In June 2022, a gastroenteritis outbreak occurred in a town in Northern Italy, possibly associated with the ingestion of norovirus from public drinking water. Noroviruses are highly infectious RNA viruses, with high stability in the environment. They are the primary cause of non-bacterial gastroenteritis worldwide, and despite the fact that the disease is mainly self-limiting, norovirus infection can lead to severe illness in the immunocompromised, the elderly and children. Immediately after the notification of the suspected norovirus outbreak, faecal specimens were collected from hospitalised patients, and water samples were collected from public drinking fountains in the affected area, to confirm the presence of norovirus. Norovirus was detected in 80 % (95 % CI 0.58-0.91) of the faecal specimens, and in 50 % (95 % CI 0.28-0.72) of the water samples using RT (reverse transcription) Real-time PCR. The identification of GII genotype in all samples confirmed public drinking water as the source of norovirus contamination. In addition, in one faeces and one water sample, the co-presence of genotypes GI and GII was detected. The strains were typed by sequencing, with most of them belonging to the genotype GII.3. Immediately after the confirmation of norovirus contamination in public drinking water, the local competent authorities applied safety measures, resulting in a decline in number of cases. Moreover, after the application of disinfection protocols in the water plant, the sampling was repeated with negative results for norovirus in the affected area. However, positive samples were found in the neighbouring area (prevalence 10.00 %, 95 % CI 0.02-0.40) and in the water spring (prevalence 50.00 %, 95 % CI 0.21-0.78), suggesting norovirus persistence and spread from the water source. Background and Aim of the Work: Hepatitis A is an infectious disease characterized by fecal-oral transmission; however, a rise in sexually-transmitted cases has been observed, particularly among "men who have sex with men". In Europe, a Hepatitis A epidemic occurred among men who have sex with men between 2016 and 2018. The aim of this study is to describe this Hepatitis A epidemic in the city of Milan and to analyze the incidence of Sexually Transmitted Diseases co-infection among Hepatitis A cases., Methods: Hepatitis A cases were traced and identified. Epidemiological data were collected and Hepatitis A vaccination was investigated. Cases were georeferenced, calculating incidence rates for each Milan Municipality. Viral genotypic analysis was carried out., Results: 353 cases were reported in Milan. Incidence rates resulted significantly higher in males (RR 18.1 CI9 5% 11.5 - 28.4). 70 cases reported foreign travel. 172 cases reported "Men  who have Sex with Men" behaviour. Genotypic analysis revealed correlation with strains of the European "Men who have Sex with Men" epidemic. Georeferencing showed asymmetric case distribution. Only 12 cases reported Hepatitis A vaccination. The Relative Risk for syphilis infection among Hepatitis A cases was 133.9 (95% CI 81.7 - 219.7) and 29.7 (95%CI 9.5 - 92.7) for gonorrhea., Conclusions: Most genotyped cases (93.7%) correlated to the European Hepatitis A epidemic among Men who have Sex with Men. Georeferencing showed a greater incidence of Hepatitis A cases in areas characterized by the homosexual community. The higher incidence of Sexually Transmitted Diseases co-infection in Hepatitis A cases correlated to clusters responsible for the European Hepatitis A epidemic, suggests increased sexual promiscuity among Men who have Sex with Men. These data support the need for Hepatitis A vaccination programs and sensitization of Men who have Sex with Men to the adoption of safe sexual practices.

Purpose: To evaluate retinal functional improvement by means of visual acuity and retinal sensibility examination after intravitreal dexamethasone implant in patients affected by cystoid macular edema secondary to retinal vein occlusion., Methods: Twenty-six consecutive patients affected by retinal vein occlusion complicated by cystoid macular edema were enrolled in this prospective interventional study. All patients underwent a baseline complete ophthalmological evaluation as well as retinal angiography, OCT examination, and microperimetry evaluation. Each patient was treated with intravitreal injection of a long-term steroid implant (Ozurdex, Allergan). Follow-up evaluations were performed at months 1, 3, and 6 and completed by OCT and MP1 examination. Clinical data underwent statistical analysis., Results: Baseline functional evaluation showed mean visual acuity of 0,63±0,42 LogMAR and retinal sensitivity of 7,93±4,73 dB (mean±standard deviation); after treatment, at day 30 we found, respectively, 0,43±0,38 LogMAR (p<0.05, compared to baseline) and 10,15±4,410 dB (p<0.05); at day 90, we found 0,44±0,32 (p<0.05) and 9.61±4,29 dB (p<0.05); at day 180, we found 0,41±0,31 (p<0.05) and 9,95±3,79 dB (p<0.05). Fixation pattern improved significantly (p<0.05), showing a stable fixation in 30% of patients at baseline, increasing to 77% of patients at day 180. Baseline morphological evaluation showed a central retinal thickness (CRT) of 398,21±181,65  μ m after treatment; we found a CRT of 222,64±95,21  μ m at day 30 (p<0.05, compared to baseline), 307,50±120,25  μ m (p<0.05) at day 90, and 294,93±135,86  μ m (p<0.05) at day 180. About 15,3% patients showed already at month 3 a recurrence of macular edema. They underwent a retreatment before month 6 as for treatment guidelines., Conclusion: Our detailed analysis showed the significative increase in retinal function in the early phases of the follow-up. Retinal sensibility showed a stronger correlation than VA in macular edema reabsorption, better underlying the progressive functional recovery and increase in quality of vision and life for the patients. The cultivation of rice, one of the most important staple crops worldwide, has very high water requirements. A variety of irrigation practices are applied, whose pros and cons, both in terms of water productivity and of their effects on the environment, are not completely understood yet. The continuous monitoring of irrigation and rainfall inputs, as well as of soil water dynamics, is a very important factor in the analysis of these practices. At the same time, however, it represents a challenging and costly task because of the complexity of the processes involved, of the difference in nature and magnitude of the driving variables and of the high variety of field conditions. In this paper, we present the prototype of an integrated, multisensor system for the continuous monitoring of water dynamics in rice fields under different irrigation regimes. The system consists of the following: (1) flow measurement devices for the monitoring of irrigation supply and tailwater drainage; (2) piezometers for groundwater level monitoring; (3) level gauges for monitoring the flooding depth; (4) multilevel tensiometers and moisture sensor clusters to monitor soil water status; (5) eddy covariance station for the estimation of evapotranspiration fluxes and (6) wireless transmission devices and software interface for data transfer, storage and control from remote computer. The system is modular and it is replicable in different field conditions. It was successfully applied over a 2-year period in three experimental plots in Northern Italy, each one with a different water management strategy. In the paper, we present information concerning the different instruments selected, their interconnections and their integration in a common remote control scheme. We also provide considerations and figures on the material and labour costs of the installation and management of the system.

Although drought is an increasing problem in agriculture, the contribution of the root-associated bacterial microbiome to plant adaptation to water stress is poorly studied. We investigated if the culturable bacterial microbiome associated with five grapevine rootstocks and the grapevine cultivar Barbera may enhance plant growth under drought stress. Eight isolates, over 510 strains, were tested in vivo for their capacity to support grapevine growth under water stress. The selected strains exhibited a vast array of plant growth promoting (PGP) traits, and confocal microscopy observation of gfp-labelled Acinetobacter and Pseudomonas isolates showed their ability to adhere and colonize both the Arabidopsis and grapevine rhizoplane. Tests on pepper plants fertilized with the selected strains, under both optimal irrigation and drought conditions, showed that PGP activity was a stress-dependent and not a per se feature of the strains. The isolates were capable of increasing shoot and leaf biomass, shoot length, and photosynthetic activity of drought-challenged grapevines, with an enhanced effect in drought-sensitive rootstock. Three isolates were further assayed for PGP capacity under outdoor conditions, exhibiting the ability to increase grapevine root biomass. Pesticide losses to water can present problems for environmental management, particularly in catchments where surface waters are abstracted for drinking water supply. The relative role of different transfer pathways (spray drift, spills, overland flow and leaching from soils) is often uncertain, and there is a need for experimental observation and modelling to ensure that processes are understood under a range of conditions. Here we examine the transport of propyzamide and carbetamide in a small (15.5 ha) headwater sub-catchment dominated by an artificially drained field with strongly undulating topography (topographic gradients >1:10). Specifically, we explore the validity of the "field-scale lysimeter" analogy by applying the one dimensional mathematical model MACRO. Although one dimensional representation has been shown to be reasonable elsewhere, the scale and topography of the monitored system challenge many of the underlying assumptions. MACRO considers two interacting flow domains: micropores and macropores. The effect of subsurface drains can also be included. A component of the outflow from the main drain was identified as originating from an upslope permeable shallow aquifer which was represented using a simple groundwater model. Predicted herbicide losses were sensitive to drain spacing and the organic carbon to water partition coefficient, K(OC). The magnitude of the peak water and herbicide transport and their timing were simulated satisfactorily, although model performance was poor following a period of one month when snow covered the ground and precipitation was underestimated by the rain gauge. Total herbicide loads were simulated adequately by MACRO, suggesting that the field-scale lysimeter analogy is valid at this scale, although baseflow contributions to flow needed to be accounted for separately in order to adequately represent hydrological response., (Copyright © 2012 Elsevier B.V. Propyzamide and carbetamide are essential for blackgrass control in oilseed rape production. However, both of these compounds can contaminate surface waters and pose compliance problems for water utilities. The transport of propyzamide and carbetamide to an instrumented field drain in a small clay headwater tributary of the Upper Cherwell catchment was monitored over a winter season. Despite having very different sorption and dissipation properties, both herbicides were transported rapidly to the drain outlet in the first storm event after application, although carbetamide was leached more readily than propyzamide. A simple conceptual model was constructed to represent solute displacement from mobile pore water and preferential flow to drains. The model was able to reproduce the timing and magnitude of herbicide losses well, lending support to its conceptual basis. Measured losses in drainflow in the month following application were 1.1 and 8.1%, respectively, for propyzamide and carbetamide. Differences were due to a combination of differences in herbicide mobility and due to the fact that the monitoring period for carbetamide was hydrologically more active. For both compounds, losses were greater than those typically reported elsewhere for other herbicides. The data suggest that drainflow is the dominant pathway for the transfer of these herbicides to the catchment outlet, where water is abstracted for municipal supply. A hysteretic spin transition is induced in a solution assembly of a mononuclear Fe(III) amphiphilic complex which exhibits only gradual spin crossover in the solid state. The hysteretic behavior is dependent on dynamic solution assembly and removal of solvent causes reversion to the original bulk solid magnetic response.

Spin crossover requires cooperative behavior of the metal centers in order to become useful for devices. While cooperativity is barely predictable in solids, we show here that solution processing and the covalent introduction of molecular recognition sites allows the spin crossover of iron(III) sal(2)trien complexes to be rationally tuned. A simple correlation between the number of molecular recognition sites and the spin crossover temperature enabled the fabrication of materials that are magnetically bistable at room temperature. The predictable behavior relies on combining function (spin switching) and structure (supramolecular assembly) through covalent interactions in a single molecular building block.

New amphiphilic and spin-labile Mn(III) complexes based on dianionic N(4)O(2)-hexadentate sal(2)trien or sal(2)bapen ligands, which contain OC(6)H(13), OC(12)H(25), or OC(18)H(37) alkoxy substituents at different positions of the salicylidene unit were prepared (H(2)sal(2)trien = N,N'''-bis(salicylidene)-1,4,7,10-tetraazadecane, H(2)sal(2)bapen = N,N'''-bis(salicylidene)-1,5,8,12-tetraazadodecane). According to electrochemical measurements, these complexes undergo two (quasi)reversible redox processes. Temperature-dependent magnetic measurements revealed a high-spin configuration for all sal(2)trien complexes (S = 2) and gradual spin crossover for sal(2)bapen complexes from high to low spin (S = 1). The chain length strongly influences the spin crossover, as C(18)-functionalization stabilizes the low spin state at much higher temperatures than shorter alkyl chains. Moreover, long alkyl chains allow for spontaneous self-assembly of the molecules, which was investigated in single crystals and in Langmuir-films at the air-water interface. Long alkyl chains (C(12) or C(18)) as well as a mutual syn-orientation of these molecular recognition sites were required for the Langmuir monolayers to be stable.

[Fe(II)(C(16)dpt)(2)(NCS)(2)].(2/3)H(2)O displays temperature-mediated spin crossover (SCO) with T((1/2)) = 290 K and the long alkyl chain substituent on the dipyridyltriazole ligand facilitates the formation of a stable Langmuir-Blodgett film at an air-water interface.

New sal₂(trien) ligands that contain alkoxy substituents of various length in meta position of the phenolate entities were coordinated to electronically and magnetically active iron(III) and cobalt(III) centers. The electrochemical and spectroscopic properties of these amphiphilic complexes are virtually unaffected upon alteration of the alkoxy substituents, thus providing a system in which the physical behavior and the metal-centered chemical activity can be tailored independently. The amphiphilic character has been exploited for preparing Langmuir monolayers at the air-water interface and for constructing Langmuir-Blodgett films, hence allowing for hierarchical assembling of electronically and magnetically active systems. While Langmuir films were stable, transfer onto solid supports was limited, which restricted the magnetic analysis of the Langmuir-Blodgett assemblies.

As a first step towards amphiphilic spin crossover (SCO) systems where the hydrophobic part of the system is introduced by a non-coordinating anion (i.e. where no modification of the ligands to introduce hydrophobic substituents is required), [Fe(II)(OH(2))(2)(C(16)SO(3))(2)] and [Co(II)(OH(2))(2)(C(16)SO(3))(2)] have been prepared and reacted with the triazole-containing ligands adpt and pldpt (C(16)SO(3) = hexadecanesulfonate anion, adpt = 4-amino-3,5-bis(2-pyridyl)-1,2,4-triazole, pldpt = 4-pyrrolyl-3,5-bis(2-pyridyl)-1,2,4-triazole). In the solid state, two HS complexes of the form [Fe(II)(Rdpt)(2)(C(16)SO(3))(2)] and two of the form [Co(II)(Rdpt)(2)(CH(3)OH)(2)](C(16)SO(3))(2) are obtained, even when a six-fold excess of ligand is used (Rdpt = adpt or pldpt). In solution, the cobalt complexes remain in this form as evidenced by colour, Visible/NIR and IR spectroscopy. For the iron complexes, there is an equilibrium in solution between the neutral high-spin form of the complex [Fe(II)(Rdpt)(2)(C(16)SO(3))(2)] and the dicationic low-spin tris form [Fe(II)(Rdpt)(3)](C(16)SO(3))(2). Polar solvents favour the dicationic form, while less polar solvents favour the neutral form (as evidenced by solution colour and solution IR spectroscopy). Visible/NIR spectroscopy and Evans' method NMR spectroscopy show the equilibrium can be shifted towards the [Fe(II)(Rdpt)(3)](C(16)SO(3))(2) form by adding additional ligand to the solution. The X-ray crystal structures of [Fe(II)(adpt)(2)(C(16)SO(3))(2)] and [Co(II)(adpt)(2)(CH(3)OH)(2)](C(16)SO(3))(2).1.33CH(3)OH are presented. [Fe(II)(adpt)(2)(C(16)SO(3))(2)] has a 2D bilayer structure with alternating layers of polar Fe(adpt)(2) centres, and hydrophobic alkyl chains. The complex cations in [Co(II)(adpt)(2)(CH(3)OH)(2)](C(16)SO(3))(2).1.33CH(3)OH form 1-D columns in the solid state. The capacity of the amphiphilic complexes [Fe(II)(pldpt)(2)(C(16)SO(3))(2)] and [Fe(II)(adpt)(2)(C(16)SO(3))(2)] to self-assemble has been probed at the air-water interface using Langmuir techniques. The pertinent pressure-area isotherms reveal only a low tendency of the complexes to form films.

Supramolecular principles have been applied for improving the spin crossover activity of metal centers due to cooperative effects in solution. Thus, incorporation of alkyloxy tails at the phenyl group of Fe(sal2trien) 2a provides amphiphilic complexes Fe(sal-OR2trien) 2b-d (b, R = C6H13; c, R = C8H17; d, R = C18H37) comprising an apolar group for supramolecular organization and a polar headgroup with potential spin crossover activity due to the presence of a spin-labile iron(III) center. Self-assembly of these complexes in solution resulted in the formation of microsize and submicrosize particles when the alkyl chain was long enough (2d) but not with shorter chains (2a-c). Solutions of 2d showed enhanced spin crossover activity as compared to complexes 2a-c, both in terms of transition temperature and steepness of the transition. This observation has been correlated to an improved cooperativity of the metal centers in 2d due to self-assembly, thus facilitating a tandem spin transition.

When river water quality fluctuates over relatively short periods of time with respect to the sampling frequency, the collection of grab samples may be inappropriate for characterising average water quality. This paper presents the results of a water quality monitoring study carried out on a stretch of the river Lambro (northern Italy) dominated by a periodically overloaded sewage treatment works (STW) located near its upstream end. Water quality was strongly influenced by a pronounced diurnal cycle in pollutant loads caused by the regular emission of untreated waste water during periods of high domestic flow (daytime). Two different sampling techniques were employed: grab sampling and 24-h composite sampling using automatic samplers. Samples were collected at the plant overflow and at several sites along the river and analysed for two common ingredients of household detergents, linear alkylbenzene sulphonate (LAS) and boron (B) and for routine water quality variables. The results obtained show that: (1) The diurnal variability of point-source-derived chemical concentrations in the river downstream of the undersized STW increased with increasing removal efficiency in sewage treatment. (2) The shape of the diurnal concentration signal remained relatively intact for a considerable distance downstream of the STW for several water quality variables, suggesting that hydrodynamic dispersion plays a relatively minor role in controlling concentration patterns in this river. (3) In-stream degradation of LAS was consistent with first order kinetics with a rate constant of 0.05-0.06 h(-1). (4) Grab sampling is a relatively inefficient methodology for capturing mean concentrations for rivers subjected to highly variable loads, especially when it is restricted to office hours. The inefficiency of grab sampling is more marked for substances (e.g. LAS) which are effectively removed during sewage treatment than for substances which are not. (5) For LAS, diurnal variability in the concentration signal decreases with distance downstream, making grab sampling an increasingly reliable methodology for estimating mean concentrations. (6) 24-h composite sampling is an efficient way of eliminating the effect of diurnal variations in load strength.

The work presents the results of a comprehensive modelling study of surface and groundwater resources in the Muzza-Bassa Lodigiana irrigation district, in Northern Italy. It assesses the impact of changes in land use and irrigation water availability on the distribution of crop water consumption in space and time, as well as on the groundwater resources. A distributed, integrated surface water-groundwater simulation system was implemented and applied to the study area. The system is based on the coupling of a conceptual vadose zone model with the groundwater model MODFLOW. To assess the impact of land use and irrigation water availability on water deficit for crops as well as on groundwater system in the area, a number of management scenarios were identified and compared with a base scenario, reflecting the present conditions. Changes in land use may alter significantly both total crop water requirement and aquifer recharge. Water supply is sufficient to meet demand under present conditions and, from the crop water use viewpoint, a reduction of water availability has a positive effect on the overall irrigation system efficiency; however, evapotranspiration deficit increases, concentrated in July and August, when it may be critical for maize crops.

Unlabelled: The role of imaging in the management of maxillofacial trauma is to describe anatomical lesions and to detect complications and associated injuries. Plain films are still useful for minimal trauma, but CT-scan is the gold standard for complex trauma. Helical CT and multidetector row CT simplify the emergency imaging of horizontal struts (skull base, orbital floor, alveolar ridge and palate). The diagnosis, and sometimes the treatment of complications may require CT cisternography, MRI and angiography., Learning Objectives: review mechanisms and classification of paranasal sinuses trauma; present the imaging techniques with special emphasis on CT; describe paranasal sinuses trauma features and pseudo-fracture patterns; describe complications and associated injuries.

The GREAT-ER (Geo-referenced Regional Exposure Assessment Tool for European Rivers) project team has developed and validated an accurate aquatic chemical exposure prediction tool for use within environmental risk assessment schemes. The software system GREAT-ER 1.0 calculates the distribution of predicted environmental concentrations (PECs) of consumer chemicals in surface waters, for individual river stretches as well as for entire catchments. The system uses an ARC/INFO-ArcView (ESRI) based Geographical Information System (GIS) for data storage and visualization, combined with simple mathematical models for prediction of chemical fate. At present, the system contains information for four catchments in Yorkshire, one catchment in Italy, and two in Germany, while other river basins are being added. Great-ER 1.0 has been validated by comparing simulations with the results of an extensive monitoring campaign for two 'down-the-drain' chemicals, i.e. the detergent ingredients boron and Linear Alkylbenzene Sulphonate (LAS). GREAT-ER 1.0 is currently being expanded with models for the terrestrial (diffuse input), air and estaurine compartments.

The objectives of the present report were to give a baseline picture of hepatitis B notification incidence rates in children before the campaign of mass vaccination for newborns and adolescents (12-13 years old), and to study the role of different risk factors. Data from a specific national surveillance system of acute viral hepatitis (SEIEVA, Sistema Epidemiologico Integrato dell'Epatite Virale Acuta) were used and acute hepatitis B cases were compared to acute hepatitis A patients with the case-control study method to estimate the associations with the considered risk factors. Since the system began, one hundred and sixty-three local health departments have joined SEIEVA covering 30% of the Italian population. The incidence of acute hepatitis B notifications among 0-14 aged children was 9 per 100,000 in 1985 and 1 per 100,000 in 1990. Such decline in incidence was observed in both the North and the South of Italy. Surgical interventions, dental therapy and household contacts with a HBsAg chronic carrier were found to be associated with acute hepatitis B. The point estimate of the odds ratio was 10 for the latter risk factor. Other preventive measures in addition to vaccination are needed to control the risk of hepatitis B infection and other parenteral diseases due to surgical intervention and dental therapy.

New nitrogen analogues of prostaglandins (11, 11a, 12, and 12a) have been synthesized starting from a 4,5-disubstituted 2-pyrrolidinone nucleus (5 and 5a) containing one side chain and a suitable functionality for elaborating the second one. These analogues had no better activity than natural prostaglandins in vitro [guinea pig ileum and trachea, rat stomach fundus strip, uterus and portal vein, ADP-induced guinea pig platelet-rich plasma (PRP) aggregation]. They similarly lacked any interesting activity in vivo [anesthetized rat blood pressure, stress, and acetylsalycilic acid (ASA) induced gastric lesions in rat].

Synthesis of alpha-halo-alpha,beta-unsaturated ketones from aldehydes as a synthetic approach to prostaglandins is investigated.

The effect of premedication with inhaled prostacyclin (PGI2) on the allergen induced immediate bronchoconstriction was investigated in 10 asthmatic patients, under single blind conditions, by means of a body plethysmograph. Statistical analysis (Dunnett's test) did not show any significant difference between PGI2 and placebo. The possible meaning of these results is discussed.

Esterification with inversion is a highly stereospecific reaction by which one-step conversion of PGF2alpha to PGF2beta is obtained with very satisfactory yield.

The efficacy of prostacyclin in preventing aspecifically and specifically induced bronchoconstriction was evaluated in a single-blind cross-over study in 3 groups of asthmatic patients. Prostacyclin was given by inhalation in doses of 250 microgram in women and 500 microgram in men. Ultrasonic mist of H2O (group 1), physical exercise on cycloergometer (group 2) and pneumoallergens (group 3) were used as bronchial stimulant agents. Prostacyclin gave a very good protection in the first 2 groups, but was ineffective in the third. An increase in heart rate was observed immediately after the inhalation of prostacyclin: this effect was particularly evident in women in whom a significant decrease in diastolic pressure was noted too.

20-methyl-13,14-didehydro-PGF2 alpha has been assayed for antagonistic activity against PGF2 alpha and PGE2 on different smooth msucle preparations in vitro. This compound may be considered a selective and probably a competitive antagonist of PGF2 alpha on rat uterus in vitro.

The bronchodilator activity of four analogues of PGE2: 13,14-didehydro-PGE2 (I), 20-methyl-13,14-didehydro-PGE2 (II), 16 S-methyl-13,14-didehydro-PGE2 (III) and 16 R-methyl-13,14-didehydro-PGE2 (IV) was studied in vivo and in vitro. The potency of III and IV when administered by aerosol to conscious guinea pigs was four times that of PGE2 in protecting against histamine-induced convulsion; I and II were less active. Compound III administered by aerosol to anaesthetized guinea pigs was six times more potent than PGE2 in protecting against i.v. histamine-induced increase of intractracheal pressure and the effect was longer-lasting. All the compounds caused relaxation of carbachol-induced tone in isolated guinea pig tracheal strips.
