determine whether there is any evidence of novelty bias in the reported treatment effects of orthodontics interventions. Materials and methods Relevant orthodontic systematic review (SRs) topics containing at least one meta-analysis on either binary or continuous outcomes with a minimum of three trials considered important areas in the field of orthodontic practice were identified. SR, meta-analysis, and primary study-level characteristics were extracted. Descriptive statistics were calculated at the SRs, meta-analysis, and at the individual study level. All SR and trial-level data were imported into the statistical software and all meta-analyses were replicated using the cumulative random-effects meta-analysis approach. Changes in the size and direction of the estimates between the first trial and the cumulative effect over time were recorded. Results Forty-seven meta-analyses were included. The total number of primary studies included within these meta-analyses was 408 (N = 408). Overall, the final effect size estimate decreased in 29 (61.7%, N = 29/47) cumulative meta-analyses whilst it increased in the remaining 18 (38.3%, N = 18/47). No association between the level of risk of bias and the cumulative absolute effect size was evident (OR 1.00; 95% CI: 0.98, 1.03; P = 0.717) after adjusting for year of the primary study (P = 0.22). Conclusions Clinicians should be wary of the results of trials reporting the effectiveness of new interventions as there is a possibility that the reported effect size will be often exaggerated. Twenty-one reports of 14 randomized control trials (RCTs) published between 1968 and 1984 comparing methods of periodontal treatment were evaluated using a system to assess the quality of RCTs based an published reports. The quality assessment system evaluates the design, implementation, and analytic methods used. An index of quality is calculated with a maximum score attainable of 1.00. The mean quality score for these 14 RCTs was 0.35±0.07 (ISD). There was no evidence of improvement in scores over lime. Ninety-five percent of the trials used the `split-mouth' protocol. Although the results of 17 reports claimed no statistically significant differences between treatments, in no case was a type II error calculated to ensure that a sample size sufficient to detect a difference of clinical interest had been used. The criteria and scoring method for a system to evaluate the quality of randomized control trials (RCTs) in dental research based on published reports is presented. This system is based on one devised for evaluation of RCTs in medicine. Items assessed in this system include randomization and blinding procedures, subject selection criteria, treatment protocols, and statistical analyses. Assessing the quality of RCTs can contribute to improved study design, implementation and reporting by investigators, and evaluation of reports by referees and editors of scientific journals.
