Abstract

This article provides current guidelines on the treatment and prevention of childhood obesity. Since factors involved in obesity change with age, the therapeutic approach in pre-school children will be different from pupils and adolescents. The treatment will also be modulated on the basis of weight excess, weight gain velocity and complications. The main goal of the treatment should be to encourage the child and his family to have healthy lifestyle. Families who are not ready for change might benefit from counselling to improve motivation before starting treatment. A detailed alimentary and behavioural history is the start point of the treatment. The strategy of the intervention is to induce changes at three levels: 1) attitudes of parents; 2) physical activity; 3) energy intake. The treatment of the adolescents should take into account the pubertal changes and the psychological aspects of this peculiar period of life. Obesity is a chronic disease and its treatment needs long-life follow-up. The long-term results of the obesity treatment are often disappointing and we have to consider consistent prevention programs for better results.

Abstract

Cognitive theory conceptualizes worry as influenced by metacognitive beliefs about worry, intolerance of uncertainty, and perceptions of control over events and reactions. This study tests the hypothesis that the effect of intolerance of uncertainty would interact with meta-cognitive beliefs on worry and perceived control. One hundred eighteen individuals with generalized anxiety disorder and 54 controls completed the Meta-Cognition Questionnaire, the Intolerance of Uncertainty Scale, the Anxiety Control Scale, and the Penn State Worry Questionnaire. Models were tested measuring interactive effects in multiple regression linear analysis. The interaction model was confirmed. The effect of intolerance of uncertainty on worry was increased by its interaction with metacognitive and control beliefs. Abstract

Introduction: CR4056 is a novel imidazoline-2 receptor ligand endowed with low affinity MAO inhibition properties. Objectives: Aims of this study was to test with different methods the anti-nociceptive effect of CR4056 in rat models of neuropathic pain, i.e. Bortezomib(Btz)-induced neuropathic pain, streptozotocin-induced diabetes and chronic constriction injury (CCI). Methods: In the Btz model (0.20mg/kg 3qwx8 iv) and in the experimental diabetes model CR4056 was administered at 6-60mg/kg po. In the CCI mononeuropathy model CR4056 was administered at 10-80 mg/kg po. Nerve conduction velocity (NCV), dynamic, Randall-Selitto, plantar and von Frey tests were used to assess the effect of CR4056. Results: After 8 weeks of treatment, Btz induced a severe reduction in NCV and this impairment was not reversed by CR4056. The behavioral assessment showed a significant mechanical allodynia in rats treated wit Btz for 8 wks and the daily administration of CR4056 significantly reduced this condition. This effect was persistent along the entire period of administration after Btz withdrawal (2 weeks). In the models of streptozotocin-induced diabetes and CCI, treatment with CR4056 significantly attenuated mechanical and thermal hyperalgesia, while CR4056 had no effect on mechanical allodynia in both experimental models. Conclusions: These results promote the use of CR4056 as a potential treatment in the chronic neuropathic pain therapy.

125. Three dimensions of the survival curve: Horizontalization, verticalization, and longevity extension

Abstract

Three dimensions of the survival curve have been developed: (1) "horizontalization," which corresponds to how long a cohort and how many survivors can live before aging-related deaths significantly decrease the proportion of survivors; (2) "verticalization," which corresponds to how concentrated aging-related (''normal'') deaths are around the modal age at death (M); and (3) ``longevity extension, `` which corresponds to how far the highest normal life durations can exceed M. Our study shows that the degree of horizontalization increased relatively less than the degree of verticalization in Hong Kong from 1976 to 2001. After age normalization, the highest normal life durations moved closer to M, implying that the increase in human longevity is meeting some resistance.

"This study analyses provincial differences in adult mortality (separately for men and women, aged 30-64 years) in Italy between 1984 [and] 1987.... By applying the fuzzy clumping method to mortality rates by cause...groups of provinces can be identified where mortality profiles (irrespective of the intensity) are not only similar for all or only for some of the mortality causes considered, but also specify the grade of similarity common to the provinces in the same group. This method, whereby any single province can belong to several groups, provides an image of the geography of mortality where homogeneous groups are not separated by fixed boundaries but may actually have provinces in common." 149. Athlete's heart in women. Echocardiographic characterization of highly trained elite female athletes.

Abstract

Unlabelled: OBJECTIVES; To define the expression of "athlete's heart" in women by determining the alterations in cardiac dimensions associated with long-term intense conditioning in elite female athletes. DESIGN; Prospective cardiovascular assessment conducted from 1986 through 1993. Subjects were evaluated using 2-dimensional, M-mode, and Doppler echo-cardiographic studies.Setting: Institute of Sports Science, Italian National Olympic Committee, Rome, Italy.Participants: A total of 600 elite female athletes (mean age, 21 years; range, 12-49 years) who had participated in vigorous training (mean duration, 9 years; range, 2-32 years) and had competed in 27 sports, including 211 athletes at the international level and 389 at the national level. A control group consisted of 65 sedentary volunteer women (mean age, 23.7 years; range, 14-41 years) who were free of cardiovascular disease and who did not participate in regular athletic training.Main Outcome Measures: Left ventricular end-diastolic cavity dimension and wall thickness.Results: Athletes demonstrated larger left ventricular end-diastolic cavity dimension (mean +/- SD) (49 +/- 4 mm) and greater maximal wall thickness (8.2 +/- 0.9 mm) than controls (46 +/- 3 mm and 7.2 +/- 0.6 mm; P < .001). These dimensions were 6% and 14% larger in athletes. Among athletes, left ventricular cavity dimension was 40 mm to 66 mm, exceeded normal limits ( > 54 mm) in 47 women (8%), and was within the range consistent with primary dilated cardiomyopathy ( > or = 60 mm) in 4 athletes (1%). Training for endurance sports, such as cycling, cross-country skiing, and rowing had the greatest effect on cavity dimension. Left ventricular wall thickness was 6 mm to 12 mm in athletes and did not exceed normal limits or extend into the borderline gray zone with hypertrophic cardiomyopathy in any subject. Compared with data from 738 previously studied male athletes, female athletes showed significantly smaller left ventricular cavity dimension (11% less; P < .001) and wall thickness (23% less; P < .001).Conclusions: Highly trained women athletes frequently demonstrate cardiac dimensional changes as an adaptation to physical training, although absolute left ventricular cavity size exceeding normal limits was evident in a minority (8%) of women athletes and was rarely (1% of athletes) within the range of dilated cardiomyopathy. Athletic training was not a stimulus for substantial increases in absolute left ventricular wall thickness, which was within normal limits for all women athletes. These findings suggest that the clinical differentiation of athlete's heart and hypertrophic cardiomyopathy appears to be a diagnostic dilemma that is limited to male athletes.
