111. Spinal Hematoma Formation Following Neuraxial Anethesia in the Anticoagulated Patient

Abstract

Spinal hematoma formation continues to be a risk factor following neuraxial anesthesia. This is largely due to the challenges that nurse anesthesia providers are faced with when patients present with altered coagulation. Anticoagulation guidelines have been formulated, but anesthesia professionals have been unable to arrive at a common consensus regarding the implementation of spinal and epidural procedures. Therefore, nurse anesthetists are required to make decisions based on their best judgment, rather than a consistent guideline, in attempt to ensure patient safety. The purpose of this independent project was to explore the association between neuraxial anesthesia, anticoagulant medications, and spinal hematoma formation. Commonly administered anticoagulants, essential laboratory data, and timing of needle placement and catheter removal were reviewed and presented to Registered Nurse Anesthetists and students in training. The ultimate goal of the presentation was to increase the knowledge and comfort level of anesthesia providers performing regional techniques . A comprehensive literature review that included: available guidelines, retrospective reviews, case reports, and prospective studies was conducted. The findings were compiled and presented in a power point format, which displayed the association between spinal hematoma formation and anticoagulation therapy. The physiologic framework of adaptation and homeostasis was used as the theoretical basis for the project. An informational pocket guide that explored the doses, frequency, and timing of anticoagulant medication administration in relation to the neuraxial anesthetic technique was developed. Relevant laboratory data that should be assessed prior to needle placement or catheter removal was also incorporated into the guide. The pocket guide was distributed to anesthesia providers and students in training to serve as a reference for the safe administration of regional anesthesia.

112. Abscisic acid and its relationship to somatic embryogenesis in Dactylis glomerata L.

Abstract

The purpose of this study was to determine if abscisic acid (ABA) was related to genotype differences of orchardgrass (Dactylis glomerata L.) for producing somatic embryos from leaf sections cultured in vitro. Leaf sections of one embryogenic (Embryogen-P) and one nonembryogenic (I- 39) genotype were exposed to various concentrations of ABA in the culture medium for differing lengths of time to determine the effect of exogenous application. Endogenous levels of the two innermost leaves were analyzed by high performance liquid chromatography (HPLC) and enzyme linked immunosorbent assay (ELISA) and compared in one embryogenic and two nonembryogenic genotypes. Comparisons were also made between basal (0-3 cm) and distal (3-6 cm) portions of the innermost leaves of one embryogenic and one nonembryogenic genotype. In the embryogenic genotype, somatic embryogenesis was enhanced by a low level (1 μM) of ABA, while higher levels (10-100 μM) inhibited embryogenesis. Duration of exposure to ABA was also found to be significant, with an initial 3 day exposure period being most effective for enhancing embryo formation. The nonembryogenic genotype showed no response to ABA application in any of the treatments. Quantification studies indicated a significant difference in ABA concentrations between the embryogenic and nonembryogenic genotypes. When quantified by immunoassay, the embryogenic genotype contained more ABA in basal and distal sections of inner leaves than did the nonembryogenic genotype, 1-39; however, the second leaf of the nonembryogenic genotype contained more ABA than the embryogenic genotype. High performance liquid chromatography quantification results of combined first and second leaves indicated a significantly higher endogenous level of ABA in the nonembryogenic genotypes than in the embryogenic genotype. There was no significant difference between the two nonembryogenic genotypes. Abscisic acid immunoassay results indicate that ABA levels decreased as much as 93% in both embryogenic and nonembryogenic genotypes during a 2 week culture period after initial plating. These results suggest that both concentration and duration of application of ABA influence somatic embryo formation.

Abstract

BACKGROUND Little is known about current clinical practice concerning peri-operative red blood cell transfusion in neonates and small infants. Guidelines suggest transfusions based on haemoglobin thresholds ranging from 8.5 to 12���g���dl-1, distinguishing between children from birth to day 7 (week 1), from day 8 to day 14 (week 2) or from day 15 (���week 3) onwards. OBJECTIVE To observe peri-operative red blood cell transfusion practice according to guidelines in relation to patient outcome. DESIGN A multicentre observational study. SETTING The NEonate-Children sTudy of Anaesthesia pRactice IN Europe (NECTARINE) trial recruited patients up to 60 weeks' postmenstrual age undergoing anaesthesia for surgical or diagnostic procedures from 165 centres in 31 European countries between March 2016 and January 2017. PATIENTS The data included 5609 patients undergoing 6542 procedures. Inclusion criteria was a peri-operative red blood cell transfusion. MAIN OUTCOME MEASURES The primary endpoint was the haemoglobin level triggering a transfusion for neonates in week 1, week 2 and week 3. Secondary endpoints were transfusion volumes, 'delta haemoglobin' (preprocedure - transfusion-triggering) and 30-day and 90-day morbidity and mortality. RESULTS Peri-operative red blood cell transfusions were recorded during 447 procedures (6.9%). The median haemoglobin levels triggering a transfusion were 9.6 [IQR 8.7 to 10.9] g���dl-1 for neonates in week 1, 9.6 [7.7 to 10.4] g���dl-1 in week 2 and 8.0 [7.3 to 9.0] g���dl-1 in week 3. The median transfusion volume was 17.1 [11.1 to 26.4] ml���kg-1 with a median delta haemoglobin of 1.8 [0.0 to 3.6] g���dl-1. Thirty-day morbidity was 47.8% with an overall mortality of 11.3%. CONCLUSIONS Results indicate lower transfusion-triggering haemoglobin thresholds in clinical practice than suggested by current guidelines. The high morbidity and mortality of this NECTARINE sub-cohort calls for investigative action and evidence-based guidelines addressing peri-operative red blood cell transfusions strategies. Abstract

Introduction Total volumes of physical activity and sedentary behaviour have been associated with cardio-metabolic risk profiles; however, little research has examined whether patterns of activity (e.g., prolonged bouts, frequency of breaks in sitting) impact cardio-metabolic risk. The aim of this review was to synthesise the evidence concerning associations between activity patterns and cardio-metabolic risk factors in children and adolescents aged 5-19 years.Materials and methods A systematic search of seven databases was completed in October 2017. Included studies were required to report associations between objectively-measured activity patterns and cardio-metabolic risk factors in children and/or adolescents, and be published between 1980 and 2017. At least two researchers independently screened each study, extracted data, and undertook risk of bias assessments.Results From the 15,947 articles identified, 29 were included in this review. Twenty-four studies were observational (cross-sectional and/or longitudinal); five were experimental. Ten studies examined physical activity patterns, whilst 19 studies examined sedentary patterns. Only one study examined both physical activity and sedentary time patterns. Considerable variation in definitions of activity patterns made it impossible to identify which activity patterns were most beneficial to children's and adolescents' cardio-metabolic health. However, potential insights and current research gaps were identified.Discussion and conclusion A consensus on how to define activity patterns is needed in order to determine which activity patterns are associated with children's and adolescents' cardio-metabolic risk. This will inform future research on the impact of activity patterns on children's and adolescents' short- and longer-term health. Refereed/Peer-reviewed
