This study aimed to characterize the natural progression and recurrence of new-onset postoperative atrial fibrillation (POAF) during an intermediate-term follow-up post cardiac surgery by using continuous event monitoring.New-onset POAF is a common complication after cardiac surgery and is associated with an increased risk for stroke and all-cause mortality. Long-term data on new POAF recurrence and anticoagulation remain sparse.This is a single-center, prospective observational study evaluating 42 patients undergoing cardiac surgery and diagnosed during indexed admission with new-onset, transient, POAF between May 2015 and December 2019. Before discharge, all patients received implantable loop recorders for continuous monitoring. Study outcomes were the presence and timing of atrial fibrillation (AF) recurrence (first, second, and more than 2 AF recurrences), all-cause mortality, and cerebrovascular accidents. A "per-month interval" analysis of proportion of patients with any AF recurrence was assessed and reported per period of follow-up time. Kaplan-Meier analysis was used to calculate the time to first AF recurrence and report the first AF recurrence rates.Forty-two patients (mean age 67.6 ± 9.6 years, 74% male, mean CHADSIn this study of continuous monitoring with implantable loop recorders, the recurrence of AF in patients who develop transient POAF is common in the first month postoperatively. Of the patients who developed postoperative AF, 76% had any recurrence in months 1 to 12, and 30% had any recurrence beyond 1-year follow-up. Current guidelines recommend anticoagulation for POAF for 30 days. The results of this study warrant further investigation into continued monitoring and longer-term anticoagulation in this population within the context of our findings that AF duration was30 minutes beyond 1 month.

In order to cope up with the increase in energy demand and decline in fossil fuels, it has become imperative to use renewable resources efficiently. Among these renewable resources, solar thermal energy is abundant in nature. Solar water heating systems are one of the most important applications of solar thermal energy. Providing internal fins to absorber tube is the technique to improve heat transfer augmentation. Hence in the present study, experiments were performed on solar flat plate collector with different cross section of absorber tubes (plain tube and internally grooved tubes with different helix angles) and by varying the mass flow rates of the working fluids. This study reports the experimental results of flat plate collector, where the working fluid is water and aqueous ethylene glycol (50 : 50). Temperature profile of grooved absorber tube will be compared with plain tube. Since conversion efficiency of solar devices is low, the present study mainly focuses on improving the efficiency of solar flat plate collector.

Background: A large number of newborns are affected by congenital heart diseases, which accounts for a high proportion of infant mortality worldwide. But there are regional variations in the prevalence and distribution pattern of congenital heart diseases. We evaluated the incidence, risk factors and diagnostic clues for congenital heart disease in newborns in our neonatal intensive care unit. Methods: A prospective observational study was conducted in the neonatal intensive care unit of Vydehi institute of medical sciences between January 2013 to December 2016. Congenital heart disease was suspected if clinical and confirmed by 2D echocardiography and doppler and colour flow imaging. Data was collected on a predesigned proforma containing information regarding gender, gestational age and weight at birth, family history, and associated malformations. Results: Out of 1974 babies screened for CHDs, 126 babies had CHDs (4.6%) of which 71(56%) were males. Most of neonates presented with Respiratory distress (52.3%), murmurs (49.5%), abnormal chest X ray (27.7%), followed by cyanosis (18.2%) in Cyanotic CHDs(100% of the cases). Conclusion: The incidence of CHD in newborns at the neonatal intensive care unit was higher than that in all live births.VSD was the most common congenital heart defect detected while Tetralogy of Fallot was commonest among the cyanotic heart defects.

The array of microstrip-fed slot-coupled patches is one such type of antenna that provides low return loss and good transient behaviour for broadband applications. A compact size slot-coupled patch antenna subarray and its phased array is proposed in this paper. Two different substrates Rogers TMM4 and Green TapeTM 951 Low Temperature Co-Fired Ceramic (LTCC) are considered for the fabrication of microstrip feedline. The feedline is fabricated on the respective substrates and then mounted on a Rogers RT/duroid 5880 laminate. Ground planes are used on both the front and backside of the antenna array. The antenna is operating in a range of 24.25 GHz - 29.5 GHz. Background Endocardial catheter ablation has been shown to be effective in patients with paroxysmal atrial fibrillation (AF), and significantly less effective in patients with persistent AF (PAF). Lately, there is a trend toward a hybrid approach in the treatment of PAF that may be a more durable treatment for patients with PAF. In this manuscript we report our experience with the convergent ablation procedure in a PAF cohort. Methods This is a single center retrospective analysis of 31 patients with PAF who underwent the convergent procedure. All patients underwent surgical epicardial ablation of the posterior left atrial through a subxiphoid approach, followed by radiofrequency endocardial ablations on the same day. Patients were followed at 6 months intervals with static electrocardiograms or implanted devices. Results Sinus rhythm was achieved intraoperatively in all patients. Recurrence was defined according to Hearlt Rhythm Society definitions. At a median follow up of 17.7 months (IQR 11-24), the recurrence of atrial tachyarrhythmia (AF and atrial flutter) by Kaplan-Meier event free survival analysis occurred in 9 (29%) patients at 1-year follow up and 15 (48%) patients at 2-year follow up with or without the use of antiarrhythmic drugs. Recurrence of AF alone occurred in 4 (13%) patients at 1-year follow up and 9 (29%) patients at 2-year follow up patients. Complication rate in perioperative period was 12.9%. Conclusion Our experience showed the hybrid procedure is a relatively safe and effective option for patients with PAF. Further studies are needed to better determine its long-term outcomes.

In percutaneous coronary intervention, the knuckle wire technique is one of the approaches to cross the long and ambiguous course of the occluded segment. However, this technique is generally used as a last alternative, when all other techniques fail. Although knuckle wiring expedites chronic total occlusion crossing, it can also complicate the percutaneous coronary intervention strategy irreversibly. Therefore, understanding the various aspects of the knuckle wire technique is a prerequisite in a chronic total occlusion setting. The authors herein intend to describe in detail the knuckle wire technique and its safe and effective approach in various chronic total occlusion wiring strategies, while befitting to the scope of a mainstream interventionist.

In this paper, a study of loading the array of SRSRRs antenna designs is conducted. By conducting parametric study on the number of the SR-SRR unit cells for loading, miniaturization ability has been realized. By increasing the number of the SR-SRR unit cells, the monopole resonance has been brought down from 4.35 GHz to 2.55 GHz with a little impedance mismatch. The explanation and validation of the above technique is done with help of the simulated field distribution and parameter extraction of resonator unit cell. The simulation of antenna models were carried out with Ansys HFSS.

Background: Allergic rhinitis (AR) is the most prevalent atopic disease in the world. It involves clinical hypersensitivity of the nasal mucosa to foreign substances mediated through immunoglobulin E antibodies. The quantitative Score for allergic rhinitis (SFAR) ranging between 0 and 16 has been in use from 2002.Methods: 240 patients diagnosed with Persistent allergic rhinitis (PAR) were taken to be part of the study after proper consent irrespective of the severity of symptoms. Each patient filled out the SFAR form before the treatment commenced. Serum IgE levels were noted and treatment was started which included oral and topical antihistamines, oral leukotriene receptor antagonists along with intranasal steroids. The IgE values were assessed along with the SFAR values side by side to look for any association. Results: It was noted that the individuals with identified triggers of allergy and those with a positive family history of allergy had a considerable higher IgE value as compared to those who did not. The ones with a trigger identified as a cause had a 54.88% higher IgE value. The ones with positive family history had 47.12% higher IgE than the ones without a history of allergy.Conclusions: Patients with well-defined trigger agents of allergy and positive family history of allergy are at a higher risk as they have a higher IgE value. They should be advised regular follow-ups and be monitored closely.

Introduction: Neonatal pneumonia is a serious respiratory infection accounting for 10% of the neonatal deaths. Objective: The aim and objective of our study was to evaluate the accuracy of lung ultrasound to diagnose neonatal pneumonia. Methods: A 3-year prospective observational study was done between October 2013 to September 2016 in the neonatal ICU of Vydehi institute of medical sciences. About 86 neonates presented with features of pneumonia of which 21 cases had no x ray or ultrasonographic features. The control group comprised 65 neonates with no pulmonary disease are taken into the control group and compared. Results: 86 neonates were included in this study with clinical features of pneumonia of which 21 cases had no x rays or ultrasonographic features, so were not taken into the study group. Of the 65 cases included in the study; 39 were males and 26 females. In the 65 cases studied, 63 (96%) had tachypnea, retractions in 42(64%) and grunting in 38(58%) of the cases. Three (4.6%), of the studied cases died while the remaining 62(93%) cases improved and were discharged. Conclusion: In conclusion, the study confirms that lung ultrasonography is useful in the diagnosis of neonatal pneumonia.

Background: Normally third portion of duodenum is always retromesenteric-retroperitoneal. Demonstration of intramesentric position of D3 on ultrasound can help in diagnosis of malrotation. Objective: The aim of this study was to evaluate the feasibility of Ultrasound in demonstrating the retroperitoneal D3 for early and effective diagnosis of malrotation. Methods: A 5-year prospective observational study was done between April 2011 to March 2016 in the neonatal ICU of Vydehi institute of medical sciences. 122 neonates presented with features of vomiting and abdominal distension. After ruling out all medical and surgical causes (other) possibility of malrotation was suspected in 41 babies were included in the study. Plain abdominal radiography, sonography and upper gasto intestinal barium examinations were performed in all the 41 suspected cases. Results: All the 41 cases were evaluated and later surgical intervention was done. 26 out of 41 cases had ultrasonographic features of malrotation, which were subsequently proved by surgery. Inversions of Superior mesenteric artery and Superior mesenteric vein were found in all the 26 cases that were surgically proved of malrotation. Other features like thickened duodenal wall greater than 2mm in 16 cases, intraperitoneal location of third part of duodenum in 13 cases, distal dilatation of the SMV in 11 cases, duodenal dilatation with tapering configuration in 10 cases and ascites in 14 cases were found. Conclusion: Ultrasonography provides good diagnostic results in neonatal intestinal malrotation. Specific Sonographic features relating to volvulus should be evaluated as potential indicators of the need for an emergent operation.

Objective: To study the incidence and risk factors predisposing to retinopathy of prematurity (ROP) in Bapuji child Health Institute NICU. Design: Prospective cohort observational study. Setting: Infants admitted to a neonatal intensive care unit of Bapuji child Health Institute NICU in a period of two years. Methods: Preterm infants with birth weight 1500g and gestation >34weeks were screened only if they had additional risk factors. Those found to have high risk ROP had laser photocoagulation. Results: The incidence of ROP in the 200 infants who were screened was 13.5%. No ROP was found in infants weighing >2000g or with a gestational age more than 36weeks. Risk factors predisposing to ROP were oxygen therapy (P=0.04), apnea [p=0.001], ventilation [0.001] , anemia [0.001],blood and blood product transfusion [0.001 ]. Conclusion: One third of the infants with ROP needed laser photo coagulation, the outcome of which was good. Risk factors predisposing to ROP were oxygen therapy, apnea, ventilation, blood transfusion, exchange transfusion.

Design of Experiments (DOE) was conducted toanalyze the impact of cutting parameters on the MaterialRemoval Rate (MRR) and Surface Roughness (SR) by usingTaguchi method. By conducting Analysis of Variance(ANOVA) the results are optimized to determine minimumsurface roughness and maximum MRR. Response SurfaceOptimizer was used for obtaining optimum settings to carryout the machining operations effectively.

To increase the energy and spectral efficiency in the field of communication, a new technique named Device to Device Communication (D2D) is proposed. This is very essential as there has been a massive growth in the number of connected devices now a days. D2D takes advantage of the physical proximity of communication devices and enables direct communications between them (i.e.) devices communicate with each other without the intermediate nodes. This proximity of devices helps in achieving extremely high bit rates, low delays, and low power consumption. Although there are many benefits of D2D, the implementation of it is very strenuous. There are a number of challenges need to be addressed before establishing a successful D2D connection. One among those is the clustering of devices. Clustering is the grouping of devices based on a particular criteria. In this paper, we first created nodes, found which devices are capable of forming a D2D link, divided them into clusters and then found the cluster head (CH) for each cluster. Finally we plotted graphs for throughput versus the threshold SNR and the distance between the links.

Idiopathic intracranial hypertension is rare in children and associated with high cerebrospinal fluid pressure, with no evidence of intracranial malformation. The etiology is unknown but it is associated with endocrine disorders, drugs, toxins and viral infections. Investigations such as lumbar puncture and imaging are usually normal. We report a child who presented with features of elevated intracranial pressure following measles vaccination. His investigations were all found to be normal and was treated with acetazolamide following which he recovered.

A Simultaneously Dual Circularly Polarized planar monopole antenna for broad band wireless applications is presented. The antenna is compact and occupies less space (60mm×60mm×l.6mm) and suitable for low cost PCB fabrication techniques. The orthogonal circular polarizations on the opposite sides of the antenna are achieved essentially by placing the two perpendicular monopoles of almost same dimensions along with strategically shaped ground. The design of a miniaturized slot antenna based on SRR and CSRR loading is presented. The SRR and CSRR were properly placed to get excited by the electromagnetic fields of the slot antenna. The miniaturization was done with out increasing lateral or longitudinal dimensions of the existing antenna. A miniaturization of approximately 22% is achieved. The radiation characteristics of the miniaturized antenna are almost similar to that of the unloaded slot with low cross polarization levels. The resonance frequency of slot antenna was brought down from 4.2 GHz to 3.3 GHz with loading. The simulations were carried out using the ANSYS HFSS.

This paper proposes an unsupervised debate stance classification algorithm. In other words, finding the side a post author is taking in an online debate. Stance detection has a complementary role in information retrieval, opinion mining, text summarization, etc. Existing stance detection techniques are not able to effectively handle two challenges: determine whether a given post is a debate or not? If the post is a debate on a given topic, correctly classify the side that the post author is taking. In this paper, we propose techniques that addresses both the above issues. Compared to existing technique, our technique gives 30% improvement in detection of whether a post is a debate or not. Our technique is able to find the side that an author is taking in a debate by 10% higher F1 score compared to existing work. We achieve this improvement by using new syntactic rules, better aspect popularity detection, co-reference resolution, and a novel integer linear programming model to solve the problem.

In this paper, we present TRAFAN, a system to analyze traffic activities of various cities in India. Traffic information is publicly available on various social networking sites. We aim to analyze the transportation data moving through the most popular social networking site Facebook. We take the data from various Facebook traffic pages as input to our system. Our system performs three important functions: fast browsing through keyword search result, compare Facebook pages and get statistical summaries about the Facebook pages. Our system can assist the government organizations to know the most chaotic traffic issue in a city. Organizations can retrieve all the problems in a city related to a traffic issue. The system also allows to compare the severity of an issue across the cities. We perform our experiment on Facebook pages containing 21,000 posts and 0.5 million reactions. We show through experimental results that our system effectively analyzes the traffic activities of cities. We present the results using information visualization techniques to improve the usability of our proposed TRAFAN interface.

INTRODUCTION: Neglected rupture of the patellar tendon is a rare disabling injury that is technically difficult to manage. Many different surgical techniques have been described for reconstruction of the disrupted extensor mechanism of the knee. In this study we describe an improved technique for the reconstruction of patellar tendon using semitendinosus and gracilis tendon grafts preserving their t ibial insertion distally and Ethibond suture augmentation instead of stainless steel wire having the added advantage of no further operative intervention needed for removal. MATERIALS AND METHODS: The study included 9 patients (7 male & 2 female) with mean age of 40.2 years (+/ - 2 SD) presented with pain, instability, difficulty in carrying out Activities of Daily Living (ADL) associated with neglected patellar tendon injury. The time since injury ranged from 3months to 2years. All the patients had loss of a ctive extension, extensor lag between 30 0 to 50 0 with an average of 41.1 0 (+/ - 2 SD) and severe functional limitation of ADL. All the patients underwent patellar tendon reconstruction using semitendinosus and gracilis tendon grafts. The functional outcome was assessed using Lysholm Knee Score, Visual Analogue Score (VAS) and IKDC scoring system. RESULTS: Post - operatively with an average follow - up of 19 months (+/ - 2 SD) all the patients had decreased amount of pain, stable knee with active extension of knee without any extension lag with flexion up to 110 0 (90 0 - 125 0 ). Out of 9 patients 7 had good and two had fair functional outcome with improvement in ADL with the IKDC score of 83.5 (+/ - SD), Lysholm Knee Score 90.8 (+/ - SD) and no/little pain on Visual Anal ogue Scale. CONCLUSION: The results of our study shows that the use of ipsilateral semitendinosus and gracilis tendon grafts preserving their insertions on tibia distally for reconstruction of neglected patellar tendon ruptures along with Ethibond suture a ugmentation provides good knee stability and functional improvement in ADL without need for allograft or prosthetic material. The use of Ethibond suture instead of stainless steel wire has added advantage of no further opera tive intervention needed for its removal.

Giant cell tumour of the bone (GCT) is a rare locally aggressive primary bone tumour with an incidence of 3% to 5% of all primary bone tumours. The most common location for this tumour is the long bone meta - epiphysis especially of the distal femur , proximal tibia , distal r adius , and the proximal humerus. Involvement of distal ulna is rare accounting for 0.45% to 3.2%. Considering local aggressive nature and high recurrence , wide resection is the treatment recommended. Instability of ulnar stump and ulnar translation of the carpals are known complications following resection of distal ulna. To overcome these problems , we attempted a newer technique of distal ulna reconstruction using proximal fibula and TFCC reconstruction using Palmaris longus tendon following wide resection of giant cell tumour of distal ulna in a 44 - year - old male. This technique of distal radio ulnar joint reconstruction has excellent functional results with no evidence of recurrence after one - year follow - up

In this paper, a combination of different wavelength injection techniques is demonstrated in a bidirectional WDM PON's implementation using arrayed waveguide grating. Wavelength reusable ONU's are provided with fiber bragg grating at particular wavelength, so that it reflects particular wavelength and transmits the remaining power, which is re-modulated as upstream by LiNbO3 modulator. Self seeding based ONU's are provided with reflective semiconductor optical amplifier, so that it not only function as amplifier, but also as a modulator for upstream generation(no need of local sources). Externally wavelength injected ONU's are provided with local source for upstream generation. The integration of all three wavelength injection techniques makes a true next generation PON technologies. The performance of implemented system has been analyzed on the basis of Min-log of BER versus OSNR, Min-log of BER versus received power. The performance of wavelength reusable based ONU's are close to externally injected ONU's performance in both downstream and upstream, so that it can be used in place of externally wavelength injected with link-length trade-off. Self-seeding based ONU's performance is close to externally wavelength injected ONU's performance below 50Km link-length in both downstream and upstream, so that it can be used in place of externally wavelength injected with upstream data rate trade-off.
