Le risque cardiovasculaire élevé retrouvé dans la polyarthrite rhumatoïde ou l'arthrite psoriasique est en grande partie imputable à l'inflammation systémique. Dans l'arthrose (OA), la survenue d'une inflammation systémique a parfois été rapportée mais l'association entre arthrose et augmentation du risque cardiovasculaire reste à préciser. Cette méta-analyse avait pour objectif d'évaluer l'incidence des infarctus du myocarde (IM) et des accidents vasculaires cérébraux (AVC) mais également les facteurs de risque cardiovasculaire chez les patients OA. Nous avons effectué une recherche dans les bases de données PubMed, EMBase et Cochrane Library afin de trouver des références d'intérêt publiées jusqu'au mois de juin 2018. Les incidences de l'IM et de l'AVC ont été calculées à l'aide d'une méta-analyse des proportions. Les différences relatives aux facteurs de risque de maladie cardiovasculaire entre les patients OA et les sujets témoins ont été exprimées sous forme de différences moyennes standardisées (DMS) selon la méthode de l'inverse de la variance. Les études analysées ont rapporté 227 IM chez 3550 patients arthrosiques (incidence, 7,5 % ; IC95 % : 3,0–13,8 %) et 616 IM chez 12 444 sujets témoins (incidence, 6,0 % ; IC95 % : 2,8–10,3 %). La méta-analyse des trois études longitudinales a révélé une augmentation significative du risque d'IM chez les patients OA (RR = 1,22 ; IC95 % : 1,02–1,45). Nous avons également retrouvé un risque d'AVC significativement élevé chez les patients OA (RR = 1,43 ; IC95 % : 1,38–1,48). En ce qui concerne les facteurs de risque de maladie cardiovasculaire, les patients OA affichaient un profil lipidique et glucidique pro-athérogène se traduisant par une élévation des concentrations en glucose, cholestérol total et LDL-cholestérol ainsi qu'un indice de masse corporel (IMC) élevé. S'agissant des marqueurs de l'athérosclérose, les patients OA montraient une majoration du risque de syndrome métabolique et une élévation de la vitesse de l'onde de pouls (VOP). Les résultats de notre méta-analyse ont révélé un risque cardiovasculaire plus élevé chez les patients arthrosiques. L'acide hyaluronique (AH) est un glycosaminoglycane constituant de base de la matrice extracellulaire du cartilage et du liquide articulaire, produit par les synoviocytes. Il assure la viscoélasticité du liquide articulaire jouant un rôle d'absorbeur de chocs et aussi la résistance du cartilage articulaire. Il a également un rôle de lubrifiant dans le liquide synovial. Dans l'arthrose, concentration et poids moléculaire diminuent. Son utilisation en clinique humaine, en rhumatologie, par injections intra-articulaires dans la gonarthrose remonte aux années 70. Les preuves d'efficacité des acides hyaluroniques intra-articulaires (AHIA) dans la gonarthrose sont nombreuses : près de 80 essais cliniques randomisés contrôlés, dont une majorité versus placebo et désormais 17 méta-analyses des essais versus placebo montrent une efficacité symptomatique avec un effet-taille sur la douleur de 0,37 à 0,63. La tolérance selon une méta-analyse récente est excellente, avec 8 à 10 % de réactions douloureuses postinjection. Aucun effet systémique n'a été signalé. Peu de cas d'arthrites septiques ont été rapportés, mais il existe des arthrites pseudo-septiques mimant une arthrite septique, cependant sans gravité. Le bénéfice-risque est donc très positif. Une vingtaine de préparations sont actuellement disponibles en France, en trois ou mono-injections, et varient par leur poids moléculaire, leur concentration, leur structure biochimique et leur prix. Malgré les preuves d'efficacité, les recommandations thérapeutiques internationales donnent un avis contrasté sur l'usage des AHIA dans la gonarthrose. Les AH sont parfois utilisés dans d'autres localisations arthrosiques : hanche, épaule, cheville, rhizarthrose, arthrose de la 1re MTP, et aussi parfois proposés dans les tendinopathies, avec des niveaux de preuves variables, nettement plus faibles ou inexistants. In osteoarthritis (OA), molecular weight and concentration are reduced. Its use in human clinic, in rheumatology, by intra-articular injections in knee OA dates back to 1970s. Evidences for IAHA efficacy in knee OA are numerous: more than 80 randomized controlled trials, of which a majority versus placebo, and now 17 meta-analyses of placebo-controlled trials show a symptomatic efficacy, with an effect size on pain reduction ranging from 0.37 to 0.63. According to a recent meta-analysis, safety is excellent, with no peculiar issue, but 8.5 to 10 % local painful reaction, redness, and modest swelling after injection. No systemic effect has been reported. Few post-injection septic arthritis cases have been reported. But there are pseudo-septic arthritis looking like septic arthritis, but with no such hard outcome. Therefore, benefit-risk of IAHA is very positive. Around 20 preparations are currently available in France, either in three- or mono-injections, differing by their molecular weight, their concentration, their biochemical structure, their price. Despite efficacy evidence, international treatment recommendations provide a most contrasted advice on IAHA use and place in knee OA therapeutic management. HA may be used in other OA locations than knee, such as hip, shoulder, ankle, thumb base, 1st metatarsal joint, and are also proposed in the treatment of tendinopathies with current low and variable level of evidence, however really weak, or non existing. L'utilisation des infiltrations repose en rhumatologie et en traumatologie du sportif sur l'utilisation des glucocorticoïdes (GC) et/ou des acides hyaluroniques (AH). Le niveau de preuve de leur efficacité ainsi que leur place dans la stratégie thérapeutique dans l'arthrose ou les tendinopathies reste débattu malgré leur large utilisation. Depuis peu de temps, les injections locales de plasma enrichi en plaquettes autologues (PRP) sont utilisées et provoquent les mêmes interrogations que les autres injections locales. En effet, différents types de PRP peuvent être obtenus. Ils sont classés selon le taux de concentration plaquettaire, le contenu en leucocytes et la présence d'un activateur plaquettaire externe. La variété de leur composition est susceptible d'entraîner une variation de leurs effets biologiques démontrés in vitro ou chez l'animal (anti-inflammatoire, anti-apoptotique...). Plusieurs méta-analyses concernant des essais randomisés et contrôlés avec PRP ont été récemment publiées. Elles ont globalement mis en évidence des effets antalgiques et fonctionnels qui persistaient jusqu'à 12 mois. L'effet taille est léger à modéré, mais pertinent cliniquement, surtout dans la gonarthrose et l'épicondylalgie latérale. Ces effets sont au moins équivalents à ceux des GC et des AH avec une tolérance similaire à celui des AH. Ces données récentes de la littérature suggèrent que les PRP ont un rapport bénéfice risque favorable et qu'ils pourraient être utilisés lorsqu'une indication d'infiltration est posée, dans la gonarthrose et l'épicondylalgie en particulier. Des études ultérieures sont nécessaires pour préciser leurs mécanismes antalgiques et leur effet chondroprotecteur à plus long terme dans la gonarthrose. Corticosteroids (CS) or hyaluronic acids (HA) are the commonest molecules used to peri- or intra-articular injections in rheumatology or in sports medicine. The level of evidence of their effectiveness to treat osteoarthritis or tendinopathy is still debated despite their broad use. Plasma rich platelets injections (PRP) have been introduced recently in these indications and are subject to discussion like local injections of CS or HA. Thus, several kinds of PRP may be gotten according to their rate of platelet concentration, their leucocytes richness or the presence of an external platelet activator. Indeed the variability of their composition could provide different biologic effects previously shown in vitro or in animal studies (anti-inflammatory, anti-apoptotic...). Several meta-analyses including randomized control trials with PRP have been recently published. They overall showed that PRP had antalgic and functional effects lasting as long as 12 months. The effect size was light to moderate but clinically relevant, in knee osteoarthritis and lateral epicondylalgia in particular. These effects were at least as good as those gotten with CS or AH. The tolerance of PRP injections were equivalent to AH. The recent data of the literature suggest that PRP have a favorable benefit risk ratio. Thus, they may be used to treat knee osteoarthritis or lateral epicondylalgia when a local injection is indicated. L'hyperuricémie est une maladie courante qui, chez un certain nombre de patients, peut se compliquer de goutte. L'arthrose est la plus fréquente des arthropathies et coexiste fréquemment avec la goutte chez un même patient. La relation entre ces deux entités doit cependant être clarifiée, en particulier l'impact de l'arthrose sur le développement de la goutte et l'impact de la goutte sur l'initiation de l'arthrose. Par ailleurs, le rôle de l'hyperuricémie dans la survenue de l'arthrose en l'absence de goutte n'est pas établi. Nous passons en revue les données qui permettent de faire le lien entre la goutte et l'arthrose, en s'attachant tout particulièrement à déterminer le rôle de l'hyperuricémie avec ou sans manifestation clinique de goutte. Compte tenu de la disponibilité des traitements de l'hyperuricémie et de la goutte mais pas celui de l'arthrose, l'implication de l'acide urique dans la pathogenèse de l'arthrose pourrait avoir des répercussions cliniques majeures. Introduction: knee osteoarthritis, a common condition in Sub-Saharan Africa, affects the quality of life of patients. Objective: Identify the factors associated with impaired quality of life. Patients and Methods: Cross-sectional and analytical study, carried out in the Department of Rheumatology at the University Teaching Hospital of Brazzaville, in two steps. The first made it possible to collect data of patients diagnosed as having knee osteoarthritis according to the the ACR criteria of 1986 and followed up from January 1st, 2013 to December 31st, 2017. The second step consisted in measuring the quality of life of patients using a specific quality-of-life questionnaire for knee and hip osteoarthritis OAKHQOL, then identifying by bivariate analysis after analysis of variance (ANOVA), the factors associated with an impairment of the quality of life (P≤ 5%). It took place from May 02 to November 02, 2018 (6 months). One hundred and nineteen patients were investigated. Results: We included in this study 103 women and 16 men (sex ratio M / F of 1/6). The average age was 56.1 ± 9 years (range 27 to 77 years). The mean evolution time was 5.1 ± 3.6 years. The knee osteoarthritis moderately impaired the overall quality of life of patients. The most affected dimensions were physical activity and pain (51.3% and 42.9% of patients between 25 and 50 respectively). The evolution time and functional impact were potentially associated with all dimensions of quality of life. Age was associated with poorer quality of life in the physical activities (p = 0.012) and social activities (p = 0.001) dimensions, as was obesity, with the social activities dimension (p = 0.004). Knorpeltherapie ist immer noch ein aktuelles Thema in der Orthopädie, wenngleich sich schon einiges an Evidenz gesammelt hat. Bevor diese Evidenz im klinischen Alltag zum Einsatz kommt, zahlt es sich aus, ihre Herkunft und ihren Wert kritisch zu beäugen. Für Knie und Sprunggelenk gibt es eine relative dichte Evidenz zur Effektivität von autologer Chondrozytentransplantation, Mosaikplasitk und Microfrakturierung. Ebenso gibt es eine solide Datengrundlage für mögliche Knorpeltherapien an der Wirbelsäule. Schulter, Ellbogen und Hüfte sind noch verhältnismäßig neue Ziele in der Knorpelbehandlung, und dementsprechend wenig Evidenz liegt vor. Als besonderes Thema bleibt die Knorpeltherapie bei Kindern und Jugendlichen, die noch am meisten Daten und Evidenz braucht. Cartilage repair is still a hot topic in orthopedic surgery, but a considerable body of evidence has been collected. Before this evidence may be applied in the clinical setting, it is worthwhile to critically appreciate its origins and value. There is relatively sound evidence for the use of autologous chondrocyte implantation, mosaicplasty, and microfacture in the knee and ankle. There is also a solid body of data for possible cartilage treatments in the spine. Shoulder, elbow, and hip are comparatively new targets for cartilage repair, and accordingly there is only little evidence. • Les différents types d'acides gras ont des effets distincts sur les signes et les symptômes de l'arthrose, comme l'ont constaté des études in vitro et in vivo. • Des études suggèrent un effet défavorable des acides gras saturés et des acides gras poly-insaturés oméga 6, avec une augmentation des marqueurs pro-inflammatoires et pro-apoptotiques. • Les acides gras poly-insaturés oméga 3 ont été associés à une diminution des marqueurs de l'inflammation et de la dégradation dans des chondrocytes et des synoviocytes. • Des études interventionnelles menées chez l'animal et chez l'être humain ont montré que les acides gras poly-insaturés oméga 3 ont des effets bénéfiques sur les symptômes et les anomalies structurales de l'arthrose. • Il existe peu d'études interventionnelles chez l'être humain à ce jour, aussi des recherches de haute qualité sont-elles nécessaires pour pouvoir établir des conclusions solides. L'association entre obésité et arthrose a un temps été considérée comme résultant uniquement d'une augmentation des charges mécaniques, mais il est probable que des facteurs systémiques interviennent également dans la physiopathologie de l'arthrose. L'excès de nutriments aboutissant à l'obésité peut provoquer une lipotoxicité qui pourrait être impliquée dans l'apparition de l'arthrose. Les différents types d'acides gras ont des effets distincts sur l'inflammation. Cette revue explore les études disponibles qui résument les effets des différents types d'acides gras sur l'arthrose et les tissus articulaires touchés. Des études chez l'animal ont montré que des acides gras poly-insaturés oméga 3 atténuaient l'expression des marqueurs de l'inflammation, de la dégradation du cartilage et du stress oxydatif dans les chondrocytes. À l'inverse, ces marqueurs ont augmenté sous la stimulation d'acides gras poly-insaturés oméga 6 et d'acides gras saturés. En outre, une diminution des douleurs et des troubles fonctionnels a été mise en évidence chez des chats et des chiens recevant une supplémentation en oméga 3. De la même manière, la plupart des études in vitro chez l'être humain révèlent une action pro-apoptotique et pro-inflammatoire des acides gras saturés. Tous les acides gras poly-instaurés ont réduit les marqueurs du stress oxydatif, et les oméga 3 ont également atténué la production de prostaglandine. Des études interventionnelles chez l'être humain utilisant une supplémentation en oméga 3 suggèrent un effet bénéfique sur la douleur et la capacité fonctionnelle, ainsi qu'une diminution des dommages structuraux. En revanche, un effet néfaste des acides gras saturés sur l'arthrose a été observé. Les études sur les acides gras mono-insaturés sont peu nombreuses et leurs résultats non concluants. L'ankylose de l'articulation temporo-mandibulaire (ATM) est définie comme une constriction permanente des mâchoires avec ouverture buccale inférieure à 30 mm mesurée entre les incisives, survenant en raison d'une fusion osseuse, fibreuse ou fibro-osseuse. L'arthrose est une cause rare de l'ankylose de L'ATM. Nous rapportons un cas d'ankylose de l'ATM d'origine arthrosique, afin de préciser les particularités diagnostiques et thérapeutiques de cette entité pathologique assez rare.

Objectives: The Tactys® prosthesis is an anatomical unconstrained sliding implant with 4 modular components, designed for proximal interphalangeal arthroplasty. The main objective of our study was to evaluate medium-term functional outcomes at a minimum follow-up of 1 year., Materials and Methods: Forty-eight implants were studied, with a mean follow-up of 4.8 years. The study retrospectively analyzed functional, clinical and radiological results. Complications and re-interventions were also analyzed., Results: Quick Disabilities of the Arm, Shoulder and Hand (QuickDASH) and Patient-Rated Wrist Evaluation (PRWE) scores improved from 40.9 to 17.7 and from 47. 1 to 11 .2, respectively (p < 0.001) at last follow-up. Seventy-seven percent of patients rated their results as good or very good, and 83% were willing to undergo the procedure again. Pain on VAS improved significantly from 7.9 to 0.8/10 (p < 0.001). There was no significant difference in range of motion (53° vs 48°), grip strength (14.9 kgF vs 14.6 kgF), or pinch strength (3.4 kgF vs 3.4 kgF). There were no cases of fracture or loosening of implants. Periarticular ossification without functional impairment was observed in 31 patients. Axial deviation was generally corrected. Three surgical re-interventions were required, including 2 implant revisions using a silicone spacer. Five-year implant survival was 94%., Conclusion: The Tactys® prosthesis was a reliable and effective option for the management of proximal interphalangeal osteoarthritis. La coxarthrose est responsable de douleurs et de raideur articulaire à l'origine d'un handicap fonctionnel important. Le seul traitement curatif à ce jour est chirurgical et consiste en la mise en place d'une prothèse totale de hanche. Néanmoins, cette option thérapeutique n'est utilisée qu'en dernier recours, après avoir épuisé les autres thérapeutiques disponibles, notamment médicales. Parmi l'arsenal thérapeutique dont dispose le clinicien, le geste intra articulaire occupe une place de choix. Plusieurs agents thérapeutiques injectables ont été évalués ; parmi ceux-ci, les plus couramment utilisés sont les corticoïdes et les acides hyaluroniques. L'infiltration intra articulaire de dérivés cortisonés apporte un bénéfice modeste sur la douleur et uniquement à court terme. La visco-supplémentation de la hanche n'a pas prouvé sa supériorité par rapport au placebo dans la coxarthrose, mais les études ayant évalué son efficacité manquent de puissance. Son intérêt pourra être rediscuté avec les résultats d'essais thérapeutiques complémentaires et mieux conçus. Osteoarthritis is responsible for pain and joint stiffness that cause significant functional impairment. To date, the only curative treatment is the surgery with total hip prosthesis. Nevertheless, this therapeutic option is only used in last course, when all of the other available therapies have been exhausted, including medical ones. The intra articular injection is often used to treat osteoarthritis. Several injectable therapeutic agents have been evaluated; the most commonly used are corticosteroids and hyaluronic acids. Intra-articular infiltration of corticoids provides a modest benefit on pain and only in the short term. Visco-supplementation at the hip is not superior to placebo in hip osteoarthritis, but studies evaluating its efficacy lacked of power. La coxarthrose est une des pathologies ostéo-articulaires les plus fréquentes et les plus invalidantes. Son traitement est complexe et comporte plusieurs modalités, que le médecin doit savoir associer afin d'optimiser sa prise en charge, en veillant à la personnaliser et à l'adapter en fonction des caractéristiques propres à chaque patient. De nombreuses sociétés savantes se sont donc intéressées à la coxarthrose et ont émis des recommandations plus ou moins spécifiques à son traitement. Comme dans l'arthrose des autres articulations, notamment celle du genou à laquelle elle est souvent assimilée, il existe une grande diversité dans les interventions qui pourraient être proposées. Toutefois, un traitement médical bien conduit se base avant toute chose sur la combinaison optimale de modalités non pharmacologiques et pharmacologiques. Il faudra, par la suite, sélectionner, dans chacun de ces deux groupes, les interventions les plus adaptés au patient, ses facteurs de risque, ses comorbidités, ses moyens, ses attentes et ses demandes...C'est justement dans ce choix qu'interviennent les recommandations internationales, puisqu'elles guident le praticien dans cette tâche en contribuant à bâtir une stratégie thérapeutique optimale, basée sur les données scientifiques les plus solides du moment, ou faute de mieux, sur les opinions d'experts reconnus. Pourtant, les différentes recommandations actuellement disponibles se distinguent à bien des niveaux, laissant transparaître quelques points d'interrogation et plusieurs sujets encore conflictuels. Il reste donc de nombreux domaines d'incertitude et de multiples points à élucider, avant de pouvoir aboutir à des recommandations qui feraient l'unanimité en bénéficiant des avancées récentes dans le domaine thérapeutique. Hip osteoarthritis is one of the most prevalent and most debilitating musculoskeletal pathologies worldwide. Its management is quite complex and relies on multiple therapeutic modalities that must be chosen and associated by the treating physician, in order to build offer his patient the best care. This optimal treatment for hip osteoarthritis should be tailored to each single patient, but, no matter how diverse it can be, it must invariably rely on 3 different types of modalities which have to be mixed and matched in the best possible way. These 3 pillars on which the doctor will build his therapeutic strategy are the non-pharmacologic, the pharmacologic and the surgical treatment modalities. However, given the great diversity of available options, one might need to be guided through this process, and to rely on solid evidence. That's why many scientific societies have issued guidelines for the management of hip osteoarthritis, aiming to make it easier and consensual. However, although these recommendations have several similarities and share many domains, they also show some discrepancies and point to conflictual questions that remain unsolved. Indeed, numerous points still need to be elucidated, and solid proofs to be gathered in order to solve these issues. Introduction: The purpose of this study is to review the pain and functional outcomes of a multimodal intervention in three patients with knee osteoarthritis (OA). This study explores how manual therapy can be delivered within an evidence-based framework for the management of knee OA. Methods: Medical records were reviewed for three patients with knee OA who underwent a standardized multimodal intervention including education, exercise, and manual therapy. Changes in pain intensity and function from baseline to post-intervention were calculated and compared to thresholds for minimal clinically important differences. Results: One participant met the threshold for clinically significant improvement in pain and two participants for function. No adverse events were reported. Conclusion: Combined education, exercise, and manual therapy delivered over a 6-week period improved function in two of the three patients reviewed. • Même s'ils mettent plusieurs semaines à récupérer, les patients âgés ayant eu une arthroplastie de la hanche ou du genou obtiennent d'excellents résultats à long terme. • La chirurgie du sujet âgé présentant plus de comorbidités entraîne une surmortalité et une augmentation du risque de complications par rapport aux sujets jeunes. • La gestion des patients âgés et de leurs comorbidités bénéficie d'une prise en charge spécifique gériatrique permettant de diminuer le taux de complications, la durée de séjour et le taux de réhospitalisation. • La réalisation d'une rééducation précoce a mis en évidence une diminution de la durée totale d'hospitalisation, ainsi que du coût dans les suites d'une prothèse totale de hanche ou de genou, sans pour autant augmenter le risque de complications. L'arthrose des membres inférieurs s'accompagne de douleurs et d'une gêne fonctionnelle, invalidantes au quotidien. Lorsque le traitement médical est dépassé, la mise en place de prothèses est le traitement de choix chez le sujet âgé. L'objectif de ce travail est de rapporter les spécificités liées au patient, aux techniques opératoires et aux implants dans le traitement chirurgical de l'arthrose des membres chez le sujet âgé. Les patients âgés ayant bénéficié d'une pose de prothèse totale de hanche ou de genou pour arthrose nécessitent souvent plusieurs semaines de récupération. Ils obtiennent d'excellents résultats à long terme au prix d'une hospitalisation plus longue et de taux de complications post-opératoires et de mortalité plus élevés. L'adaptation de la stratégie chirurgicale et le type d'implants utilisés doivent être adaptés aux risques spécifiques des sujets âgés. La prise en charge doit intégrer une cogestion gériatrique avant et après la chirurgie afin de réduire le taux de complications mineures. Osteoarthritis of the lower limbs causes pain and discomfort in daily activities. After medical treatment, knee or hip replacements are the treatment of choice in the elderly. The aim of this work is to report the specificities related to the patient, the surgical techniques and the implants in the surgical treatment of osteoarthritis of the lower limbs in the elderly population. Elderly patients who had hip or knee replacement for osteoarthritis often require several weeks of recovery. They achieve excellent long-term results at the cost of longer hospitalization and higher rates of postoperative complications and mortality. The surgical strategy and the type of implants used must be adapted to the specific risks of the elderly. L'arthrose associée au syndrome métabolique est un phénotype clinique défini par ses facteurs de risque (obésité et syndrome métabolique) et qui se caractérise par une inflammation de bas grade chronique. L'obésité est un facteur de risque reconnu de gonarthrose, mais aussi d'arthrose digitale. Le syndrome métabolique augmente le risque d'arthrose avec à la fois un effet cumulatif des maladies métaboliques mais aussi un rôle indépendant et propre de chaque maladie métabolique (diabète, dyslipidémie ou hypertension artérielle). Plusieurs mécanismes expliquent l'augmentation des cas incidents d'arthrose chez le sujet obèse. Tout d'abord, l'excès de masse grasse est responsable d'une augmentation des contraintes mécaniques. Le tissu adipeux est aussi responsable de la production endocrine de médiateurs pro-inflammatoires (cytokines, adipokines, acides gras, radicaux libres oxygénés) ayant un rôle néfaste sur les tissus articulaires. Plus récemment, la dysbiose et la sarcopénie du sujet obèse ont été impliquées dans cette association. L'arthrose, qu'elle soit métabolique ou pas, est associée à une surmortalité cardiovasculaire qui peut s'expliquer par la sédentarité mais aussi par les facteurs de risques communs ainsi que par l'inflammation de bas grade, présente en cas de maladie métabolique. Introduction: Physicians are often challenged with accessing relevant up-to-date arthritis information to enable the delivery of optimal care. An online continuing medical education programme to disseminate arthritis clinical practice guidelines (CPGs) was developed to address this issue. Methods: Online learning modules were developed for osteoarthritis (OA) and rheumatoid arthritis (RA) using published CPGs adapted for primary care (best practices), input from subject matter experts and a needs assessment. The programme was piloted in two rural/remote areas of Canada. Knowledge of best practice guidelines was measured before, immediately after completion of the modules and at 3-month follow-up by assigning one point for each appropriate best practice applied to a hypothetical case scenario. Points were then summed into a total best practice score. Results: Participants represented various professions in primary care, including family physicians, physiotherapists, occupational therapists and nurses (n = 89) and demonstrated significant improvements in total best practice scores immediately following completion of the modules (OA pre = 2.8/10, post = 3.8/10, P < 0.01; RA pre = 3.9/12, post = 4.6/12, P < 0.01). The response rate at 3 months was too small for analysis. Conclusions: With knowledge gained from the online modules, participants were able to apply a greater number of best practices to OA and RA hypothetical case scenarios. The online programme has demonstrated that it can provide some of the information rural/remote primary care providers need to deliver optimal care; however, further research is needed to determine whether these results translate into changes in practice. Objective: A minimally invasive technique to prevent soft tissue problems using a calcaneal nail (Calcanail®, FH Orthopedics, Heimsbrunn, France) for calcaneal fractures or in subtalar joint arthrodesis is described. Indications: Displaced extra-articular calcaneal fractures involving the tuberosity fragment and in displaced intra-articular calcaneal fractures with impression and/or displacement of the subtalar joint surface. Subtalar joint arthrodesis for posttraumatic subtalar osteoarthritis. Contraindications: Severely displaced fractures type Sanders IV with the purpose of internal fixation; peripheral calcaneal fractures; general contraindications for operative treatment. Surgical technique: Minimally invasive reduction of calcaneal fractures using a special distractor and a graft pusher through a calcaneal working channel. Fixation performed via the insertion of the Calcanail® and the two locking screws, optional additional screws. Subtalar joint arthrodesis with a Calcanail® used with optional three locking screws to fix the talus and calcaneus creating an angular stable construct. Postoperative management: Mobilization and restricted weight-bearing for 6 weeks in the patient's own shoes after fracture fixation or in a walker after arthrodesis. Results: Preliminary results of 69 cases from three surgical centers have already been published. From 2013–2017, the technique was used in 48 of our own patients (42 calcaneal fracture reduction and fixation; 6 for subtalar joint arthrodesis). Mean postoperative hospital stay was 7 days for fracture reduction without any need of additional operations. During follow-up, 6 implant removals and 2 secondary subtalar fusions were noticed. Hintergrund: Die Entwicklung einer posttraumatischen Gonarthrose ist häufig Folge von Gelenkfrakturen oder Kapsel‑/Bandverletzungen sowie von direkten Knorpel- oder Meniskusschäden. Auch extraartikuläre Frakturen mit Achs- bzw. Rotationsfehlstellungen prädisponieren für eine beschleunigte Arthroseentwicklung. Die Patienten sind zumeist jünger, und es bestehen häufig komplexe Ausgangsbefunde, die das operative Ergebnis wesentlich beeinflussen.Material und Methode: Die Besonderheiten im Hinblick auf Diagnostik und Therapie werden in diesem Beitrag wiedergegeben. Zudem wird ein Überblick über operative Techniken, Implantatwahl und zu erwartende Ergebnisse gegeben.Ergebnis und Diskussion: Neben dem präoperativen Infektausschluss stellen die Analyse der Fehlstellung, der Bandstabilität sowie das Ausmaß der knöchernen Defekte wesentliche Faktoren der Operationsplanung dar. Es muss entschieden werden, ob und wann Osteosynthesematerialien entfernt werden müssen, ob eine ein- oder zweizeitige Achskorrektur erfolgen muss und welche Art der Prothese benötigt wird. Zudem spielen das Weichteilmanagement und die Wahl des Zugangs eine wesentliche Rolle.Schlussfolgerung: Die Behandlung der posttraumatischen Gonarthrose ist deutlich komplexer und komplikationsträchtiger verglichen mit der Primärendoprothetik. Es werden besondere Anforderungen an Vorhaltung und Erfahrung mit verschiedenen Primär- und Revisionssystemen gestellt. Abstract Objective Osteoarthritis (OA) is one of the most common degenerative diseases especially in the knee joint. The definitive method for the treatment of this disease is not known. In recent years, use of platelet-derived products has been considered as a new therapeutic approach because of its low cost, easy to use, and minimum side effects. Serum rich of growth factors (SRGF) is one of the biological compounds used to healing and regeneration. Its effects may improve in combination with antioxidants such as vitamin C. This vitamin increases the synthesis of proteoglycans by chondrocytes. The present study investigated effect of xenogenous SRGF in combination with vitamin C on the monosodium iodoacetate-induction osteoarthritis in rats. Methods Animals were randomly categorized into three groups including OA, SRGF, and vitamin C + SRGF. Treatments were performed with 3 time intra-articular injection in weekly intervals. Knee samples were taken after two weeks of the last treatment for histopathologic investigations. Results In the OA group, surface fibrillation and irregularity, multiple clefts, loss of chondrocytes, proteoglycan depletion with Toluidine blue staining were detected. In the treated group with SRGF/vitamin C, the severity of degenerative lesions was decreased. Chondrocytes had proliferated and matrix proteoglycan increased in compared to the SRGF and OA groups. Also, osteoarthritis stage was markedly reduced in this group rather than two other groups. Joint pain attributable to osteoarthritis (OA) is complex and influenced by a myriad of factors beyond local joint pathology. Current practice continues to predominantly adopt a biomedical approach to OA despite emerging evidence of the importance of a more holistic approach. This paper will summarise evidence for the presence of multidimensional pain profiles in knee joint pain and the presence of subgroups characterized by systemic features such as psychological distress, high comorbidity load or sensitisation of the nervous system. These factors have the potential to influence patient outcomes making them relevant for clinicians and highlighting the necessity of a broader multifactorial approach to assessment and treatment. This review describes the current state of the evidence for treatments of people with knee OA-related pain, including those receiving strong recommendations from current clinical guidelines, namely exercise, weight loss, self-management advice and pharmacological approaches. Other pain-modulating treatment options are emerging such as sleep and psychological interventions, pain education and multisensory retraining. The evidence and rationale for these newer therapeutic approaches is discussed. Finally, this review will highlight some of the limitations of current international guidelines for the management of OA and make recommendations for future research. [ABSTRACT FROM AUTHOR]

Ankylosis of the temporomandibular joint (TMJ) is a joint stiffness with an oral aperture of less than 30 mm measured between the incisors, occurring because of a bony, fibrous or fibro-osseous fusion. Arthrosis is a rare cause of the ankylosis of the temporomandibular joint. Résumé Objectif Évaluer les facteurs prédictifs de réponse selon un score IRM d'inflammation de la hanche (HIMRISS) dans un échantillon de patients atteints de coxarthrose recevant une injection d'acide hyaluronique (AH). Méthodes Au total, 60 patients atteints de coxarthrose ont été inclus. Les résultats cliniques ont été évalués par le score WOMAC à l'inclusion, puis trois mois après une injection d'AH. Quatre lecteurs ont évalué les lésions médullaires osseuses (LMO) et la synovite avant l'injection par le score HIMRISS sur les IRM de hanche. La fiabilité interlecteur du score HIMRISS était de 0,86 pour le HIMRISS total, 0,64 pour celui des LMO acétabulaires, 0,83 pour les LMO fémorales et 0,78 pour le score de synovite-épanchement. Les associations entre les caractéristiques à l'IRM et les données cliniques ont également été évaluées. Les associations entre caractéristiques à l'IRM et réponse à l'injection d'AH ont été analysées par régression logistique (univariée et multivariée) selon la réponse WOMAC50 à trois mois. Résultats Une réponse WOMAC50 a été obtenue chez 45,5 % des patients. Cinq effets indésirables ont été signalés. À l'inclusion, on a observé une corrélation significative entre le score WOMAC fonctionnel et le score HIMRISS de synovite-épanchement (r = 0,27, p = 0,03). Dans l'analyse univariée, les LMO fémorales selon l'évaluation binaire (p = 0,025), le score HIMRISS des LMO fémorales (p = 0,0038), le score HIMRISS des LMO acétabulaires (p = 0,042) et le score HIMRISS total (p = 0,0092) ont été associés négativement à la réponse WOMAC50. Dans l'analyse multivariée ajustée selon l'âge et l'IMC, les scores HIMRISS des LMO fémorales (p = 0,02) et total (p = 0,016) ont été associés négativement à la réponse. À un seuil HIMRISS < 15, 82 % des patients étaient répondeurs, avec une spécificité de 0,97, une sensibilité de 0,39 et des valeurs prédictives positives et négatives de 0,91 et 0,64 respectivement. Conclusion Les scores HIMRISS, total et en sous-domaines, constituent une méthode fiable permettant d'identifier les patients atteints de coxarthrose répondeurs aux injections d'AH.
