Abstract

Aims/Introduction: A gold standard in the diagnosis of diabetic polyneuropathy (DPN) is a nerve conduction study. However, as a nerve conduction study requires expensive equipment and well‐trained technicians, it is largely avoided when diagnosing DPN in clinical settings. Here, we validated a novel diagnostic method for DPN using a point‐of‐care nerve conduction device as an alternative way of diagnosis using a standard electromyography system. Materials and Methods: We used a multiple regression analysis to examine associations of nerve conduction parameters obtained from the device, DPNCheck™, with the severity of DPN categorized by the Baba classification among 375 participants with type 2 diabetes. A nerve conduction study using a conventional electromyography system was implemented to differentiate the severity in the Baba classification. The diagnostic properties of the device were evaluated using a receiver operating characteristic curve. Results: A multiple regression model to predict the severity of DPN was generated using sural nerve conduction data obtained from the device as follows: the severity of DPN = 2.046 + 0.509 × ln(age [years]) − 0.033 × (nerve conduction velocity [m/s]) − 0.622 × ln(amplitude of sensory nerve action potential [µV]), r = 0.649. Using a cut‐off value of 1.3065 in the model, moderate‐to‐severe DPN was effectively diagnosed (area under the receiver operating characteristic curve 0.871, sensitivity 70.1%, specificity 87.7%, positive predictive value 83.0%, negative predictive value 77.3%, positive likelihood ratio 5.67, negative likelihood ratio 0.34). Abstract

Nonenzymatic glycosylation of structural proteins and the formation of advanced glycosylation end products AGES) have been involved in the pathogenesis of diabetic complications. We examined the effect of aminoguanidine, a potent inhibitor of AGE formation, on functional and structural abnormalities in peripheral nerve of streptozocin-induced diabetic rats. Diabetic rats were treated with dally injections of 25 mg/kg body wt s.c. aminoguanidine (AG)-sulfate for 16 wk and compared to untreated diabetic rats. AG treatment improved motor nerve conduction velocity in 12- and 16-wk diabetic rats despite no changes in body weight, blood glucose, and [HbA.sub.lc] levels. AG treatment inhibited an accumulation of fluorescent AGE in diabetic nerves, and morphometric analysis of the sural nerve showed a partial effect on myelinated fiber size and axonal atrophy. These findings suggest that AG may have a beneficial effect on diabetic neuropathy and nonenzymatic glycosylation of peripheral nerve proteins. Abstract

Aims/Introduction: Diabetic peripheral neuropathic pain (DPNP) affects the functionality, mood and sleep patterns of patients with diabetes. Mirogabalin, an α2δ ligand with a slower dissociation for α2δ‐1 versus α2δ‐2 subunits, showed efficacy and safety in a randomized, double‐blind, placebo‐controlled, 14‐week study in Asian patients with DPNP. This open‐label extension study evaluated the long‐term safety and efficacy of mirogabalin in Asian patients with DPNP. Material and Methods: This 52‐week open‐label extension study was carried out in Japan, Korea and Taiwan in patients with DPNP. Patients received mirogabalin, initiated at 5 mg twice daily and increased to a flexible maintenance dosage of 10 or 15 mg twice daily. Adverse events were monitored throughout the study. Patients provided a self‐assessment of pain using the Short‐Form McGill Pain Questionnaire. Results: Of the 214 patients who entered the study, 172 (80.4%) completed the extension study. Of 172 patients who completed the study, 149 received the highest dosage of mirogabalin (15 mg twice daily). The most common treatment‐emergent adverse events were nasopharyngitis, diabetic retinopathy, peripheral edema, somnolence, diarrhea, increased weight and dizziness. Most treatment‐emergent adverse events were mild or moderate in severity. The incidence of treatment‐emergent adverse events leading to treatment discontinuation was 13.1%. The visual analog scale and all other Short‐Form McGill Pain Questionnaire subscales (sensory score, affective score, total score and present pain intensity) generally decreased over time from baseline until week 52. Conclusions: This extension study showed the safety and efficacy of a long‐term flexible dosing regimen of mirogabalin 10 or 15 mg twice daily in patients with DPNP. [ABSTRACT FROM AUTHOR]

Abstract

Objective: Our objective was to do an epidemiologic survey of patients with multifocal motor neuropathy (MMN) in comparison with those with amyotrophic lateral sclerosis (ALS) in Japan. Methods: In this retrospective study, we examined 46 patients with MMN and 1,051 patients with ALS from major neuromuscular centers in Japan from 2005 to 2009. Diagnosis was based on the European Federation of Neurological Societies/Peripheral Nerve Society (EFNS/PNS) and the revised El Escorial criteria. The efficacy of intravenous immunoglobulin (IVIg) was also taken into consideration in the diagnosis of MMN. Results: The ratio of MMN to ALS patients (0-0.10) varied among the centers, but mostly converged to 0.05. The prevalence was estimated to be 0.29 MMN patients and 6.63 ALS patients per 100,000 population. Conclusions: The frequency of MMN patients was around 1 out of 20 ALS patients, and MMN was possibly underdiagnosed in some centers.

127. Carbon Ion Radiotherapy in Hypo-Fractionation Regimen and Single Dose for Stage I Non-small-Cell Lung Cancer

Abstract

It has been more than one decade since we started carbon ion radiation therapy (CIRT) for non-small-cell lung cancer (NSCLC) in November 1994. From 1994 to 1999, we conducted a phase I/II clinical trial for stage I NSCLC with CIRT and demonstrated an optimal dose of 90 GyE in 18 fractions over 6 weeks and 72 GyE in 9 fractions over 3 weeks for achieving more than 90% local control with minimal pulmonary damage. In the following phase II study from 1999 to 2003, the total dose was fixed at 72 GyE in 9 fractions over 3 weeks and at 52.8 GyE for stage IA and at 60 GyE for stage IB in 4 fractions over 1 week. Targets were irradiated from four oblique direc- tions. A respiratory-gated irradiation system was used for all irradiation sessions. On these two phase II schedules combined, the 5-year local control rate for 13 1 primary tumors of 129 patients was 91.5%. The local control rate for Tl and T2 tumors was 96.3 and 84.7%, respectively. While there was significant difference in control rate between Tl and T2, there was no significant difference in histology between squamous and non-squamous type. The 5-year cause-specific survival rate of the patients was 67.0% (IA: 84.4, IB: 43.7), and their overall survival was 45.3% (IA: 53.9, IB: 34.2). No adverse effects greater than grade III occurred in the lung. In this way, the treatment period and fractionation were shortened and lessened from 18 fractions over 6 weeks to 9 fractions over 3 weeks and further to 4 fractions over one week. Finally it reached a single dose. Since 2003, 210 patients have already been treated with CIRT in single dose increasing 28, 32, 34, 36, 38, 40, 42, 44, 46, and 48 GyE. Compared with the previous fractionation regimen, CIRT in single- dose is demonstrating low morbidity and high QOL. The 5-year local control rate of 131 tumors with doses more than 36 GyE was higher than80%. The 5-year cause-specific and overall sur- vival rate of 131 patients were 1.5 and 52.6%, respectively. Of the whole evaluate, we will finally recommend that CIRT in single dose is the best for the treatment of the peripheral type of stage I NSCLC.

130. Cytological characteristics of pulmonary pleomorphic and giant cell carcinomas

Abstract

Objective: To establish cytological features of pulmonary pleomorphic carcinoma (PC) or giant cell carcinoma (GC), we evaluated the cytological characteristics of these tumors using a multidisciplinary approach. Study Design: Samples from 13 surgically resected and histologically confirmed PC or GC patients were collected from our institutes. Eight cases without prior chemotherapy before surgery were selected, and cytological features were analyzed. Results: The background contained numerous lymphocytes and neutrophils. The tumor cells were arranged in flat loose clusters, but some were in fascicles. The shape of the tumor cell was spindle or pleomorphic, and the sizes of the tumor cells varied by more than 5-fold. The tumor cells had an abundant, thick and well-demarcated cytoplasm. The location of the nucleus was centrifugal, and the nucleus was oval or irregularly shaped. Multinucleated giant cells were frequently observed. The size of the nucleus was more than 5 times that of normal lymphocytes, and its size also varied by more than 5-fold. The nuclear membrane was thin, and nuclear chromatin was coarsely granular, while the nucleolus was single and round. Conclusion: PC or GC has characteristic cytological features, however, spindle cells tended to be hardly observed in cytological specimens in some cases.

131. Carbon Ion Radiotherapy for Lung Cancer

Abstract

Although stage I non-small cell lung cancer can often be cured by surgical resection, in some cases, surgery cannot be performed or is refused by the patient. There is a critical need to develop safe and effective treatments to reduce the mortality of lung cancer in such patients. Carbon ion radiotherapy for lung cancer was first performed in Novenber of 2009, and 918patients have been treated as of December 2010. For peripheral type stage Ilung cancer, the treatment period was gradually shortened from 6 weeks(18fractions)to 3 weeks(9 fractions)to 1week(4 fractions)to determine the safety and efficacy of the treatment. Aclinical trial examining the treatment of patients in a single day is just ending. In the phase II clinical trial, the adverse effect on normal tissue was 91.8% in a five years local control rate, not showing a pulmonary response grade of 3 or more, which indicates clinically problematic cases. It appears that carbon ion radiotherapy contributes to the reduction of lung cancer mortality, and may reprsent a valid alternative local therapy for inoperable cases.

Abstract

In June 1994, the world's first clinical center offering carbon ion radiotherapy opened at the National Institute of Radiological Science (NIRS), Japan. Among several types of ion species, carbon ions were chosen for cancer therapy because they were judged to have the most optimal properties in terms of superior physical and biological characteristics. As of March 2010, 5,196 patients have been registered for carbon ion radiotherapy. Clinical results have shown that carbon ion radiotherapy has the potential to provide a sufficient radiation dose to the tumor, while having acceptable morbidity in the surrounding normal tissues. Tumors that appear to respond favorably to carbon ions include locally advanced tumors as well as histologically non-squamous cell tumor types such as adenocarcinoma, adenoid cystic carcinoma, malignant melanoma, hepatoma, and bone/soft tissue sarcoma. By taking advantage of the unique properties of carbon ions, treatment with small fractions within a short treatment period has been successfully carried out for a variety of tumors. This means that carbon ion radiotherapy can offer treatment for larger numbers of patients than is possible with other modalities over the same time period.

133. Endobronchial ultrasound-guided transbronchial needle aspiration for lymph node staging in patients with non-small cell lung cancerin nonoperable patients pursuing radiotherapy as a primary treatment

Abstract

Introduction: Carbon ion radiotherapy (CIRT) is a promising modality with excellent localization and significant biologic effects on tumors. Nevertheless, success depends primarily on accurate staging before radiotherapy. Surgical interventions should be avoided in patients considered for CIRT because they usually have multiple comorbidities. The aim of this study was to evaluate the effectiveness of endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) for lymph node staging in patients with non-small cell lung cancer before CIRT. \nMethods: From April 2005 to December 2007, 49 patients with non-small cell lung cancer considered for CIRT with abnormal positron emission tomography-computed tomography (PET-CT) accumulations in the mediastinum and/or hilum were evaluated by EBUS-TBNA. The convex probe EBUS was used for EBUS-TBNA. \nResults: There were 38 men and 11 women. Their mean age was 75.2 years (range: 55-87). Based on PET-CT, clinical staging was four with N1 disease, 42 with N2 disease, and three with N3 disease. By histology, 26 patients had adenocarcinoma, 19 had squamous cell carcinoma, and four had other histologies. All positive lymph nodes on PET-CT were aspirated (range: 1-5; average 2.55 lymph nodes/patient). EBUS-TBNA diagnosed 43 cases as N0 disease and as a result underwent CIRT. Forty of the 43 cases remained in stable condition without local recurrences (follow-up 6-46 months). The diagnostic accuracy of EBUS-TBNA for lymph node staging was 93.9%. \nConclusions: EBUS-TBNA offers accurate minimally invasive lymph node staging in patients who are candidates for CIRT. 134. Carbon Ion Therapy for Patients with Locally Recurrent Rectal Cancer

Abstract

Purpose: To evaluate the tolerance for and effectiveness of carbon ion radiotherapy in patients with locally recurrent rectal cancer. Patients and Methods: We conducted a phase I/II dose escalation study of carbon ion radiotherapy . One hundred patients with 105 sites of locally recurrent cancer receiving carbon ion radiotherapy were analyzed. Forty-eight relapses originated in the presacral region, 28 in the pelvic sidewalls, 16 in the perineal region and 8 in the colorectal anastomosis. The total dose ranged from 67.2 to 73.6 gray equivalent (GyE) and was administered in 16 fixed fractions over 4 weeks (4.2to 4.6 GyE/fraction). Results: None of 75 patients treated with the highest total dose of 73.6 GyE experienced National Cancer Institute-Common Toxicity Criteria grade 3 to 5 acute reactions. The local control rate in patients treated with 73.6 GyE in the present study was 97% at one year and 92% at 3 years. Dose escalation was then halted at this leval. The median survival time in patients treated with 73.6 GyE was 54 months (range, 7 to 65 months), and the 1-and 3-year overall survival rates were 71% at 3 years and 39% at 5 years, respectively. Conclusion: Carbon ion radiotherapy seems to be a safe and effective modality in the management of locally recurrent rectal cancer, providing good local control and offering a survival advantage without unacceptable morbidity., II NIRS-CNAO Joint Symposium on Hadrontherapy

136. Carbon Ion Radiotherapy in a Hypofraction Regimen for Stage I Non-Small Cell Lung Cancer

Abstract

From 1994 to 1999,we conducted a phase I/II clinical trial for patients with stage I non-small cell lung cancer(NSCLC)using carbon ion beams alone,demonstrating optimal doses of 90.0GyE in 18 fractions over 6 weeks(Protocol#9701),which led to more than 95%local control with minimal pulmonary damage.In the present study,the total dose was fixed at 72.0GyE in 9 fractions over 3 weeks(Protocol#9802),and at 52.8GyE for stage IA and 60.0GyE for stage IA and 60.0GyE for stage IB in 4 fractions in 1 week(Protocol#0001). Following this schedule, we conducted a phase II clinical trial for stage I NSCLC from 1999 to 2003. We also conducted a phase I/II single fractionation clinical trial(Protocol#0201) as a dose escalation study. The total dose was initially 28.0GyE in 2003, and it was increased to 46.0GyE in 2008. This article describes the intermediate steps. Most targets were irradiated from four oblique directions. A respiratory-gated irradiation system was used for all sessions. Local control and survival were by the Kaplan-Meier method. statistical testing,the Log-rank test was used. The local control rate for all patients(#9802 and #0001) was 91.5%,and for those with T1 and T2 tumors was 96.3% and 84.7%,respectively. while there was significant difference(P=0.0156) in the tumor control rate between patients with T1 and T2 tumors, there was no significant difference(P=0.1516) between squamous cell carcinomas and non-squamous cell carcinomas. The 5-year cause-specific survival rate was 67.0% (IA:84.4,IB:43.7), and overall survival was 45.3%(IA:53.9,IB:34.2). No adverse effects greater than grade 2 ocurred in the lungs. In a single fractionation trial, the 3-year local control rate for 117 patients was 81.9%, and the control rates for patients with T1 and T2 tumors were 84.7% and 78.0%,respectively. No adverse effects greater than grade 2 ocurred in the lungs. Carbon beam radiotherapy, an excellent new modality in terms of high QOL and ADL,was proven to be a valid alternative to surgery for stage I cancer, especially for elderly and inoperable patients.

Abstract

Adenocarcinoma of the pancreas continues to be a significant source of cancer mortality in Japan, resulting in approximately 19,000 deaths a year. It is the fifth leading cause of cancer-related deaths in Japan, with a less than 5% 5-year expected survival rate. About 70-75% of patients with pancreas cancer present with locally advanced disease or distant metastases and have a median survival time of only 6 months. For unresectable pancreas cancer, the median survival time with external beam radiation (EBRT) was better than with surgical bypass or stents alone. The median survival of EBRT alone was 4 to 7 months. The median survival with combined EBRT and chemotherapy for locally unresectable tumor was 8 to 10 months, better than with EBRT alone. Local failure of these combined therapies was still 26 to 48%. On the other hand, surgery with curative intent is undertaken in 15-20% of patients. Even after resection, the predicted 5-year survival rates are still less than 20%. Local recurrence in the pancreatic bed is seen in 50% of the patients undergoing presumed curative resection. We examined the effect of carbon ion therapy in terms of reducing the rate of local recurrence in patients with locally advanced adenocarcinoma of the pancreas or undergoing resection for adenocarcinoma of the pancreas.

145. Specification of Carbon Ion Dose at the National Institute of Radiological Sciences(NIRS)

Abstract

The clinical dose distributions of therapeutic carbon beams, currently used at NIRS HIMAC, are based on in-vitro Human Salivary Gland (HSG) cell survival response and clinical experience from fast neutron radiotherapy. Moderate radiosensitivity of HSG cells is expected to be a typical response of tumours to carbon beams. At first, the biological dose distribution is designed so as to cause a flat biological effect on HSG cells in the spread-out Bragg peak (SOBP) region. Then, the entire biological dose distribution is evenly raised in order to attain a RBE (relative biological effectiveness) = 3.0 at a depth where dose-averaged LET (linear energy transfer) is 80 keV/mum. At that point, biological experiments have shown that carbon ions can be expected to have a biological effect identical to fast neutrons, which showed a clinical RBE of 3.0 for fast neutron radiotherapy at NIRS. The resulting clinical dose distribution in this approximation is not dependent on dose level, tumour type or fractionation scheme and thus reduces the unknown parameters in the analysis of the clinical results. The width SOBP and the clinical / physical dose at the center of SOBP specify the dose distribution. The clinical results analysed in terms of TCP were found to show good agreement with the expected RBE value at higher TCP levels. The TCP analysis method was applied for the prospective dose estimation of hypofractionation.
