Abstract

AIMS: Penumbral selection is best-evidence practice for thrombectomy in the 6-24 hour window. Moreover, it helps to identify the best responders to thrombolysis. Multimodal computed tomography (mCT) at the primary centre-including noncontrast CT, CT perfusion, and CT angiography-may enhance reperfusion therapy decision-making. We developed a network with five spoke primary stroke sites and assessed safety, feasibility, and influence of mCT in rural hospitals on decision-making for thrombolysis. METHODS: Consecutive patients assessed via telemedicine from April 2013 to June 2018. Clinical outcomes were measured, and decision-making compared using theoretical models for reperfusion therapy applied without mCT guidance. Symptomatic intracranial hemorrhage (sICH) was assessed according to Safe Implementation of Treatments in Stroke Thrombolysis Registry criteria. RESULTS: A total of 334 patients were assessed, 240 received mCT, 58 were thrombolysed (24.2%). The mean age of thrombolysed patients was 70 years, median baseline National Institutes of Health Stroke Scale was 10 (IQR 7-18) and 23 (39.7%) had a large vessel occlusion. 1.7% had sICH and 3.5% parenchymal hematoma. Three months poststroke, 55% were independent, compared with 70% in the non-thrombolysed group. Abstract

Plasmodium vivax is the most widely distributed human malaria parasite. Outside sub-Saharan Africa, the proportion of P. vivax malaria is rising. A major cause for concern is the re-emergence of Plasmodium vivax in malaria-free areas. Oman, situated in the south-eastern corner of the Arabian Peninsula, has long been an area of vivax malaria transmission but no locally acquired cases were reported in 2004. However, local transmission has been registered in small outbreaks since 2007. In this study, a local outbreak of 54 cases over 50 days in 2014 was analyzed retrospectively and stained blood slides have been obtained for parasite identification and genotyping. The aim of this study was to identify the geographical origin of these cases, in an attempt to differentiate between imported cases and local transmission. Using circumsporozoite protein (csp), merozoite surface protein 1 (msp1), and merozoite surface protein 3 (msp3) markers for genotyping of parasite DNA obtained by scrapping off the surface of smears, genetic diversity and phylogenetic analysis were performed. The study found that the samples had very low genetic diversity, a temperate genotype, and a high genetic distance, with most of the reference strains coming from endemic countries. We conclude that a small outbreak of imported malaria is not associated with re-emergence of malaria transmission in Oman, as no new cases have been seen since the outbreak ended.

109. The Effective of Flipped Classroom Instruction on Students' Academic Performance in Dubai Private Schools

Abstract

This study aims to investigate the extent of the impact of applying the FCI flipped classroom method on the student’s academic level and academic achievement in private schools in Dubai. The study also investigates teachers ’opinion and their views on the effectiveness of applying FCI and whether it can be used in various subjects and stages of the study. This study was conducted in Dubai in the private school sector, where 51 teachers were involved from two different schools that follow the American curriculum in the education system. The mixed approach (MMR) was also followed in the research method, where a questionnaire was used for the data collection in the quantitative approach, and the focus interview (FGD) was used for the qualitative approach. To collect data, the researcher prepared a questionnaire consisting of 25 open-ended questions and 3 other open-ended questions. In addition to holding a discussion session through the ZOOM application with a group of teachers. A paper was prepared to contain a set of (6 questions) related to the research objectives on the application of the flipped classroom, and the participants took the time and space necessary for discussion. The results were collected and analyzed through the SPSS program, and the results showed that the use of the flipped classroom method in Dubai Private Schools has a positive impact on both the academic level of students, the work of classroom activities, the promotion of teamwork among the students, the transformation of the teacher's role from lecturer to directed to students during The educational session. There were also some opinions indicating that the technical problems related to the Internet and the problems of electronic devices may impede the work of the students of distance learning, and some opinions preferred to reserve the flipped classroom method on literary subjects more than scientific materials.

111. The potential role of Helicobacter pyloriin chronic rhinosinusitis with or without nasal polyposis

Abstract

Background: Certain gastrointestinal and extra-intestinal diseases are caused by the gram-negative bacterium Helicobacter pylori(H. pylori). We aimed to determine the potential H. pylorirole in chronic rhinosinusitis (CRS) with or without nasal polyposis. This cross-sectional study was conducted on 80 subjects of any age and sex diagnosed with CRS (CRSWNP and CRSSNP) who had failed medical treatment and scheduled for surgery. Patients were further categorised into two equal groups: study group (n= 40) were diagnosed with CRS with or without nasal polyposis (CRSWNP and CRSSNP) who had failed medical treatment and scheduled for surgery, and control group (40 patients) were admitted for septoplasty. All patients had clinical assessment, physical assessment, diagnosis of rhinosinusitis (according to EPOS), based on symptoms, rhinoscopic/endoscopic findings, CT scan findings, questionnaire for gastroesophageal reflux disease (GERD), and questionnaire for laryngopharyngeal reflux (LPR). Samples were collected in the operating room during surgery from both groups and PCR tissue was done. Results: The study group patients demonstrated significantly higher history of GERD, LPR, and H. pylori. H. pyloripatients had significantly lower ages than those without (P= 0.03). Patients with H. pylorirevealed significantly higher GERD history (P< 0.001) and LPR (P= 0.002) than those without H. pylori. History of GERD (P= 0.003), LPR (P= 0.043), and H. pyloripresence (P= 0.028) were significant predictors, controlling for the abovementioned variables. Conclusions: We concluded that a significant correlation was observed between the H. pyloripresence in the sinonasal mucosa and nasal polyps and the development of chronic rhinosinusitis with or without nasal polyps.

Abstract

The main concept behind business intelligence (BI) is how to use integrated data across different business systems within an enterprise to make strategic decisions. It is difficult to map internal and external BI’s users to subsets of the enterprise’s data warehouse (DW), resulting that protecting the privacy of this data while maintaining its utility is a challenging task. Today, such DW systems constitute one of the most serious privacy breach threats that an enterprise might face when many internal users of different security levels have access to BI components. This thesis proposes a data masking framework (iMaskU: Identify, Map, Apply, Sign, Keep testing, Utilize) for a BI platform to protect the data at rest, preserve the data format, and maintain the data utility on-the-fly querying level. A new reversible data masking technique (COntent BAsed Data masking - COBAD) is developed as an implementation of iMaskU. The masking algorithm in COBAD is based on the statistical content of the extracted dataset, so that, the masked data cannot be linked with specific individuals or be re-identified by any means. The strength of the re-identification risk factor for the COBAD technique has been computed using a supercomputer where, three security scheme/attacking methods are considered, a) the brute force attack, needs, on average, 55 years to crack the key of each record; b) the dictionary attack, needs 231 days to crack the same key for the entire extracted dataset (containing 50,000 records), c) a data linkage attack, the re-identification risk is very low when the common linked attributes are used. The performance validation of COBAD masking technique has been conducted. A database schema of 1GB is used in TPC-H decision support benchmark. The performance evaluation for the execution time of the selected TPC-H queries presented that the COBAD speed results are much better than AES128 and 3DES encryption. Theoretical and experimental results show that the proposed solution provides a reasonable trade-off between data security and the utility of re-identified data.

Abstract

We present the case of a 19-year-old female with severe hypogammaglobulinemia after having had treatment with rituximab for idiopathic thrombocytopenic purpura requiring intravenous immunoglobulins. She was admitted with the diagnosis of left-sided pneumonia with parapneumonic effusion. The patient was treated with piperacillin/tazobactam after having a poor response to co-amoxiclav. The patient had been tested for immunoglobulin levels, and the levels were very low. She has a history of ITP for which she received steroids. She also received rituximab for the same on four separate occasions, and the last one was about 1 year ago.
