Abstract

THE ACCURATE collection of a 24-hour urine sample is an essential part of many diagnostic protocols and clinical research studies. In young infants, such collections present, at the very least, a difficult nursing problem, since constant supervision is necessary in order to ensure complete collections. Inaccurate specimens of 24-hour urines could lead to spurious test results with possible serious consequences for the patient.Most present methods utilize some form of collecting apparatus which must be tightly applied to the perineum. These range from the "bird feeder" types to variants of a colostomy bag. Skin ulceration, urinary leakage, and fecal contamination are the most frequently encountered problems with these apparatuses. A hammock assembly designed by Hepner and Lubchenco1 has proved useful for separate urine and stool collections in newborn infants. 119. Impact of the Onset of Puberty on Self-Image and Behavior.

Abstract

The impact of the simultaneous occurrence of physiological, cognitive, and social changes associated with the onset of puberty was examined in terms of adolescents' self-image and behavior. As a result of having to cope with these multiple changes, it was hypothesized that young adolescents who have recently experienced the onset of puberty would have a poorer self-image and exhibit more behavior problems than both prepubescent youngsters and more mature adolescents. It was further hypothesized that stage of development would be more highly associated with self-image and behavior than would chronological age. Measures of self-image, delinquent behavior, school behavior and behavior at summer camp were obtained from 97 male subjects attending summer camp in August, 1982. Instruments employed included questionnaires, camp counselor reports, and interviews of subjects and campers. Subjects were assigned to one of five stages of pubertal development based on Tanner's criteria for pubic hair development. Results indicate that grouping adolescents according to stage of development rather than age is a more powerful means of underst and ing their self-image and certain behaviors. The data support the hypothesis that adolescents who have just experienced the onset of puberty do experience a disturbance in self-image in relation to other groups of young people at various stages of pubertal development. Evidence for the mediating influences of social factors including family relationships and peer group interaction was also found. The hypothesized relationship between stage of development and behavior was only partially supported. While the onset of puberty was found to be associated with a rise in behavior problems among young adolescents, the hypothesized reduction in behavior problems with further maturation was not evidence. The findings of this study indicate that young adolescents experience a significant, but not overwhelming disturbance in self-image and that this disturbance is related to physiological development. The intensity of this disturbance and its manifestation in behavior is mediated by social factors.

Abstract

Objective: To identify complications amenable to prevention in adults with glycogen storage disease (GSD) types Ia, Ib, and III and to determine the effect of the disease on social factors.Design: Case series and clinical review.Setting: Referral medical centers in the United States and Canada.Patients: All patients with GSD-Ia (37 patients), GSD-Ib (5 patients), and GSD-III (9 patients) who were 18 years of age or older.Measurements: Ultrasound or radiographic studies identified liver adenomas, nephrocalcinosis, or kidney stones. Radiographic studies identified osteopenia. Reports of the clinical examination, serum chemistry results, and social data were obtained.Results: For patients with GSD-Ia, problems included short stature (90%), hepatomegaly (100%), hepatic adenomas (75%), anemia (81%), proteinuria or microalbuminuria (67%), kidney calcifications (65%), osteopenia or fractures or both (27%), increased alkaline phosphatase (61%) and gamma-glutamyltransferase (93%) activities, and increased serum cholesterol (76%) and triglyceride (100%) levels. Hyperuricemia was frequent (89%). Patients with GSD-Ib had severe recurrent bacterial infections and gingivitis. In patients with GSD-III, 67% (6 of 9) had increased creatinine kinase activity. Four of these patients had myopathy and cardiomyopathy.Conclusions: For GSD-Ia, hyperuricemia and pyelonephritis should be treated to prevent nephrocalcinosis and additional renal damage. For GSD-Ib, granulocyte-colony-stimulating factor may prevent bacterial infections. For GSD-III, more data are required to determine whether the myopathy and cardiomyopathy can be prevented.
