102. Totally endoscopic mitral valve repair: a case report

Minimum incision techniques in cardiac surgery have been on the rise after their inclusion in videothoracoscopy. Video-assisted minimally invasive cardiac surgery is considered by many centers in the world to be the approach of choice for treatment of mitral and aortic valve diseases. The aim of this is study is to report a case of valve repair in a patient with mitral restenosis after surgical repair 12 years before. Through a minimally invasive and totally endoscopic procedure, it was possible to successfully perform a mitral commissurotomy with only videoscopic support, and without any incisions larger than those needed to introduce the trocars for instrumentation.

Summary Objective We evaluated and compared the analytical and clinical performance of the TnI-Ultra immunoassay for cardiac Troponin I (cTnI) measurement, carried out on the fully automated ADVIA Centaur and ADVIA Centaur CP ® platforms (Siemens Medical Solutions Diagnostics SrL), and that of Access AccuTnI™, carried out on the UniCell ® DxI 800 platform (Beckman Coulter, Inc., Fullerton). Methods We assayed 645 plasma Li-heparin samples of healthy subjects and 230 of patients with cardiovascular diseases (70 with a cTnI value higher than the decision level for acute myocardial infarction) by TnI-Ultra and Access AccuTnI™ immunoassays. Results The total imprecision (CV) profile of TnI-Ultra method was determined by repeatedly measuring 19 plasma pools, with different concentrations of TnI, in 20 different runs using two reagent lots; the CV ranged from 25% (at 0.02 μg/L) to less than 5% (for levels greater than 0.4 μg/L). On average, functional sensitivity (i.e. concentration measured with an imprecision of 10% CV) of TnI-Ultra method was 0.057 μg/L. The 99th percentile of normal distribution, calculated by enrolling a control group of 645 healthy subjects (278 males and 367 females; mean age of 47.4 ± 16.2 years, range 15–89 years), was 0.072 μg/L. Therefore, the ratio between functional sensitivity and 99th percentile values was less than 1 (i.e., 0.057/0.072 = 0.79), while for Access AccuTnI™ resulted 0.07/0.06 = 1.16. A close linear regression was found between the cTnI values obtained with the two methods only when the samples of patients with cardiac disease were taken into account (TnI-Ultra = −0.192 + 1.434 AccuTnI T ; R = 0.976; n = 230). A significant bias was found between the two methods by means of Bland-Altman plot (mean difference 38.9%), but when 368 healthy subjects were considered, no significant linear regression was found between these two methods ( R = 0.002, p = 0.9624). Conclusion Our results indicate that the TnI-Ultra immunoassay carried out on the fully automated ADVIA Centaur platforms shows analytical characteristics well in agreement with the quality specifications recommended for cardiac troponin assay.

BACKGROUND The aim of this study was to evaluate the distribution of cardiac troponin I (cTnI) values, measured by the ADVIA TnI-Ultra method (Siemens Medical Solutions Diagnostics SrL) in healthy subjects and to characterize its relation to gender, age, as well as to N-terminal fragment of pro-brain natriuretic peptide (NT-proBNP). METHODS A Caucasian population of 692 healthy subjects (311 males and 381 females) with a mean (SD) age of 45.3 (17.3) years [range 11-89 years; females 46.5 (17.3) years, males 43.8 (17.1) years] was enrolled. The presence of cardiac or systemic acute or chronic diseases was excluded by history and accurate clinical evaluation. 105. Standardization of video-assisted cardiac surgery technique: initial experience

Minimally invasive cardiac surgery has been performed in major worldwide centers, including procedure such as valves, coronary and congenital surgery.To demonstrate our first works with noninvasive cardiac surgery by mean of the experience gained with general and thoracic surgery.Whenever possible to carry out a minimally invasive cardiac surgery, this was the approach of choice. The left thoracoscopy was used in four cases: (1) symptomatic coronary-pulmonary fistula ligation; (2) implant of an epicardial electrode into the left ventricle for resynchronization; (3) excision of pericardial giant lipoma in the left atrium, and (4) resection of hemangiolipoma in the mediastinum. Right thoracoscopy with extracorporeal circulation through cardiopulmonary bypass via femoral vein and artery and cardiac arrest in ventricular fibrillation with moderate hypothermia were carried out in the following cases: (5) patient with mitral stenosis after surgical repair with Carpentier ring 12 years before. An anterior and posterior commissurotomy without thoracotomy was successfully made; (6) patient with idiopathic dilated cardiomyopathy, high-response atrial fibrillation, and severe mitral insufficiency, underwent mitral repair surgery with Gregori's ring and ablation of the pulmonary veins with radio-frequency catheter. (MAZZE modified).No death occurred in this series and the surgical result in all cases was highly satisfactory. All patients were discharged from hospital with a mean time of 5.5 +/- 5 days after intervention.Our initial experience in this field proves the effectiveness and the viability in introducing this type of technique.

Objectives: To determine pediatric reference values for thyrotropin using Advia® Centaur™ analyzer. Design and methods: The study was conducted in a large regional hospital on TSH results obtained from 5741 females and 4332 males aged 0–17 years. After the exclusion of the results outside 4 standard deviations, we calculated the Health Related Limits (HRLs) following the indirect Kairisto's procedure and using the software GraphROC™. Results: The lower HRL of TSH concentration was 0.70 mU/L in the years 0–11 and 0.50 mU/L in the following years. The upper TSH HRL was 6.9 mU/L in males vs. 5.7 mU/L in females in the first year and 6.7 mU/L vs. 5.3 in the period 1–2 years. The upper HRLs in females and males were similar in the following years and the upper HRL in the 13–17 years class was 3.8 mU/L. Conclusions: The indirect methods appear reliable for calculating the pediatric HRLs for TSH.

The assessment and management of congestive heart failure relies increasingly on the measurement of B‐type natriuretic peptide (BNP). However, the effective contribution of this biochemical test in the clinical decision making is influenced by reliability of the measure, which also depends on several preanalytical issues. Since there is controversy on the influence of the matrix and the storage conditions on BNP measurement, we compared results of BNP in serum, K2 ethylene diamine tetra‐acetic acid (EDTA) plasma and lithium heparin plasma fresh samples and in matching samples stored at −20 and −80°C for 1 week. BNP measured on the Bayer Advia Centaur was systematically underestimated in heparin plasma (−47%) and serum (−62%) when compared to K2 EDTA plasma. According to the established 100 ng/L cutoff value, 25% and 37% of the fresh samples collected in heparin plasma or serum were misclassified from the reference K2 EDTA fresh specimen, respectively. When compared to the fresh specimens, the mean and interindividual bias observed for samples stored at either −20°C or −80°C was, overall, modest for K2 EDTA plasma (−2%) and heparin plasma (+6% and −4%, respectively), though it appeared clinically meaningful in serum (+47% and +28%, respectively). Although we can not rule out that other BNP assays using different antibodies may be not affected from degradation during storage to the same extent, results of our investigation demonstrate that K2 EDTA plasma is the most suitable specimens for BNP testing on fresh and frozen samples stored at either −20°C or −80°C for up to 1 week. In this paper, we study the fluid-structure interaction in a weakened basilar artery. The aim is to study how the wall shear stress changes in space and time because of the weakening, because spatial and temporal changes are thought to be possible causes of aneurysm and vascular deseases. The arterial wall, in its natural configuration, is modeled as a hyperelastic cylinder, inhomogeneous along its axis, in order to simulate the axis-symmetric weakening. The fluid is studied exploiting a recent approach for quasi-one-dimensional flows in slowly varying ducts, which allows to write the averaged equations of mass and energy balance on the basis of the velocity profile in a straight duct. The unknowns are the wall pressure, the average velocity, and the wall radial displacement. The problem is solved in two parts: first, the stationary non-linear coupled problem is solved, and an intermediate configuration is obtained. Then, we study the variation of the basic unknowns about the intermediate configuration, considering time dependence over the cardiac cycles. The results suggest that, with a 10% reduction of the main elastic modulus, the shear stress in the weakened zone changes its sign and doubles the maximum stress value detected in the healthy zone. We consider fracture problems for one-dimensional bodies, such as rods, through energy minimization. For an elastic-brittle body β occupying in the reference configuration the domain Ω, the energy depends on both some unknown closed crack set K and a displacement field u, smooth on Ω ∖ K. On Ω ∖ K a bulk energy density and on K an interface energy density are defined. As a result of this assumption the total potential energy is a nonconvex functional. We propose a new numerical method for the search of minimal states of rods by minimizing the energy both on K and u. To seek a minimizer (\( \tilde K \); ũ) a nonlinear gradient iterative procedure is employed. During the process of minimization both the displacement field u and the set K evolve toward local minima. To illustrate our approach, some examples are reported.

112. Evaluation of a new fully automated one-step C-peptide chemiluminescence assay (LIAISON C-Peptid)

The determination of C-peptide, a 31 amino acid fragment of proinsulin which is a byproduct of insulin formation, is used as a marker for insulin secretion. Clinically, the determinations are performed to detect autonomous insulinoma, factitious hypoglycemia, and in general to assess the function of beta-cells in patients with diabetes mellitus. The analysis is frequently performed by radioimmunoassays (RIA), which have several disadvantages, for instance the use of radioactivity and time and resource requirements. We performed an evaluation of a new fully automated chemiluminescence assay (LIAISON C-Peptid, Byk-Sangtec) at two clinical sites, in Germany and Italy, with regard to imprecision and clinical relevance of the obtained data, and the correlation with a standard RIA method and another chemiluminescence test. The new assay showed a good correlation with the RIA (r = 0.950) and the chemiluminescence assay (r = 0.967). The intra-assay variability and inter-assay variability was 3.5% and 8.7% in Germany, and 2.4% and 9.6% in Italy. The clinical evaluation of samples derived from 19 oral glucose tolerance tests, 13 insulin suppression tests, and 2 insulin secretion stimulation tests revealed a clinical specificity of 100%, i.e. all cases resulted in the same clinical diagnosis with all tests. With regard to the practical performance of the assays, the new chemiluminescence test, as a single-step fully automated method, offered the advantage of being a non-radioactive, less complex and much faster method than the RIA and also had timely advantages over the comparative chemiluminescence test. In general, the new LIAISON chemiluminescence assay compared favorably with the RIA and comparative chemiluminescence test and offers an attractive alternative for C-peptide analysis.

Recent guidelines have defined that the decisional cut-off of cardiac troponin (cTn) would recognize "the normal" subjects, and in theory they do not have "measurable" values of cTn. The 99th percentile of this population is chosen as cut-off but using an assay with an analytical CV10% at this value. Objectives of this study were to set the decisional limits for cTroponin I (cTnI) measured by the ADVIA:Centaur, and to evaluate the analytical precision around these limits. 120 normal plasma samples were tested for cTnI levels. The 99th percentile defined the decisional level, according to the recent guidelines. The precision was estimated by 10 replicates for 21 samples. The 99th percentile was 0.17 microg/L, with an analytical CV10%. Since this analytical method achieves the recommended analytical precision, the cTnI decision level for myocardial damage by the ADVIA:Centaur is 0.17 microg/L.

This paper describes a new method for the analysis of masonry vaults based on the masonry-like material model. In the present work, restricted to the equilibrium of vaults, we look for a compressive thrust surface, contained within the extrados and intrados surfaces of the structure, carrying the applied loads and transferring them to the abutments of the vault. The optimal shape of such surface is obtained through minimization of a convenient form of relaxed energy.

Chemotherapy is an established approach for several malignancies, but its utility may be hampered by induced cardiac toxicity possibly leading to heart failure, with a negative impact on the patient's quality of life and ultimately survival. Prospective left ventricular systolic function monitoring has demonstrated that cardiotoxicity could be subclinically present for many months or years before its overt manifestation. Although considered irreversible, some reports suggested recovery or delayed progression of cardiac dysfunction by preventive cardioactive therapies. Thus, the identification of earlier instrumental or biochemical markers of cardiac injury able to predict heart failure remains a major task. Diastolic indexes as a primary expression of hemodynamic dysfunction after cardiac damage, analyzed by means of conventional or newer Doppler technologies (tissue Doppler, color M-mode, etc.) are discussed. Moreover, brain natriuretic peptides, troponins and endothelin-1, as possible sensitive/specific markers/predictors of subclinical cardiotoxicity are reviewed in order to update and possibly improve the strategy for the detection and clinical management of chemotherapy-related cardiotoxic effects.

In light of recommendations on the quality (1) and clinical use (2) of troponin assays, we evaluated the analytical performance of the ADVIA Centaur and ADVIA CP® platforms (TnI-Ultra, Siemens Medical Solutions Diagnostics SrL) for measurement of cardiac troponin I (cTnI). The chemiluminescent TnI-Ultra method uses 2 monoclonal capture antibodies directed to epitopes at amino acids 41–49 and 87–91 and a tracer polyclonal goat antibody labeled with acridinium ester, directed against amino acids 27–40 (1)(3)(4). Two clinical laboratories participated in the study: the CNR Institute of Physiology in Pisa and the San Bortolo Hospital in Vicenza. The limit of detection (limit of the blank) for the TnI-Ultra method was calculated as the concentration corresponding to a signal of 3 SD above the mean of 60 replicates (obtained in 4 different runs and pooled together) for the calibrator in which cTnI was absent; a mean cTnI concentration of 0.006 μg/L was found. Age and close living conditions are known to be risk factors for the acquisition of Helicobacter pylori (HP) infection. It is unknown whether institutionalization of asymptomatic, elderly subjects is an additional risk factor and whether gastric function and nutritional status are affected by the HP infection. The study sample comprised 102 subjects over 65 years of age: 52 living in a nursing home and 50 at home. No subject had symptoms or previous pathology related to the upper digestive tract. In all subjects, serum levels of specific anti-HP antibodies were determined. Gastric function was evaluated by levels of pepsinogen A (PGA), pepsinogen C (PGC) and gastrin. The nutritional status of the subject was evaluated by measuring: albumin, haemoglobin, iron, ferritin, transferrin, vitamin B12, and folic acid in blood, and body mass index and mid-arm muscle area. With the aim of verifying the effect of omeprazole treatment on theophyline serum concentration in elderly peptic ulcer patients, we studied 10 male subjects aged >65 years (mean age = 75.2, range = 67--86) with chronic obstructive bronchopneumonia and endoscopically diagnosed duodenal ulcer in acute phase. All subjects were treated with a slow-release formulation of theophylline 200 mg b.i.d. plus omeprazole 20 mg daily for 4 weeks. In all subjects serum concentrations of azote, creatinine, theophylline were determined at the beginning and after 1 and 4 weeks; at the beginning and end of the study, pepsinogen group A (PGA), pepsinogen group C(PGC) and gastrin were measured. Statistical analysis was performed with the Student's t-test for paired data. The results showed no statistically significant differences after 1 and 4 weeks of omeprazole treatment in serum concentrations of theophylline (T(0) = 7.4, T(1 week) = 7.5, T(4 weeks) = 6.0, p = ns), azote (T(0) = 45.2, T(1 week) = 30.5, T(4 weeks) = 36.1, p = ns), creatinine (T(0) = 1.27, T(1 week) = 1.02, T(4 weeks) = 1.16, p = ns), PGA (T(0) = 99.5, T(4 weeks) = 126.2, p = ns), and PGC (T(0) = 10.6, T(4 weeks) = 12.1, p = ns); however serum gastrin increased from T(0) = 70.2 plus minus 13.2 to T(4 weeks) = 130.3 plus minus 18.3 (p < 0.0001). It was concluded that (1) serum concentration of theophylline is not affected by the concomitant omeprazole treatment lasting 1 month in elderly patients suffering from chronic obstructive bronchopneumonia and peptic ulcer, (2) modifications of dosages of theophylline and/or omeprazole are not necessary in the elderly with normal renal function, (3) the increase in fasting serum gastrin after 4 weeks of treatment may indicate that omeprazole 20 mg daily is efficacious in inhibiting gastric acid secretion in the elderly people.

In order to investigate the main sites of action of phototherapy in the treatment of neonatal jaundice we studied (a) the in vivo and in vitro relationship between the hematocrit and the effectiveness of phototherapy, and (b) the effect of varying the skin area exposed to light. The results show that the hematocrit does not influence in vivo the efficacy of phototherapy, while they confirm that the total skin surface exposed to light is important in determining the effectiveness of light treatment. The authors have also studied the possible action of phototherapy on bilirubin solutions placed into the postmortem brain or the stomach. The results suggest that blue light does not penetrate strongly enough to photomodify the exposed pigment.

Different doses (0.5-20 mg/kg) of hematoporphyrin (HP) have been injected intraperitoneally into normal rats and rats affected by Yoshida ascites hepatoma. About 80% of HP reaching the liver was recovered in the extracellular compartment after liver perfusion, the ratio of extra- to intracellular HP being essentially independent of the administered dose. Similar data were obtained at different times after injection of 20 mg/kg HP. Intracellular HP largely accumulates in the mitochondria and in the membrane components of the nuclear fraction of isolated hepatocytes. Kinetic studies suggest that the cell receptors of highest affinity for HP are present in the external membrane. The latter result obtains for ascites hepatoma cells in an even more evident way, although the latter cells exhibit secondary HP binding sites probably constituted by cytoplasmatic proteins. Moreover, the clearance of intracellular HP from malignant cells occurs at a remarkably lower rate as compared with HP clearance from liver cells.

O solo escolhido foi uma areia de duna costeira, não coesiva e de granulometria uniforme, a fim de destacar a ação pozolânica cimentante da cal mais cinza volante quando misturadas com água. Foram estudadas três misturas com proporções diferentes. A primeira composta de 83% de areia de duna, 13% de cinza volante e 4% de cal. A segunda mistura possuindo os mesmos teores da primeira com respeito à cal, cinza volante e areia de duna, mas contendo o acréscimo de 1% de cimento portland de alta resistência inicial, como aditivo. A terceira mistura possuindo 53% de areia de duna, 30% de brita, 4% de cal, 13% de cinza volante e mais 1% de cimento portland de alta resistência inicial, novamente como aditivo. Todas as misturas foram estudadas somente na umidade ótima de compactação. Os corpos de prova foram moldados em cilindros tripartidos de 10 cm de diâmetro por 20 cm de altura. As idades de cura adotadas foram de 7, 14, 22, 28, 30, 38 e 90 dias. Foram realizados ensaios de resistência à compressão axial simples e resistência a tração por compressão diametral, ensaios de durabilidade pelo método da ABCP, pelo método Iowa e por um método estudado pelo autor, e além disso ensaios de determinação do módulo de elasticidade por métodos estático e dinâmico. A resistência a compressão axial simples alcançou valores de até 137 kgf/cm2 e a resistência a tração por compressão diametral 22 kgf/cm2. Os valores do módulo de elasticidade estático chegaram até a 150.000 kgf/cm2 enquanto os valores do método dinâmico alcançaram até 178.000 kgf/cm2. Os ensaios de durabilidade mostraram que a adição de cimento e a substituição da areia por brita mais a adição de cimento melhoram a qualidade das misturas estudadas. O estudo de laboratório mostrou que as misturas ora estudadas apresentam características suficientemente boas para que possam ser aplicadas na construção de base e sub-base de pavimentos. Entretanto, provavelmente, cuidados especiais devem ser tomados durante a construção. Study of durability, elastic modulus, split tensile strength and unconfined axial compression strength of a sandy soil stabilized with lime and fly ash, and also, with the cement addition and replacing part of sand with crushed stone. The selected soil was a coastal dune sand, cohesionless and with uniform gradation, in order to enhance the cementitious pozzolanic action of lime plus fly ash when mixed with water. Three mixtures with different proportions wers studied. The first mix had 83% dune sand, 13% fly ash and 4% lime. The second mixture had the same contents of lime, fly ash and dune sand, but having 1% of high initial strength portland cement as additive. The third mixture had 53% dune sand, 30% crushed stone, 4% lime,13% fly ash and 1% of high initial strength portland cement as additive. All mixes were studied at optimum moisture content only. Specimens were molded in cylinders 10 cm diameter and 20 cm height split longitudinally into three parts. Curing ages adopted were: 7, 14, 22, 28, 30, 38 and 90 days. Tests performed were the following: unconfined compressive strength, split tensile strength, durability according to ABCP ("Associação Brasileira de Cimento Portland") method, Iowa method, and the Author's method, and elastic moduli determinations by static and dynamic methods. Unconfined compressive strength reached values up to 137 kgf/cm2 and tensile strength obtained through diametrical compression (split tensile strength) 22 kgf/cm2. Static elastic modulus attained values up to 150,000 kgf/cm2 and the dynamic modulus 178,000 kgf/cm2. Durability testing have shown that adding cement and replacing sand by crushed stone together with the addition of cement improved the quality of mixtures studied. Laboratory research have shown that the mixes studied present good enough characteristics so that they could be aplied in the construction of base and sub-base courses of pavements. However special provisions, probably, should be considered during construction.

The lymphoproliferative response to phytohemagglutinin of lymphocytes and spontaneous motility, chemotaxis and phagocytic activity of granulocytes were studied in the newborn and adult blood in the presence of bilirubin and photobilirubin. All of these activities were inhibited to the same extent by these substances. In addition, a significant difference between newborn and adult values was found. In conclusion, the authors suggest that phototherapy does not decrease bilirubin cellular toxicity.

In this paper, we study the equilibrium of masonry vaults, assuming that the material has infinite friction and no cohesion (i.e. it is No-Tension in the sense of Heyman). With Heyman's assumptions, the equilibrium of a structure composed of this ideal masonry material, can be studied with limit analysis. In particular, the present study is concerned with the application of the safe theorem of limit analysis to masonrylike vaults, that is, curved constructions modelled as continuous unilateral bodies. On allowing for singular stresses in the form of line or surface Dirac deltas, statically admissible stress fields concentrated on surfaces (and on their folds) lying inside the masonry, are considered. Such surface and line structures are unilateral membranes/ arches, whose geometry is described a la Monge, and their equilibrium can be formulated in Pucher form. It is assumed that the load applied to the vault is carried by such a (possibly folded) membrane structure S. The geometry of the membrane S, that is of the support of the singularities, is not fixed, in the sense that it can be displaced and distorted, provided that one keeps it inside the masonry. Two particular case studies are analyzed to illustrate the method: A cross vaults of the Gothic Cathedral of Caserta, and a modern timbrel spiral stair built by the Guastavinos in New York. © Civil-Comp Press, 2015.

The equilibrium problem for masonry-like materials in the Heyman sense can be formulated looking for a minimum of the complementary energy functional defined in the field of admissible stress tensors, belonging to the class of negative semi-definite tensors. The classical mixed boundary problem for a masonry panel is developed assuming that the stress is uniaxial so it is possible to express the complementary energy as a function of the slope of compressive rays which characterise the uniaxiality of the stress field. In this work, a family of proximity curves defined starting from a Bézier curve with three control points to model the slope function is employed. Two structural cases are solved, pure shear displacements and shear-flexural-tensile ones, showing the usefulness of the method when applied to the analysis of masonry piers.

New dialyzers are designed to optimize the convective and diffusive components of solute transport. Asahi Kasei Medical Co., Ltd. has developed a new high flux dialyzer series called Rexeed™ with improved flow distributions. We evaluated the in vivo dialytic performance of two dialyzers of the Rexeed™ series: Rexeed-18A and Rexeed-25A (1.8 m2 and 2.5 m2). We calculated the clearance for urea, creatinine, phosphate and b2-microglobulin both in high flux dialysis (HFD) and in 15 liter postidiluitional on-line hemodiafiltration (HDF) mode. With n=3 patients in high flux HD at blood flow 450, 400, 350 and 250 ml/min we found remarkably high clearance for urea (347±4%,305±0%,288±5%,230±3%, for Rexeed-18A and 361±3%,329±0%,313±1%,234±3% for Rexeed-25A), creatinine (282±10%,234±0%, 221±8%, 174±8%, for Rexeed-18A and 276±6%,245±0%,226±9%,172±13% for Rexeed-25A), phosphate (347±0%,316±0%,275±4%,202±16%, for Rexeed-18A and 364±3%,365±0%,286±3%,224±2% for Rexeed-25A) and b2-microglobulin (133±21%,124±0%,118±12%,98±11%, for Rexeed-18A and 159±8%,169±0%,157±8%,129±7% for Rexeed-25A) With n=2 patients in HDF at blood flow 300 ml/min we found remarkably high clearance for urea (268±2%, for Rexeed-18A and 283±2% for Rexeed-25A), creatinine (183±6% for Rexeed-18A and 205±9% for Rexeed-25A), phosphate (245±3%, for Rexeed-18A and 270±2% for Rexeed-25A) and b2-microglobulin (166±12%, for Rexeed-18A and 192±4% for Rexeed-25A). Our preliminary evaluation describes the characteristics and the performances of a new polysulfone-based hemodialyzer series called Rexeed™. Several innovative features have been implemented by the manufacturer. These constructive approaches seem to have produced a positive effect on the dialyzer performance at the bedside.

Abstract Background: Few reports exist on the relationship of the left ventricular diastolic dysfunction (LVDD) with its most important features including enlargement of the left atrium and left ventricular hypertrophy (LVH), and with the right ventricular (RV) function. Objective: To determine the correlation between the left atrial size and the RV function and dimensions in patients with and without LVDD and LVH. Methods: Fifty patients were included, 25 (40% men) of them with LVDD, aged 67.1 ± 10.6 years (study group) and 25 without LVDD (52% men) aged 49.9 ± 16.3 years (control group). Patients underwent transthoracic echocardiography with evaluation of the left atrial size and volume (LAV), LVDD, LVH, and RV function and dimensions. P-values < 0.05 were considered statistically significant. Results: LAV > 34 mL/m2 and left atrial size > 40 mm were associated with lower absolute values of tricuspid annular plane systolic excursion (TAPSE) and RV lateral S' (p ≤ 0.001, Pearson's correlation coefficient -0.4 and -0.38, respectively) in the study group. Patients in the study group showed higher incidence of LVH (p = 0.02) and greater left atrial diameter (p = 0.03) compared with the control group. In addition, greater left atrial diameter (p = 0.02) and LAV (p = 0.01) values were found in patients with LVDD grade II compared with LVDD grade I. Conclusions: The present study determined, for the first time, the correlation of left atrial enlargement with progressive RV dysfunction in patients with LVDD.

Dissertação (mestrado) - Universidade Federal de Santa Catarina, Centro Tecnológico, Programa de Pós-Graduação em Engenharia Civil, Florianópolis, 2011 Este trabalho apresenta a análise, baseada em estudos de laboratório, do comportamento de misturas entre solo e resíduos de cerâmica vermelha, visando seu emprego em camadas de pavimentos de rodovias e ruas com baixo volume de tráfego. A indústria da cerâmica vermelha é responsável pela produção de um grande volume de tijolos, blocos e telhas e, como resultado do controle de qualidade e danos de manuseio, a quantidade de resíduos gerados é significativa. Na Região Sul de Santa Catarina, onde são produzidos cerca de 100 milhões de peças de tijolos e telhas por mês, aproximadamente 5% desta produção é descartada por algum tipo de defeito ocorrido durante o seu processo de produção ou transporte. Para a realização deste trabalho foram coletados 12 m³ deste resíduo, dos quais 6 m³ se tratavam de cacos ou resíduos de tijolos e 6 m³ de cacos ou resíduos de telhas. De forma separada, estes materiais foram fragmentados com a ajuda de um rolo compactador e o material resultante foi coletado, analisado e utilizado como agregado em misturas com três tipos de solo. Utilizaram-se três tipos de solos, coletados em jazidas nos municípios de Gravatal, Tubarão e Criciúma, que foram submetidos aos ensaios tradicionais de pavimentação, e aos ensaios da metodologia MCT. As misturas solo-agregado foram compostas com diferentes dosagens e submetidas a ensaios de Granulometria, Compactação, CBR e Módulo de Resiliência. Os resultados destes ensaios foram analisados e constatou-se que as misturas apresentaram comportamentos diferentes, que variaram de acordo com as dosagens dos materiais e o tipo de solo utilizado. Os melhores resultados foram observados nas misturas realizadas com 50% de solo e 50% de material cerâmico. A mistura composta por 50% de solo coletado na jazida de Tubarão e 50% de cacos de telhas foi utilizada como camada de sub-base no cálculo de dimensionamento de um pavimento localizado na Rua Hilário José de Mello, no município de Tubarão. A partir dos resultados obtidos, concluiu-se que os resíduos de cerâmica vermelha podem ser utilizados em misturas com solo, na construção de camadas de pavimentos com baixo volume de tráfego

Um Sistema de Gerência de Pavimentos pode ser definido como sendo um sistema atividades interconectadas que envolve a identificação e a seleção de estratégias, com a finalidade de manter o nível de serviço dos pavimentos pertencentes a uma rede rodoviária. Este sistema está embasado em princípios de engenharia econômica para maximizar o retorno dos investimentos efetuados em uma rede rodoviária através de uma análise de benefício-custo. Este trabalho tem o objetivo de realizar um estudo experimental para a introdução de um Sistema de Gerência de Pavimentos para a rede rodoviária da Província de Maputo em Moçambique, levando em conta a nova abordagem estratégica desenvolvida pelo Governo Moçambicano para o setor Rodoviário. O estudo consistiu no uso da abordagem tradicional de análise para previsão e desempenho de pavimentos e sua incorporação no Sistema de Gerência RoadSoft-GIS. O RoadSoft-GIS é um sistema único, de baixo custo de gerenciamento de pavimentos, que foi idealizado para ser usado em agências com baixo volume de tráfego e em redes rodoviárias pequenas, como a rede rodoviária Moçambicana. A pesquisa foi realizada na rede rodoviária pertencente à Província de Maputo, que concentra cerca de 55% do tráfego total do País e para onde a maioria dos investimentos são destinados. Para isso, foram coletados dados estruturais e funcionais, por mais de 10 anos (1995-2008), e inseridos em modelos de previsão de desempenho, para entender o estado atual da rede e também projetar seu desempenho futuro. Através de análises estatísticas, foram estabelecidas correlações entre parâmetros para entrada no RoadSoft-GIS, demonstrando a sua adaptabilidade e, consequentemente, seu uso apropriado na análise da rede rodoviária de Maputo. Uma vez incorporados os dados no RoadSoft-GIS, foi utilizada a rodovia com maior volume de tráfego para avaliação do seu estado atual ao longo dos próximos 20 anos, baseando-se na atual política para o setor rodoviário do Governo Moçambicano. Esta avaliação foi realizada através de um processo de otimização e seleção da melhor estratégia para os trabalhos a serem realizados na rodovia. O processo de coleta, armazenamento, análise e correlacionamento para incorporação no RoadSoft-GIS demonstrou a adequação do sistema para a rede rodoviária de Maputo, o que abre as portas para um futuro uso do mesmo por parte dos agentes do setor rodoviário Moçambicano, apesar de alguns ajustes serem necessários nos modelos de previsão de desempenho do sistema, assim como a expansão para o uso de outros tipos de pavimento. A Pavement Management System involves the identification of the optimum strategies at various management levels, and maintaining a pavement at an adequate level of serviceability, based on sound economic and civil engineering standards, aiming to maximize the return of investment on a specific road or road network environment, justifying the cost-benefit of such strategies. The present study aimed to do an experimental research in order to adequate a Pavement Management System to the Mozambican Road Network, taking in to consideration the new Strategic Approach on Roads being developed by the Mozambican government. The intention was to use traditional pavement performance analysis and incorporate all the information into the RoadSoft-GIS system, which is a very unique low-cost survey pavement management system that was specially idealized to be used in to a low traffic and low income agencies. The main focus was Maputo province road network which accrues almost 55% of the country traffic volume and were the most of the govern investments are. There was collected road data from ten years in order to evaluate the condition of the network. 1995 These evaluations were done based on structural, functional and subjective analysis with the input granted by the different data collected. Pavement prediction models were then tested in order to understand the present stage of deterioration of the pavement, and thereafter linear correlation and statistical analysis were conducted in order to understand and incorporate these predictions into the RoadSoft GIS system, which they have shown to be quite conservative but valuable to the actual Maputo Road Network condition. Finally based on the actual expenditure budget for roads awarded by the government several simulations were done in order to the adequateness of the RoadSoft-GIS to the Maputo Road Network needs with regards of the best approach to the various works to be undertaken in order to reduce the number of failing pavement due to lack of maintenance approach by the Govern. By collecting the data, store and analyze, it has shown a possible future way to see as model, as well, the suitability of the RoadSoft-GIS, as prominent pavement management system, but further research should subsequently be done in order to refine its logistics models to adequate to other pavement types.

O presente trabalho objetiva apresentar os estudos efetuados com dados provenientes de fontes de informações de volumes de tráfego e pesagens da Rodovia Presidente Dutra, BR-116 no trecho de ligação entre as cidades de São Paulo e Rio de Janeiro. Para tanto, foram analisados os dados obtidos por contagens feitas no ano de 2009 em analisadores de tráfego, que são laços indutores, distribuídos ao longo da rodovia, volumes de veículos comerciais contados nas seis praças de pedágio durante o período de 1997 a 2009 classificados por número de eixos, dados provenientes de duas balanças dinâmicas em um período de três meses com pesagens classificadas por eixo. Dados a respeito de áreas de aplicação de carga foram obtidos em atividade de campo, assim como cronometragem de tempo de percurso de veículos comerciais em condições de trechos em rampas, realizada na Serra das Araras. Todos os dados foram avaliados de maneira a caracterizar o comportamento da frota, determinar fatores de tráfego como tipo de veículos, cargas totais e por eixos, pressão de inflação de pneus, áreas de contato entre pneu e pavimento e velocidades de deslocamento, que concorrem à obtenção dos parâmetros de análise de estruturas de pavimento. Dentre os resultados obtidos, destaca-se a distribuição de cargas por eixo e por tipo de veículo, evidenciando as faixas de cargas que são praticadas para cada situação. Os estudos revelaram porcentagens significativas na ordem de 30% de eixos pesados acima da carga máxima legal e abaixo do limite adicionado da tolerância de 7,5% permitida. Os dados provenientes dos analisadores de trafego mostram que, de maneira geral, os veículos comerciais compõem em torno de 32% da frota circulante e destes veículos, aproximadamente 75% dos veículos comerciais trafegam na faixa mais carregada. Os dados de área de contato entre pneu e pavimento e cronometragem dos tempos de percurso em rampas permitem análises mais aprofundadas de estruturas de pavimento, considerando-se simulações e análises termoviscoelásticas, evidenciando a aplicação dos parâmetros obtidos nesta pesquisa. Os dados foram concedidos pela Companhia de Concessões Rodoviárias - CCR Novadutra, portanto tratam de uma frota volumosa que quase não apresenta sazonalidades anuais geralmente observadas pelos escoamentos de produções agrícolas This research aims to present studies done with data provided by information sources related to traffic volume and vehicles weight from Presidente Dutra Highway, BR-116, wich connects São Paulo City and Rio de Janeiro City. It has been taken data from analyzers devices with magnetic loops all highway long for 2009, data collection from six tool boots during the period from 1996 to 2009, three mouths data from two weight-in- motion stations with values sorted by axles. Data related to contact area of tire and pavement and timing of heavy trucks passing by places with steep grade has been done at Serra das Araras in Rio de Janeiro State. All data were evaluated in order to characterize the behavior of the fleet, to determine traffic factors such as type of vehicle, total loads and axles, tire inflation pressure, contact area between tire and pavement and travel speeds, needed as parameters obtained for structure pavement analysis. Among the obtained results, there is the distribution of axle loads and by type of vehicle, showing the ranges of loads that are applied to each situation. The studies revealed significant percentages of around 30% of axle with loads over maximum legal load limit permitted and under the limit added to 7.5% tolerance allowed. Data from traffic analyzers show, in general, that commercial vehicles sum about 32% of fleet vehicles, and approximately 75% of commercial vehicles traveling in the heavy traffic lane. Tire contact area between tire and pavement and timing at steep grade make possible to analyze pavement structures deeply, considering thermoviscoelastics simulations and analysis, highlighting obtained parameters in this research. Data collect had been provided by CCR NovaDutra and it is a very big sample with a huge traffic volume and with almost insignificant annual seasonality from harvest production transport

Na presente dissertação desenvolve-se uma avaliação dos Fatores de Veículos em Rodovias Federais no Estado do Paraná, baseado em pesquisas realizadas através de pesagens em 7 Postos de Pesagens de Veículos, em rodovias federais concessionadas, durante o ano de 2002. O objetivo principal da pesquisa é a criação de um banco de dados, suficiente para permitir avaliações mais atualizadas das cargas por eixo nas rodovias, e, principalmente, propiciar uma atualização dos valores dos Fatores de Veículos. Os atuais valores de FV calculados pelo DNER (atual DNIT), baseados em pesagens de 1986, estão desatualizados, face à introdução de novos tipos de veículos transportadores de cargas que adentraram no mercado nos últimos tempos, além de modificações ocorridas na legislação de trânsito no Brasil. Para calcular os valores atualizados dos Fatores de Veículos buscaram-se equações originais dos Fatores de Equivalência de Cargas das metodologias norte-americanas da American Association of State Highway and Transportation Officials (AASHTO) e da United State Army Corp of Engineers (USACE). Ao todo foram coletadas aproximadamente 150.000 pesagens de veículos da frota circulante nas rodovias federais BR-277 (Guarapuava - Paranaguá) e da BR-376 (Norte do Paraná - Curitiba), que constituem dois dos mais importantes corredores rodoviários do Estado do Paraná. Os dados tabulados fornecerão aos projetistas de pavimentos, valores mais atualizados dos Fatores de Veículos, melhorando a adequação nos projetos de estruturas de pavimentos, principalmente no que se refere ao cálculo do número N. Ainda dentro do trabalho estão contidas duas análises paramétricas mostrando e analisando a modificação do carregamento ocorrido no trecho da BR-277 (Curitiba-Campo Largo) em 1986 e 2002, e avaliação de um dimensionamento do pavimento, reavaliando o tráfego e introduzindo novos valores de Fatores de Veículos, de um trecho construído e entregue ao tráfego no final de 2002. Por último a apresentação do programa computacional MEPDG, conhecido como DG/2002, inserido no Guia da AASHTO/2002, como uma ferramenta mais poderosa e atual no dimensionamento estrutural de pavimentos em geral, baseados em critérios mecanísticos-empíricos.

145. Análise da influência da duplicação de uma rodovia no tipo e gravidade dos acidentes de trânsito: estudo de caso na Rodovia SC-401

É indiscutível a importância de estudos sobre os acidentes de trânsito em nosso país. A segurança viária tem como um de seus objetivos analisar as condições das vias, buscando alternativas que minimizem os riscos de acidentes. Este trabalho tem por objetivo estudar os efeitos da duplicação da Rodovia SC-401 nos acidentes de trânsito, comparando os acidentes ocorridos durante três anos antes do início das obras e durante três anos após o término das obras de duplicação. A SC-401 consiste na rodovia de ligação entre o Centro de Florianópolis e as praias do Norte da Ilha de Santa Catarina, tratando-se de uma rodovia que apresenta aumento significativo no volume de tráfego nos meses de verão. Com o estudo foi possível verificar as mudanças ocorridas nos tipos de acidentes registrados antes e após as obras de duplicação. O trecho em estudo foi dividido em sete segmentos e oito interseções que foram analisadas separadamente quanto ao número de acidentes e a severidade dos mesmos. Foi determinado o acréscimo ou diminuição de cada tipo de acidente para cada segmento e interseção e foram estudadas as prováveis causas. Foram calculadas duas taxas de acidentes: a primeira dividindo-se o número de acidentes pelo Volume Médio Diário Anual (VMDA) do segmento ou interseção, e a segunda dividindo a Unidade Padrão de Severidade (UPS) pelo VMDA. Foi observado, de forma geral, que nos segmentos duplicados houve uma diminuição da gravidade dos acidentes e aumento da taxa de acidentes em quase toda rodovia estudada. Este estudo pretende fornecer subsídios a estudos futuros, intensificando o interesse para a análise dos efeitos causados à segurança viária após intervenções em uma rodovia, demonstrando a necessidade de estudos do tipo antes e depois.

O objetivo deste estudo foi verificar a eficiência de serviços de restauração de pavimento, englobando a análise dos projetos e a execução dos serviços, através de uma análise comparativa do comportamento estrutural e funcional antes e depois dos serviços concluídos. Foram levantados dados de projeto, de controle do comportamento funcional e estrutural, anteriores e posteriores aos projetos, realizados ensaios em laboratório, simulações de comportamento e estimativa de vida de fadiga com programas computacionais. Todos os resultados obtidos foram analisados, juntamente com a análise visual obtida através de inspeção em campo. O produto final, após a análise estrutural e funcional de todos os segmentos, com suas perspectivas de vida remanescente, atendimento ao previsto nos projetos de restaurações, definição dos problemas ocorridos, suas causas e necessidade de intervenções corretivas, foi a conclusão da eficiência ou não das intervenções realizadas. Programa de Pós-Graduação em Engenharia Civil Este trabalho propõe um método para a avaliação das condições funcionais de revestimentos de pavimentos flexíveis urbanos, para fins de manutenção. Além dos defeitos de superfície, normalmente considerados, o método proposto leva em conta a macrotextura do revestimento e a composição e o volume de tráfego por faixa. Para os revestimentos flexíveis foi definido um Índice de Funcionalidade do Pavimento - IFP, baseado no método proposto por FREITAS (2002), utilizando as ponderações de defeitos preconizadas pelo procedimento DNIT 006/2003-PRO. Sendo a área de estudo uma área urbana, levou-se em conta a composição do tráfego. Considerou-se três tipos de veículos: automóveis, ônibus e caminhões, diferindo das metodologias convencionais que desconsideram o tráfego de automóveis ou assumem baixos percentuais de volumes de tráfego para estes. Em áreas urbanas, onde o tráfego de automóveis é majoritário, algumas vias com altos percentuais de veículos pesados (caminhões e ônibus) merecem ser destacadas das demais, tendo em vista que estes contribuem mais significativamente para o desgaste superficial dos revestimentos asfálticos. Todas estas informações foram armazenadas e processadas em ambiente SIG, com o objetivo de facilitar o manuseio, o processamento, a visualização e a análise dos dados coletados. A determinação e a visualização dos segmentos viários em condições mais desfavoráveis é realizada, comprovando-se a utilidade do método proposto para fins de priorização dos trechos mais necessitados de intervenções de manutenção. O método proposto mostrou-se vantajoso, como uma complementação dos demais, no aperfeiçoamento dos Sistemas de Gerência de Manutenção de Pavimentos Urbanos - SGMPU. Concluiu-se que o método proposto oferece um aperfeiçoamento em relação à avaliação funcional dos pavimentos e em relação ao tratamento do tráfego veicular. Sugestões são dadas no sentido do aperfeiçoamento dos métodos analisados, no contexto da formulação de Sistemas de Gerência de Pavimentos Urbanos que atendam as necessidades das cidades brasileiras.

Neste trabalho foi realizada a avaliação da macrotextura de revestimentos de concreto asfáltico usinado a quente de um grupo de rodovias da malha viária estadual de Santa Catarina. Foram escolhidos trechos de rodovias situadas na região insular do município de Florianópolis, abrangendo seis rodovias (SC 400, SC 401, SC 402, SC 406 Norte, SC 406 Sul e Via Expressa Sul). A avaliação consistiu na classificação e comparação da macrotextura com a lâmina d'água provável sobre a pista. A classificação foi feita por meio de dois ensaios de campo, o Ensaio de Mancha de Areia e de Drenabilidade. A flecha na trilha de roda também foi medida para verificar a possibilidade de ocorrência de poças ou corredeiras ao longo da pista. A lâmina d'água foi calculada e comparada à altura média da macrotextura de cada revestimento, com o objetivo de verificar se a rugosidade seria capaz de expulsar a água da superfície do pavimento. Com os valores da espessura da lâmina d'água, pôde-se calcular a velocidade limiar crítica de hidrolanagem para cada trecho analisado e constatar que as velocidades estão muito próximas à velocidade de tráfego permitida na rodovia.

149. Análise do comportamento de pavimentos de rodovias estaduais de Goiás: estudo de caso

O conhecimento do comportamento dos pavimentos é parte fundamental para a administração de uma rede rodoviária. Baseado nesta afirmação, este trabalho de dissertação consistiu na análise do comportamento dos pavimentos da rede rodoviária estadual de Goiás, representados por uma amostra, por meio da observação das linhas de tendências de comportamento e pela retroanálise de bacias de deflexão para obtenção dos módulos elásticos das camadas do pavimento e do subleito. Os dados utilizados nesta pesquisa foram obtidos junto ao órgão responsável pela malha rodoviária estadual, a Agência Goiana de Transportes e Obras Públicas (AGETOP) e são provenientes de dois levantamentos realizados em nível de rede, nos anos de 1996 e 2001, e outros dois em nível de projeto, nos anos de 1998 e 2000. A amostra selecionada para esta pesquisa foi composta apenas por pavimentos que nunca sofreram intervenção significativa, sendo a mesma representada por 236 trechos, totalizando 3.976 quilômetros de rodovias. Baseando-se numa metodologia apresentada por NUÑEZ e SHAHIN em 1986, a amostra foi dividida em duas "famílias", dependendo do tipo de revestimento apresentado, Tratamento Superficial Duplo (TSD) ou Concreto Betuminoso Usinado a Quente (CBUQ). Para cada família foram determinadas as linhas de tendência de comportamento dos pavimentos em relação aos parâmetros Deflexão, Irregularidade Longitudinal, Afundamento de Trilha de Roda e Trincamento. As linhas de tendência obtidas foram comparadas com equações de previsão de desempenho existentes. A determinação dos módulos elásticos das camadas dos pavimentos e do subleito "in situ", foi feita a partir da retroanálise das bacias de deflexão medidas em campo provenientes de 49 trechos com o auxílio do programa computacional ELSYM-5. Com os resultados obtidos foi possível verificar a evolução dos parâmetros caracterizadores da condição dos pavimentos em relação ao tráfego suportado para os dois tipos de revestimentos predominantes na rede rodoviária estadual de Goiás.

Levantamentos de condição de superfície têm sido largamente utilizados como alternativa da determinação do desempenho, manutenção e reabilitação de pavimentos de concreto. Dentre estes levantamentos destacam-se os levantamentos de defeitos, de deflexões e de aderência. Com o objetivo de verificar a condição funcional e estrutural dos pavimentos de concreto através dos levantamentos realizados, foram analisadas as condições de superfície através da determinação do Índice de Condição do Pavimento, das condições de derrapagem através dos resultados do equipamento Griptester, da macrotextura através dos ensaios de mancha de areia e drenabilidade, bem como, da condição estrutural através de métodos não destrutivos com a utilização do equipamento do tipo Falling Weight Deflectometer. Foram realizadas as análises dos procedimentos dos levantamentos mencionados e do resultado dessas avaliações determinou-se o comportamento das estruturas. Por último, com base em critérios técnicos foi elaborado um fluxograma para tomada de decisão para seleção de serviços adequados de manutenção e/ou reabilitação de pavimentos de concreto viários.
