Objectives The management of upper gastrointestinal bleeding (UGIB) has seen rapid advancements with revolutionising innovations. However, insufficient data exist on the necessary number of emergency endoscopies needed to achieve competency in haemostatic interventions.Design We retrospectively analysed all oesophagogastroduodenoscopies with signs of recent haemorrhage performed between 2015 and 2022 at our university hospital. A learning curve was created by plotting the number of previously performed oesophagogastroduodenoscopies with signs of recent haemorrhage against the treatment failure rate, defined as failed haemostasis, rebleeding and necessary surgical or radiological intervention.Results The study population included 787 cases with a median age of 66 years. Active bleeding was detected in 576 cases (73.2%). Treatment failure occurred in 225 (28.6%) cases. The learning curve showed a marked decline in treatment failure rates after nine oesophagogastroduodenoscopies had been performed by the respective endoscopists followed by a first plateau between 20 and 50 procedures. A second decline was observed after 51 emergency procedures followed by a second plateau. Endoscopists with experience of 51 emergency oesophagogastroduodenoscopies performed (p=0.039) or consultants (p=0.041).Conclusions Our data suggest that a minimum number of 20 oesophagogastroduodenoscopies with signs of recent haemorrhage is necessary before endoscopists should be considered proficient to perform emergency procedures independently. Endoscopists might be considered as advanced-qualified experts in managing UGIB after a minimum of 50 haemostatic procedure performed. Implementing recommendations on minimum numbers of emergency endoscopies in education programmes of endoscopy trainees could improve their confidence and competency in managing acute UGIB.

Abstract Parasitic weeds in the genera Orobanche, Phelipanche (broomrapes) and Striga (witchweeds) have a devastating impact on food security across much of Africa, Asia and the Mediterranean Basin. Yet, how climatic factors might affect the range expansion of these weeds in the context of global environmental change remains unexplored. We examined satellite‐based environmental variables such as surface temperature, root zone soil moisture, and elevation, in relation to parasitic weed distribution and environmental conditions over time, in combination with observational data from the Global Biodiversity Information Facility (GBIF). Our analysis reveals contrasting environmental and altitude preferences in the genera Striga and Orobanche. Asiatic witchweed (Striga asiatica), which infests corn, rice, sorghum, and sugar cane crops, appears to be expanding its range in high elevation habitats. It also shows a significant association with heat‐moisture coupling events, the frequency of which is rising in such environments. These results point to geographical shifts in distribution and abundance in parasitic weeds due to climate change.

Mapping the demand for ecosystem services (ES) is a profoundly debated research topic that needs to be investigated further to overcome spatial discrepancies between supply and demand in the literature. This study proposes a holistic approach to valuing ES, which is demanded by local people living adjacent to Sabah’s protected area in the southwestern part of Sabah, Malaysia. To assess the local people's demand for ecosystem services, we applied the 'ES Matrix Assessment' method. Timber, global climate regulation, recreation, and ecotourism are selected to illustrate the changes in ES demand patterns reported in the Klias Peninsula region. To identify the ES required by local people based on land-cover type, we used a weighted arithmetic mean approach. Then, using multiple regressions, we identified socio-demographic characteristics that influence demand for ES obtained from the Klias Peninsula's tropical forest. The 6-point Likert scale results showed that timber received medium (3) to highly relevant demand (4) among local people in the forest ecosystem, while climate regulation demand was the most highly relevant demand (5) in the forest ecosystem, and recreation and ecotourism are highly demanded in the forest ecosystem and water-based area. Overall, ethnicity, source(s) of income, distance from the protected area, length of residence, and education level have influenced the population's perception of ecosystem service demand in peat swamp forest, mangrove, and forest land, and these factors are statistically significant at the 1% to 5% levels. Our approach possesses the advantage of being intuitively straightforward, making it easy to convey to stakeholders and decision-makers across various ecosystem service (ES) applications. Therefore, our approach, while relatively simple, remains realistic and easy to apply, effectively raising awareness about the utility of the ecosystem services concept for stakeholders and policymakers.

Artificial intelligence (AI) has the potential to revolutionize healthcare, for example via decision support systems, computer vision approaches, or AI-based prevention tools. Initial results from AI applications in healthcare show promise but are rarely translated into clinical practice successfully and ethically. This occurs despite an abundance of “Trustworthy AI” guidelines. How can we explain the translational gaps of AI in healthcare? This paper offers a fresh perspective on this problem, showing that failing translation of healthcare AI markedly arises from a lack of an operational definition of “trust” and “trustworthiness”. This leads to (a) unintentional misuse concerning what trust (worthiness) is and (b) the risk of intentional abuse by industry stakeholders engaging in ethics washing. By pointing out these issues, we aim to highlight the obstacles that hinder translation of Trustworthy medical AI to practice and prevent it from fulfilling its unmet promises.

The paper briefly informs about content, procedures, and objectives of a study of driving dynamics of selected Aircraft Rescue and Firefighting (hereinafter ARFF) vehicles that have been used in practice to ensure fire safety at the Leoš Janáček Ostrava Airport. Two characteristic types of ARFF vehicles were selected for testing. Specifically, the vehicles were Mercedes-Benz 3360A 6x4.2 Actros (hereinafter M-B Buffalo) and Rosenbauer 36.705 Panther 6x6 CA5 ECE HRET (hereinafter Rosenbauer Panther). The driving dynamics testing focused on crisis braking in a straight direction, and on driving in a circle at speeds approaching the safe limit for a vehicle rollover.

Abstract A 66-year-old female presented in the emergency department with Blue-Toe-Syndrome (BTS) and signs of osteitis of her left big toe. Imaging workup of the peripheral vasculature showed no findings. Upon invasive angiography, severe focal stenosis of the dorsalis pedis artery (DPA) could be seen at the talonavicular joint. Complete regression of the stenosis was inducible by dorsal extension in the ankle joint. Further imaging revealed an underlying subluxation of the talonavicular joint as cause of the arterial compression. Entrapment of the DPA is a rare condition and most often described in relation to connective tissue bands or variant muscular tendons (McCabe et al. 70:213–8, 2021; Weichman et al. 24:113, 2010; Smith et al.58:212–4, 2013; Griffin et al. 20:325–8; 2012). In the presented case, bony compression of the PDA due to cranial subluxation of the talus was seen as the cause of BTS and osteitis of the phalanx of the first toe.

Abstract The realization of lossless metasurfaces with true chirality crucially requires the fabrication of three-dimensional structures, constraining experimental feasibility and hampering practical implementations. Even though the three-dimensional assembly of metallic nanostructures has been demonstrated previously, the resulting plasmonic resonances suffer from high intrinsic and radiative losses. The concept of photonic bound states in the continuum (BICs) is instrumental for tailoring radiative losses in diverse geometries, especially when implemented using lossless dielectrics, but applications have so far been limited to planar structures. Here, we introduce a novel nanofabrication approach to unlock the height of individual resonators within all-dielectric metasurfaces as an accessible parameter for the efficient control of resonance features and nanophotonic functionalities. In particular, we realize out-of-plane symmetry breaking in quasi-BIC metasurfaces and leverage this design degree of freedom to demonstrate an optical all-dielectric quasi-BIC metasurface with maximum intrinsic chirality that responds selectively to light of a particular circular polarization depending on the structural handedness. Our experimental results not only open a new paradigm for all-dielectric BICs and chiral nanophotonics, but also promise advances in the realization of efficient generation of optical angular momentum, holographic metasurfaces, and parity-time symmetry-broken optical systems.

Aims: Understand and illustrate differences and common methods in surveys of arable weed vegetation from the two scientific disciplines Vegetation science and Weed science; analyse the relationship between study aims and the employed methodology; assess in how much detail methodologies are reported and whether this changed over time. Study area: Europe. Methods: Literature review, classification of studies according to their reported aims and according to the journal scope. Results: Survey methods were reported in greater detail in studies aiming to describe management effects on weed vegetation compared to phytosociological studies. Methods employed in vegetation science and weed science differ in plot sizes, surveyed field parts and the seasonal timing of the survey. Conclusions: We recommend for future weed surveys to record and report on plot size and position relative to field limits, recording date, abundance scale, as well as the crop grown in a field. This information should also be retained when digitising published data and compiling large databases. A data standard should be developed in an interdisciplinary process.

The problem of poor reproducibility of scientific findings has received much attention over recent years, in a variety of fields including psychology and neuroscience. The problem has been partly attributed to publication bias and unwanted practices such as p-hacking. Low statistical power in individual studies is also understood to be an important factor. In a recent multisite collaborative study, we mapped brain anatomical left-right asymmetries for regional measures of surface area and cortical thickness, in 99 MRI datasets from around the world, for a total of over 17,000 participants. In the present study, we revisited these hemispheric effects from the perspective of reproducibility. Within each dataset, we considered that an effect had been reproduced when it matched the meta-analytic effect from the 98 other datasets, in terms of effect direction and significance threshold. In this sense, the results within each dataset were viewed as coming from separate studies in an "ideal publishing environment," that is, free from selective reporting and p hacking. We found an average reproducibility rate of 63.2% (SD = 22.9%, min = 22.2%, max = 97.0%). As expected, reproducibility was higher for larger effects and in larger datasets. Reproducibility was not obviously related to the age of participants, scanner field strength, FreeSurfer software version, cortical regional measurement reliability, or regional size. These findings constitute an empirical illustration of reproducibility in the absence of publication bias or p hacking, when assessing realistic biological effects in heterogeneous neuroscience data, and given typically-used sample sizes.

The mixed state transport properties of type-II superconductors are strongly influenced by the dynamic behavior of quantized magnetic fluxoids around the critical temperature (T_{c}), where a combination of normal and superconducting properties is exhibited. To understand the mixed state transport properties of type-II superconducting NbN ultrathin films (2D) we measured sheet resistance (R_{xx}^{M}) and Hall resistance (R_{xy}^{M}) of a 5-nm-thick NbN film around T_{c} (10.75 K) at temperatures 10.40, 10.68, and 10.77 K. Hall resistance (HR) was measured in external out-of-plane and in-plane magnetic fields up to 6 T, using 100 µA and 1 mA driving current in Van der Pauw geometry. The electric field of applied bias and Lorentz force of applied external magnetic field causes a movement of the normal conducting electrons within each fluxoid. The moving fluxoids cause dissipation and generation of Hall voltage. We developed a macroscopic analysis of the Hall resistance arising from fluxoids, to advance the differentiation between dissipating current and superconducting currents in type-II superconductors at T_{c}. We have extracted the number of normal conducting carriers per fluxoid and areal density and mobility of the fluxoids in dependence on the external magnetic field. This differentiation provides valuable insights into the dissipation mechanisms observed during transport measurements, e.g., after localized heating due to single photon absorption in nanostructured type-II superconductors. Furthermore, the developed macroscopic analysis of Hall resistance of fluxoids shows promising potential for investigating the fundamental aspects of fluxoid-defect interactions in type-II superconductors.
