Abstract

The reasons for 50 requests for reversal of tubal sterilisation were studied prospectively. Our results agree with those in the rest of the literature and show that within a population of women who have been sterilised there is a group who have "a strong likelihood to ask for reversal of sterilisation". These results make one think of the role of the doctor and can help by using scores that have been worked out previously to improve the indications for sterilisation in case legislation will be forthcoming.

Abstract

The places microsurgery and in vitro fertilization (IVF) hold, respectively, in the treatment of tubal infertility are evaluated. "Optimal" and "acceptable" results of the two techniques are compared, and future results are talked about. Most of the indications for microsurgery are still valid, some are debatable (salpingostomies with bad prognosis, repeated surgery, tubal transplant, etc.), and some new ones emerge. Technically speaking, IVF introduces some changes in surgery of the ovary and surrounding tissues and gives rise to new techniques. Diagnostic and therapeutic management is changed insofar as initial assessment, preoperative coelioscopy and, above all, postoperative monitoring are concerned. The microsurgeon, who is to remain a reproduction specialist, undergoes few personal changes, but his team must be organized on a multi-field basis in order to cope with the new techniques.

Abstract

In this review paper the respective places of microsurgery and in vitro fertilization in the treatment of tubal infertility are evaluated. The best and current results of the two methods are compared and their future is considered. Most of the indications for microsurgery remain, while some (e.g. poor prognosis salpingostomy, repeated tuboplasty, tubal transplantation) require discussion and new ones are appearing. The advent of in vitro fertilization has introduced some technical changes in surgery of the ovaries and surrounding tissues and has given rise to new techniques. It has also modified the initial investigations in infertile women and the use of pre-operative and second-look laparoscopy. It has exerted little influence on microsurgeons specialized in problems of reproduction but has obliged their teams to be reorganized on a multidisciplinary basis in order to cope with new techniques.

Abstract

Two hundred and twenty-three cases notes of patients with toxaemia of pregnancy were analysed in order to try to establish the ability to predict through clinical and biological assessments of the mother the prognosis for the fetus. Statistical analysis methods were chosen to work out the classification of the parameters and to lead to an index of severity that could be established by a multifactorial discriminating analysis. This study confirms that there is an inversion in the circadian rhythm of arterial blood pressure, with an increase in the evening and at night. It shows how bad prognostically blood pressure instability is. On the other hand oedema of the legs is of no significance. Contrary to the view that has been held for a long time, an increase in weight is directly proportional to fetal parameters. As far as parameters are concerned as a whole (both clinical and biological) a rise in blood urea is more discriminating at every stage of the illness and precedes an increase in the platelet count, the changes in creatinine clearance and night blood pressure. Maternal changes are better correlated with fetal variability before the 35th week of the pregnancy whereas after that date the prognostic value of these readings is of little importance.

Abstract

With reference to an observation of porphyria variegata, which was complex and unusual as all such observations are, the direct responsibility of rifampicin is underscored. Porphyria variegata should be considered in two situations. The first of these is a dramatic acute neuroabdominal picture: the diagnosis of acute hepatic porphyria is established by the measurement of urinary porphyrins; the second step is to distinguish between the three acute hepatic porphyrias by looking for cutaneous manifestations and determining the respective proportions of coproporphyrins and protoporphyrins in the stools. In porphyria variegata, fecal protoporphyrins are significantly increased. The second situation is cutaneous involvement suggestive of late-onset cutaneous porphyria: porphyria variegata as well as hereditary coproporphyria in the cutaneous phase must be considered. Diagnosis can be established only through measuring fecal porphyrins. Porphyria variegata is a genetic enzymatic disorder inherited on an autosomal dominant basis. A study of the family is required in all cases, but the conventional methods for detecting heterozygotes for porphyria variegata are not satisfactory. Carriers will be unequivocally distinguished from healthy subjects only through measuring the defective enzyme activity.

Abstract

Water vapor and temperature spatial distribution and their temporal evolution are among the most important parameters in numerical weather forecasting and climate models. The operational relative humidity/temperature profiling in meteorology is carried out mostly by radio sondes. Sondes provide profiles with high vertical resolution but suffer from systematic errors and low temporal resolution. The temporal resolution is also a limitation for the now-casting, which has become more and more important for meteorological alerts and for the aviation. Recently, some of national meteorological services have introduced Raman lidars for additional operational humidity/temperature profiling. The lidars allow monitoring of water vapor mixing ratio and temperature with high vertical and temporal resolutions. Here the design and measurement results from the Raman Lidar for Meteorological Observation (RALMO) developed by the Ecole Polytechnique Federal de Lausanne (EPFL) and operated by MeteoSwiss is presented as an illustration of the potential of Raman lidars in operational meteorology. The first applications of lidar data in numerical weather forecasting is also discussed.

147. Inequalities in heart failure care in a tax‐financed universal healthcare system: a nationwide population‐based cohort study

Abstract

Abstract Aims Data on socioeconomic‐related differences in heart failure (HF) care are sparse. Inequality in care may potentially contribute to a poor clinical outcome. We examined socioeconomic‐related differences in quality of HF care among patients with incident HF with reduced ejection fraction (EF) (HFrEF). Methods and results We conducted a nationwide population‐based cohort study among patients with HFrEF (EF ≤40%) registered from January 2008 to October 2015 in the Danish Heart Failure Registry, a nationwide registry of patients with a first‐time primary HF diagnosis. Associations between individual‐level socioeconomic factors (cohabitation status, education, and family income) and the quality of HF care defined by six guideline‐recommended process performance measures [New York Heart Association (NYHA) classification, treatment with angiotensin‐converting‐enzyme inhibitors (ACEI)/angiotensin receptor blockers (ARB), beta‐blockers and mineralocorticoid receptor antagonists, exercise training, and patient education] were assessed using multiple imputation and multivariable logistic regression controlling for potential confounders. Among 17 122 HFrEF patients included, 15 290 patients had data on all six process performance measures. Living alone was associated with lower odds of NYHA classification [adjusted OR (aOR) 0.81; 95% confidence interval (CI): 0.72–0.90], prescription of ACEI/ARB (aOR 0.76; 95% CI: 0.68–0.88) and beta‐blockers (aOR 0.84; 95% CI: 0.76–0.93), referral to exercise training (aOR 0.75; 95% CI: 0.69–0.81), and patient education (aOR 0.73; 95% CI: 0.67–0.80). Compared with high‐level education, low‐level education was associated with lower odds of NYHA classification (aOR 0.93; 95% CI: 0.79–1.11), treatment with ACEI/ARB (aOR 0.99; 95% CI: 0.81–1.20) and beta‐blockers (aOR 0.93; 95% CI: 0.79–1.09), referral to exercise training (aOR 0.73; 95% CI: 0.65–0.82), and patient education (aOR 0.86, 95% CI: 0.75–0.98). An income in the lowest tertile was associated with lower odds of NYHA classification (aOR 0.67; 95% CI: 0.58–0.79), prescription of ACEI/ARB (aOR 0.80, 95% CI: 0.67–0.95) and beta‐blockers (aOR 0.88, 95% CI: 0.86–1.01), referral to exercise training (aOR 0.59, 95% CI: 0.53–0.64), and patient education (aOR 0.66; 95% CI: 0.59–0.74) compared with an income in the highest tertile. Overall, no systematic differences were seen when the analyses were stratified by sex and age groups. Conclusions Living alone, low‐level education, and income in the lowest tertile were associated with reduced use of recommended processes of HF care among Danish HFrEF patients with a first‐time primary HF diagnosis. Efforts are warranted to ensure guideline‐recommended HF care to all HFrEF patients, irrespective of socioeconomic background.

International audience; The topic of this research paper is a discussion on the relevance of the sole use of absolute dating for the culturalattribution of bone artefacts. Lyngby-axes are an artefact-type indicative of a single culture of the Late-Glacial, thoughradiometric dates suggest that a wider time period can be represented. It is discussed whether the direct dating of theutilised animal-based material always accurately reflects the age of the technology used to implement the artefact asa relevant cultural item. The technological approach used here points to the gathering of fossil antler, targeted asa convenient raw material for this tool.

Abstract

International audience; Associated to the Ahrensbourgian horizon of the Stellmoor site (Germany), 18 worked reindeer antlers represent the so-called « Lyngby-axes » of the site available for study. Techniques of manufacture have been observed and described. Albeit a bad state of preservation, the pieces show specific patterns related to their use as heavy duty tool, either in direct percussion or acting as an intermediate piece. Compared to the archaeological frame where these tools have been found, this result is contextualized and hypothesis of use are suggested.
