Abstract

To evaluate a combined indocyanine green-enhanced optical imaging/radiography system for the detection of arthritic joints in a rat model of antigen-induced arthritis.Arthritis of the knee and ankle joints was induced in 6 Harlan rats, using peptidoglycan-polysaccharide polymers. Three rats served as untreated controls. Optical imaging of the knee and ankle joints was done with an integrated optical imaging/radiography system before and up to 24 hours following intravenous injection of 10 mg/kg indocyanine green. The fluorescence signal intensities of arthritic and normal joints were compared for significant differences, using generalized estimating equation models. Specimens of knee and ankle joints were further processed and evaluated by histology.Immediately after administration, indocyanine green provided a significant increase in the fluorescence signal of arthritic joints compared with baseline values (P0.05). The fluorescence signal of arthritic joints was significantly higher compared with that of nonarthritic control joints at 1-720 minutes after intravenous injection (P0.05). Fusion of indocyanine green-enhanced optical imaging and radiography allowed for anatomic coregistration of the inflamed tissue with the associated joint. Hematoxylin and eosin staining confirmed marked synovial inflammation of arthritic joints and the absence of inflammation in control joints.Indocyanine green-enhanced optical imaging is a clinically applicable tool for detection of arthritic tissue. Using relatively high doses of indocyanine green, long-term enhanced fluorescence of arthritic joints can be achieved. This may facilitate simultaneous evaluations of multiple joints in a clinical setting. Fusion of indocyanine green-enhanced optical imaging scans with radiography increases anatomic resolution.

113. Diffusion imaging of the vertebral bone marrow.

Abstract

Diffusion-weighted MRI (DWI) of the vertebral bone marrow is a clinically important tool for the characterization of bone-marrow pathologies and, in particular, for the differentiation of benign (osteoporotic) and malignant vertebral compression fractures. DWI of the vertebral bone marrow is, however, complicated by some unique MR and tissue properties of vertebral bone marrow. Due to both the spongy microstructure of the trabecular bone and the proximity of the lungs, soft tissue, or large vessels, substantial magnetic susceptibility variations occur, which severely reduce the magnetic field homogeneity as well as the transverse relaxation time T*2, and thus complicate MRI in particular with echoplanar imaging (EPI) techniques. Therefore, alternative diffusion-weighting pulse sequence types such as single-shot fast-spin-echo sequences or segmented EPI techniques became important alternatives for quantitative DWI of the vertebral bone marrow. This review first describes pulse sequence types that are particularly important for DWI of the vertebral bone marrow. Then, data from 24 studies that made diffusion measurements of normal vertebral bone marrow are reviewed; summarizing all results, the apparent diffusion coefficient (ADC) of normal vertebral bone marrow is typically found to be between 0.2 and 0.6 × 10−3 mm2/s. Finally, DWI of vertebral compression fractures is discussed. Numerous studies demonstrate significantly greater ADCs in osteoporotic fractures (typically between 1.2 and 2.0 × 10−3 mm2/s) than in malignant fractures or lesions (typically 0.7-1.3 × 10−3 mm2/s). Alternatively, several studies used the (qualitative) image contrast of diffusion-weighted acquisitions for differentiation of lesion etiology: a very good lesion differentiation can be achieved, particularly with diffusion-weighted steady-state free precession sequences, which depict malignant lesions as hyperintense relative to normal-appearing vertebral bone marrow, in contrast to hypointense or isointense osteoporotic lesions. Abstract

Osteoporosis is defined as a progressive systemic skeletal disease characterized by diminishing bone mass and deterioration of the microarchitecture of bone tissue, resulting in a loss of mechanical strength and susceptibility to fracture. Typical fractures associated with osteoporosis (fragility fractures) are found in the spine, hip and wrist. Osteoporosis can be classified as primary or secondary. According to the time of manifestation primary Osteoporosis is termed as juvenile, post-menopausal or senile. A wide spectrum of causes may lead to secondary osteoporosis. The most common conditions include osteoporosis due to drugs (e.g. corticoids, phenytoin) renal or hepatic insufficiency, hyperparathyreoidism, hyperthyroidism, metastases or hematologic bone marrow neoplasia. Abstract

Purpose: Preservation of the joint line in total knee arthroplasty (TKA) has shown to be an important factor for the long-term outcome, especially in revision TKA. For unicompartmental knee arthroplasty (UKA), the role of the joint line has neither been investigated nor is it consciously respected during implantation. Thus, the aim was to establish and validate a standardised measurement method to determine the joint line in UKA. Methods: As there is no established method to evaluate changes in the joint line radiologically, we introduced two methods and correlated them. The methods were first validated in a cadaver model by a controlled rotational study. Then, the joint line of 29 patients with an UKA (Oxford, Biomet, Bridgend, UK) was determined on pre- and post-operative radiographs. Both methods were tested by intra- and inter-rater reliability. Results: Both methods showed a good intra- and inter-rater reliability. Furthermore, there was only little bias in agreement between both methods and raters. Measurements of the 29 UKA patients revealed that the joint line was more distally by a mean of 4.4 ± 1.2 mm after surgery. Conclusions: The study provides for the first time a reliable and standardised measurement tool to determine the changes in the joint line after implantation of an UKA. The instrument should be used in further studies to evaluate the impact of the joint line on the long-term outcome, the load in the two non-replaced knee compartments and on the ligaments.
