Abstract

Food security is a fundamental human right yet many people are food insecure, even in high-income countries. Reviewed here is the evidence for the physical, economic, sociocultural, and political environmental influences on household food security in high-income countries. The literature was evaluated using the ANGELO framework, which is a lens developed for understanding the environmental factors underpinning the obesity pandemic. A review of the literature identified 78 articles, which mostly reported on cross-sectional or qualitative studies. These studies identified a wide range of factors associated with food security. Foremost among them was household financial resources, but many other factors were identified and the complexity of the issue was highlighted. Few studies were prospective and even fewer tested the use of interventions other than the supplemental nutrition assistance program to address food security. This indicates a solution-oriented research paradigm is required to identify effective interventions and policies to enhance food security. Abstract

Population food and nutrition monitoring plays a critical role in understanding suboptimal nutrition at the population level, yet current monitoring methods such as national surveys are not practical to undertake on a continuous basis. Supermarket sales data potentially address this gap by providing detailed, timely, and inexpensive monitoring data for informing policies and anticipating trends. This paper reviews 22 studies that used supermarket sales data to examine food purchasing patterns. Despite some methodological limitations, feasibility studies showed promising results. Abstract

New Zealand has a strategy of eliminating SARS-CoV-2 that has resulted in a low incidence of reported coronavirus-19 disease (COVID-19). The aim of this study was to describe the spread of SARS-CoV-2 in New Zealand via a nationwide serosurvey of blood donors. Samples (n = 9806) were collected over a month-long period (3 December 2020–6 January 2021) from donors aged 16–88 years. The sample population was geographically spread, covering 16 of 20 district health board regions. A series of Spike-based immunoassays were utilised, and the serological testing algorithm was optimised for specificity given New Zealand is a low prevalence setting. Eighteen samples were seropositive for SARS-CoV-2 antibodies, six of which were retrospectively matched to previously confirmed COVID-19 cases. A further four were from donors that travelled to settings with a high risk of SARS-CoV-2 exposure, suggesting likely infection outside New Zealand. The remaining eight seropositive samples were from seven different district health regions for a true seroprevalence estimate, adjusted for test sensitivity and specificity, of 0.103% (95% confidence interval, 0.09–0.12%). The very low seroprevalence is consistent with limited undetected community transmission and provides robust, serological evidence to support New Zealand's successful elimination strategy for COVID-19. Abstract

Key Points Question Is a brief parenting program delivered solely via text message effective in improving parental competence and mental health literacy in parents of adolescents? Findings In this randomized clinical trial of 221 parents and caregivers, significant group differences were observed between those receiving the text-messaging intervention and the control group. Participants who received the text-messaging program reported higher levels of parental competence, improved knowledge of help seeking, improved parent-adolescent communication, and lower levels of parental distress at 1 and 3 months of follow-up compared with the control group. Meaning The text-messaging program appears to be an effective and feasible way to reach and support a large number of parents to improve parental competence and may represent a less expensive option for service delivery., This randomized clinical trial evaluates the effectiveness of a text-messaging program, MyTeen, for promoting parental competence and mental health literacy for parents of adolescents., Importance There is global pressure to respond to the burden posed by adolescent mental health problems. The National Mental Health Commission has made a call for investment in mobile health services directed at prevention and early intervention to relieve the demand on targeted mental health services that are costly to provide. Parents and primary caregivers play a significant role in the lives of adolescents and are important targets for such efforts. Currently, there is no evidence for the effectiveness of programs delivered solely via text message for parents of adolescents. Objective To evaluate the effects of a text-messaging program (MyTeen) on promoting parental competence and mental health literacy for parents of adolescents. Design, Setting, and Participants A parallel 2-group randomized clinical trial was conducted in New Zealand. A total of 221 parents and primary caregivers of adolescents aged 10 to 15 years were recruited from March 19 to August 17, 2018, via community outreach and social media and were randomly allocated 1:1 into the control or the intervention group. Statistical analysis was performed on the principle of intention to treat with adjustment for baseline factors and ethnicity. Intervention A text-messaging program for parents of adolescents (age 10-15 years) to promote parental competence and mental health literacy. Participants received 1 daily text message over 4 weeks. Main Outcomes and Measures Parental competence, assessed at 1 month after randomization by the Parenting Sense of Competence Scale. Results In total, 221 participants (214 [96.8%] female) were randomized, 109 to the intervention group and 112 to the control group; 201 participants (91%) completed the trial at 3 months. Significant group difference was observed on the primary outcome at the end of 1 month of intervention, with participants reporting a higher level of parental competence than those in the control group (estimated mean difference, 3.33 points; 95% CI, 1.37-5.29 points; P = .002). Introduction The mobile-phone-based Bedfont iCO TM Smokerlyzer ® is of unknown validity and reproducibility compared to the widely-used piCO + Smokerlyzer ® . We aimed to compare the validity and reproducibility of the iCO TM Smokerlyzer ® with the piCO + Smokerlyzer ® among patients reducing or quitting tobacco smoking. Material and Methods Methadone-maintained therapy (MMT) users from three centers in Malaysia had their exhaled carbon monoxide (eCO) levels recorded via the piCO + and iCO TM Smokerlyzers ® , their nicotine dependence assessed with the Malay version of the Fagerström Test for Nicotine Dependence (FTND-M), and daily tobacco intake measured via the Opiate Treatment Index (OTI) Tobacco Q-score. Pearson partial correlations were used to compare the eCO results of both devices, as well as the corresponding FTND-M scores. Abstract

Both housing structure and occupant use of homes influence health outcomes and carbon emissions. However, it remains unclear how these elements interact. We conducted semi-structured interviews with 13 technical experts in the New Zealand housing and health sectors. We applied the general inductive method for qualitative data analysis and generated frameworks for 'housing structure' and 'occupant behaviour' including: (i) issues or barriers, (ii) solutions or facilitators and (iii) potential interventions. Addressing the location, aspect and design of the house were seen to offer the greatest gains in energy efficiency, indoor temperature and air quality. Insulation, heating and ventilation were the most critical technologies for promoting health. Raising consumer expectations of housing standards was thought to be critical to boost demand and drive supply for better quality housing. Participants proposed that sharing personal stories, combining information with active strategies (demonstration, skill rehearsal) and providing incentives were promising strategies to realise the full potential of health-enhancing home design. Schools and the local community were identified as suitable settings. We conclude that public health interventions should not only aim to build homes that maximise health and environmental outcomes but must also pay attention to how occupants behave and interface with their houses.
