105. Freeze-drying wet digital prints: An option for salvage?

On the occasion of the collapse of the Historical Archive of the City of Cologne in March 2009 and the ensuing salvage effort, questions were raised about the use of freeze-drying for soaked digital prints, a technique that has not yet been evaluated for these materials. This study examines the effects of immersion, air-drying, drying in a blotter stack, freezing and freeze-drying on 35 samples of major digital printing processes. The samples were examined visually before, during and after testing; evaluation of the results was qualitative. Results show that some prints were already damaged by immersion alone (e.g. bleeding inks and soluble coatings) to the extent that the subsequent choice of drying method made no significant difference any more. For those samples that did survive immersion, air-drying proved to be crucial for water-sensitive prints, since any contact with the wet surface caused serious damage. Less water-sensitive prints showed no damage throughout the entire procedure, regardless of drying method. Some prints on coated media suffered from minor surface disruption up to total delamination of the surface coating due to the formation of ice crystals during shock-freezing. With few exceptions, freeze-drying did not cause additional damage to any of the prints that hadn't already been damaged by freezing. It became clear that an understanding of the process and materials is important for choosing an appropriate drying method.

The Ilisarov ring fixator is an extremely versatile external fixation device. There is a wide range of indications for its use, e.g. fractures, pseudarthroses and non-unions, large defects, deformities and limb shortening. The device consists of rings or half-rings, which are connected to each other by threaded rods or plates and to the bone by wires under tension. This design allows neutral fixation, compression or distraction of the bone fragments. Because of its low axial stiffness at low loads and increased stiffness at higher loads, this fixator promotes osteogenesis and reduces strain on the tissues in functional treatment. Surgical intervention for its application is minimally invasive. This is a useful addition to the orthopaedic surgeon's armamentarium, providing proper attention is paid to preoperative planning, the principle rules of frame design and wire insertion technique, and postoperative management.

In 338 consecutive human kidney transplants since 1972 uretero-neocystostomy was performed in a typical manner. The technique is a modification of Leadbetter's ureterosigmoid anastomosis and it is used for all cases of ureteral reimplantation at our institution. The rate of operative complications belonging to the urinary tract is 4.8%. In 29% out of 31 reexamined patients a vesico-ureteral reflux to the transplant appeared. We believe that the demonstrated method is useful for reconstruction of the urinary tract in renal transplants.

In cases of urinary fistulae as well as lymphoceles the only ultrasound-guided percutaneous nephrostomy has led to a good result. In cases of ureterostenosis and distal fistulae the renal function could be preserved by percutaneous nephrostomy. The anterograde pyelography was exact in demonstrating localization of the stenosis which led to a better operative procedure. The interventional ultrasound therefore represents an important option in the treatment of operative complications after kidney transplantation.

Most of the temporary dressing used in the treatment of extensive burn wounds pose immunological permeability, or transparency problems. All available dressings must be removed, and removal is often painful and traumatic. The development of biodegradable wound dressings made of copolymers of lactic and caproic acid was part of a research program (BMFT/FRG). Four parameters determine the properties of the copolymers: Purity, mole ratio of monomers, molecular weight, and sequence. Films of these copolymers can be made transparent, permeable, and flexible. They are degraded by hydrolysis and the normal ways of metabolism.

Abstract The adoption of precision farming (PF) was studied on the basis of personal interviews conducted at several agricultural exhibitions in Germany. Between 6.65% and 11% of the interviewed farmers used PF. The majority used data collection techniques such as GPS-based area measurement and soil sampling rather than variable rate application techniques such as site-specific sowing and fertilising. Roughly half the farmers interviewed knew about PF. About 7–10% of them intended to start using PF in the future. The results indicated that a large number of farmers did not even know what PF meant. In order to get more insight into this situation, several interviews were conducted with farmers already using PF techniques. A further study in 2005 of PF education in Germany indicated that, especially at vocational and technical schools, the subject was only covered to a small extent although the aim was to give a better coverage in future. At higher education levels, such as at universities and technical colleges, the teaching of PF was clearly better established. In order to promote awareness of PF among farmers, information and teaching materials adapted to the relevant educational levels were developed and tried out at training events. The main topics addressed were parallel tracking systems, site-specific nitrogen fertilising, yield mapping in grain production and the use of PDAs in crop farming. Finally, preliminary survey results are presented showing how knowledge about PF can best lead to its adoption and transfer into daily practice. [ABSTRACT FROM AUTHOR]

Abstract The adoption of Precision Farming (PF) in Germany has been studied by several mail surveys, telephone interviews and personal interviews with farmers, advisors, teachers and representatives of the PF industry. The intention was to monitor how PF techniques have entered the German market over the years and geographic location. The farmers were interviewed about their experiences with PF technology and their attitudes and barriers towards it. Those farmers who had not yet used PF technologies were asked for the reason and on which condition they would probably start with PF. Although the number of PF users slightly increased between 2001 and 2006, the results of all the surveys indicate that there are still various barriers regarding PF. Those farmers who had already used some PF technologies struggled with many problems at the beginning, but after overcoming these problems, they were generally content with the introduction of PF technologies. However, the majority of the interviewed farmers hesitated to introduce PF techniques mainly because of the high costs of the technology. Most of the interviewed teachers at vocational and technical schools stated that PF was not yet a subject in courses. The interviews with the advisors show that most of them do not offer any advisory service in the field of PF. Finally, the results of the interviews with representatives of the agricultural engineering industry confirm the statements of the other surveys. [ABSTRACT FROM AUTHOR]

Zusammenfassung Ellengelenkfrakturen sind relativ selten und machen etwa 7% aller Frakturen aus. Bei Beschränkung auf den proximalen Unterarm betreffen etwa 38% aller Ellengelenkfrakturen das Olekranon und 20–30% das Radiusköpfchen. Bei den ellengelenknahen Unterarmfrakturen hat sich die Einteilung nach AO nicht durchgesetzt. Hier finden v. a. Klassifikationen Anwendungen, die auch eine therapeutische Konsequenz nach sich ziehen. Bei der Einteilung der Olekranonfraktur verwenden wir die Klassifikation nach Schatzker, die den Frakturverlauf und die Anzahl der Knochenfragmente berücksichtigt und somit die Art der Osteosynthese vorgibt. Bei der Radiusköpfchenfraktur hat sich die Klassifikation nach Mason durchgesetzt. Hier werden je nach Typ das konservative Vorgehen, das operative Verfahren oder eine Resektion empfohlen. Die Frakturen des Processus coronoideus werden nach Regan und Morrey eingeteilt. Dabei wird über die Größe des Fragments indirekt auf die Instabilität des Gelenks geschlossen und dementsprechend das operative Vorgehen empfohlen. Neben der Monteggia-Verletzung wird als Sonderformen noch auf die Essex-Lopresti-Verletzung und die terrible triade of the elbow eingegangen. [ABSTRACT FROM AUTHOR]

Abstract From September 1997 to December 2000, 25 fractures and 20 psaeudarthroses in 44 patients were treated by fixed-angle plating; in 92% of these cases a minimally invasive tunneling technique was used. Fifteen fractures had initially been stabilized by means of an external fixator, the internal fixator being implanted as a second step once consolidation of the soft tissue had been achieved. Revision of a pseudarthrosis was always combined with autologous bone grafting. Follow-up examinations were possible in all 44 patients after an average of 22 (range 9–48) months, and all fractures and pseudarthroses were found to have healed. No secondary loss of reduction or implant failure was seen. In 4 patients deep infection at the plate occurred, but was successfully treated by early implant removal. Full weight-bearing was possible after a mean of 15.2 (range 8–26) weeks for the fracture patients and of 17.1 (mean; range 8–52) weeks for the pseudarthrosis patients. These results show that the multidirectional fixed-angle internal plate fixator implanted by a minimally invasive technique has proved very reliable in the treatment of fractures and pseudarthroses of the distal tibia. Abstract Hexapod robot kinematics have been exploited to develop an external fixator that is adjustable in all six spatial degrees of freedom. As with other robot systems, bones can be moved with a high degree of precision. A manually controlled fixator has so far been used with appropriate software in 127 patients. Secondary fracture reduction was performed successively, if necessary over several days, without causing pain to the patients. Ilizarov’s system for combined correction and distraction was especially useful. Electric drive motors were used in 4 patients. A fixator featuring load measurement capabilities was also constructed and was used as the basis for an ‘intelligent’ fixator, which will allow automatically controlled fracture and deformity treatment in the future. Abstract In recent years, operative management of intra-articular calcaneus fractures has gained increasing momentum. Precise anatomical reduction with reconstruction of tuber angle, tuber length and, especially, subtalar joint are required. The mobile Siremobil Iso-C3D is an important aid to the accurate reconstruction of the anatomical structures; dislocation persisting after reposition can be detected and corrected at a very early stage. From October 2002 to May 2004, 49 patients with fractures of the calcaneus underwent internal fixation of intra-articular fractures of the calcaneus with a fixed-angle titanium plate system with intraoperative 3D imaging. On average, the tuber angle was improved by 16°. In 18 cases, the intraoperative 3D imaging disclosed dislocations, which it was possible to correct. Secondary dislocation of the central fragment did not occur in any of these cases. [ABSTRACT FROM AUTHOR]

Abstract In our opinion implants with multidirectional blocked screws are the ideal treatment for periprosthetic fractures even in the elderly and critically ill patient. The operation is not time-consuming and the blood loss is relatively low. In osteoporotic bone those fixateur intern systems with multidirectional screws give a high stability so that we rapidly achieve joint motion. With the possibility of fixing screws in various angles and the possibility of contouring the plate to the bone we even achieve a rigid fixation in the presence of a voluminous prostheses. 123. Topkin ®-Eine resorbierbare Folie aus Lactid-Caprolacton.

Zusammenfassung Die resorbierbare Folie Topkin
® (Fa. Biomet-Merck), welche aus einem Kopolymer aus D,L-Lactid und e-Caprolacton besteht, wurde bei 71 Spalthautentnahmewunden und 42 Spalthautempfängerbezirken als Wundabdeckung eingesetzt. Die Größe der Spalthautentnahmewunden betrug bis zu 900 cm2 , die Ausdehnung der konditionierten Hautweichteildefekte bis 1200 cm2 . Zum Vergleich wurden 29 Empfängerbezirke mit Fettgazeverbänden versorgt. Die Schmerzhaftigkeit der Behandlung wurde auf einer visuellen Analogskala (0?100) gemessen und die Zeitdauer für die Verbandswechsel registriert. Bei den Spalthautentnahmewunden kam es zu 2 Komplikationen (Nachblutung, lokale Infektion). Im Bereich der Spalthautempfängerbezirke traten in 5 Fällen lokale Komplikationen auf (1 überschießende Granulation, 2 verbliebene Restdefekte, 1 lokaler Infekt sowie 1 Schorfbildung). Bei 29 Patienten, deren Spalthautempfängerbezirke mit herkömmlichen Fettgazeverbänden behandelt wurden, wurden 8 Wundheilungsstörungen (4 Restdefekte, 3 oberflächliche Transplantatnekrosen und 1 lokaler Infekt) beobachtet. Bei 4 Wunden wurden deutliche Narbenbildungen beobachtet. Die Zeit der Wundheilung bei der Behandlung mit resorbierbarer Folie und Fettgazeverbänden war nicht signifikant unterschiedlich. Die maximale Schmerzhaftigkeit der Folienbehandlung betrug bei den Spalthautentnahmewunden 10,9, bei den kontaminierten Wunden 2,9. Der pflegerische Aufwand für die Wundbehandlung betrug bei den ersten 20 Patienten 4 min/Tag, nach Ãnderung des Regimes bei den weiteren durchschnittlich 2 min/Tag. [ABSTRACT FROM AUTHOR]- Published

Zusammenfassung Alle Fälle der BGW-Bezirksverwaltungen Hamburg, Delmenhorst und Magdeburg mit Unfalldatum nach dem 1.1.1994, bei welchen von Februar 1999?Juni 2002 Rentenbegutachtungen nach Sprunggelenkfrakturen anstanden, wurden untersucht. Insgesamt wurden 205 Gutachten mit Hilfe von verletzungsspezifischen Erfassungsbögen ausgewertet. Die Verletzungen wurden anhand der Unfallröntgenaufnahmen nach der AO-Klassifikation eingeteilt. Die funktionellen Ergebnisse waren überwiegend gut. Bei Weber-C-Frakturen ergeben sich Ansatzpunkte für eine Verbesserung der Versorgungsqualität, da hier in 13,5% Syndesmosenerweiterungen als präarthrotische Deformität verblieben waren! Nahezu 50% der Patienten mit Sprunggelenkfrakturen ohne Begleitverletzung waren mehr als 3 Monate, etwas über 1/5 der Patienten sogar über 5 Monate arbeitsunfähig. Diese hohe Rate langer Arbeitsunfähigkeitszeiten war im Rahmen der Studie weder medizinisch noch durch besondere berufliche Belastungen begründet, sodass hier u. E. bedeutende Einsparungen möglich sein könnten. Die standardisierte, EDV-gestützte Gutachtenauswertung ist somit geeignet, die Ergebnisqualität im Bereich der gesetzlichen Unfallversicherung festzustellen. Sie ermöglicht einen repräsentativen, Klinik übergreifenden Vergleich der Behandlungsergebnisse als Basis für Maßnahmen der Qualitätssicherung und die Erfassung der Auswirkungen von Maßnahmen zur Steuerung und Überwachung des Heilverfahrens.
