Preterm births are the leading cause of neonatal death in the United States. Previously, a spontaneous preterm birth (sPTB) predictor based on the ratio of two proteins, IBP4/SHBG, was validated as a predictor of sPTB in the Proteomic Assessment of Preterm Risk (PAPR) study. In particular, a proteomic biomarker threshold of -1.37, corresponding to a ~two-fold increase or ~15% risk of sPTB, significantly stratified earlier deliveries. Guidelines for molecular tests advise replication in a second independent study. Here we tested whether the significant association between proteomic biomarker scores above the threshold and sPTB, and associated adverse outcomes, was replicated in a second independent study, the Multicenter Assessment of a Spontaneous Preterm Birth Risk Predictor (TREETOP). The threshold significantly stratified subjects in PAPR and TREETOP for sPTB (p = 0.041, p = 0.041, respectively). Application of the threshold in a Kaplan-Meier analysis demonstrated significant stratification in each study, respectively, for gestational age at birth (p < 001, p = 0.0016) and rate of hospital discharge for both neonate (p < 0.001, p = 0.005) and mother (p < 0.001, p < 0.001). Above the threshold, severe neonatal morbidity/mortality and mortality alone were 2.2 (p = 0.0083,) and 7.4-fold higher (p = 0.018), respectively, in both studies combined. Thus, higher predictor scores were associated with multiple adverse pregnancy outcomes.

Increased intake of dietary antioxidants such as anthocyanins, which are enriched in colourful fruits, is a promising alternative to reduce the risk of degenerative diseases such as Alzheimer’s Disease (AD). Since Amyloid β (Aβ) is one of the key components contributing to AD pathology, probably by reactive oxygen species (ROS) induction, this study investigated the preventive effect of anthocyanin-rich bilberry extract (BE) and its anthocyanin fraction (ACN) on ROS generation and cell toxicity. The results showed a significant and concentration-dependent decrease in neuroblastoma cell (SH-SY5Y) viability by BE or ACN, whereas no cell toxicity was observed in HeLa cells. Incubation with BE and ACN for 24 h diminished the generation of induced ROS levels in SH-SY5Y and HeLa cells. In addition, low concentrations of BE (1–5 µg/mL) showed protective effects against Aβ-induced cytotoxicity in SH-SY5Y cells. In conclusion, our results suggest antioxidant and protective effects of BE and ACN, which could potentially be used to delay the course of neurodegenerative diseases such as AD. Further studies are needed to clarify the high potential of anthocyanins and their in vivo metabolites on neuronal function.

Marine environments wherein long-term microbial oxygen consumption exceeds oxygen replenishment can be associated with oxygen minimum zones (OMZ). The Bay of Bengal OMZ (BOB-OMZ) is one of the most intense OMZs globally. To assess the contribution of bacterial oxygen consumption to oxygen loss in BOB-OMZ, we measured bacterial production (BP), temperature, salinity, and dissolved oxygen (DO) in the whole water column. We then compared the estimated bacterial oxygen demand (BOD) with diapycnal oxygen supply (DOS) at depths of 50–200 m in the southern BOB in January 2020. The average BP was 3.53 ± 3.15 μmol C m−3 h−1 in the upper 200 m of four stations, which was lower than those reported in other tropical waters. The vertical distribution of BP differed between the open ocean and nearshore areas. In the open ocean, temperature and DO were the most important predictors for BP in the whole water column. In the nearshore areas, when DO increased sharply from the suboxic state, extremely high BP occurred at 200 m. The average estimated BOD/DOS could reach up to 153% at depths of 50–200 m, indicating advection and anticyclonic eddies probably are important DO replenishment pathways in the BOB.

The A-energy of a graph G, denoted by EA(G), is defined as sum of the absolute values of eigenvalues of adjacency matrix of G. Nikiforov in Nikiforov (2016) proved that EA(G¯)−EA(G)≤2μ¯1and EA(G)−EA(G¯)≤2μ1for any graph G and posed a problem to find best possible upper bound for EA(G)−EA(G¯), where μ1and μ1¯are the largest adjacency eigenvalues of G and its complement G¯respectively. We attempt to provide an answer by giving an improved upper bound on a class of graphs where regular graphs become particular case. As a consequence, it is proved that there is no strongly regular graph with negative eigenvalues greater than −1. The obtained results also improves some of the other existing results.

The spatial distribution of compositional elements in milky, dough, and mature rice, together with the structure-function relationship was studied using synchrotron-FTIR. The dough stage aleurone and endosperm had high lipid, protein (amide I and II), and hemicellulose. The amylopectin chain length distribution of starch exhibited small differences among developmental stages. Dough starch featured a greater portion of A and B1 chains, implying a more crystalline and high in resistant starch. The protein in milky-to-dough stage aleurone and endosperm had higher contents of α-helixes than β-sheets, resulting in higher protein digestibility and availability. There were high intensities of the glycosidic peaks of hemicellulose and cellulose, pectin, carbohydrates, and β 1–6 glucans in milky-to-dough stages. They corresponded to higher total, insoluble, and soluble dietary fiber contents in milky-to-dough stage than in mature stage. Young-stage rice was found to be more nutritious and could thus be a functional food source.

Acute bacterial endocarditis is a rapid, difficult to manage, and frequently lethal disease. Potent antibiotics often cannot efficiently kill Staphylococcus aureus that colonizes the hearts valves. S. aureus relies on virulence factors to evade therapeutics and the hosts immune response, usurping the hosts clotting system by activating circulating prothrombin with staphylocoagulase and von Willebrand factor-binding protein. An insoluble fibrin barrier then forms around the bacterial colony, shielding the pathogen from immune cell clearance. Targeting virulence factors may provide previously unidentified avenues to better diagnose and treat endocarditis. To tap into this unused therapeutic opportunity, we codeveloped therapeutics and multimodal molecular imaging to probe the host-pathogen interface. We introduced and validated a family of small-molecule optical and positron emission tomography (PET) reporters targeting active thrombin in the fibrin-rich environment of bacterial colonies. The imaging agents, based on the clinical thrombin inhibitor dabigatran, are bound to heart valve vegetations in mice. Using optical imaging, we monitored therapy with antibodies neutralizing staphylocoagulase and von Willebrand factor-binding protein in mice with S. aureus endocarditis. This treatment deactivated bacterial defenses against innate immune cells, decreased in vivo imaging signal, and improved survival. Aortic or tricuspid S. aureus endocarditis in piglets was also successfully imaged with clinical PET/magnetic resonance imaging. Our data map a route toward adjuvant immunotherapy for endocarditis and provide efficient tools to monitor this drug class for infectious diseases.

BackgroundPreterm birth remains a common and devastating complication of pregnancy. There remains a need for effective and accurate screening methods for preterm birth. Using a proteomic approach, we previously discovered and validated (Proteomic Assessment of Preterm Risk study, NCT01371019) a preterm birth predictor comprising a ratio of insulin-like growth factor-binding protein 4 to sex hormone-binding globulin.ObjectiveTo determine the performance of the ratio of insulin-like growth factor-binding protein 4 to sex hormone-binding globulin to predict both spontaneous and medically indicated very preterm births, in an independent cohort distinct from the one in which it was developed.Study designThis was a prospective observational study (Multicenter Assessment of a Spontaneous Preterm Birth Risk Predictor, NCT02787213) at 18 sites in the United States. Women had blood drawn at 170/7 to 216/7 weeks' gestation. For confirmation, we planned to analyze a randomly selected subgroup of women having blood drawn between 191/7 and 206/7 weeks' gestation, with the results of the remaining study participants blinded for future validation studies. Serum from participants was analyzed by mass spectrometry. Neonatal morbidity and mortality were analyzed using a composite score by a method from the PREGNANT trial (NCT00615550, Hassan et al). Scores of 0-3 reflect increasing numbers of morbidities or length of neonatal intensive care unit stay, and 4 represents perinatal mortality.ResultsA total of 5011 women were enrolled, with 847 included in this planned substudy analysis. Increasing human-elephant conflict (HEC) has been attributed to the rising human population, their needs including agriculture, and the fragmentation and reduction of forest cover (Fernando et al. 2011). Nineteen administrative districts out of 25 in Sri Lanka have reported HEC (Prakash et al. 2020). During 2010 to 2019, a total of 14,516 incidents of HEC have been reported in Sri Lanka including the death of 807 people, injuries to 579 people, 10,532 incidents of property damage and 2,631elephants deaths (Prakash et al. 2020). HEC mitigation attempts by the state include distributing elephant thunder flashes (large firecrackers), capture and translocation, elephant drives, erecting electric fences and paying compensation (Prakash et al. 2020). Electric fences are one of the most effective methods for mitigating HEC (Fernando et al. 2008). However, in Sri Lanka they have been mostly erected around protected areas of the Department of Wildlife Conservation (DWC) restricting elephant movements (Fernando & Leimgruber 2011). In addition to the impossibility of restricting elephants to protected areas using electric fences, some elephants whose movements are obstructed by electric fences may search for new locations and thereby electric fences could even aggravate HEC (Gunaratne & Premarathne 2005). Wild elephants occur on both sides of many of the electric fences that have been constructed by the DWC, hence do not prevent elephant depredation (Fernando et al. 2008). Many of the weaknesses in the current approach to electric fences as a HEC mitigation measure could be remedied if electric fences were erected around human dwellings (Fernando 2020) and especially if individuals had full ownership of them. Here we present the results of 60 electric fences erected around private properties of villagers frequently affected with HEC.

Radar based sensing and communication systems sharing a common spectrum have become a potential research problem in recent years due to spectrum scarcity. The spectrum sharing radar (SSR) is a new technology that uses the total available bandwidth (BW) for both radar based sensing and communication. Unlike traditional radar, the SSR divides the total available BW into radar-only and mixed-use bands. In a radar-only band, only radar sensor signals can be transmitted and received. In contrast, radar and communication signals can both be transmitted and received in the mixed-use band. Taking such BW sharing into account, this paper investigates the performance of SSR in an information-theoretic sense. To evaluate performance, mutual information (MI), spectral efficiency (SE) and capacity (C) metrics are used. Initially, this paper considered a clean environment (no multipath) in order to evaluate performance metrics in the mixed-use band with and without successive interference cancellation. Following that, this paper addresses the performance of BW allocation by allocating low to high BW in mixed-band. Furthermore, the performance metrics are extended to account for the multipath environment, and the same analogy as in a clean environment is used. In addition, the MI and SE of traditional radar system is taken into account when comparing the performance of SSR with and without the use of the SIC. Finally, MI and capacity results show that using the SIC scheme in a mixed-use band yields performance comparable to traditional radar and communication system. In terms of SE, the SSR with SIC scheme outperforms traditional radar and communication system.

Objective An observational study was conducted to evaluate (1) the incidence rates of infection-related ventilator-associated complication (IVAC) and possible ventilator-associated pneumonia (PVAP) among mechanically ventilated patients of adult medical and surgical intensive care units (ICUs) and (2) the pathogen distribution in patients with PVAP.

Here, the authors use quantitative ultrafast interferometric pump-probe microscopy to track photoexcitations with sub-10 nm spatial precision in three dimensions and 15 fs temporal resolution to study the spatiotemporal dynamics of singlet exciton fission in polycrystalline pentacene films.

Objective The performance of Xpert Carba-R assay for the direct identification of carbapenemases directly from positive blood culture vials was evaluated. Materials and Methods In total, 176 positively flagged blood culture vials, yielding carbapenem-resistant GNB (CR-GNB), were enrolled for the detection and differentiation of blaKPC, blaNDM, blaVIM, blaOXA-48, and blaIMP using Xpert Carba-R. Results Klebsiella pneumoniae (76/176, 43.1%), Acinetobacter baumannii complex (67/176, 38%), and Escherichia coli (29/176,16.4%) were the predominant isolates. Overall, NDM production was the commonest (61/176, 34.6%), followed by the co-production of NDM + OXA-48 and the absence of any CR gene (44/176, 25%), followed by OXA-48 (27/176, 15.3%). In CR K. pneumoniae, the co-production of NDM + OXA-48 was most frequent (34/76, 44.7%), whereas in the A. baumannii complex, no CR gene was detected in the majority of isolates (38/67, 56.7%). bla NDM was the commonest gene in E. coli (18/29, 62%) and A. baumannii complex (26/67, 38.8%). Conclusion Xpert Carba-R can identify the molecular mechanism of CR within hours after a blood culture turns positive and, thus, has the potential for optimization of antimicrobial therapy, choosing appropriate novel β-lactam combination agents, as well as infection control interventions.

Granulicatella adiacens, a nutritionally variant Streptococcus (NVS), is part of the normal commensal flora of human mouth, genital, and intestinal tracts and rarely causes disease. It has been mostly reported from bacteremia and endocarditis cases, but rarely can cause vertebral osteomyelitis, pancreatic abscess, otitis media, and endovascular, central nervous system, ocular, oral, bone and joint, and genitourinary infections. Due to requirement of fastidious culture conditions and non-specific colony morphology, serious diagnostic difficulties may arise in cases of NVS infections. Here, we are reporting a rare fatal infection of G. adiacens presented with empyema complicated to sepsis and necrotizing fasciitis. Clinicians should be aware of the pathogenic potential of Granulicatella adiacens (a normal commensal flora of human mouth, genital and intestinal tracts). Appropriate supplemented media and a reliable detection system should be used to identify these fastidious organisms. We present this rare case to bring awareness among clinicians regarding such a rare but potentially fatal infection.
