Abstract

Main Recommendations ESGE recommends offering stone extraction to all patients with common bile duct stones, symptomatic or not, who are fit enough to tolerate the intervention. Strong recommendation, low quality evidence. ESGE recommends liver function tests and abdominal ultrasonography as the initial diagnostic steps for suspected common bile duct stones. Combining these tests defines the probability of having common bile duct stones. Strong recommendation, moderate quality evidence. ESGE recommends endoscopic ultrasonography or magnetic resonance cholangiopancreatography to diagnose common bile duct stones in patients with persistent clinical suspicion but insufficient evidence of stones on abdominal ultrasonography. Strong recommendation, moderate quality evidence. ESGE recommends the following timing for biliary drainage, preferably endoscopic, in patients with acute cholangitis, classified according to the 2018 revision of the Tokyo Guidelines: - severe, as soon as possible and within 12 hours for patients with septic shock - moderate, within 48-72 hours - mild, elective. Strong recommendation, low quality evidence. ESGE recommends endoscopic placement of a temporary biliary plastic stent in patients with irretrievable biliary stones that warrant biliary drainage. Strong recommendation, moderate quality of evidence. ESGE recommends limited sphincterotomy combined with endoscopic papillary large-balloon dilation as the first-line approach to remove difficult common bile duct stones. Strong recommendation, high quality evidence. ESGE recommends the use of cholangioscopy-assisted intraluminal lithotripsy (electrohydraulic or laser) as an effective and safe treatment of difficult bile duct stones. Strong recommendation, moderate quality evidence. ESGE recommends performing a laparoscopic cholecystectomy within 2 weeks from ERCP for patients treated for choledocholithiasis to reduce the conversion rate and the risk of recurrent biliary events. Abstract

In this paper we propose to extend the concept of the Internet of Things to the musical domain leading to a subfield coined as the Internet of Musical Things (IoMUT). IoMUT refers to the network of computing devices embedded in physical objects (Musical Things) dedicated to the production and/or reception of musical content. Musical Things, such as smart musical instruments or smart devices, are connected by an infrastructure that enables multidirectional communication, both locally and remotely. The IoMUT digital ecosystem gathers interoperable devices and services that connect performers and audiences to support performer-performer and audience-performers interactions, not possible beforehand. Abstract

The role of excessive alcohol intake in the development of chronic pancreatitis (inflammation of the pancreas) has been recognized for over 100 years. Alcoholism is also known to be causally involved in the development of cirrhosis of the liver, a chronic degenerative disease of that organ. In recent years, it has also been suggested that cigarette smoking may have an independent causal action in pancreatitis, but the degree to which it is involved remains controversial. Because smoking and drinking frequently coexist as social behaviors, it is difficult to partition the independent risk associated with each behavior. To investigate the relative contributions of alcohol and tobacco as risk factors for cirrhosis of the liver and pancreatitis, a case control study was carried out. Subjects were 103 male patients suffering from alcoholic cirrhosis, 145 patients with chronic pancreatitis, and 264 healthy control patients. Patients with chronic pancreatitis were younger as a group than those with cirrhosis (42 versus 61 years). More of the patients with chronic pancreatitis (94 percent) than with cirrhosis (83 percent) were both drinkers and smokers. Using a multivariate statistical predictive tool (multiple logistic regression), it was determined that daily intake of alcohol is a significant risk factor for developing both pancreatitis and cirrhosis of the liver, whereas smoking is related to the development of pancreatitis, but not cirrhosis. Abstract

International audience; Acoustic design of performance spaces typically places the performers' needs relatively low in the hierarchy of requirements in comparison to the quality of sound for an audience. While there are a number of studies relating to solo performers' and symphony orchestras' preferred acoustic environments, there is a paucity of literature on objective measurements of the impact of acoustic spaces on smaller ensembles. This study aims to bridge this gap by building a methodology for the analysis of changes in ensemble musical expression caused by different acoustic environments. The study finds that there is an association between reverberation time and global tempo for a colocated piano duo and provides a basis for future studies in this area. This work extends previous research in the area of acoustics and musical performance.
