OBJECTIVE. Human Papillomavirus (HPV)-related squamous cell carcinoma of the oropharynx (HPV-OPSCC) usually originates from the reticulated epithelium lining the crypts of Waldeyer’s ring. The increasing epidemiologic evidence of this type of tumors and the peculiar positive prognosis, suggested to introduce a dedicated staging system (1). The 8th edition of the American Joint Committe on Cancer (AJCC) staging manual presented a new classification to discriminate HPV-related cancers based on p16 over-expression, since it demonstrated to be an important biomarker (2). The detection of HPV-DNA could not be used as a defining factor due to its cost and lack of universal applicability. The main purpose of this preliminary study was to evaluate the concordance between the results of immunohistochemistry for p16 overexpression and the HPV-DNA detection performed with a commercial kit as control. MATERIALS AND METHODS. We retrospectively collected 32 cases of OPSCC diagnosed in 2016 at the Academic Hospital of Cattinara in Trieste, whose samples had been tested with Real Time PCR (Pyro Mark System Q96 IDTM - CE-IVD Diatech Pharmacogentetics Iesi Italy). The study was performed using tumor samples fixed in buffered formalin, embedded in paraffin and stored at room temperature for 6 to 18 months. All samples were selected by a pathologist based on sections stained with hematoxylin and eosin to confirm the adequacy of the tissue. The samples were tested for p16 by immunohistochemistry (CINtech® p16 Histology E6H4 ROCHE) and the positivity was assessed when nuclear expression was ≥+2/+3 intensity and ≥75% distribution according to the 8th edition of the AJCC staging manual. In the context of oropharyngeal cancer, the optimal cut-off for p16 overexpression is ≥+2/+3 intensity (+/- cytoplasmic staining) with ≥75% distribution. RESULTS. Seven cases were excluded because of lack of histological stored material for p16 immunohistochemical analysis (only cytological specimen). Among the remaining 25 cases, there were 5 HPV-positive and 20 HPV-negative tumors. The sensibility and specificity of the p16 immunostaining was equal to 100% (95%, confidence interval 83.4%-100%) and 100% (95%, confidence interval 83.4%-97.8%), respectively; the positive predictive value was 100% (95%, confidence interval 83.4%-100%) and the negative predictive value 100% (95% confidence interval, 83.4%-100%). CONCLUSIONS. In agreement with the literature, p16 overexpression demonstrated to be a reliable parameter to identify the cases of OPSCC in which performing an in-depth analysis by means the detection of HPV-DNA would be indicated. Indeed, it is widely known that the association of p16 immunostaining and HPV-DNA detection by quantitative PCR increases the prognostic discriminator (3). We intend to carry on this study adopting a prospective design increasing the sample size and the test accuracy.

Human papillomavirus (HPV)-associated head and neck squamous cell carcinoma (HNSCC) is an entity with peculiar clinical and molecular characteristics, which mainly arises from the reticulated epithelium lining the crypts of the palatine tonsils and the base of the tongue. The only head and neck site with a definite etiological association between persistent high-risk (HR) HPV infection and development of SCC is the oropharynx. HPV-positive malignancies represent 5-20% of all HNSCCs and 40-90% of those arising from the oropharynx, with widely variable rates depending on the geographic area, population, relative prevalence of environment-related SCC and detection assay. HPV-16 is by far the most common HR HPV genotype detected in oropharyngeal SCC (OPSCC), and the only definitely carcinogenic genotype for the head and neck region. Patients with HPV-induced OPSCC are more likely to be middle-aged white men, non-smokers, non-drinkers or mild to moderate drinkers, with higher socioeconomic status and better performance status than subjects with HPV-unrelated SCC. HPV-induced HNSCCs are often described as non-keratinizing, poorly differentiated or basaloid carcinomas, and are diagnosed in earlier T-category with a trend for a more advanced N-category, with cystic degeneration, than the HPV-unrelated carcinomas. HPV positivity is associated with better response to treatment and modality-independent survival benefit. Treatment selection in HPV-related oropharyngeal carcinoma is becoming a critical issue, and although there is no evidence from randomized, controlled trials to support a treatment de-escalation in HPV-positive SCC, some investigators argue that intensive combined modality strategies may represent an overtreatment.Il carcinoma squamoso testa-collo associato al papillomavirus umano (HPV) è una patologia con peculiari caratteristiche cliniche e molecolari che origina principalmente dall'epitelio criptico delle tonsille palatine e linguale. L'orofaringe rappresenta, infatti, l'unica sede per la quale a tutt'oggi sussista un sicuro nesso eziologico tra infezione da HPV e insorgenza di un carcinoma squamoso. I tumori maligni HPV-positivi rappresentano il 5-20% di tutti i carcinomi testa-collo e il 40-90% di quelli che originano dall'orofaringe, con tassi variabili di prevalenza in funzione dell'area geografica, del peso relativo degli altri fattori di rischio, della popolazione considerata e dei metodi di rilevamento del virus. Il paziente con tumore HPV-associato è più spesso un maschio di razza bianca, di mezza età, non-fumatore, nonbevitore o bevitore occasionale, presenta uno status socio-economico più elevato e un performance status migliore rispetto al soggetto con carcinoma HPV-negativo. Dal punto di vista istologico i tumori HPV-associati sono più spesso descritti come carcinomi non cheratinizzanti, scarsamente differenziati, con aspetti basaloidi e si presentano con una categoria T meno avanzata e una categoria N più avanzata, con aspetti di degenerazione cistica, rispetto ai tumori HPV-negativi. I carcinomi HPV-associati presentano una miglior risposta al trattamento e hanno una sopravvivenza migliore indipendentemente dal tipo di trattamento. La selezione del trattamento dei carcinomi orofaringei HPV-correlati sta diventando un punto critico poiché, nonostante non vi siano evidenze derivate da trials randomizzati controllati per giustificare una de-intensificazione del trattamento nei carcinomi squamosi HPV-positivi, alcuni ricercatori sostengono che una strategia di trattamento multimodale intensificata possa costituire in tali casi un over-treatment.

PURPOSE: A complete economic evaluation requires accurate data concerning the resources used, outcomes, and utilities (patient's preferences) to properly value the cost utility of human papillomavirus (HPV) vaccination strategies. This study was designed to measure the utility loss in health states affected by a broad range of HPV-induced pathologies in both sexes in Italy. As a secondary objective, risk factors influencing the viral transmission and development of HPV infections were also investigated. METHODS: Patients with a diagnosis of several HPV-induced pathologies including atypical squamous cells of undetermined significance (ASC-US), cervical intraepithelial neoplasia (CIN), cervical and anal-colorectal cancer, head and neck squamous cell carcinoma (HNSCC) and anogenital warts (AWs) were evaluated. Utilities, quality of life, and risk factors were elicited using a standardized and computer-guided administration of time trade-off, European Quality of Life 5 Dimensions (EQ-5D), 3 levels, and risk factor questionnaires. Utilities were measured at 6 clinical research centers across Italy. A group of healthy subjects was used as a control. A mean number of 20 healthy subjects was used as a control for each pathology group. FINDINGS: Overall, 600 respondents were eligible for analysis: 465 patients (mean [SD] age, 44.0 [16.3] years) and 135 controls (mean [SD] age, 44.0 [13.2] years). With the exception of anal and HNSCC cancer, no statistically significant differences were observed between case and control groups, in terms of either age or quality of life at the time of interview. The patients' perception of their health condition at baseline was equal to an EQ-5D score of 0.87 (0.22). The mean (SD) value of utilities associated with the HPV-induced pathologies corresponded to 0.83 (0.24), 0.78 (0.27), 0.83 (0.22), 0.81 (0.27), 0.58 (0.31), 0.51 (0.26), and 0.69 (0.30) for ASC-US, AWs, CIN 1 (mild), CIN 2-3 (moderate to severe), cervical cancer, anal cancer and HNSCC, respectively. Utility lost due to AWs was significantly higher in females compared with males (0.71 [0.29] vs 0.83 [0.25]; P = 0.018). Having >5 sexual partners increased the risk of acquiring HPV-induced infections as much as 2.52-fold (P = 0.004), whereas for smoking or the age at start of sexual activity younger than 18 years, the risk increased by ~1.62-fold (P = 0.034). High levels of education were associated with a statistically significant protective effect (P < 0.001). IMPLICATIONS: Risk factors and utilities elicited in this study can be used as part of future economic assessments of other HPV vaccination strategies, including an immunization program for preadolescents of both sexes in Italy.

Acinic cell carcinoma (ACC) is a rare tumor generally involving the parotid gland and infrequently the minor salivary glands with the potential for both local recurrence and metastatic spread when tracked for decades. The biological behavior of ACC cannot be predicted on the basis of histological features, and surgical stage is still the best predictor of clinical outcome. Only 5 cases of ACC of the larynx have been reported in the English literature. The authors present a case of a rare transglottic ACC in a 74-year-old woman. At admission, a submucosal mass involving the left arytenoid and adjacent aryepiglottic fold was noted. A CT scan of the head and neck region showed a mass of the left hemilarynx involving the paraglottic space and extending from the aryepiglottic fold to the sinus piriformis. Definitive histopathological examination showed an ACC with a large amount of clear cells. The patient was treated by radiotherapy alone (66 Gy in 7 weeks) with complete remission.

To evaluate whether there is an association between deep-sea fishing and common chronic disease.The study was cross-sectional, simultaneously considering groups of fishermen and non-fishermen. Information on life-style and work was collected by means of questionnaires, and clinical data were collected by specialists in: Internal Medicine (general clinical examination), Cardiology (ECG, measurement of arterial pressure), Pneumology (measurement of spirometric volumes), ENT (clinical examination of the ear, nose and throat, including audiometry), Ophthalmology (examination of lens). Beside the common statistical methods, the logistic stepwise regression analysis was used in order to find the risk factors of the diseases, and to correct the risk estimates for the confounding variables.Fisherman had prolonged hours of continuous work, which were found to be correlated with high cigarette and alcohol consumption. Significant associations were found between, on the one hand, work accidents, noise-induced hearing loss, solar keratosis, cataracts, obstructive bronchitis, rhino-sinusitis, otitis media with tympanic perforation, ECG alterations, and, on the other hand, various aspects of fisherman occupation, mainly fishing in high sea and work duration as fisherman.Deep-sea fishing is a stressful and risky work; a reduction in the number of years at sea with reduced exposure to noise, poor weather conditions and sun, and a lower consumption of cigarettes and alcohol might result in fewer skin, eye respiratory and cardiovascular diseases, and injuries.
