The abscopal effect is defined as the systemic regression of distant neoplastic lesions induced by localized treatment. Although the first case reports date back to the beginning of the twentieth century, it remains a very rare event. In recent years, research and reporting on the subject has increased thanks to the development of new immune-checkpoint inhibitors (ICIs) and stereotactic body radiotherapy, as a consequence of molecular and clinical synergism. This work describes an extremely particular presentation of metastatic laryngeal cancer, with mediastinal abdominal nodes and bone progressive disease after PD-1 inhibitor failure, which resulted in reductions of bone pain and abdominal and thoracic lymphadenopathies and an improvement in clinical conditions after treatment with concurrent palliative radiotherapy on the bulky mediastinal node and ICI beyond progression, configuring an important abscopal response.Laryngeal carcinoma is the most common cancer site of the aerodigestive tract and accounts for 25–30% of all head and neck cancer cases. Multimodality treatment (chemotherapy plus surgery and radiotherapy) is pivotal in localized or locally advanced disease, while immunotherapy is reserved for relapsed and metastatic settings. Nivolumab resulted in median overall survival of 7.5 months, progression-free survival of 2.0 months and an estimated 1-year survival rate of 36%; however, the response rate is very poor, at about 14%. Integrating immunotherapy with radiotherapy may prolong the duration of response and enhance efficacy.

The present study was conducted on milk samples from 30 Italian Mediterranean buffaloes, and as many goats, sheep and cattle. Milk samples were subjected to chemical-nutritional analysis and compared with commercially available milk samples. In the experiments, the management conditions could have influenced this parameter, determining the observed values. The higher fat content in sheep's milk differed significantly from that in the milk of the other animals analyzed. A higher lactose content was found in the milk of buffaloes and sheep, than in cows and goats. The highest cholesterol content was observed in cow and buffalo milk. These differences were statistically significant. To avoid excessive cholesterol intake, it is necessary to pay attention to both the quantity and quality of fats contained in food, bearing in mind that the introduction of large quantities of dairy products in the diet can cause a significant intake in cholesterol levels. Furthermore, when selecting the type of milk to be consumed, it is necessary to consider that the amount of total cholesterol is dependent on both the animal of origin and on the integrity of the lipid fraction. To complete the study, a chemical-nutritional analysis of milk samples normally marketed, both of vegetable and animal origin, was also included. Observing the results, some differences appear evident in the chemical-nutritional composition of goat and cow milk compared to the raw milk analyzed. In particular, goat's milk has a higher percentage of lipids. However, the differences observed herein were not significant and could be explained by the manufacturing process the samples were subjected to from the stable to the packaging industry. From the results obtained in the present study in the compositional analyses performed, and as also obtained from previous studies, it was found that sheep and goat milk, in particular, may be a valid substitute for cow's milk.

In this article, we present a control architecture for a robotic manipulator finally aimed at helping people with severe motion disabilities in performing daily life operations, such as manipulating objects or drinking. The proposed solution allows the user to focus the attention only on the operational tasks, while all the safety-related issues are automatically handled by the developed control architecture. The user commands the manipulator sending high-level commands via a P300-based brain–computer interface. A perception module, relying on an RGB-D sensor, continuously detects and localizes the objects in the scene, tracking the position of the user and monitoring the environment for identifying static and dynamic obstacles, e.g., a person entering in the scene. A lightweight manipulator is controlled relying on a task-priority inverse kinematics algorithm that handles task hierarchies composed of equality-based and set-based tasks, including obstacle avoidance and joint mechanical limits. This article describes the overall architecture and the integration of the implemented software modules, that are based on common frameworks and software libraries, such as the robotic operating system (ROS), BCI2000, OpenCV, and PCL. The experimental results on a use case scenario using a Kinova 7DOFs Jaco2 robot helping a user to perform drinking and manipulation tasks show the effectiveness of the developed control architecture.

Colorectal cancer (CRC) is one of the most frequent tumours and one of the major causes of morbidity and mortality globally. Its incidence has increased in recent years and could be linked to unhealthy dietary habits combined with environmental and hereditary factors, which can lead to genetic and epigenetic changes and induce tumour development. The model of CRC progression has always been based on a genomic, parametric, static and complex approach involving oncogenes and tumour suppressor genes. Recent advances in omics sciences have sought a paradigm shift to a multiparametric, immunological-stromal, and dynamic approach for a better understanding of carcinogenesis and tumour heterogeneity. In the present paper, we review the most important preclinical and clinical data and present recent discoveries in the field of transcriptomics, proteomics, metagenomics and radiomics in CRC disease.

Background Evidence links sedentary behaviours (SB) with poorer health outcomes; i.e. higher screen time is associated with poorer fitness and cardiometabolic health, as well as unfavourable measures of adiposity, mental health and social behaviour in children. The WHO guidelines for children recommend reaching at least an average of 60 min per day of physical activity (PA). Several studies show a correlation between parental education (PE) and children PA levels. This study aims to analyse the influence that PE has on childrens' sedentary lifestyle and PA habits in Italian society. Methods We conducted a cross-sectional analysis among 428 children aged 6-11 recruited in a primary school of Calderara di Reno (Italy) in 2017. Children's and parents' information were collected at baseline (i.e. age, BMI, occupation, instruction). PA level and SB were evaluated using a weekly diary investigating screen time (PC and TV), sports practice and active play (i.e: running, jumping, riding a bike, etc.). We investigated the relationship between lifestyles and PE (high school degree or lower vs college degree or higher) through multiple linear regression models adjusted for age and gender. Results Our models show that fathers' educational level is not related to children's SB. PA habits and weekly sports activities are not related to PE. On the other hand, higher mothers' educational level shows to be linked to a decreased TV screen time (p = 0.003) and time spent using a computer (p = 0.035). Notably, older children spend more time using PCs or devices (p = 0.001), net of confounding variables. Conclusions Our findings show that mothers' educational level positively influences children's lifestyle. This result might be related to the fact that, in the Italian context, mothers are usually the principal children's caregiver influencing their daily routine. In light of this, mothers might be considered a primary target when implementing public health interventions to reduce children's SB. Key messages Higher mothers’ educational level is positively associated with healthier lifestyles in children, in particular in determining a less sedentary behaviour. Fathers' degree of education does not display any impact on children's attitude toward sports and PA.

Purpose In the past decade, more than 700 million people are affected by some kind of disability or handicap. In this context, the research interest in assistive robotics is growing up. For people with mobility impairments, daily life operations, as dressing or feeding, require the assistance of dedicated people; thus, the use of devices providing independent mobility can have a large impact on improving their life quality. The purpose of this paper is to present the development of a robotic system aimed at assisting people with this kind of severe motion disabilities by providing a certain level of autonomy. Design/methodology/approach The system is based on a hierarchical architecture where, at the top level, the user generates simple and high-level commands by resorting to a graphical user interface operated via a P300-based brain computer interface. These commands are ultimately converted into joint and Cartesian space tasks for the robotic system that are then handled by the robot motion control algorithm resorting to a set-based task priority inverse kinematic strategy. The overall architecture is realized by integrating control and perception software modules developed in the robots and systems environment with the BCI2000 framework, used to operate the brain–computer interfaces (BCI) device. Findings The effectiveness of the proposed architecture is validated through experiments where a user generates commands, via an Emotiv Epoc+ BCI, to perform assistive tasks that are executed by a Kinova MOVO robot, i.e. an omnidirectional mobile robotic platform equipped with two lightweight seven degrees of freedoms manipulators. Originality/value The P300 paradigm has been successfully integrated with a control architecture that allows us to command a complex robotic system to perform daily life operations. The user defines high-level commands via the BCI, letting all the low-level tasks, for example, safety-related tasks, to be handled by the system in a completely autonomous manner.

The management of prostate cancer recurrence following external beam radiotherapy is not defined yet. Stereotaxic body re-irradiation therapy, has shown encouraging results for local and biochemical control. From April 2017 to December 2020, 29 patients with prostate cancer recurrence were collected, joining the retrospective studies CyPro (prot. 46/19 OSS) and CLARO (Prot.19/20 OSS) trials. Patients were treated with Cyberknife (CK), or VMAT (Volumetric Modulated Arc Technique) system by IGRT (Image-Guided Radiation Therapy)/Clarity. By comparing the re-irradiation of two groups, we investigated urinary (GU), rectal (GI) toxicities and biochemical control. Furthermore, we dosimetrically compared the two techniques by rival plans. The VMAT-IGRT Clarity treatments were replanned with an optimized template developed for prostate VMAT-SBRT in FFF mode keeping the dose and fractionation scheduled for CK Group at 30 Gy in 5fx, prescribing at 80% isodose. 23% of patients experiencing grade 2 acute GU and 6% grade 2 acute GI in the CK group. 17% Acute GU toxicity was recorded in the VMAT-Clarity group. 8% grade 2 late toxicity was recorded in the VMAT-Clarity group. The dosimetric analysis shows that the VMAT-FFF will allow to deliver a dose biologically equivalent than with the CK, and reduce the toxicities that could arise.

The success of future long-term manned missions in Space is strictly related to the development and setup of sustainable closed artificial ecosystems, defined as Bioregenerative Life Support Systems (BLSSs). In these BLSSs, the cultivation of higher plants represents the core element, given its role in primary resources (e.g., oxygen, water) regeneration and in fresh food production, also helping to counteract the emergence of Space-induced diseases in astronauts. However, Space farming may be constrained by Space ionizing radiation for its influence on plant growth and biomass production, depending on radiation properties and plant intrinsic factors (e.g., type of radiation, dose, plant species, cultivar, developmental stage at the time of irradiation). This study aimed to evaluate the effects of the different types and doses of low and high-LET (Linear Energy Transfer) radiation on morpho-anatomical traits and antioxidant content of Brassica rapa L. subsp. sylvestris var. esculenta microgreens. Dry seeds were exposed to different doses (0-control, 0.3, 1, 10, 20, and 30 Gy) of X-rays (the reference Low-LET radiation) at Istituto Nazionale Tumori - IRCCS - Fondazione G. Pascale, and to two types of Heavy Ions, namely 12C and 56Fe (to simulate the High-LET radiation) at different doses (0-control, 0.3, 1, 10, 20, and 25 Gy) at GSI Helmholtzzentrum für Schwerionenforschung GmbH. After the irradiation, seeds were sowed and microgreens were cultivated in a growth chamber under controlled environmental conditions, monitoring germination and survival, as well as growth performances. At the harvest, morpho-biometric traits, such as stem elongation, fresh and dry biomass, and total leaf area were measured. Leaf functional anatomical traits (e.g., lamina thickness, localization of phenolics, stomatal frequency) were quantified through light and epifluorescence microscopy and digital image analysis. The antioxidant charge of microgreens was evaluated considering antioxidant capacity, and polyphenols, chlorophylls, and carotenoid content. Results highlighted that the responses of microgreens, from irradiated seeds, were dose and radiation-type specific. This study furnished useful insights to evaluate plant radio-resistance at early stages of development, namely seedling establishment, that is a very critical phase in the plant life cycle. Gained information is useful to support the decision actions about the choice of suitable species to be cultivated in the BLSSs in Space and the definition of the shielding requirements for Space cultivation facilities.Part of the results presented here is based on the experiment Bio_08_DeMicco, which was performed at the SIS18 at the GSI Helmholtzzentrum fuer Schwerionenforschung, Darmstadt (Germany) in the frame of FAIR Phase-0.

Space exploration always has fascinated scientists, and in the last decades, the interest in this goal has increased exponentially. However, the realization of crewed space exploration missions or the permanence of human settlements on orbital stations or planetary habitats requires Life Support Systems (LSS) based on the interaction among abiotic and biotic elements, resembling terrestrial ecosystems. As on Earth, plants in Space would be involved in air regeneration through photosynthesis, water recovery, waste processing and food production, providing the astronauts with renewed resources and fresh food.The extraterrestrial environment may be considered one the most extreme for organisms’ survival because of several Space factors constraining biological life (e.g., altered gravity, fluid-dynamic and microgravity interaction, modified pressure, temperature). Among them, ionizing radiation influences severely plant growth and metabolic processes. Our study explores the response of tomato (Solanum lycopersicum L. ‘Microtom’), one of most widely cultivated crops, to different doses of sparsely ionizing radiation (IR), with two main goals: 1) to evaluate the morpho-functional mechanisms conferring radioresistance to guarantee their ecological role in closed, controlled environments; 2) to assess the possibility to use IR as a “biostimulant” to produce plant-derived functional food richer in functional compounds.Different doses of X-rays (0.3, 1, 10, 20, and 30 Gy, 6 MV energy), were delivered on dry seeds (DS) and germinated seeds (GS) and compared to not-irradiated controls, to define a dose-response curve and check possible negative/positive outcomes on seedlings. After the irradiation treatments, seeds/seedlings were transferred to a climatic chamber and cultivated under controlled environmental conditions of light, temperature, relative humidity and photoperiod. For our study, we adopted a multidisciplinary approach that merges anatomical analyses with measurements of photosynthetic efficiency and biochemical traits, including polyphenols and other antioxidant compounds linked with the nutritional value of derived food. The growth and photosynthetic performance of DS and GS plants were followed during the whole plant life cycle, while anatomical traits and fruit antioxidant properties were determined at full maturity of the specific organs. The results showed that the outcomes of radiation are dose-specific and dependent on the irradiated target stage, being SS plants more high-performing in photosynthetic activity and antioxidant content in fruits than SGs. Furthermore, some doses of X-rays act as a booster of bioactive compounds in fruits of both SS and SG plants. The outcomes of this research will be helpful to optimize crop production in Space and controlled environment agriculture systems. Moreover, the fine analysis of the relations between anatomical, eco-physiological and biochemical traits will furnish valuable insights to understand mechanisms of plant acclimation to stress, useful to manage cultivation factors to improve resource use optimization in controlled environments cultivations on Earth in line with sustainable development goals.

Background Spindle cell oncocytomas are extremely rare neoplasms of the sellar, parasellar, and suprasellar regions that can frequently mimic pituitary adenomas. Fewer than 50 cases have been ever reported in the literature, and there is no consensus on best treatments to be provided. Case Description We hereby present a challenging case of sellar and suprasellar spindle cell oncocytoma in a patient of 64 years. The patient, who presented with hydrocephalus, hypopituitarism, and visual deficit, underwent urgent transsphenoidal (TNS) resection of the mass, which was aborted for massive life-threatening bleeding. The patient received ventriculoperitoneal shunt with relief of symptoms. An endovascular embolization of tumor feeders from the distal portion of the right internal maxillary artery, in particular the sphenopalatine artery, was then performed and a second-look TNS surgery was feasible. The patient was discharged in optimal clinical condition, recovered both endocrinologic and visual deficits, and is now in follow-up. Conclusions We found that the oncocytoma was radiologically and clinically comparable with a pituitary adenoma, except for higher representation of vasculature. According to our recent experience and review of the literature, we believe that surgery (transsphenoidal or transcranial approach) is the recommended treatment in those who are symptomatic and preoperative embolization might be a suitable option to reduce intraoperative bleeding and increase radicality.

Preparedness for the ongoing coronavirus disease 2019 (COVID-19) and its spread in Italy called for setting up of adequately equipped and dedicated health facilities to manage sick patients while protecting healthcare workers, uninfected patients, and the community. In our country, in a short time span, the demand for critical care beds exceeded supply. A new sequestered hospital completely dedicated to intensive care (IC) for isolated COVID-19 patients needed to be designed, constructed, and deployed. Along with this new initiative, the new concept of "Pandemic Radiology Unit" was implemented as a practical solution to the emerging crisis, born out of a critical and urgent acute need. The present article describes logistics, planning, and practical design issues for such a pandemic radiology and critical care unit (e.g., space, infection control, safety of healthcare workers, etc.) adopted in the IC Hospital Unit for the care and management of COVID-19 patients.

Interventional oncology (IO) procedures have become extremely popular in interventional radiology (IR) and play an essential role in the diagnosis, treatment, and supportive care of oncologic patients through new and safe procedures. IR procedures can be divided into two main groups: vascular and non-vascular. Vascular approaches are mainly based on embolization and concomitant injection of chemotherapeutics directly into the tumor-feeding vessels. Percutaneous approaches are a type of non-vascular procedures and include percutaneous image-guided biopsies and different ablation techniques with radiofrequency, microwaves, cryoablation, and focused ultrasound. The use of these techniques requires precise imaging pretreatment planning and guidance that can be provided through different imaging techniques: ultrasound, computed tomography, cone-beam computed tomography, and magnetic resonance. These imaging modalities can be used alone or in combination, thanks to fusion imaging, to further improve the confidence of the operators and the efficacy and safety of the procedures. This article aims is to provide an overview of the available IO procedures based on clinical imaging guidance to develop a targeted and optimal approach to cancer patients.

Childhood obesity is an established health problem, and there is a growing need for health promotion interventions focused on healthy behaviors in collaboration with parents and schools. The Mediterranean diet (MD) could help to tackle obesity, but it is essential to maintain a good level of physical activity (PA) and limit time spent in sedentary activities (ST). To explore family determinants, adherence to the MD and PA levels as potential predictors of a child’s health-related behaviors, we performed a cross-sectional analysis of 368 Italian primary school children with a mean age of 8.95 years (SD = 1.43). Data were collected from May to June 2017 using a weekly diary, an interactive tool to assess the child’s and parents’ lifestyle. The child’s degree of adherence to the MD was calculated using the KIDMED index. Adherence to the MD was high, medium and poor in 5.2%, 62.5% and 32.3% of children, respectively. Higher maternal educational level was positively associated with children’s MD and PA (p < 0.05) and negatively correlated to ST. Maternal fruit and vegetable consumption was positively related to the MD and negatively related to ST (p < 0.05). Maternal PA was positively associated with the MD (p < 0.001). Paternal PA, and fruit and vegetable consumption, were positively associated with children’s PA (p < 0.05). Our results underline the need for future studies, mainly focused on school-based and family-based interventions, to promote healthy lifestyles and nutritional habits.

Background: Healthy eating and active lifestyle habits are essential for a child’s development, wellbeing, and health. School setting and family environment play a crucial role in shaping these habits and this could be reflected in different behavior patterns during weekdays and weekends. Methods: We investigated primary school children’s lifestyle habits through a cross-sectional analysis of 428 Italian primary school children, with a mean age of 8.99 years (±1.43). Data were collected from May to June 2017 using a weekly diary to assess children’s lifestyles. Results: Children who eat their morning snack and lunch at school three or more times during the weekdays were 5.47 times more likely (95% CI 3.02, 10.2) to consume adequate snacks and 7.79 times more likely (95% CI 4.43, 14.5) to have adequate meals than those who did not. Conclusion: Consumption of vegetables, lunch, and snacks are significantly more adequate during the weekdays as compared to the weekends. Physical activity levels did not differ between weekdays and weekends. Moreover, children spent more time engaged in physical activities than in front of a screen during both the weekdays and the weekends. The present results are good indicators of the importance of the school canteen in defining correct eating habits. Family-based and school-based interventions could represent valuable integrative strategies for promoting a healthy lifestyle in children.
