Abstract

We aimed to explore the impact of the COVID-19 pandemic on cardiac diagnostic testing and practice and to assess its impact in different regions in Europe. The online survey organised by the International Atomic Energy Agency Division of Human Health collected information on changes in cardiac imaging procedural volumes between March 2019 and March/April 2020. Data were collected from 909 centres in 108 countries. Centres in Northern and Southern Europe were more likely to cancel all outpatient activities compared with Western and Eastern Europe. There was a greater reduction in total procedure volumes in Europe compared with the rest of the world in March 2020 (45% vs 41%, p=0.003), with a more marked reduction in Southern Europe (58%), but by April 2020 this was similar in Europe and the rest of the world (69% vs 63%, p=0.261). Regional variations were apparent between imaging modalities, but the largest reductions were in Southern Europe for nearly all modalities. In March 2020, location in Southern Europe was the only independent predictor of the reduction in procedure volume. However, in April 2020, lower gross domestic product and higher COVID-19 deaths were the only independent predictors. The first wave of the COVID-19 pandemic had a significant impact on care of patients with cardiac disease, with substantial regional variations in Europe. This has potential long-term implications for patients and plans are required to enable the diagnosis of non-COVID-19 conditions during the ongoing pandemic.

117. Cocaine- and opiate-related fatal overdose in New York City, 1990–2000

Abstract Background In New York City (NYC), the annual mortality rate is higher for accidental drug overdoses than for homicides; cocaine and opiates are the drugs most frequently associated with drug overdose deaths. We assessed trends and correlates of cocaine- and opiate-related overdose deaths in NYC during 1990–2000. Methods Data were collected from the NYC Office of the Chief Medical Examiner (OCME) on all fatal drug overdoses involving cocaine and/or opiates that occurred between 1990–2000 (n = 8,774) and classified into three mutually exclusive groups (cocaine only; opiates-only; cocaine and opiates). Risk factors for accidental overdose were examined in the three groups and compared using multinomial logistic regression. Results Overall, among decedents ages 15–64, 2,392 (27.3%) were attributed to cocaine only and 2,825 (32.2%) were attributed to opiates-only. During the interval studied, the percentage of drug overdose deaths attributed to cocaine only fell from 29.2% to 23.6% while the percentage of overdose deaths attributed to opiates-only rose from 30.6% to 40.1%. Compared to New Yorkers who fatally overdosed from opiates-only, fatal overdose attributed to cocaine-only was associated with being male (OR = 0.71, 95% CI 0.62–0.82), Black (OR = 4.73, 95% CI 4.08–5.49) or Hispanic (OR = 1.51, 95% CI 1.29–1.76), an overdose outside of a residence or building (OR = 1.34, 95% CI 1.06–1.68), having alcohol detected at autopsy (OR = 0.50, 95% CI 0.44–0.56) and older age (55–64) (OR = 2.53 95% CI 1.70–3.75)). Conclusion As interventions to prevent fatal overdose become more targeted and drug specific, understanding the different populations at risk for different drug-related overdoses will become more critical.

Abstract

Abstract Background Evidence on the negative outcomes of untreated mental disorders during pregnancy and in the first year after childbirth on women’s and children’s health has stimulated interest in how to develop and sustain high-quality mental health care during the perinatal period. In Italy, there is a lack of knowledge about how mental health services support women with perinatal mental disorders (PMDs). This study aims to describe the adoption of good practices for the prevention and care of PMDs by the Italian mental health departments (MHDs). Methods This is a nationwide cross-sectional survey conducted online using LimeSurvey. Starting from the Ministry of Health Registry’s 127 MHDs were invited to participate between February and March, 2023. Characteristics of the participating MHDs were reported as descriptive statistics. Results One hundred nineteen MHDs participated, with a response rate of 93.7%. Regarding the prevention of PMDs, 69 (58.0%) MHDs offer preconception counseling, whereas only 6 (5.0%) have information material for this purpose. Written integrated care pathways for PMDs are not available in 94 (79.7%) MHDs. A reference professional for psychopharmacological treatment during pregnancy or breastfeeding is available in 55 (46.2%) MHDs, while a specific treatment plan for women with PMDs is adopted by 27 (22.7%) MHDs. Thirty-four (28.6%) MHDs have established an outpatient clinic for PMDs, whereas there are no inpatient psychiatric facilities designed for mothers and infants (mother-baby units). Conclusions There is a need to improve the care of women with PMDs in Italy. The provision of pre-conception counseling, integrated care pathways, and specialist skills and facilities for PMDs should be prioritized.

Abstract

Call-by-Push-Value (CBPV) is a programming paradigm subsuming both Callby-Name (CBN) and Call-by-Value (CBV) semantics. The essence of this paradigm is captured by the Bang Calculus, a (concise) term language connecting CBPV and Linear Logic. This paper presents a revisited version of the Bang Calculus, called $\lambda !$, enjoying some important properties missing in the original formulation. Indeed, the new calculus integrates permutative conversions to unblock value redexes while being confluent at the same time. A second contribution is related to nonidempotent types. We provide a quantitative type system for our $\lambda !$-calculus, and we show that the length of the (weak) reduction of a typed term to its normal form plus the size of this normal form is bounded by the size of its type derivation. We also explore the properties of this type system with respect to CBN/CBV translations. We keep the original CBN translation from $\lambda$-calculus to the Bang Calculus, which preserves normal forms and is sound and complete with respect to the (quantitative) type system for CBN. However, in the case of CBV, we reformulate both the translation and the type system to restore two main properties: preservation of normal forms and completeness. Last but not least, the quantitative system is refined to a tight one, which transforms the previous upper bound on the length of reduction to normal form plus its size into two independent exact measures for them.

Abstract

Tetrodotoxin (TTX) is a potent neurotoxin that was first identified in pufferfish but has since been isolated from an array of taxa that host TTX-producing bacteria. However, determining its origin, ecosystem roles, and biomedical applications has challenged researchers for decades. Recognized as a poison and for its lethal effects on humans when ingested, TTX is primarily a powerful sodium channel inhibitor that targets voltage-gated sodium channels, including six of the nine mammalian isoforms. Although lethal doses for humans range from 1.5-2.0 mg TTX (blood level 9 ng/mL), when it is administered at levels far below LD50, TTX exhibits therapeutic properties, especially to treat cancer-related pain, neuropathic pain, and visceral pain. Furthermore, TTX can potentially treat a variety of medical ailments, including heroin and cocaine withdrawal symptoms, spinal cord injuries, brain trauma, and some kinds of tumors. Here, we (i) describe the perplexing evolution and ecology of tetrodotoxin, (ii) review its mechanisms and modes of action, and (iii) offer an overview of the numerous ways it may be applied as a therapeutic. There is much to be explored in these three areas, and we offer ideas for future research that combine evolutionary biology with therapeutics. The TTX system holds great promise as a therapeutic and understanding the origin and chemical ecology of TTX as a poison will only improve its general benefit to humanity.

132. The population of Galactic planetary nebulae: a study of distance scales and central stars based on the second GAIA release

Abstract

We matched the astrometry of central stars (CSs) of spectroscopically-confirmed Galactic planetary nebulae (PNe) with DR2 Gaia parallaxes ($p$), finding 430 targets in common with $p>0$ and $|\sigma_{\rm p}/p|<1$. A catalog of PNe whose CSs have DR2 Gaia parallaxes is presented in Table 1. We compared DR2 parallaxes with those in the literature, finding a good correlation between the two samples. We used PNe parallaxes to calibrate the Galactic PN distance scale. Restricting the sample to objects with 20$\%$ parallax accuracy, we derive the distance scale ${\rm log}(R_{\rm pc})=-(0.226\pm0.0155)\times{\rm log}(S_{\rm H\beta})-(3.920\pm0.215)$, which represents a notable improvement with respect to previous ones. We found that the ionized mass vs. optical thickness distance scale for Galactic PNe is not as well constrained by the Gaia calibrators, but gives important insight on the nature of the PNe, and is essential to define the domain for our distance scale application. We placed the CSs whose distance has been determined directly by parallax on the HR diagram, and found that their location on the post-AGB H-burning evolutionary tracks is typical for post-AGB stars., Comment: The Astrophysical Journal, in press

Skew Boolean algebras (skew BA) and Boolean-like algebras (nBA) are one-pointed and n-pointed noncommutative generalisation of Boolean algebras, respectively. We show that any nBA is a cluster of n isomorphic right-handed skew BAs, axiomatised here as the variety of skew star algebras. The variety of skew star algebras is shown to be term equivalent to the variety of nBAs. We use skew BAs in order to develop a general theory of multideals for nBAs. We also provide a representation theorem for right-handed skew BAs in terms of nBAs of n-partitions.

Abstract

During the peak phase of the COVID-19 pandemic, alterations of standard operating procedures were necessary for health systems to protect patients and healthcare workers and ensure access to vital hospital resources. As the peak phase passes, re-activation plans are required to safely manage increasing clinical volumes. In the context of cardiovascular magnetic resonance (CMR), re-activation objectives include continued performance of urgent CMR studies and resumption of CMR in patients with semi-urgent and elective indications in an environment that is safe for both patients and health care workers.

Abstract

In response to the coronavirus disease 2019 (COVID-19) pandemic, The Cardio-Oncology and Imaging Councils of the American College of Cardiology offers recommendations to clinicians regarding the cardiovascular care of cardio-oncology patients in this expert consensus statement. Cardio-oncology patients-individuals with an active or prior cancer history, and with or at risk of cardiovascular disease-are a rapidly growing population, who are both at increased risk of infection by COVID-19 and experiencing severe and/or lethal complications. Recommendations for optimizing screening and monitoring visits to detect cardiac dysfunction are discussed. In addition, judicious use of multimodality imaging and biomarkers are proposed to identify myocardial, valvular, vascular, pericardial involvement in cancer patients. The difficulties of diagnosing the etiology of cardiovascular complications in patients with cancer and COVID-19 are outlined, along with weighing the advantages against risks of exposure, with the modification of existing cardiovascular treatments and cardiotoxicity surveillance in patients with cancer during the COVID-19 pandemic.

149. Manipulation and use of sound waves for engineering applications

Abstract

Over the last decade, the sound wave manipulation has become a focal point for many engineering applications, such as negative refraction, subwavelength imaging, cloaking and one-way transmittance. Only recently the sound wave manipulation has been deeply investigated to achieve full sound absorption because of the growing interest in noise mitigation and cancellation. However, the achieving of such challenging results is usually constrained by the general properties of naturally available materials, and this is the main reason for the emergence of engineered structures and metamaterials. In this thesis, the sound wave manipulation has been studied and applied in two main fields: material properties evaluation and perfect sound absorption. In the first case, a new approach based on the impedance tube test rig is proposed to evaluate not only the acoustic properties but also the elastic properties of the tested material. The proposed method results an innovative non-destructive approach to evaluate the out-of-plane elastic modulus for composite and porous materials. Moreover the sound manipulation due to the sound wave and resonator structures interaction has been investigated with the main challenging purpose of low frequencies sound absorption using subwavelength structure. Therefore, in the following chapters of the thesis the developed new metamaterials are based respectively on Microperforated panel (MPP), elastic membrane, Graphene Oxide (GO) and Honeycomb structures. The sound absorption performances were studied through numerical and analytical models which helps to understand the absorption mechanism for each proposed metamaterial. Prototypes were manufactured for each of proposed metamaterial unit cells and their absorption performances experimental tested using an impedance tube test rig. The results demonstrated how the proposed metamaterials represents subwavelength and tuneable devices which allows almost perfect and broadband absorption at low frequencies.
