Abstract Purpose: To investigate the prophylactic and therapeutical effects of sildenafil in a model of acute radiation proctitis (ARP). Methods: All experimental procedures of this study was examined by histopathological, immunohistochemical and transmission electron microscopic analysis. Results: Our histopathological evaluations indicated significant increases in lesion severity, cryptic apsis, cryptitis, cryptic distortion, reactive atypia and infiltration depth of the control (proctitis) group. While the prophylaxis group and the treatment group had significantly lower scores. High-dose group showed similar results as prophylaxis group. Histopathological findings of the prophylaxis group was more significant than the treatment group. Immunoreactivities of IL-1β, FGF-2, TNF- α and HIF-1α increased in the control group especially in the epithelial and cryptic regions. On the contrary, sildenafil application caused significant decreases of inflammatory markers in all treatment groups, specifically better results in the prophylaxis group. Conclusion: The sildenafil has anti-inflammatory effects on ARP, as well as protective effects against ARP and the protective effect of sildenafil surpasses its therapeutic effect histopathologically.

Petersen fıtığı gastrojejunostomi anastomozu bacağı ile transvers mezokolon arasındaki boşluğa ince bağırsakların migrasyonu ile oluşan spesifik ve nadir bir internal herni türüdür. Öncelikle peptik ülser ya da gastrik kanser cerrahisi için yapılan subtotal gastrektomiler sonrası bildirilirken Roux-en-Y gastrik bypass (RYGB) ve biliopankreatik diversiyon ameliyatlar sonrası görülme sıklığı artmıştır. Bu olgu sunumuzda peptik ülser nedeniyle distal gastrektomi RYGB anastomoz yapıldıktan 20 yıl sonrası gelişen Petersen fıtığını sunmayı amaçladık. Acil servise karın ağrısı ve kusma şikâyetleri ile başvuran olgumuzun yapılan radyolojik tetkikleri sonucu ileus ve internal herniasyon ön tanısıyla yapılan laparatomisinde distal gastrektomi ve RYGB uygulanmış olduğu görüldü. Eksplorasyona devam edildiğinde rou bacağı ile transverkolon mezokolon arasındaki boşluğa (Petersen Boşluğu) ince bağırsakların fıtıklaştığı izlendi ve rezeksiyona gerek kalmadan redükte edilerek defekt onarıldı. Ülser cerrahisinden son dönemde uzaklaşılmasına rağmen dünyada artan sayıda obezite cerrahisi yapılmaya başlanmış olup, RYGB cerrahisi sonrası oluşan internal herniasyonlar da literatürde önem kazanmaya başlamıştır. Laparoskopik ya da açık cerrahide bu potansiyel defektlerin çeşitli yöntemlerle kapatılması postoperatif komplikasyon oranını düşürecektir.

Amaç: Kraniyotomi operasyonlarında ekstübasyon sırasında gelişebilecek istenmeyen hemodinamik değişiklikleri ortadan kaldırmak için, çalışmamızda lidokain+lidokain, lidokain+esmolol etkisini karşılaştırmayı amaçladık.Gereç ve Yöntem: Kraniyotomi uygulanacak ASA I-III grubu 60 hasta çalışmaya dahil edildi. Anestezi idamesi ise sevofluran-remifentanil kombinasyonu ile sağlandı. Postoperatif analjezi 2 mg kg-1 intravenöz tramadol ile sağlandı. Ameliyat bitimine 10 dakika kala hemodinamik parametreler kaydedildikten sonra tüm hastalara 1mg kg-1 bolus lidokain intravenöz yapıldı. Hastalar rastgele iki gruba ayrılarak Grup I’ de (grup Lidokain+Lidokain) 0,025mg kg sa-1 lidokain, Grup II’ de (grup Lidokain+Esmolol) 50 µg kg dk-1 esmolol infüzyonu başlandı. Hastaların lidokain ve esmolol infüzyonları sonrası 1.-5. ve 10. dakikada, hemodinamik parametreleri kaydedildi. Bulgular: Grup I’ de preoperatif döneme göre ekstübasyon sonrasında hemodinamik değişikliklerde anlamlı artış gözlendi. Grup II’ de esmolol infüzyonu sonrası 10. dakika ve ekstübasyon sonrasındaki hemodinamik değişikliklerde preoperatif döneme göre anlamlı artış belirlendi. Grup II’ de ekstübasyon zamanı, göz açma, sözel yanıt, oryantasyon süresi grup I’ den anlamlı olarak daha yüksekti.Sonuç: Kraniyotomi uygulanan olgularda ekstübasyon öncesinde uygulanan lidokain+lidokain infüzyonunun lidokain+esmolol infüzyonuna göre ekstübasyon sırası ve sonrasında daha stabil bir hemodinami sağladığı, ekstübasyon süresinin daha kısa olduğu kanısına varıldı.

This paper considers the reference signal generation problem for the multi-functional operation of single-phase dynamic voltage restorers. For this purpose, a single-phase quasi type-1 phase-locked loop (QT1-PLL) is proposed. The pre-loop filter part of this PLL is composed of a frequency-fixed delayed signal cancellation method and a two-stage all-pass filter. Thanks to the frequency-fixed nature, the pre-loop filter is easy to implement and can provide rejection of any measurement offset. Moreover, this PLL benefits from the excellent harmonic robustness property of the conventional QT1-PLL. Small-signal modeling and gain tuning procedures are detailed in this paper. In order to track the reference voltage signals generated by the proposed PLL, a super-twisting sliding mode controller is also presented, which helps to achieve fast dynamic responses. Laboratory-scale prototype-based experimental studies were conducted to validate the developed reference generator and the controller. Experimental results show that the proposed method is fast in detecting and compensating any grid voltage anomalies to maintain constant load voltage despite voltage sag, swell, and harmonic distortions.

A plethora of research emphasizes the importance of performance feedback as an adaptive way to improve the outcomes of undergraduate students. However, research on negative performance feedback (i.e. communicating the results of a critical assessment related to student performance) and the potential unintended negative consequences remains fragmented and non-comprehensive. The current systematic review provides an integrative synthesis of the literature that details the unintended negative consequences of negative performance feedback on undergraduate students. It also identifies the contexts in which these consequences are more likely to occur. Using the 36 articles that fit the study's search criteria, we found negative effects on self-efficacy, cognition, affect, and behavior. These factors were moderated by the characteristics of the feedback provider and the feedback receiver, the feedback style, and demographic variables. Based on our review, we propose a new integrative model. This study contributes to the literature and practice of teaching and learning by providing educators with an up-to-date review of negative feedback. It also provides a multidisciplinary examination and identifies future research directions. [ABSTRACT FROM AUTHOR]

The leading position of English as a global language has evidently been continuing for several decades. This pivotal role has inevitably been influencing the agenda of English language teaching and teacher education in most domains of the profession. Although English as a lingua franca (ELF) implications and practices on teaching and teacher education are increasingly researched, they still remain a fairly untrodden territory. For this very aim, this study endeavored to shed lights on the role of teacher education programs through exploration of pre-service language teachers' perceptions on ELF related issues through a questionnaire and interviews. The findings revealed that although a large number of participants accepted the realities of ELF, however, they stated that their perspectives and teaching practices were largely shaped by inner circle native norms of English. The study has significant implications for teacher education programs involving language instructors, students, curriculum designers and material developers.

Amaç: Bu prospektif ve randomize çalışma, elektif cerrahi geçirecek 65 yaş ve üzeri ASA II-III hastalarda, sevofluran ve desfluran anestezisinde tek doz uygulanacak esmololün hemodinamik değişiklikler, entübasyona bağlı Bispektral indeks (BIS) değişiklikleri ve derlenme üzerine etkilerini karşılaştırmayı amaçlamaktadır. Gereç ve Yöntem: 65 yaş üzeri ASA II-III elektif cerrahi geçirecek 80 hasta 4 gruba ayrıldı. Tüm hastalara standart monitörizasyon ve BIS monitörizasyonu uygulandıktan sonra Grup I deki hastalara sevofluran ve intravenöz(IV) salin, Grup II deki hastalara desfluran ve IV salin, Grup III deki hastalara sevofluran ve tek doz esmolol (1 mg kg-1), Grup IV deki hastalara desfluran ve tek doz esmolol (1 mg kg-1) uygulandı. Hastaların indüksiyon ajan uygulanması sonrası, esmolol uygulanması sonrası, kas gevşetici uygulanması sonrası, entübasyon esnasında, entübasyon sonrası oksijen saturasyonu, sistolik kan basıncı, diyastolik kan basıncı, kalp atım hızı ve bispektral indeks değerleri kaydedildi. Bulgular: Tek doz esmolol uygulamasının sevofluran ve desfluran anestezisinde endotrakeal entübasyona bağlı BIS değerlerindeki artışı engellediği görüldü. Gruplar karşılaştırıldığında esmolol uygulanan gruplarda ekstübasyon, göz açma ve derlenme süreleri daha kısa bulundu. Sonuç: İleri yaş grubu hastalarda desfluran ve sevofluran anestezisinde tek doz uygulanan esmolol trakeal entübasyona yanıt olarak gelişen sempatik deşarjı ve BİS değerlerindeki artışı baskılamaktadır.

Kaposi sarcoma (KS) is a rare condition mostly seen in immunosuppressed patients due to a syndrome or organ transplantation. However, few cases have been reported in patients with rheumatologic diseases treated with long-term corticosteroid and cyclophosphamide. We present a case in which the subject developed KS following a course of immunosuppressive therapy for their systemic vasculitides.

The analgesic benefit of melatonin and vitamin C as primary or adjuvant agents has been reported in various studies; however, their analgesic effects in the treatment of postoperative pain remain unclear. Thus, we aimed to evaluate the effect of single preoperative dose of oral melatonin or vitamin C administration on postoperative analgesia. In this study, we recruited 165 adult patients undergoing elective major abdominal surgery under general anesthesia. Patients were randomly divided into three equal (n = 55) groups. One hour before surgery, patients received orally melatonin (6 mg) in group M, vitamin C (2 g) in group C, or a placebo tablet in group P. Pain, sedation, patient satisfaction, total morphine consumption from a patient-controlled analgesia device, supplemental analgesic requirement, and the incidence of nausea and vomiting were recorded throughout 24 h after surgery. The mean pain score and total morphine consumption were found significantly lower in both M and C groups compared with group P (p < 0.001). There were no significant differences between group M and C with respect to pain scores (p = 0.117) and total morphine consumption (p = 0.090). Patients requested less supplemental analgesic and experienced less nausea and vomiting in groups M and C compared with group P. In conclusion, preoperative oral administration of 6 mg melatonin or 2 g vitamin C led to a reduction in pain scores, total morphine consumption, supplemental analgesic requirement, and the incidence of nausea and vomiting compared with placebo.
