109. ADHD and risk for subsequent adverse childhood experiences: understanding the cycle of adversity.

Abstract

Background: Children with adverse childhood experiences (ACEs) are more likely to develop Attention‐Deficit/Hyperactivity Disorder (ADHD). The reverse relationship – ADHD predicting subsequent ACEs – is vastly understudied, although it may be of great relevance to underserved populations highly exposed to ACEs. Methods: Participants were 5‐ to 15‐year‐olds (48% females) with (9.9%) and without ADHD (DSM‐IV criteria except age of onset) in a longitudinal population‐based study of Puerto Rican youth. In each wave (3 yearly assessments, W1‐3), ten ACEs (covering parental loss and maladjustment and child maltreatment) were examined, plus exposure to violence. Logistic regression models examined ADHD (including subtypes) and subsequent risk for ACEs. Also considered were interactions by age, sex, number of W1 ACEs, and recruitment site. Results: Children with W1 ADHD were more likely to experience subsequent adversity (OR: 1.63; 95% CI: 1.12–2.37) accounting for child age, sex, public assistance, maternal education, site, disruptive behavior disorders, and W1 ACEs. Inattentive (OR: 2.00; 95% CI: 1.09–3.66), but not hyperactive/impulsive or combined ADHD, predicted future ACEs. Conclusions: ADHD predicts subsequent risk for ACEs, and the inattentive presentation may confer the most risk. Inattentive presentations could pose a bigger risk given differences in symptom persistence, latency to access to treatment, and treatment duration. The present study suggests a pathway for the perpetuation of adversity, where bidirectional relationships between ADHD and ACEs may ensnare children in developmental pathways predictive of poor outcomes. Understanding the mechanism underlying this association can help the development of interventions that interrupt the cycle of adversity exposure and improve the lives of children with ADHD. Abstract

Sexual attraction (SA), the earliest stage of sexual orientation, is scarcely studied. This prospective study examined, over 3 years, prevalence, changes in SA, and the role of context, among 946 Puerto Rican youth, aged 11-13 years at initial assessment in the South Bronx (SBx), New York City, and Puerto Rico (PR). Overall, 98.1% of boys and 95.3% of girls reported opposite-sex only SA at some point, whereas 13.8% of girls and 12.0% of boys reported any-same SA. Opposite-sex only SA increased over time, whereas other SAs decreased except for any same-sex SA among SBx girls. Girls in the SBx and younger youth in PR reported more any same-sex SA. Abstract

Spiritism is an institution of religious belief that still exists in some parts of the world. In areas where it is practiced, it would be interesting to know the relationship it has to mental health in the community. Unfortunately, due to such factors as the covert practice of spiritism, little is known about this relationship. Reincarnation is one of the basic beliefs of spiritism. Spiritists tell of fluids that surround the body. The fluids may be sick, leading to mental or physical illness. In Puerto Rico, where spiritism is thought to be practiced by somewhere between 30 and 60 percent of the population, there is a greater report of psychiatric symptoms than in other North American regions. It is also true that those who utilize the services of spiritists have a greater incidence of mild to moderate psychiatric disorders. Spiritists are used as an outlet and are consulted as mental health professionals. To better understand the relationship between spiritism and psychiatric symptomatology, an island-wide study of Puerto Ricans was conducted. Out of a random sample, 119 (18 percent) had consulted a spiritist and 534 never had. Subjects were rated with a psychiatric symptom scale and sociodemographic data were collected. It was found that, contrary to findings from previous studies, use of a spiritist did not correlate with higher incidence of risk for diagnosable psychiatric illness, but signs of depression were more likely in those subjects who reported that they had consulted a spiritist. It was also found that spiritists may play an important social-support role for Puerto Ricans. They do not appear to play a major role in mental health care, however. Abstract

Among maltreated children, elevated use of non-routine (for illness or injury) services may coexist with underutilization of preventive services. Besides physical health problems, lack of contact with primary care may preclude the identification and delivery of appropriate interventions. We examined health service utilization in the longitudinal Boricua Youth Study of Puerto Rican children residing in the South Bronx (SBx), New York City (n = 901), and San Juan metropolitan area, Puerto Rico (n = 1,163). Parents and children (M age = 9 years) reported on child physical abuse, sexual abuse, and neglect. Parents reported if their child had been to illness, injury, and well-child visits in the past year. In the SBx site, caretakers were more likely to report the children seeing a doctor for a well-child visit (90%) compared with children in Puerto Rico (71%). Children in Puerto Rico were more likely to visit a doctor for an injury in the past year compared with children in the SBx (39% vs. 24%). Twenty-one percent of children in the SBx reported maltreatment versus 16% in Puerto Rico. Adjusting for sociodemographic factors, compared with non-maltreated children, those who experienced two or more types of maltreatment were more likely to have an illness visit in Puerto Rico (prevalence ratio [PR] = 1.5, 95% confidence interval [CI] = [1.1, 2.2]) and the SBx (PR = 1.8, 95% CI = [1.1, 3.0]), or an injury visit (PR = 4.1, 95% CI = [1.9, 8.9]) in Puerto Rico only. Children in the SBx who reported only one type of maltreatment were less likely to use services for injuries than non-maltreated children (PR = 0.42, 95% CI = [0.2, 0.9]). No relation between maltreatment and well-child visits was noted. Children who experience maltreatment may frequently come in contact with health care providers, presenting opportunities for intervention and the prevention of further maltreatment. Abstract

Background: Parenting behaviors have been shown to moderate the association between sensation seeking and antisocial behaviors. Methods: Data were obtained from the Boricua Youth Study, a longitudinal study of 2,491 Puerto Rican youth living in the South Bronx, New York, and the metropolitan area of San Juan, Puerto Rico. First, we examined the prospective relationship between sensation seeking and antisocial behaviors across 3 yearly waves and whether this relationship varied by sociodemographic factors. Second, we examined the moderating role of parenting behaviors—including parental monitoring, warmth, and coercive discipline—on the prospective relationship between sensation seeking and antisocial behaviors. Results: Sensation seeking was a strong predictor of antisocial behaviors for youth across two different sociocultural contexts. High parental monitoring buffered the association between sensation seeking and antisocial behaviors, protecting individuals with this trait. Low parental warmth was associated with high levels of antisocial behaviors, regardless of the sensation seeking level. Among those with high parental warmth, sensation seeking predicted antisocial behaviors, but the levels of antisocial behaviors were never as high as those of youth with low parental warmth. Conclusions: Study findings underscore the relevance of person–family context interactions in the development of antisocial behaviors. Future interventions should focus on the interplay between individual vulnerabilities and family context to prevent the unhealthy expression of a trait that is present in many individuals. Abstract

Aggression (e.g., assaulting others, bullying, oppositionality; AGG) and non-aggressive rule-breaking (e.g., lying, stealing, vandalism; RB) appear to constitute meaningfully distinct dimensions of antisocial behavior. Despite these differences, it is equally clear that AGG and RB are moderately-to-strongly intercorrelated with one another. To date, however, we have little insight into the sampling and methodologic characteristics that might moderate the association between AGG and RB. The current study sought to evaluate several such moderators (i.e., age, sex, informant, and society) in a sample of 27,861 parent-adolescent dyads from 25 societies. AGG and RB were assessed with the well-known Child Behavior Checklist and Youth Self-Report (Achenbach & Rescorla, 2001). Results revealed small effects of informant and adolescent sex, such that the association between AGG and RB was stronger for parents' reports than for adolescents' self-reports, and for boys than for girls. The association also varied by society. Unexpectedly, the specific operationalization of 'aggression' emerged as a particularly strong moderator, such that the association was stronger for a general measure of AGG than for a more focused measure of physical aggression per se. Abstract

Previous research has documented the association between the Latino cultural idiom of distress, ataques de nervios (i.e., “attacks of nerves”), and unexplained neurological symptoms among adults. However, the associations between ataques and somatic complaints in children have not been sufficiently explored. Aims: In this study, we assessed the relation between this anxiety‐related experience, henceforth ataques, and somatic complaints in a probability sample of Puerto Rican youth, ages 5–13 years, living in San Juan, Puerto Rico (N = 1353) and in the South Bronx, New York (N = 1138). When both sites were combined, children with ataques were significantly more likely to have either a lifetime prevalence of asthma or headaches, and tended to have more stomach aches and a history of epilepsy or seizure than children without ataques. Further within site analyses showed a similar patterning of complaints for the South Bronx sample as for the combined sample. However, children in San Juan with ataques were only slightly more likely to experience headaches, and at risk for injury, than those without ataques. In addition, comparisons between ataque sufferers across sites indicated that children in San Juan with ataques were at elevated risk for serious illness or injury in comparison to those in the South Bronx with ataques. Ataques are significantly associated with a wide range of physical complaints in Puerto Rican youth. However, their pattern of associations differs by context.

Abstract

To date, no community study has examined the psychiatric aspects and or sequelae of Lyme disease (LO) among children. As part of a community epidemiologic study of psychiatric disorders among children ages 9 through 17 in a Lyme endemic county, parents were asked whether their child had ever been diagnosed as having LD, and 10.1% (36/357) responded yes to the LO question. Of the 36, 29 also agreed to take part in a follow-up interview. Sixteen of the 29 children had had physician-diagnosed LO as well as either an erythema migrans rash or a positive serology. Fifteen of these 16 received treatment within I month of symptom onset; none of these 15 children were symptomatic longer than 4 months. Only one child had physical symptoms at the time of the interview; she was not treated until 4 month~ after symptom onset. This child experienced 5 years of mtermittent arthritis, cognitive deficits, emotional problems, severe fatigue, and a deterioration in school performance. Courses of oral antibiotics were at first associated with a good response, followed by a resurgence of symptoms month
