The ongoing coronavirus disease 2019 (COVID-19) pandemic may result in cardiovascular complications such as myocarditis, while encephalitis is a potentially life-threatening COVID-19-associated central nervous system complication. This case illustrates the possibility of developing severe multisystem symptoms from a COVID-19 infection, despite having received the COVID-19 vaccine within the year. Delay in treatment for myocarditis and encephalopathy can lead to permanent and possibly fatal damage. Our patient, a middle-aged female with a complicated medical history, initially came in without characteristic manifestations of myocarditis such as shortness of breath, chest pain, or arrhythmia, but with an altered mental status. Through further laboratory tests, the patient was diagnosed with myocarditis and encephalopathy, which were resolved within weeks through medical management and physical/occupational therapy. This case presentation describes the first reported case of concomitant COVID-19 myocarditis and encephalitis after receiving a booster dose within the year.

The concentration and distribution of trace metals were determined in sediment cores from the Khuan Khi Sian wetland, Thailand. The sediment cores were collected from seven stations in the dry and wet seasons in 2022. The concentration of Pb, As, and Cd in the dry season were in the range 0.00–60.16, 0.00–6.68, and 0.00–0.92 mg/kg (dry weight), respectively. Meanwhile, the concentration of Pb, As, and Cd in the wet season were in the range 0.00–12.12, 0.00–3.86, and 0.00–0.92 mg/kg (dry weight), respectively. The vertical profiles of metal concentrations in core sediment show a general increase from bottom to top. Average concentrations of heavy metals in the sediments of the Khuan Khi Sian wetland are found to be lower than the sediment quality guideline. In the sediment cores, only As in the dry season exceeded the U.S. EPA standard. The calculated enrichment factor (EF) and the geoaccumulation index (Igeo) indicate that the sediments were moderately polluted with As in some locations. According to the Ri analysis, Pb was low risk but the criteria of ecological risk of As and Cd are considerable and they are considered high risk. This is potentially due to agricultural activities and land use around the wetland areas and municipalities. The concentration of As and Cd should be of concern and subject to regular monitoring.

Background:-The association of glaucoma with cataracts has become more frequent because of increase in life expectancy and the increased risk of cataract development in the patients with glaucoma. For this reason, and to reduce the trauma and the time taken for patient rehabilitation, the prevailing trend is to perform a combined procedure, taking care of both pathologies simultaneously. Material and Methods:-The present prospective study was conducted in the Postgraduate Department of Ophthalmology SKIMS Medical College Bemina in 51 patients from October 2016 to April 2018. All patients underwent sutureless phacotrabeculectomy (Sheikh's Technique). Postoperative evaluation was done on the first postoperative day and thereafter at 1 week, 4 weeks, 2 months and 6 months. Results:-Mean age of the patients was 60.04 ±9.6 years. 50% of patients were in age group 60-70 years. The postoperative mean intraocular pressure (mmHg) was 13.67±5.28 mmHg at 1 week, 14.41 ± 4.68 mmHg at 1 month, 14.53 ±5.52 mmHg at 2 months, and 14.71 ±4.1 mmHg at 6 months At the 6th month follow up visit, the mean number of antiglaucoma medications per treated eye was 0.14± 0.52 which was a significant decrease from pre-op number (2.94±0.61). In terms of snellen BCVA, most of the patients (80%) had a visual acuity of 6/18 or better at 6 month follow-up in the operated eye. Conclusion:-suturelessphacotrabeculectomy is a safe and effective option in the management of cataract associated with glaucoma. [ABSTRACT FROM AUTHOR]

This paper focuses on the hybridization of two renewable energy sources (i.e., PV Panel and Bi-Cycle Dynamo) and the use of a Multi-Input SEPIC converter for highly efficient output from them. This paper has also presented the comparative assessment between the conventional Single-Input Single Output SEPIC (Single Ended Primary Inductor Converter) and Multi-Input SEPIC converter based on performance analysis. Both the Multi-Input SEPIC that is designed in this literature powered by the hybrid architecture of power sources and the Single-Input SEPIC are run in boost mode. The project is mainly developed with a view to facilitating the isolated rural islands and regions detached from the on-grid connections. The findings and assessments of this study are corroborated by the MATLAB/Simulink simulation results and optimally designed prototype built for miniature applications. The Multi-Input SEPIC topology has been developed in such a way that it is functional with an input voltage of 12.1V exactly and gives an output voltage of DC 53V approximately at the output terminal. To get the maximum voltage at the output from the designed circuit, the duty cycle of the converter recorded is almost 81.49%. The renewable energy sources (RES) that are used to build the prototype are Photovoltaic Panel and Bi-Cycle Dynamo. Due to the limitations of the PV Panels to generate power during the night and gloomy weather, the Bi-Cycle dynamo works as the backup power source. In performing the hardware and software analyses, various intermittent situations, solar irradiation, seasonal change, day-night phase, and other parameters are taken into consideration on the input terminal of the converter. The coronavirus disease 2019 (COVID-19) is a global health emergency of unprecedented proportions. Countries around the world have taken extraordinary steps to control the disease. The preventive measures face challenges in low and lower middle income countries (LICs and LMICs). Especially the marginalized communities, e.g., women are the hardest hit of the virus. This study took Bangladesh as a representative LMIC and aimed to determine the level of knowledge, perception, attitude, and preparedness related to COVID-19 among the adult women in the country. Using a comprehensive questionnaire, we channeled a cross-sectional study among adult women in Bangladesh. Participant's self-reported data on the knowledge, attitude, and preparedness were tabulated and analyzed using suitable statistical tools. A total of 1,869 adults from 61 districts of Bangladesh took part in this study. Ninety seven percentage of the participants claimed to have heard of COVID-19 before it arrived in Bangladesh. Regarding the general knowledge related to COVID-19's causal agent, symptoms, and treatment, the positive response rate was nearly 80%, with a mean of 10.68 ± 1.72. Younger and educated women had better knowledge levels compared to the older and lower-educated participants (p < 0.01). More efforts are required to educate women with older age and lower socioeconomic status. An overall positive attitude and perception were observed, although a significant proportion of the participants opined that the Government's efforts in controlling the outbreak were not adequate. Although the participants had a satisfactory level of knowledge and a positive attitude in adopting preventive measures against COVID-19, greater efforts are needed from the healthcare authorities and Government.

An exploration of a gold-coated dual-core with hexagonally arranged circular air holes SPR-PCF sensor is presented in this paper. Chemically inactive and stable material gold is provided as plasmonic material in this design. Placement of the gold layer outside of the PCF structure simplifies the detection process of this sensor and also assures fabrication feasibility. The performance of this sensor is investigated in terms of wavelength sensitivity, amplitude sensitivity, resolution, sensor length and linearity response by using the Finite Element Method (FEM) based COMSOL Multiphysics software. Maximum wavelength sensitivity is found to be 10,700 nm/RIU for analyte sensing range between 1.39 to 1.40 by using wavelength interrogation method as well as maximum amplitude sensitivity reaches about 1770 RIU-1 for analyte RI 1.39 by using amplitude interrogation method. Besides, the sensor's resolution is found to be 9.34×10-6. The outcome of alternating structural parameters such as gold layer thickness, pitch, air hole diameter, different shapes of air hole in the core is also discussed as well. The reported sensor might be a fruitful aspirant in the field of biological sample detection, organic chemical sensing and biomolecule recognition. Currently, dengue fever and human cervical cancer are two of the most dangerous diseases around the world. Dengue fever is caused by four serotypes of dengue virus (DENV)- 1, 2, 3 and 4 and most of the human cervical cancer cases are found to be caused by human papillomavirus type-16 (HPV-16). In this study, potential epitope-based subunit vaccines were designed using numerous tools of reverse vaccinology, targeting the DENV-1 envelope protein E and HPV-16 major capsid protein L1. Reverse vaccinology is a genome-based approach of vaccine designing, identifying potential antigenic epitopes of a pathogen. After obtaining the target protein sequences, their antigenicity and physicochemical properties were determined. Thereafter, the possible T cell and B cell epitopes were predicted and after analyzing their antigenicity, allergenicity, toxicity, and conservancy, as well as docking results (docked with MHC class-I and class-II alleles), the best epitopes were selected for vaccine construction. Thereafter, two best vaccine constructs (one for each of the virus) were selected for further analysis based on analyzing the docking results (docked with various MHC alleles and TLR molecules) of the initially constructed six vaccines. The molecular dynamics simulation of the two selected docked complexes with the two best vaccines showed quite satisfactory results. Finally, after codon adaptation, the vaccine constructs were inserted into the pET-19b plasmid vector for E. coli strain K12 using an in silico cloning approach. However, more in vitro and in vivo studies might be required on the suggested vaccines for their proper validation.

Bartonella bacilliformis, a causative agent of Bartonellosis or Carrión's disease, has become resistant to antibacterial agents. To suggest effective novel drug targets, a comprehensive in silico subtractive genomic approach was employed in the genome of B. bacilliformis subsp. ver097. Different bioinformatics tools and online servers were utilized to determine human homologous proteins of the pathogen, as well as proteins associated with common metabolic pathways of pathogen and host. Only seven proteins were associated in pathogen specific pathways, which were further screened to determine drug or vaccine targeted membrane proteins. ‘ABC transporter permease’ and ‘Flagellar biosynthesis protein FlhA’ were found to be novel drug targets which showed the highest level of antigenicity. Moreover, ‘Flagellar biosynthesis protein FlhA’ exhibited broad spectrum conservancy with other Bartonella bacilliformis strains. Consequently, it might be used for development of novel drugs and therapeutic components for the successful treatment of infections caused by Bartonella bacilliformis.

Having been obtained values ​​of crude were processed statistically by using the statistical bags SPSS which include mean and standard deviation and test r. The results were discussion and put some recommendations that suit to the current search for the women long jumpers in Kurdistan.

Introduction: The elderly patients are at a greater risk for hypotension after induction of anesthesia. Perfusion index (PI) predicted patients vascular responses in many situations. This study hypothesized that baseline value of PI would predict hypotension in geriatric patients after anesthetic induction as population at risk for postinduction hypotension. Methods: This observational, prospective study involved 30 patients aged >65 years who were scheduled for elective procedures under general anesthesia. From baseline to 15 min after propofol induction of anesthesia, the PI, heart rate, and blood pressure (BP) were recorded every minute. Hypotension was defined as a decrease greater than 30% in systolic BP from baseline. Results: Early hypotension within 3 min after induction was experienced in 53.3%, while 36.7% had late hypotension within 15 min. Receiver operating characteristic (ROC) curves for PI as a predictive test of hypotension at 3 min showed good predictability of the baseline PI for early postinduction hypotension with area under receiver operating characteristic curve (AUC) 0.97, sensitivity 88%, specificity 93%, positive predictive value (PPV) 93%, negative predictive value (NPV) 87%, and a cutoff value of =1.3. After intubation, the predictive value decreased, with PI > 0.91 to predict late hypotension with sensitivity 91%, specificity 58%, PPV 56%, NPV 92%, and AUC 0.66. Conclusions: PI could predict early hypotension following propofol induction in the geriatric population. So, it can be considered a valuable tool in monitoring this vulnerable population after induction of anesthesia. [ABSTRACT FROM AUTHOR]

The present work includes preparation and spectral study of some transition metal complexes of Mn,Co,Ni,Cu(II)and Zn(II)with new Mannich base Z-1-(3-methoxy phenyl amines)methl-3-(p-tolyl)indolin-2-one that behaved as donating bi dentate Lewis base via nitrogen and oxygen atoms .The new Mannich base (L)was prepared in tow step , the first is condensation of indol -2,3-dion with 4-methylaniline ,and the second followed the reachion with formaldehyde and m-anisidine.All the complexes and the Ligand have identified on the basis of C.H.N.elemental analyses , flume atomic absorption ,NMR,IR,Uv- visible ,mass spectra. Furthermore the magnetic moment by Fardy method and molar conductivity measurement in DMF solvent revealed the tetrahedral geometry of [MLCl2].n H2O formal ,n= 1,2,for Co,Ni,Cu+2 .
