Objective To evaluate, through a biomechanical assay, the maximum load, energy, and displacement necessary for the occurrence of fractures in synthetic models of femurs after the removal of cannulated screws and the performance of a reinforcement technique with polymethylmethacrylate (PMMA) in different combined positions. Methods In total, 25 synthetic bones were used, and they were divided into 4 groups: the control group (CG), with 10 models without perforation, and the test groups (A, B and C), with 5 models each. The test groups were fixed with cannulated screws using the Asnis technique, and they had the synthesis removed, and two of the holes formed by the reinforcement technique with PMMA were filled. The biomechanical analysis was performed simulating a fall on the large trochanter using a servo-hydraulic machine. Results All specimens of the CG and of groups A, B and C presented basal-cervical fracture of the femoral neck, except for a single model in group B, which presented a longitudinal fracture. An average of 5.4 mL of PMMA were used to reinforce the groups with filling. According to the analysis of variance (ANOVA) and the Tukey multiple comparison test, at the level of 5%, we observed that the CG presented significant differences in relation to groups A and C in the following parameters: maximum load, energy up to the fracture, and displacement. Conclusion We observed that groups A and C, when compared to the CG, showed significant differences in the observation of displacement, maximum load, and energy until the fracture.

Popularly known as Embiratanha, Pseudobombax marginatum is a widely distributed species among biomes, exclusive to South America, which belongs to the Malvaceae family. In folk medicine it is used as an anti-inflammatory in the treatment of ulcers, gastritis and back pain. This study was conducted to identify and quantitate the polyphenolic compounds present in the methanol extracts of the bark, roots, leaves, flowers and fruit of P. marginatum, as well as to evaluate the antioxidant capacity, and in addition α-glucosidase and acetylcholinesterase inhibition. The individual constituents of the methanol extract were identified and quantitated by HPLC-UV. The antioxidant capacities of the samples were evaluated by the DPPH and hypoxanthine/xanthine oxidase assays. Inhibition of α-glucosidase and acetylcholinesterase enzymes of the methanol extracts was evaluated in vitro. Eight polyphenolic compounds were detected and quantified in methanol extracts from the various botanical parts of P. marginatum.The highest concentration of individual phenolics were 6-methoxyquercetin glucoside (87.20 g/kg) and procyanidin B2 (60.53 g/kg) in the leaf extracts. In the DPPH assay, the strong antioxidant capacity was evident in the methanolic extracts of the bark (IC50 = 2.1 ± 0.01 µg/mL). The hypoxanthine/xanthine oxidase assay confirmed most of the results obtained with DPPH. The lowest IC50 values ​​in relation to α-glucosidase inhibition were obtained for the bark (IC50 = 0.87±0.01 μg/mL) and leaf extracts (IC50 = 1.14±0.19 μg/mL). The hexane extracts of the leaves (12 mm) and methanol (10 mm) of the fruits indicated the most promising data of anticholinesterase activity. Methanol extracts of P. marginatum botanical parts may contribute to the formulation of effective drugs for the treatment of Type 2 diabetes. Further studies are needed on their extracts and their therapeutic action. FUNCAP Conhecida popularmente como Embiratanha, Pseudobombax marginatum é uma espécie de ampla distribuição entre biomas, exclusiva da América do Sul, que pertence à família Malvaceae. Na medicina popular é utilizada como anti-inflamatório no tratamento de úlceras, gastrites e dores lombares. Este estudo foi realizado para identificar e quantificar os compostos polifenólicos presentes nos extratos de metanol da casca, raízes, folhas, flores e frutos de P. marginatum, bem como avaliar a capacidade antioxidante e, além disso, a inibição da α-glicosidase e acetilcolinesterase. Os constituintes individuais do extrato de metanol foram identificados e quantificados por CLAE-UV. As capacidades antioxidantes das amostras foram avaliadas pelos ensaios de DPPH e hipoxantina/xantina oxidase. A inibição das enzimas α-glicosidase e acetilcolinesterase dos extratos de metanol foram avaliadas in vitro. Oito compostos polifenólicos foram detectados e quantificados em extratos de metanol das várias partes botânicas da P. marginatum. As maiores concentrações de fenólicos individuais foram o glicosídeo de 6-metoxiquercetina (87,20 g/kg) e procianidina B2 (60,53 g/kg) nos extratos das folhas. No ensaio DPPH, a forte capacidade antioxidante foi evidente nos extratos metanólicos da casca (IC50 = 2,1 ± 0,01 µg/mL). O teste de hipoxantina/xantina oxidase confirmou a maioria dos resultados obtidos com DPPH. Os menores valores de IC50 em relação à inibição da α-glicosidase foram obtidos para a casca (IC50= 0,87±0,01 μg/mL) e extratos de folhas (IC50 = 1,14±0,19 μg/mL). Os extratos hexanicos das folhas (12 mm) e metanólicos (10 mm) dos frutos indicaram os dados mais promissores de atividade anticolinesterasica. Os extratos de metanol das partes botânicas de P. marginatum podem contribuir para a formulação de medicamentos eficazes para o tratamento da diabetes tipo 2. Mais estudos são necessários sobre seus extratos e sua ação terapêutica.

O grupo de gestão executou as atividades do programa com êxito, em função de sua capacidade de governo e do apoio político e administrativo da Secretaria Estadual de Saúde, ainda que não tenha havido incremento de recursos financeiros para potencializar os componentes do programa. Evidenciou-se o envolvimento dos atores da coordenação da supervisão e da supervisão médica na tentativa de reorientar a formação médica, apostando em dispositivos pedagógicos e outros recursos educacionais. We present a unique case of displaced simultaneous bilateral fractures, Garden 3 type, in a 49 year woman treated with non-cemented total hip arthroplasty. The patient showed a Harris hip score of 86 on the right hip and a 81 on the left side on the fourth postoperative year, besides a bilateral Trendelenburg gait, more pronounced on the right side. She needed a cane to walk, and felt pain in the left thigh. The X-ray showed a shortening of 0.9 cm and a left femoral varus. The other arthroplasty components showed good osseointegration and position. We found that the use of the uncemented total hip arthroplasty to treat a simultaneous bilateral fracture in renal osteodystrophy patients has satisfactory results at a four year follow-up. According to the OCEBM Levels of Evidence Working Group, this study is graded as a Level of Evidence IV.

This case report describes the surgical treatment and one-year follow-up of an adult male patient, who was treated for a severe anterior open hip dislocation fracture with no sign of femoral head necrosis and maintaining a Harris Hip Score (HHS) of 93.

Introduction: Fracture of the femoral neck in a young adult with an ipsilateral transfemoral amputation is uncommon, and there is no consensus regarding the best treatment. Methods: We present a unique case of a displaced right femoral neck fracture treated with closed reduction and percutaneous screw fixation in a twenty-eight-year-old woman with a preexisting ipsilateral transfemoral amputation. Results: We observed satisfactory healing of the fracture with a slight varus deformity and no radiographic signs of osteonecrosis at one year. Conclusion: In a young adult with a preexisting transfemoral amputation and an acute femoral neck fracture, reduction and fixation of the fracture may be a reasonable treatment option.

No Brasil, existe uma distribuição desigual de médicos atuando na Atenção Primária à Saúde em regiões de difícil acesso. Em 2011, foi criado o PROVAB que se fundamenta no apoio e incentivo para que profissionais de saúde atuem de forma supervisionada na atenção básica de municípios com carência de profissionais. Este estudo teve como objetivo avaliar o grau de implantação do PROVAB no Estado da Bahia. Trata-se de uma pesquisa avaliativa do tipo qualitativa, exploratória, e elegeu-se o componente 1b da Análise de Implantação. No que concerne à descrição do grau de implantação do PROVAB, foram considerados 18 critérios reunidos em três dimensões, a saber: condições funcionais e de trabalho, pedagógica e gestão. Os elementos críticos do processo de implantação foram analisados mediante o uso do triângulo de governo proposto por Matus. Para cumprir os objetivos desse estudo foram analisados documentos do PROVAB e realizadas entrevistas semiestruturadas com membros da Comissão Coordenadora Estadual do PROVAB na Bahia. As evidências foram sistematizadas em uma matriz de análise derivada do modelo lógico do PROVAB, que contém critérios e padrões para as dimensões estabelecidas. A matriz e o modelo lógico foram apreciados por um grupo de especialistas através da técnica de consenso simplificada. Os documentos foram analisados a partir da técnica de conteúdo de Bardin. Os resultados evidenciaram que o PROVAB no Estado da Bahia alcançou 84% de implantação, dos 18 critérios avaliados, 13 (72%) foram considerados implantados, 4 (22%) implantados parcialmente e 1 (6%) não foi implantado. Foi evidenciado que o grupo de gestão do PROVAB conseguiu executar as atividades de implantação do PROVAB com capacidade técnica e habilidades gerenciais que permitiram alcançar êxito, principalmente, devido a capacidade de governo do grupo e ao apoio político e administrativo da Secretaria de Saúde do estado da Bahia. Entretanto, não foi evidenciado o incremento de recursos financeiros para potencializar os componentes do Programa. Evidenciou-se o envolvimento dos atores da coordenação da supervisão e da supervisão médica na tentativa de reorientar a formação médica apostando em dispositivos pedagógicos e recursos educacionais.

Objective:To statistically analyze the results obtained from biomechanical tests on fixation of femoral neck fractures of Pauwels III type, in synthetic bone, using the dynamic hip system with an anti-rotation screw, versus a control group.Methods:Ten synthetic bones from a Brazilian manufacturer (model C1010) were used and divided into two groups: test and control. In the test group, fixation of an osteotomy was performed with 70° of inclination at the level of the femoral neck, using DHS with an anti-rotation screw. The resistance of this fixation was evaluated, along with its rotational deviation at 5 mm of displacement (phase 1) and at 10 mm of displacement (phase 2), which was considered to be failure of synthesis. In the control group, the models were tested in their entirety until femoral neck fracturing occurred.Results:The test values in the test group (samples 1-5) in phase 1 were: 1512N, 1439 N, 1205 N, 1251N and 1273 N, respectively (mean = 1336 N; standard deviation SD] = 132 N). The rotational deviations were: 4.90°, 3.27°, 2.62°, 0.66° and 0.66°, respectively (mean = 2.42°; SD = 1.81°). In phase 2, we obtained: 2064 N, 1895 N, 1682 N, 1713 N and 1354 N, respectively (mean = 1742 N; SD = 265N). The failure loading values in the control group were: 1544 N, 1110 N, 1359 N, 1194 N and 1437 N, respectively (mean = 1329 N; SD = 177N). The statistical analysis using the Mann-Whitney test showed that the test group presented maximum loading at a displacement of 10 mm, i.e. significantly greater than the failure loading of the control group (p = 0.047).Conclusion:The mechanical resistance of the test group was significantly greater than that of the control group. Objetivo:Analisar estatisticamente resultados obtidos em ensaios biomecânicos de fixação de fratura do colo femoral tipo Pauwels III, em osso sintético, com o uso do sistema dinâmico do quadril (DHS) com parafuso antirrotatório vs um grupo controle.Métodos:Foram usados dez ossos sintéticos, de um fabricante nacional, do modelo C1010, divididos em dois grupos: teste e controle. No grupo teste foi feita fixação de osteotomia, com 70° de inclinação em nível de colo femoral, com o uso de DHS com parafuso antirrotatório. Avaliou-se a resistência dessa fixação e seu desvio rotacional em 5 mm de deslocamento (fase 1) e em 10 mm de deslocamento, considerado como falência da síntese (fase 2). No grupo controle, os modelos foram ensaiados em sua integridade até que ocorresse a fratura do colo femoral.Resultados:Os valores do ensaio no grupo teste na fase 1, nas amostras de 1 a 5, foram: 1.512 N, 1.439 N, 1.205 N, 1.251 N e 1.273 N, respectivamente (média = 1.336 N; desvio padrão DP] = 132 N). Os desvios rotacionais foram: 4,90°; 3,27°; 2,62°; 0,66° e 0,66°, respectivamente (média = 2,42°; DP = 1,81°). Na fase 2, obtivemos: 2.064N, 1.895 N, 1.682N, 1.713 N e 1.354N, respectivamente (média = 1.742 N; DP = 265 N). Os valores da carga de falência no grupo controle foram: 1.544N, 1.110N, 1.359N, 1.194N e 1.437N, respectivamente (média = 1.329N; DP = 177 N). A análise estatística pelo teste de Mann-Whitney demonstrou que o grupo teste apresentou carga máxima, em 10 mm de deslocamento, significativamente maior do que a carga de falência do grupo controle (p = 0,047).Conclusão:A resistência mecânica do grupo teste foi significativamente superior à do grupo controle.

OBJECTIVE: To analyze statistically results obtained between biomechanical assays on fixation of femoral neck fracture type Pauwels III, on synthetic bone, using 7.5 mm non parallel cannulated screws and control group. METHODS: Ten synthetic bones from a national brand were used. Test group: fixation of 70(o) tilt femoral neck osteotomy was performed using three 7.5 mm non parallel cannulated screws. We analyzed the resistance of this fixation with 5 mm of displacement, and rotational deviation (phase 1) and with 10 mm offset (phase 2). Control group: the models were tested in their integrity until the femoral neck fracture occurred. RESULTS: the values of the test group in phase 1, in sample 1-5 had a mean of 517N and SD = 96N. Rotational deviations showed a mean of 3.79° e SD = 2. 03°. In phase 2, mean was 649N and SD = 94N. The values of the maximum load in the control group were: 1544N, 1110N, 1359N, 1194N, 1437N; respectively. Statistical analysis between the groups showed a statistically significant lower value in the test group. CONCLUSION: the analysis of mechanical resistance between the groups has determined statistically significant value for the test group. Level of Evidence III, Case-control Study.

Aim: This in vitro study evaluated the effect of using a dental hypersensitivity treatment on the bond strength to dentin of etch-and-rinse and self-etching simplified adhesive systems. Methods: 40 healthy molars were used. The crowns of teeth were removed at the dentinoenamel junction, and remaining roots were sectioned in the mesiodistal direction along their long axis, separating the buccal and lingual/palatal halves. The segments obtained were ground, keeping the buccal or lingual/palatal face turned downwards in order to flatten the surface and produce a standardized smear layer. Prepared specimens were randomly distributed into 4 groups (n=10), according to the combination of surface treatment with the desensitizing dentifrice Colgate Sensitive Pro-Relief (Colgate Palmolive®) (test group - with dentifrice and control group - without dentifrice) and type of simplified bonding agent (etch-and-rinse and self-etching agents). Resin composite blocks were fabricated on the dentin surfaces, after application of bonding agents, and specimens were sectioned into rectangular stick-shaped specimens with cross-sectional area of approximately 0.8mm2 . For microtensile bond strength tests, 4 sticks were randomly selected from the central region of specimens, and fixed to a universal test machine. Data were analyzed statistically by the Mann-Whitney U test (a=0.05). Results: The bond strength obtained in the group without desensitizer (control) + etch-and-rinse agent was significantly higher compared with the other groups. Under both experimental conditions, the self-etching agent showed weak bond strength to dentin. Conclusions: Change in the dentin substrate by obliteration of tubules in the process of relieving dentinal sensitivity could reduce the bond strength of simplified etch-and-rinse and self-etching bonding agents.

Satisfactory healing of the osteoporotic fracture is critically important to functional recovery, morbidity, and quality of life. Some therapies for osteoporosis may affect the processes associated with bone repair. For example, bisphosphonates in experimental models are associated with increased callus size and mineralization, reduced callus remodeling, and improved mechanical strength. Local and systemic bisphosphonate treatment may improve implant fixation. No negative impact on fracture healing has been observed, even after major surgery or when administered immediately after fracture. For the osteoanabolic agent teriparatide, case reports and a randomized trial have produced mixed results, but they are consistent with a positive impact of teriparatide on fracture healing. Some of the agents currently being developed for osteoporosis, notably sclerostin and DKK1 antibodies have shown a beneficial effect on fracture healing. At this point, therefore, there is no evidence that osteoporosis therapies are detrimental to fracture healing with some promising experimental evidence for positive effects on healing, notably for those agents whose actions are primarily anabolic. A consolidação adequada da fratura osteoporótica é essencial para recuperação funcional, morbidade e qualidade de vida. Alguns tratamentos para osteoporose podem afetar os processos associados ao reparo ósseo. Por exemplo, os bisfosfonatos, em modelos experimentais, estão associados com aumento do tamanho do calo e a mineralização, reduzindo o remodelamento do calo e melhorando a força mecânica. Tratamento com bisfosfonato, local ou sistêmico, pode melhorar a fixação de implantes. Não foi observado impacto negativo na consolidação da fratura, mesmo após uma cirurgia maior ou quando administrado imediatamente após a fratura. Relatos e um estudo randomizado com o osteoanabólico teriparatida produziram resultados mistos, mas que são consistentes com o impacto positivo da teriparatida na consolidação da fratura. Algumas das drogas que estão sendo desenvolvidas no momento para osteoporose, como anticorpos para esclerotina e DKK1, têm mostrado efeito benéfico na consolidação da fratura. No estágio atual, portanto, não há evidência de que terapias para osteoporose impeçam a consolidação da fratura, mas existem algumas evidências experimentais promissoras de efeito positivo na consolidação, especialmente daqueles agentes cuja ação é primariamente anabólica.

To evaluate, through biomechanical testing, the resistance to and energy required for the occurrence of proximal femoral fracture in synthetic bone after removal of a proximal femoral nail model (PFN), comparing the results obtained with a reinforcement technique using polymethylmethacrylate (PMMA).Fifteen synthetic bones were used: five units for the control group (CG), five for the test group without reinforcement (TGNR), and five for the test group with reinforcement (TGR). The biomechanical analysis was performed simulating a fall on the trochanter using a servo-hydraulic machine. In the GC, the assay was performed with the PFN intact. In the TGNR and TGR groups, a model of PFN was introduced and the tests were performed in the TGNR, after simple removal of the synthesis material, and in the TGR, after removal of the same PFN model and filling of the cavity in the femoral neck with PMMA.All groups presented a basicervical fracture. The CG presented a mean of 1427.39 Newtons (N) of maximum load and 10.14 Joules (J) of energy for the occurrence of the fracture. The TGNR and TGR presented 892.14 N and 1477.80 N of maximum load, and 6.71 J and 11.99 J of energy, respectively. According to the Kruskal-Wallis ANOVA, there was a significant difference in the maximum load (The simple removal of a PFN in synthetic bone showed a significant reduction of the maximum load and energy for the occurrence of fracture, which were re-established with a reinforcement technique using PMMA.Avaliar, por meio de ensaio biomecânico, a resistência e a energia necessária para ocorrência de fratura do fêmur proximal em osso sintético após retirada de um modelo de haste de fêmur proximal (PFN) e comparar os resultados obtidos com técnica de reforço com polimetilmetacrilato (PMMA).Foram usados 15 ossos sintéticos: cinco unidades para o grupo controle (GC), cinco para o grupo teste sem reforço (GTS) e cinco para grupo teste com reforço (GTC). A análise biomecânica foi feita e simulou queda sobre o trocânter com máquina servo-hidráulica. No GC, o ensaio foi feito com sua integridade intacta. Nos grupos GTS e GTC, foi introduzido um modelo de PFN e os ensaios foram feitos no GTS, após simples retirada do material de síntese, e no GTC, após retirada do mesmo modelo de haste e preenchimento do pertuito no colo com PMMA.Todos os grupos apresentaram fratura basocervical. O grupo GC apresentou média 1.427,39 Newtons (N) de carga máxima e 10,14 Joules (J) de energia para a ocorrência da fratura. Os grupos GTS e GTC apresentaram 892,14 N e 1.477,80 N de carga máxima e 6,71 J e 11,99 J de energia, respectivamente. Segundo a Anova de Kruskal–Wallis, existe diferença significativa na carga máxima (p = 0,009) e na energia (p = 0,007) entre esses grupos.A simples retirada de um PFN em osso sintético apresentou redução significativa da carga máxima e da energia para a ocorrência de fratura, que foram reestabelecidas com uma técnica de reforço com PMMA.

The Witches' Broom Disease, caused by the hemibiotrophic basidiomycete fungus Moniliophthora perniciosa, drastically reduced the production of cocoa in Brazil. Phytosanitation, chemical control, genetically resistant strains, and biological control still leave flaws in the disease eradication process. Effort has been expended in the elucidation of molecular targets, in particular, the structural components of the fungal cell wall, such as the beta(1,3)-D-glucan synthase. This enzyme is essential for the cellular construction of the wall, as it catalyses the formation of beta(1,3)-D-glucans. Protein structure homology modeling approaches are able to determine the structure of proteins without performing experimental steps, considering the barriers related to experimental methods for the structural determination of molecular targets. The presence of the conserved catalytic residues in members of the same glycosyltransferase family and overall structural analysis suggests that they catalyze glycosyl transfer reactions by similar mechanisms. Therefore, the objective of this study was to determine the three-dimensional model of the enzyme beta(1,3)-D-glucan synthase of M. perniciosa by homology modeling. Both procedures were performed to build the models: a comparative modeling by satisfaction of spatial restraints in MODELLER and a modeling by assembly of rigid bodies in the SWISS-MODEL software. The models were elected based on analysis of the stereochemistry quality and a quantitative assessment of similarity from the obtained models and to templates. A reasonable structural model was obtained of the beta(1,3)-D-glucan synthase enzyme (BegS1). The BegS1 model showed two distinct α/β domains, features of the inverting glycosyltransferase family, and the topology of the folded structure showed 7 beta-strands and 13 alpha-helices. The BegS1 model showed the presence of a catalytic cavity formed by the conserved aspartic acid residues (Asp326, 345, 353, and 354 DDxD motif) implicated in substrate binding and/or catalysis. In the BegS1 model, this cavity is near a loop region, as was observed in the GT-2 family structure. It is encouraging to find that the model for BegS1 agrees well with structures from the GT-2 enzyme family. © 2012 Wiley Periodicals, Inc.

OBJETIVO: Descrever através de estudo em cadáver a disposição anatômica do terço proximal da artéria cincunflexa femoral medial (ACFM). MÉTODOS: Foi avaliada a disposição anatômica do terço proximal da ACFM, através de dissecção em 12 quadris de cadáver, que após terem sua altura aferida em metros (M), foram determinados alguns parâmetros: Ângulo de lateralização da ACFM (Â), Profundidade da ACFM em relação a origem superomedial do músculo quadrado femoral (P), distância da ACFM em relação a inserção superolateral do músculo quadrado femoral (D). RESULTADOS: Quando comparamos a média dos parâmetros entre os sexos notamos: Masculino Â= 43.6 graus, feminino Â= 38.3 graus. Masculino D= 6mm, feminino D= 9.5mm. Masculino P= 20.8 mm, feminino P= 18.3mm. A média dos parametros  e P são menores no sexo feminino porém o parâmetro D no sexo feminino é maior. CONCLUSÃO: Devemos ter como parâmetro superficial de segurança para o terço proximal da ACFM o ponto de inserção superolateral do músculo quadrado femoral. Que o parâmetro D da ACFM no sexo feminino é maior do que no sexo masculino. Que desinserir o músculo quadrado femoral em sua origem no ísquio pode oferecer maior segurança a ACFM do que faze-lo em sua inserção femural. Nível de evidência IV, série de casos. METHODS: We evaluated the anatomic arrangement of the proximal third of the ACFM through dissection in 12 cadaver hips. After measuring their height in meters (M), several parameters were determined: angle of lateralization of ACFM (Â), Depth ACFM in relation to the superomedial origin of the quadratus femoris muscle (P), and distance of ACFM from the superolateral insertion of the muscle quadratus femoris (D). RESULTS: In the comparison of mean parameters between the sexes, it was noted that: Male  = 43.6 degrees, female  = 38.3 degrees. Male D = 6mm, female D = 9.5mm. Male P = 20.8 mm, female P = 18.3mm. The average for parameters A and P were lower in females, but parameter D was higher in females. CONCLUSION: We should use, as the secure surface parameter for the proximal third of the ACFM, the superolateral insertion point of the quadratus femoris muscle. The fact that the parameter D of ACFM is greater in females than in males. Disconnecting the quadratus femoris muscle at its point of origin in the isquio may offer greater security to ACFM than doing it at the point of femoral insertion. Level of Evidence.

Objective: To analyze statistically results in biomechanical testing of fixation of femoral neck Pauwels type III fractures, on synthetic bone, with dynamic condylar screw (DCS) and control group. Methods: Ten synthetic bones of a national brand were used. Test Group: fixation was performed after osteotomy at 70o tilt using DCS plate with four holes. We analyzed the resistance of this fixation with 5 mm displacement and rotational deviation (Step 1) and with10 mm (Step 2). Control group: the models were tested in their integrity until the femoral neck fracture occurred. Results: The values of the test group in Step 1 showed a mean of 974N and SD = 114N. In Stage 2, we obtained on average 1335N and SD = 98N. The values in the control group were: 1544N, 1110N, 1359N, 1194N, 1437N, respectively. Statistical analysis using the Mann-Whitney test for comparison of the maximum force (N) between the test group and the control, in Step 2, demonstrated that there is no significant difference between the DCS and control plates (p = 0.91). Conclusion: There is no significant difference between the DCS boards and the control group exposed to full resistance. Level of Evidence III, Case Control.

O uso das restaurações cerâmicas é constantemente indicado e utilizado nos consultórios odontológicos devido as suas qualidades estéticas e estruturais, no entanto mesmo sendo um material com excelentes propriedades mecânicas podem apresentar fraturas. A substituição total da peça protética tem custo elevado, e necessita de bastante tempo clínico. Assim, o reparo das cerâmicas utilizando resina composta fotopolimerizável tem sido uma boa alternativa em casos de fratura. O objetivo desse trabalho é avaliar, por meio de uma revisão de literatura, a influência do condicionamento ácido no reparo de cerâmicas com resina composta. A coleta de dados foi realizada através da seleção de artigos publicados nas bases de dados Scielo, LILACS e Bireme, utilizando os seguintes descritores: tratamento de superfície, resina composta e cerâmicas odontológicas. De acordo com os achados pesquisados, o reparo da superfície cerâmica fraturada pode ser realizado intra-oralmente através do tratamento mecânico e químico da superfície fraturada. Para cerâmicas ácido sensíveis é relatado na literatura a efetividade do condicionamento com ácido fluorídrico na promoção de alteração superficial, no entanto, a longevidade desta adesão entre cerâmica e resina composta após o condicionamento ácido ainda é questionável.

OBJECTIVE: To analyze the in vitro mechanical strength of the DHS-AF®. METHODS: We evaluated the in vitro resistance of a sliding bolt modified by the addition of a connector system, DHS-AF®. RESULTS: The changes performed by the authors allow the exchange by the surgeon of the sliding bolt without the need to remove the plate and disassemble the entire device to reposition it properly. We conducted a static bending test to evaluate maximum strength, stiffness and ductility of the system. DHS-AF® showed satisfactory mechanical properties when compared to other conventional devices which use the same principle. CONCLUSION: Based on these results, the authors propose the use of this new implant in further in vivo studies. Level of Evidence III, Analytical Study.

O presente trabalho visa avaliar a degradação microestrutural e variações nas propriedades mecânicas do aço 1,25Cr-0,5Mo após de 25 anos de uso em um sistema de tubulações de distribuição de vapor operando sob condições de alta pressão (10 MPa) em temperaturas elevadas (510ºC). Nesse sistema de tubulações, após 21 anos de operação, detectou-se uma trinca seguida de vazamento em uma região de concentração de tensões em um dos coletores de condensado. Desde então, tem-se monitorado por ensaios não destrutivos, a presença de trincas neste sistema. Com base nesses ensaios, em alguns testes acelerados de fluência e em análises metalográficas, foi efetuada a substituição programada de todos os coletores em outubro de 2006. A análise metalográfica de dois coletores substituídos, de um trecho de tubo e de uma curva removidos após 25 anos de operação mostra estágios avançados de degradação da microestrutura acompanhada de processo de fluência. Neste contexto, os principais objetivos deste trabalho foram concentrados nas caracterizações microestrutural e mecânica de amostras coletadas do trecho reto de tubulação, curvas e coletores. Para tanto, um conjunto de amostras foi submetido aos tratamentos térmicos de revenido e normalização seguido de revenido para avaliar a capacidade de regeneração microestrutural e mecânica. As amostras foram submetidas aos ensaios de tração à temperatura ambiente e a 510°C, ensaio de dureza e ensaio de impacto charpy antes e após os tratamentos térmicos. O trabalho foi complementado pela determinação dos principais micromecanismos de fratura. The aim of this work is to evaluate the microstructural degradation and the mechanical properties variation of a 1.25Cr-0.5Mo steel pipeline steam after nearly 25 years of operation at 510°C and loaded by internal pressure of 10 MPa. A failure occurred after 21 years of operation caused by a crack in a stress concentration area in one of the \"T\" components of this pipe. Since then, non destructive tests have been done to evaluate the damage growth. Based on these inspections, creep tests and microstructural replication all the \"T\" components were planned to be replaced in 2006. The metallographic analysis in two \"T\" components, a piece of pipe and an elbow removed 25 years of operation shows high microstructural degradation followed by creep process. In this context, the main objective of this work was the microstructure and mechanical properties characterization of the \"T\" components, the piece of pipe and the elbow. According to this, some samples were tempered and normalized-andtempered to evaluate the capability of microstructure and mechanical regeneration. These samples were tried by stress-rupture tests on ambient and 510°C temperature, hardness tests and charpy V-notch tests, before and after heat treatments. The analyses were supplement by the determination of the principal fracture mechanisms present.

OBJETIVO: Apresentar os resultados obtidos em ensaios de aplicabilidade de uma placa do tipo sistema dinâmico do quadril modificado, que permite a manipulação ou troca do parafuso deslizante, sem que haja necessidade da retirada de todo o sistema. MÉTODO: Foram utilizadas cinco placas do tipo sistema dinâmico do quadril modificado (DHS- AFpat.req.), fabricadas por uma empresa nacional, produzidas em aço inoxidável austenítico ASTM F 138, com quatro furos e ângulo de 135o, as quais foram aplicadas a cinco segmentos de osso sintético de fêmur proximal da marca Synbone. Todos os implantes foram fixados ao osso sintético, seguindo as técnicas cirúrgicas descritas pela fundação AO (Arbeitsgemeinschaft für Osteosynthesefragen). RESULTADOS: Todas as placas do sistema dinâmico do quadril modificado (DHS-AF) permitiram facilidade na manipulação e a troca do pino deslizante sem a necessidade de retirada da placa. CONCLUSÃO: Os testes in vitro de aplicabilidade do DHS-AF nos forneceram resultados promissores e nos levaram a acreditar que, após avaliações biomecânicas, para confirmar a sua segurança, podem ser reproduzidos in vivo. OBJECTIVE: This report considers the results of the applicability test of a modified dynamic hip system developed by the authors, which allows either the manipulation or the exchange of the sliding screw, without the need to remove the plate and take all the system apart in order to change its size or position. METHODS: Five modified plates - DHS-AF - manufactured with austenitic stainless steel ASTM F 138, with 4 holes and a 135o angle were inserted in five segments of synthetic bone of proximal femur (Synbone). All implants were fixed to the femur following the surgical techniques described by AO foundation (Arbeitsgemeinschaft für Osteosynthesefragen). RESULTS: The modified dynamic hip system (DHS-AF) allowed ease in handling and exchange of sliding screw without the need for plate removal. CONCLUSION: In vitro test of the applicability of DHS-AF afforded promising results and led us to believe that after biomechanical evaluation to confirm its safety it may be reproduced in vivo.

Efforts have been expended to elucidate the molecular targets, such as beta(1,3)-D-glucan synthase, a glycosyltransferase that is essential for the cell wall construction, this enzyme catalyzes the formation of 1,3-?-D-glycans structural compounds. Bioinformatics methods are able to determine the structure of proteins without performing experimental steps, considering the barriers related to experimental methods for structure determination of molecular targets. Therefore, the objective of this study was determine the three-dimensional model of the enzyme beta(1,3)-D-glucan synthase of M. perniciosa by homology modeling and study the micelial growth inhibiting of the fungus by organics compounds and vegetal extracts with potential use in cocoa plantation. To build the models, was implemented both procedures: a comparative modeling by satisfaction of spatial restraints in MODELLER and a modeling by assembly of rigid bodies in the SWISS-MODEL software. Was obtained reasonable for beta(1,3)-D-glucan synthase enzyme (BegS1), agree well with general structure from GT-2 enzyme family. In studies of fungal inhibiting growth, the MIC was considered one which there was 100% inhibition of mycelial growth. Extracts derived from Lippia alnifolia and the substance HQN showed promising results with MIC lowest values for M. perniciosa fungus. Objectives: Calcium hydroxide cements have been largely used in deep cavities due to their abilities to stimulate dentin formation. However, their resistance can be relatively low and their solubility relatively high, in many instances. This study evaluated water sorption and solubility of different calcium hydroxide cements, in order to show alterations that may reduce their effectiveness. Material and methods: Five discs (20 mm in diameter and 1.5 mm thick) of three different materials (Biocal®, Dycal® and Hidro C®) were prepared with the aid of a ring-shaped metallic matrix. After being stored at 37°C for 24 h, the discs were weighed on a precision weight scale, dehydrated and weighed again. Immediately after weighing, discs were stored for a week in 50 mL of distilled water at 37°C and, then, weighed again, dehydrated and submitted to a new weighing. The loss of soluble material and its water sorption was obtained from the difference between the initial and the final dry mass of each disc, after 1 week of immersion in water. OBJETIVO: Avaliar isolada e comparativamente placas do tipo sistema dinâmico do quadril (DHS) de dois fabricantes nacionais, analisar estatisticamente seus resultados e demonstrar a falta de determinantes para sua fabricação. MÉTODOS: Foram realizados ensaios estáticos de flexão em cinco placas DHS do fabricante I (grupo I) e em igual quantidade do mesmo modelo do fabricante II (grupo II), sendo todas fabricadas em aço inoxidável austenítico ASTM F 138, com quatro furos e angulação de 135º. Utilizou-se máquina servohidráulica MTS, modelo Test Star II®, com capacidade de carga de 10 toneladas e controle de deslocamento. Foram obtidos dados da carga aplicada (P) em função do deslocamento vertical do pistão (L), cuja velocidade foi 5 mm/min. Os ensaios foram interrompidos após atingir a deflexão vertical máxima especificada pelas normas dos ensaios. RESULTADOS: Grupo I: resistência de flexão, 161,4 ± 17,2 kgf rigidez, 64,5 ± 1,8 kgf/mm, ductilidade, > 25,4 mm. Dissertação (mestrado) - Universidade Federal de Santa Catarina, Centro Tecnológico.Programa de Pós-Graduação em Ciência da Computação Este trabalho apresenta uma proposta de um protocolo de comunicação em nível de aplicação para possibilitar a realização de teleconferências radiológicas multiponto que seja compatível com redes de banda menor encontradas no interior de nosso país permitindo a transferência de pacotes de dados e imagens. O protocolo foi batizado de "Protocolo Cyclops de Teleconferência - PCT". Para validação do PCT, foi desenvolvido um sistema voltado a telemedicina ou especificamente para a área da teleradiologia. O sistema proposto denomina-se "Sala de Laudos Virtual Multiponto - SLVM", onde seu principal objetivo é compartilhar entre os usuários participantes de uma mesma sessão, arquivos de imagem no padrão DICOM 3.0 e suas ações durante a utilização do sistema. Durante toda a sessão de utilização do sistema, os usuários poderão se comunicar através de chat e também através voz sobre IP. O protocolo PCT permite a transferência apenas de dados referentes a ações dos usuários por chat, mouse e envio de arquivos DICOM. A aplicação desenvolvida para validar o protocolo, Sala de Laudos Virtual Multiponto, permite também a comunicação entre os participantes através de voz sobre IP, portanto, foi adotado um protocolo já existente denominado REMKO que transmite voz sobre conexões UDP. Esta solução permite a criação de um ambiente cooperativo entre médicos localizados geograficamente distantes, utilizando a tecnologia Internet existente para elaboração conjunta de laudos e diagnósticos dos pacientes através de informações e embasamento das imagens compartilhadas entre os membros participantes. O resultado final deste trabalho é um ambiente multiponto que permite que dois ou mais usuários médicos possam efetuar laudos em conjunto. Este sistema é parte integrante do projeto Cyclops Brasil onde possui cooperação binacional com a Universidade de Kaiserslautern da Alemanha com a Universidade Federal de Santa Catarina. A parceria objetiva o desenvolvimento de soluções voltadas para informática médica.

O processo de trabalho adotado pelas empresas do ramo de confecções tem aumentado a competitividade nos últimos anos. Grandes transformações e pressões por parte do mercado, têm levado as empresas do setor a repensar o seu sistema de produção como um todo. A empresa foco deste estudo procura um modelo que se adapte às suas necessidades. Este trabalho procurou discutir a aplicação do conceito de polivalência em uma empresa do ramo de confecções na cidade Maringá, Estado do Paraná. Na pesquisa de campo foram feitos relatos sobre o método de trabalho adotado na empresa, confrontando um modelo anterior de produção com um novo modelo apresentado. Concluiu-se que a polivalência pode ser considerada uma ferramenta importante para a empresa na busca da produtividade em suas tarefas, não deixando de lado outras fases do processo de administração da produção, como planejamento, programação e controle.

Forty patients with intertrochanteric fractures of the femur were treated by external fixation between July 1992 and April 1994. They were selected because of their high surgical risk, and evaluated by radiographs immediately after operation, and at 30 and 90 days. The results indicate that external fixation is a useful method of treatment for this type of patient.

Objective To evaluate, through biomechanical testing, the resistance to and energy required for the occurrence of proximal femoral fracture in synthetic bone after removal of a proximal femoral nail model (PFN), comparing the results obtained with a reinforcement technique using polymethylmethacrylate (PMMA). Methods Fifteen synthetic bones were used: five units for the control group (CG), five for the test group without reinforcement (TGNR), and five for the test group with reinforcement (TGR). The biomechanical analysis was performed simulating a fall on the trochanter using a servo-hydraulic machine. In the GC, the assay was performed with the PFN intact. In the TGNR and TGR groups, a model of PFN was introduced and the tests were performed in the TGNR, after simple removal of the synthesis material, and in the TGR, after removal of the same PFN model and filling of the cavity in the femoral neck with PMMA. Results All groups presented a basicervical fracture. The CG presented a mean of 1427.39 Newtons (N) of maximum load and 10.14 Joules (J) of energy for the occurrence of the fracture. The TGNR and TGR presented 892.14 N and 1477.80 N of maximum load, and 6.71 J and 11.99 J of energy, respectively. According to the Kruskal-Wallis ANOVA, there was a significant difference in the maximum load (p = 0.009) and energy (p = 0.007) between these groups. Conclusion The simple removal of a PFN in synthetic bone showed a significant reduction of the maximum load and energy for the occurrence of fracture, which were re-established with a reinforcement technique using PMMA. Resumo Objetivo Avaliar, por meio de ensaio biomecânico, a resistência e a energia necessária para ocorrência de fratura do fêmur proximal em osso sintético após retirada de um modelo de haste de fêmur proximal (PFN) e comparar os resultados obtidos com técnica de reforço com polimetilmetacrilato (PMMA). Métodos Foram usados 15 ossos sintéticos: cinco unidades para o grupo controle (GC), cinco para o grupo teste sem reforço (GTS) e cinco para grupo teste com reforço (GTC). A análise biomecânica foi feita e simulou queda sobre o trocânter com máquina servo-hidráulica. No GC, o ensaio foi feito com sua integridade intacta. Nos grupos GTS e GTC, foi introduzido um modelo de PFN e os ensaios foram feitos no GTS, após simples retirada do material de síntese, e no GTC, após retirada do mesmo modelo de haste e preenchimento do pertuito no colo com PMMA. Resultado Todos os grupos apresentaram fratura basocervical. O grupo GC apresentou média 1.427,39 Newtons (N) de carga máxima e 10,14 Joules (J) de energia para a ocorrência da fratura. Os grupos GTS e GTC apresentaram 892,14 N e 1.477,80 N de carga máxima e 6,71 J e 11,99 J de energia, respectivamente. Segundo a Anova de Kruskal-Wallis, existe diferença significativa na carga máxima (p = 0,009) e na energia (p = 0,007) entre esses grupos. Conclusão A simples retirada de um PFN em osso sintético apresentou redução significativa da carga máxima e da energia para a ocorrência de fratura, que foram reestabelecidas com uma técnica de reforço com PMMA.

Investigation of oral administration of Saccharomyces boulardii in healthy volunteers demonstrates several cellular and humoral changes in peripheral blood. Among its effects are the increase of erythrocytes, leucocytes, polymorphs, neutrophils, complement components C3, C5, C3d, serum anticomplementary activity and leucocyte chemokinesis, specially when autologous serum and antigen have been added to the culture medium and decrease of complement haemolytic activity (CH50, classic and alternative pathways). We have also demonstrated that in vitro S. boulardii was able to activate complement directly, to fix C3b to its surface and that its phagocytosis by mononuclear cells was complement-dependent. The overall changes in serum proteins suggested changes of acute phase proteins typical of an inflammatory process. Furthermore S. boulardii had no mitogenic response of lymphocyte populations. Our results demonstrated that S. boulardii activates the reticuloendothelial system and complement system and suggest that S. boulardii merits therapeutic trial in a variety of clinical situations.
