The prediction of the dynamic response of Unreinforced Masonry Structures (URMS) is a very complex task, since it is governed by material degradation and cyclic hysteric behaviour. Procedures based on nonlinear static analyses have been proposed for the seismic assessment of URMS, without properly considering hysteretic energy dissipation during the dynamic response. Even though dynamic nonlinear analyses provide satisfactory simulations of the seismic response, its application requires considerable computational effort and high user expertise for the accurate definition of the material properties, making it unsuitable for practical applications. However, simplified macro-element strategies, capable of simulating in-plane and outof-plane nonlinear responses, could represent a satisfactory engineering solution in the dynamic context. In this paper, a simple and practitioners-friendly calibration strategy to consistently link target panel-scale mechanical properties (that can be found in national standards) to model material-scale mechanical properties is presented. Simple masonry panel geometries, with various boundary conditions, are utilized to test numerical models and calibrate their mechanical properties. The calibration is successfully conducted through five different numerical models (most of them available in commercial software packages) suitable for nonlinear modelling of masonry structures, using nonlinear static analyses. Firstly, the panel stiffness calibration is performed, focusing the attention to the shear stiffness. Secondly, the panel strength calibration is conducted for several axial load ratios by attempts using as reference the target panel strength deduced by well-known analytical strength criteria. The results in terms of panel strength for the five different models show that this calibration strategy appears effective in obtaining model properties coherent with Italian National Standard and Eurocode. Open issues remain for the calibration of the post-peak response of masonry panels, which still appears highly conventional in the standards.

Endometriosis is a common gynecologic disorder characterized by ectopic endometrial tissue growth outside the uterine cavity. Although usually occurring in pelvic organs, endometrial lesions may involve urinary tract. Renal endometriosis is extremely rare and it has only occasionally been reported in the past. We report two cases of patients with renal cystic lesions, incidentally found at imaging techniques during oncologic follow-up for gastric sarcoma and melanoma, initially misinterpreted as complicated haemorrhagic cysts and then histologically characterized as renal localizations of extragenital endometriosis.

BACKGROUND: Patients with systemic sclerosis (SSc) often complain reduced capacity at submaximal exercise; conversely physical capacity in performing daily duties has never been measured in SSc. OBJECTIVES: To evaluate this performance and its correlates, in patients with SSc compared to healthy controls, in a free-living setting. METHODS: Twenty-seven outpatients with stable SSc and 11 controls were recruited. Physical activity was assessed by portable multiple sensor device (SenseWear Armband) worn for at least six days. Physical activity duration (PAD; in minutes) for non-sedentary activities and physical activity level (PAL= total daily energy/resting energy expenditure) per day were calculated. Nutritional status was estimated by bioelectrical impedance analysis, and pulmonary arterial hyperthension excluded by echocardiography. RESULTS: Daily physical activities (243+/-145 minutes per day versus 397+/-142 min, respectively; p=0.005) and PAL were significantly reduced in SSc compared to controls (1.5+/-0.4 vs 2+/-0.7, respectively; p=0.019). 74% of SSc patients showed PAL

Background and Aims. An ageing lung is characterised by distal airspace enlargement without alveolar wall destruction: therefore the anatomical distinction between senile lung and emphysema is clear-cut. In clinical settings the definition of precise boundaries between normalcy and pathology is more difficult with the risk of overdiagnosis. CT is an important diagnostic advancement in the field of COPD. Most methods for the evaluation of emphysema are based on the detection and measurement of areas characterised by a density level below a threshold assumed to characterize parenchymal destruction. Methods. Our retrospective study included 47 healthy subjects (65-91 years), 36 never smokers and 11 former smokers. As a reference sample we recruited 9 patients with emphysema (69-81 years). Thoracic scan was performed by single slice spiral CT and acquired without contrast enhancement. For each scan and on both lungs we sampled eighteen regions of interest in the upper, middle and lower field. Mean lung density (MLD) and lower limit of normal (LLN) of density distribution were calculated. Results. MLD for the whole study sample was -846±41 HU. -901 HU was the LLN of density distribution in the study sample. No significant correlation was noted between age and MLD. A system for low-energy photon spectrometry using a planar germanium detector with appropriate specifications is presented. A spectrometric background investigation has been carried out with various detector shielding arrangements. The characteristics of the system for measurements of 210Pb in air-particulate matter on filters have been determined.

Materiali e Metodi: La nostra popolazione è costituita da 25 di età compresa tra 50 e 81 anni (media 63.95 +/- 9.54) sottoposti ad esame RM della prostata con apparecchiatura da 1.5 T mediante l'ausilio di bobina endorettale. Oltre al protocollo morfologico (T1w-T2w) , la valutazione spettroscopica e dinamica post-contrastografica, sono state acquisite sequenze DWI sul piano assiale con valori di b pari a 800/mm2. E' stato calcolato il valore di ADC in regioni di interesse poste in corrispondenza di aree di zona periferica (ZP) con aspetto normale ed aree che presentavano segnale ipointenso enne sequenze T2w e/o che mostravano restrizione della diffusione dell'acqua nelle sequenze DWI. All'esame istologico, ottenuto tramite biopsia e/o prostatectomia radicale, la diagnosiè stata di carcinoma prostatico (CaP) in 18 pazienti, di prostatite+PIN in 4 pazienti e di fibrosi post-radioterapia (RT) in 3 pazienti. RISULTATI. Le medie di valori ADC sono risultate 1.57+/-0.28 per la ZP normale, 0.72 +/- 0.11 per il CaP, 0.97 +/- 0.07 per la prostatite+PIN, 0.89 +/- 0.13 per l fibrosi post-RT. Introduction: Decreased airway distensibility (AD) in response to deep inspirations, as assessed by HRCT, has been associated with the severity of asthma and COPD. Aims: The current study was designed to compare the magnitude of AD by HRCT in individuals with asthma and COPD with comparable degrees of bronchial obstruction, and to explore factors that may influence it. Results: We enrolled a total of 12 asthmatics (M/F:7/5) and 8 COPD (7/1) with comparable degree of bronchial obstruction (FEV1% predicted mean±SEM: 69.1 ± 5.2% and 61.2 ± 5.0%, respectively; p = 0.31). Each subject underwent chest HRCT at FRC and at TLC. A total of 701 airways (range 20 to 38 airway per subject; 2.0 to 23.1 mm in diameter) were analyzed. AD did not differ between asthmatics and COPD (mean ± SEM: 14 ± 3.5% and 17 ± 4.3%, respectively; p = 0.58). In asthmatics, AD was significantly associated with FEV1% predicted (r2 = 0.45, p = 0.018). We found a significant correlation between the change in lung volume and the change in AD by HRCT (r2 = 0.64, p = 0.002). In COPD, we found significant correlations between AD and the RV% predicted (r2 = 0.51, p = 0.046) and the RV/TLC (r2 = 0.68, p = 0.01). Conclusions: AD was primarily affected by the dynamic ability to change lung volumes in asthmatics, and by static lung volumes in COPD. © 2013 Informa Healthcare USA, Inc.

Introduction. Acrylic cement pulmonary embolism is a potentially serious complication following vertebroplasty. Case Report. A 70-year-old male patient was treated with percutaneous vertebroplasty for osteoporotic nontraumatic vertebral collapse of L5-S1. Asymptomatic pulmonary cement embolism was detected on routine postoperative chest radiogram and the patient was treated with enoxaparin, amoxicillin, and dexamethasone. At the followup CT scan no further migration of any cement material was reported; and the course was uneventful. Discussion. The frequency of local leakage of bone cement is relatively high (about 80–90%), moreover, the rate of cement leakage into the perivertebral veins (seen in up to 24% of vertebral bodies treated) with consequent pulmonary cement embolism varies from4.6 to 6.8% (up to 26% in radiologic studies); the risk of embolism is increased with the liquid consistency of the cement and with the treatment of some malignant lesions. Patients may remain asymptomatic and develop no known long-term sequelae. Conclusions. Our ancedotal case illustrates the need for close monitoring of patients undergoing percutaneous vertebroplasty and emphasizes the importance of prompt and correct diagnosis and treatment, even if actually there is no agreement regarding the therapeutic strategy. 1. Introduction Bone cement embolism is a severe and potentially lifethreatening complication of cement (polymethylmethacrylate, PMM) vertebroplasty. We report a case of asymptomatic PMM pulmonary embolism following a surgical vertebroplasty. 2. Case Report A 70-year-old male patient with a complex medical history of coronary heart disease and hypertension (bicameral pacemaker dependent, left carotid artery stent, and triple aortocoronary bypass) was admitted to our university hospital for osteoporotic nontraumatic vertebral collapse of L5-S1 and spondylotic degeneration of vertebral. Preoperative serum chemistries and electrocardiogram were normal.

Sialography is considered a low invasive procedure, due to cannulation of salivary duct orifice. The aim of this study was to evaluate the morbidity of sialography using a nonionic dimeric contrast medium. METHODS: A questionnaire was proposed to 90 patients who underwent parotid and submandibular sialography to reveal related symptoms as: pain, swelling, impairment to daily activities and taste alteration. Pain and swelling intensity was assessed using a visual analogue scale (VAS). RESULTS: Fifty-two out of ninety questionnaires were returned. Sialography was performed in 30/52 patients in the submandibular gland, 22/52 in the parotid gland. Pain during sialography was complained by 28/52 patients with a 2.4 mean VAS score. Pain after sialography, was reported by 17/52 patients with a 1.3 mean VAS score. Swelling was reported by 25/52 patients with a 2.4 mean VAS score; 48/52 patients returned immediately to daily activities; 8/52 complained different taste alterations. CONCLUSION: Main related symptoms for submandibular and parotid sialography are pain and swelling, spontaneously resolving after 24-48 hours in most of cases. The statistical analysis also demonstrated in patients who underwent submandibular sialography the onset of pain during the procedure is significantly related to an increase of pain after the examination. Thus in patients, who referred severe pain during sialography, antiinflammatory drugs are recommended to control the onset of pain after sialography.

This study sought to validate the Warrick score in the assessment of lung involvement in systemic sclerosis (SSc) and correlate the results with functional abnormalities. In addition, we propose the use of high resolution computed tomography (HRCT) indices of alveolitis and fibrosis to discriminate between different stages of the disease.Thirty-one patients with SSc (16 with the diffuse form and 15 with the limited form) underwent functional and HRCT evaluations of the lung. The semiquantitative evaluation of radiological involvement, as proposed by Warrick, provides a score for each lesion based on the severity and the extent of the pulmonary damage. In addition to the total score, indices for alveolitis and fibrosis were created. The association between each score and functional abnormalities was tested.The total Warrick score was 16+/-7.7 [mean+/-standard deviation (SD)]. The total score correlated inversely with total lung capacity (TLC) (percentage of predicted TLC) (r=-0.38; p=0.04), diffusion lung capacity for carbon monoxide (DLCO) (percentage of predicted DLCO) (r=-0.43; p=0.02) and with forced expiratory volume in 1 second (FEV1) (percentage of predicted FEV1) (r=-0.36; p=0.05). The alveolitis and fibrosis indices were 1.9+/-1.1 and 14.2+/-7.4, respectively. The alveolitis index correlated significantly with percentage of predicted DLCO (r=-0.46; p=0.01) but not with percentage of predicted TLC (r=-0.28; p=0.13). The fibrosis index correlated both with percentage of predicted DLCO (r=-0.38; p=0.04) and with percentage of predicted TLC (r=-0.35; p=0.05).Our findings confirm the diagnostic role of HRCT in assessing the extent of lung damage in SSc. Furthermore, the two indices (alveolitis and fibrosis) provide new tools for evaluating the relationship between pulmonary involvement and systemic impairment in SSc.
