International audience; A panel of pediatric experts met to develop recommendations on the technical requirements specific to pediatric controlled donation after planned withdrawal of life-sustaining therapies (Maastricht category III). The panel recommends following the withdrawal of life-sustaining therapies protocol usually applied in each unit, which may or may not include immediate extubation. The organ retrieval process should be halted if death does not occur within 3 h of life-support discontinuation. Circulatory arrest is defined as loss of pulsatile arterial pressure and should be followed by a 5-min no-touch observation period. Death is declared based on a list of clinical criteria assessed by two senior physicians. The no-flow time should be no longer than 30, 45, and 90 min for the liver, kidneys, and lungs, respectively. At present, the panel does not recommend pediatric heart donation after death by circulatory arrest. The mean arterial pressure cutoff that defines the start of the functional warm ischemia (FWI) phase is 45 mmHg in patients older than 5 years and/or weighing more than 20 kg. The panel recommends normothermic regional perfusion in these patients. The FWI phase should not exceed 30 and 45 min for retrieving the pancreas and liver, respectively. There is no time limit to the FWI phase for the lungs and kidneys. The panel recommends routine sharing of experience with Maastricht-III donation among all healthcare institutions involved in order to ensure optimal outcome assessment and continuous discussion on the potential difficulties, notably those related to the management of normothermic regional perfusion in small children.

The mandible has various unnamed accessory foramina, and surgery is often performed in the symphyseal area. The aim of this study was to analyze the anatomical characteristics of mandibular lingual foramina with the objective of preventing clinical complications during implant surgery on an edentulous mandible.A total of 100 cone beam computed tomography scans of completely edentulous patients were included in this study. For each canal, nine measurements were recorded in millimeters: seven length or height measurements and two diameter measurements. The placement of a standard implant was simulated and whether the implant passed through the canal was noted. The results were analyzed by t-test and chi-squared at a significance level of 0.05. Pearson correlation analysis was used to assess the relationship between variables.We identified 309 foramina: 236 medial lingual foramina and 73 lateral lingual foramina. We found no significant relationship between the number of foramina and the age of the individuals, or between diameter and the age or gender of the individuals. The mandibular canal was injured in 32.7 % of implant placement simulations.The risk of injury to neurovascular bundles is increased in edentulous patients due to vertical bone resorption.

International audience; Depuis toujours, le climat et la santé des hommes sont liés. Les disettes sont causées par la sécheresse ou les intempéries qui génèrent aussi des épidémies. Ainsi, jusqu’au xxe siècle, l’Homme subissait les affres climatiques et la mortalité des famines. Mais l’industrialisation de la production agroalimentaire et les changements de régime nutritionnel qui en découlent ont inversé ce rapport. L’agriculture est, pour partie, à l’origine du dérèglement climatique et les Hommes meurent, maintenant, davantage des conséquences sanitaires de l’obésité que de famine. Aussi, depuis le début du xxie siècle, les politiques françaises de santé publique fixent des objectifs d’évolution de la santé nutritionnelle. En parallèle, la protection de l’environnement a fait émerger la notion de santé environnementale. La protection du climat tient donc autant à notre changement de mode de production de l’alimentation qu’à l’évolution de la composition de notre assiette qui doit privilégier les légumineuses sur la viande bovine. Est ainsi apparue la notion de santé durable issue de la rencontre des objectifs de l’agriculture durable avec la responsabilisation du consommateur face à la protection de sa santé. Les politiques de santé publique convergent avec les politiques agricoles pour consacrer la notion de santé durable. À l’avenir, elles pourraient asseoir la notion de santé alimentaire assise sur la protection de la santé nutritionnelle comme sur celle du climat.

International audience; After causing mass disasters that claimed the lives of tens of thousands of soldiers from countries around the world, the two Great Wars left some of them lost and missing. In France, these corpses reside in a legal vagueness where they belong neither to forensic anthropology nor archeology. Nevertheless, the process of identification and determining the cause of death requires the use of modern forensic anthropology by applying biological profiling and DNA analysis. New genomic methods also provide insight into the health statuses of these military populations, providing new perspectives on these periods of humanitarian crisis.

International audience; English Abstract: Mass exhumations and the mass unearthing of dead bodies are quite new phenomena in the history of humankind. The ‘forensic turn’ has led various disciplines to pay renewed attention to the dead and not only to the death. For a couple of decades, social anthropologists have been urged to (re)consider the materiality of death. This return of the dead bodies en masse has consequently made them look carefully at the ways in which corpses are dealt with in various situations of disasters or mass crime, but also made them aware of the various silences and denial mechanisms lastingly surrounding mass graves and buried corpses. This afterword aims to reflect on the way these silences and denial have shaped our disciplinary field and maintained some of its blind spots.; Les exhumations et la mise au jour massives de cadavres sont des phénomènes nouveaux dans l’histoire de l’humanité. Le tournant médico-légal a conduit diverses disciplines à accorder une attention renouvelée aux morts et non plus seulement à la mort. Pour leur part, les anthropologues sociaux ont été incités depuis quelques décennies à (re)considérer la matérialité de la mort. Ce retour en masse des cadavres les a, par conséquent, amenés à se pencher sur la manière dont les cadavres sont traités dans des situations de catastrophes ou de crimes de masse, mais aussi à prendre en compte les divers silences et mécanismes de déni qui entourent durablement les fosses communes et les cadavres enterrés. Cette postface vise à réfléchir à la manière dont ces silences et ces dénis ont façonné notre champ disciplinaire et maintenu certains de ses angles morts.

International audience; Antenatal ear examination is an integral part of the thorough examination of the fetal face. The discovery of an anomaly, whether it is made by chance or during a complementary in-depth examination, leads the practitioner to determine its isolated or associated character, in order to characterise its possible belonging to a syndromic entity. In this context, the realization of genetic analysis more precise and wider allowing a return of the results in a time compatible with an evolutive pregnancy, gives to the geneticist a central role in the management of these couples. The main challenge lies in obtaining a set of concordant clinical and biological clues, enabling the genetic results identified to be interpreted correctly, the optimised functioning of the ultrasound practitioner - geneticist duo is therefore fundamental. This results in a complex information to deliver, in the fact that the clinical translation of an ear anomaly in antenatal can go from an isolated aesthetic anomaly to a genetic syndrome with neurodevelopmental disorder. The objective of this work is to describe, from a methodological analysis of antenatal ears, the accessible malformative entities, isolated or associated, and to discuss the problems in the need or not to propose their screening.

The most significant polymorphism associated with dog size occurs in the region of theIGF1gene and concerns a single base change in a neighbouring lncRNA. The “small” (C) allele of this SNP is mostly found in small modern breeds and canids (foxes, coyotes, jackals) while the “large” (T) dominates in wolves and large dogs. However, the small allele is also present at low level in ancient wolves and is shown to represent the ancestral allele in canids, which has been recently selected in small dog breeds obtained by human selection.

International audience; The study of the human virome is still in its infancy, especially with regard to the viral content of the blood of people who are apparently disease free. In this study, the genome of a new giant virus that is related to the amoeba-infecting pathogen Marseillevirus was recovered from donated blood, using high-throughput sequencing. Viral antigens were identified by an immunoconversion assay. The virus was visualized with transmission electron microscopy and fluorescence in situ hybridization and was grown in human T lymphocytes. Specific antibody reactions were used to identify viral proteins in blood specimens from polymerase chain reactive-positive donors. Finally, we tested 20 blood specimens from additional donors. Three had antibodies directed against this virus, and 2 had circulating viral DNA. This study shows that giant viruses, which are missed by the use of ultrafilters, are part of the human blood virome. The putative pathogenic role of giant viruses in humans remains undefined.

Context: Cyclophosphamide is used for renal and major extrarenal involvement in systemic lupus erythematosus (SLE) and is associated with a risk of premature ovarian failure. There are no data available about the relation between anti-Müllerian hormone (AMH) serum levels and the probability of subsequent pregnancy in SLE patients. Objective: We analyzed AMH levels and the probability of pregnancy in SLE women exposed to cyclophosphamide. Design and Setting: We conducted a matched cohort study in referral centers for SLE. Patients: Fifty-six cyclophosphamide-exposed SLE women younger than 40 years of age and 56 control SLE women matched for age within 6 months participated in the study. Main Outcome Measures: AMH was measured in samples from the PLUS study (ClinicalTrials.gov no. NCT00413361). All patients were interviewed in May 2012 regarding their obstetric status. Results: The mean age ± SD of the 112 patients was 31.6 ± 5.8 years. The mean AMH level was low (1.21 ± 1.01 ng/mL) and was significantly lower in patients exposed to cyclophosphamide (P = .03) and in patients older than 30 years (P = .02). During a median follow-up (interval between sampling and the interview) period of 4.2 (range, 2.5–4.8) years, 38 patients sought to become pregnant, and 32 (84.2%) succeeded. In the univariate analysis, the risk of failure was associated with cumulative cyclophosphamide dose (P = .007) and older age (P = .02), but not with AMH. Conclusion: We confirmed that AMH levels are low in SLE patients and decrease significantly with age and cyclophosphamide exposure. Nonetheless, the risk of failure to conceive was low and was predicted by cyclophosphamide exposure and age, but not by AMH levels.

THE AGEING BODY AND CARERS: TOWARDS CO-DEPENDENCY? The dependency of the ageing body is often recognised and designated by carers in terms of the degree, seriousness and frequency of the care called for. In this article we show how carers have recourse to several types of action corresponding to a precise ethics of caring. But even if the action is effective, psychological feedback is important for the carer, who is not indifferent to dependency: this is what we call a co-dependency effect between carers and those they care for. Each needs the other; each is in a state of dependency that seemingly exceeds and/or transcends a relationship of reciprocity.; La dépendance du corps vieillissant est souvent reconnue et désignée par les soignants selon le degré, la gravité et l’intensité du soin à prodiguer. Nous montrons dans cet article comment les soignants s’appuient sur plusieurs types de gestes pour agir selon une éthique précise du soin. Mais si l’action est efficace, le feedback psychologique pour le soignant est important car il ne reste pas indifférent à la dépendance : c’est ce que nous appelons un effet de co-dépendance entre les soignants et les soignés ; chacun a besoin de l’autre, chacun est bien dans une dépendance de l’un envers l’autre qui dépasse vraisemblablement et/ou va au-delà de la relation de réciprocité.

Les lésions traumatiques du squelette en pratique pédiatrique sont fréquentes, faites de macrotraumatismes et de microtraumatismes répétés. Le diagnostic et le bilan de cette pathologie sont, dans l'immense majorité des cas, réalisés grâce aux clichés standards, surtout pour le squelette appendiculaire. Les fractures de l'enfant sont plus souvent diaphysométaphysaires incomplètes qu'épiphysométaphysaires. Elles touchent principalement le membre supérieur par ordre de fréquence. Il existe de nombreuses images pièges, variantes de la normale ; la consultation d'ouvrages de référence est fondamentale. Les clichés standards en incidences orthogonales sont la règle et permettent de diagnostiquer et de faire le bilan précis de la majorité de ces fractures. Les clichés comparatifs ne doivent pas être systématiques mais réservés à quelques rares situations. L'échographie a peu de place dans cette indication chez l'enfant. Le scanner et l'imagerie par résonance magnétique (IRM) sont à réserver à la compréhension de fractures complexes ou de leurs complications évolutives potentielles (épiphysiodèse). En revanche, l'IRM présente un intérêt évident dans certaines indications communes à l'enfant et à l'adulte, comme les lésions méniscoligamentaires, les fractures occultes, les fractures de fatigue, voire les fractures apophysaires. D'autres indications sont propres à la pathologie pédiatrique, en particulier les lésions intéressant la physe ou cartilage de croissance. Le développement de la pratique du sport chez l'enfant et l'adolescent est à l'origine d'une pathologie traumatique particulière.L'imagerie par résonance magnétique nucléaire (IRM) est la méthode d'imagerie appropriée pour l'étude de la maturation cérébrale, en particulier de la myélinisation. L'IRM apporte aussi une approche fonctionnelle du développement cérébral par l'utilisation de séquences de diffusion et de la spectrométrie du proton. Dans cet article, les techniques IRM sont détaillées pour les périodes à la fois pré- et postnatales. L'effet de la maturation sur le signal de résonance magnétique nucléaire doit être également pris en compte afin de comprendre la maturation normale et par conséquent son influence sur les différentes pathologies rencontrées avant et après la naissance. L'IRM étant aveugle en termes de signal sur les séquences conventionnelles après 2ans, une attention particulière est donc donnée aux images de diffusion et à la spectrométrie de protons.

The effectiveness of the so-called skele- tal markers of activity as functional indicators is widely debated. Among them, certain morphological features of the anterior aspect of the femoral head-neck junction (Poirier's facet, cervical fossa of Allen, etc.) have been considered in relation to some behaviors and specific activities (e.g., squatting, horseback riding, etc.). How- ever, disagreement on terminology and descriptions, the absence of standardized scoring methods and poor knowledge of the variability and distribution of these features make it difficult to interpret their meaning. The aim of this study is to analyze the variability of the anterior aspect of the femoral neck through a new scor- ing method taking into account three main traits: Poi- rier's facet, plaque, and cribra (including the Allen's fossa). This method has been applied to a sample of 225 adult individuals of both sexes coming from an identified modern skeletal collection, achieving low intraobserver and interobserver error values. The results highlight some significant trends: plaque, almost always bilateral, appears to be a normal condition of the femur, being present in approximately 90% of the individuals. Cribra is more frequent in females and decreases with age. Poi- rier's facet shows a very low frequency. This method allows the representation of both the anatomical diver- sity of the region already described in literature and part of the variability never considered before. Our results suggest caution in considering these features as markers related to specific activities.

Coordination éditoriale : François Félix; Depuis le baron Pierre de Coubertin au moins, le sport véhicule des valeurs entendues comme universelles : le fair-play, la conscience de la règle, le respect de l’adversaire, la dignité des personnes, le self-control, le sens de l’effort individuel et collectif, le plaisir du jeu… Des valeurs comme autant de règles de civilité, et que garantiraient les associations faîtières internationales, qui ont autour d’elles rédigé des codes et des éthiques.Pourtant, force est de constater des débordements de ces règles et le non-respect des valeurs dans bien des domaines du sport : tricheries, dopage cycliste ou athlétique, insultes racistes à l’endroit des sportifs voire entre sportifs eux-mêmes, contestation de l’arbitrage, violences des hooligans, exploitation des enfants par de grandes marques dans la fabrication du marketing du sport, vente de joueurs mineurs, discriminations hommes-femmes, lotos sportifs et matchs arrangés, harcèlement sexuel entre entraîneurs et entraînés, etc., la liste est malheureusement longue, qui nous place devant un certain échec de l’éducation.Autant de débordements et pratiques illicites qui provoquent des réactions morales dans la population, mais face auxquels on cherche en général des solutions juridiques et techniques, avec pour conséquence que ces valeurs sportives en viennent de plus en plus à passer pour des idéaux auxquels tendre (et qui se font alors de plus en plus lointains ou inatteignables) plutôt que pour des règles directrices et des normes comportementales de principe.Pour autant, l’exemplarité du sport n’a pas disparu de l’horizon éducatif ; en outre, bien des sportifs en sont venus à promouvoir d’autres valeurs que la compétition: le bien-être, les sensations pour elles-mêmes, une meilleure conscience corporelle de soi, une confrontation renouvelée aux éléments naturels… faisant émerger une autre forme d’éthique sportive. C’est donc un champ très large qui s’offre à la réflexion, et que parcourt cet ouvrage.

L'étude archéo-anthropologique réalisée sur les inhumations de l'église Saint-Pierre de Moirans a permis d'effectuer des comparaisons entre différentes chapelles (secteurs I, II, III, IV) et le choeur (secteur VII) du point de vue du recrutement et des pratiques funéraires (mode d'inhumation et organisation de l'espace). L'analyse du recrutement funéraire, réalisée sur les individus provenant des sépultures puis intégrant les os issus du comblement, indique que le profil de mortalité est conforme au schéma de mortalité archaïque dans les secteurs I, II et IV. Dans ces chapelles du collatéral nord, les enfants (même ceux décédés en période périnatale) étaient inhumés avec les adultes et les adolescents. Ce résultat soutient l'hypothèse de chapelles familiales, attestées dans les textes pour celle du secteur IV. Une autre chapelle se distingue par une prédominance masculine. Il est attesté par les textes qu'il s'agit du lieu de sépulture de la confrérie des pénitents. Quant au choeur, il n'était pas destiné uniquement au clergé, puisque des femmes et des enfants de tous âges y étaient inhumés. Certaines règles telles que l'orientation de la tombe et l'inhumation en cercueil bien qu'homogènes sont parfois contournées à des fins pratiques liées à la gestion de l'espace ou au statut social du défunt.

International audience; Anelloviruses are small negative-sense single-stranded DNA viruses with genomes ranging in size from 1.6 to 3.9 kb. The family Anelloviridae comprised 14 genera before the present changes. However, in the last five years, a large number of diverse anelloviruses have been identified in various organisms. Here, we undertake a global analysis of mammalian anelloviruses whose full genome sequences have been determined and have an intact open reading frame 1 (ORF1). We established new criteria for the classification of anelloviruses, and, based on our analyses, we establish new genera and species to accommodate the unclassified anelloviruses. We also note that based on the updated species demarcation criteria, some previously assigned species (n = 10) merge with other species. Given the rate at which virus sequence data are accumulating, and with the identification of diverse anelloviruses, we acknowledge that the taxonomy will have to be dynamic and continuously evolve to accommodate new members.

Since direct oral anticoagulants have been marketed, the management of major bleeding in emergency departments has become more challenging. In 2013 and 2016, the Working Group in Perioperative Haemostasis made several proposals for this. The objective of the present study was to evaluate conformity with these proposals in daily clinical practice. We performed a retrospective single-center cohort study addressing the management of major bleeding in patients treated by direct oral anticoagulant in an emergency department. Patients were included between January 1, 2015 and November 30, 2017. Among 3,365 patients admitted to the emergency department for bleeding, a total of 191 were treated by direct oral anticoagulant and 55 (28.8%) with major bleeding were included in the present study. The majority of patients received rivaroxaban (n = 44, 80.0%) and were treated for atrial fibrillation (n = 47, 85.0%). Bleeding concerned gastrointestinal (n = 30, 54.6%) and (n = 14, 25.5%) intracranial location. Twenty-one patients (38.2%) were treated by prothrombin complex concentrate, and 31 patients (56.4%) were transfused. Intervention for hemostatic control was performed for 30 patients (54.6%). Thirty-three patients (60.0%) presented a deviation from protocol. There is no significant difference for mortality at 3 months according to whether or not there was a deviation from protocol. Nearly two-thirds of patients with major bleeding associated with direct oral anticoagulant administration had protocol deviation, but there was no effect on mortality.

Aim This paper describes the characteristics of the new advanced practice nursing roles in France, as well as their challenges and perspectives, and compares the French model with the recommendations of the International Council of Nurses. Background Advanced practice nursing is particularly well established in English-speaking countries. Since 2018, France has become the second French-speaking region to legalize and regulate advanced practice nurses. Source of evidence The International Council of Nurses and French government websites, and scientific databases (PubMed, CINALH, Cochrane Library) were explored. Feedback from French nursing academics was also requested. Discussion The advanced practice nursing model in France is described according to the scope and conditions of professional practice. The educational program leads to a State diploma with master's degree, which it is mandatory to be a registered nurse. Remuneration depends on the sector of practice in the public hospital, primary care or private sector. Although there is no national strategy for the implementation of advanced practice nursing roles, research projects are being initiated to guide and evaluate the practice. Based on concordance analysis with the recommendations of the International Council of Nurses, the French advanced practice nursing model appears to be similar to the nurse practitioner model. Conclusion Adjustments in the scope of practice and education can be expected as the implementation of these roles is evaluated. Implications for nursing practice This is a historical evolution of the nursing profession in France, for which communication with patients and healthcare professionals is essential. Implications for nursing policy The implementation of advanced practice nursing roles in clinical settings requires the development of national strategies to support initiatives and ensure the sustainability of these roles.

Summary Traumatic bone defects (BD) are rare in children. There are no pediatric series in the literature on this topic. The aim of this first pediatric series was to determine the epidemiological characteristics and evaluate the results of different treatments in this entity. Material and methods This retrospective multicenter study evaluated diaphyseal bone defects in cases in which bone reconstruction was performed. BD was either initial and associated with trauma or secondary, resulting from infected non-union. Results The series included 27 patients (17 boys and 10 girls), mean age 11.4 years old (3–16) (20 traffic accidents). At the outset of all patients’ history was an open fracture (one stage 1, seven stage 2, 11 stage 3A and seven stage 3B, 1 NR). BD involved 13 tibias, 9 femurs, three humerus, one radius and one ulna. Bone defects were initial in 20 cases and secondary in seven cases. They were less than 2 cm in two cases, between 2 and 5 cm in 9 cases, between 5 and 10 cm in 10 cases and more than 10 cm in six cases. Treatment of BD was immediate in one case and delayed in 26 cases. Techniques used included: induced membrane in 10 cases, bone transport in seven cases, bone autograft in eight cases, vascularized fibular transfer in one case, no bone reconstruction in one case. Union was obtained in 27 patients. Union was obtained within a mean 12.3 months BD (3–62). Fifteen patients presented with sequellae. Discussion Traumatic bone defects have a better prognosis in children than in adults. The thicker, more active and richly vascularized periosteum in children is an important prognostic factor. Treatment of BD requires good initial bone stabilization. Reconstruction depends on the integrity of the periosteum. In case of an intact periosteum, bone reconstruction does not seem necessary in young children. If one part of the periosteum is intact, a simple autograft seems sufficient even with extensive bone defects. In the absence of the periosteum or especially in case of infection, the induced membrane technique seems preferable, with bone transport or a vascularized bone transfer. Pour apprivoiser la mort et se soustraire aux désordres de la putréfaction, rituels et techniques thanatologiques ont été élaborés.Construire une anthropologie du cadavre implique une connaissance des comportements sur une longue histoire et une approche pluridisciplinaire. C’est aussi aborder la mort en dissolvant le sujet dans un objet répondant à des lois bio-physico-chimiques dont il faut s’accommoder lors des rituels.C’est enfin remettre le sujet ─le défunt ─ au centre du fait social. Pour aborder ces questions, la Rencontre autour du cadavre propose trois axes :─ entropie du cadavre, principes biologiques, perception et tolérance,─ un cadavre, une chaîne opératoire, des outils, des savoirs,─ le cadavre, questions et gestions sociales, symbolismes et représentations.Les journées de Marseille ont permis une rencontre entre des disciplines très différentes ─ anthropologie, archéologie, sociologie, histoire, ethnologie, psychanalyse, droit... ─ avec chacune ses propres méthodes, corpus documentaires ou systèmes de représentation.

International audience; Introduction The role of fine-needle aspiration cytology (FNAC) in the management of parotid tumours is still the subject of controversy. The purpose of this study was to determine the diagnostic value of FNAC in our institution in order to define its place in the diagnostic strategy.Patients and methods :This retrospective study was based on 249 patients who had undergone preoperative FNAC before being operated in our institution between 2001 and 2008. All examinations were performed and interpreted by the same experienced pathologist.Results, Among the 249 patients included in this study, 187 (75%) had a benign tumour and 62 (25%) had a malignant tumour. No complications of FNAC were observed. Cytological findings were non-contributory in 47 patients (18%). The sensitivity of FNAC for the diagnosis of malignancy was 80% with a specificity of 89.5%. Among the 11 false-negative results, lymphomas and low-grade mucoepidermoid carcinomas were the most common histological types. Among the 16 false-positive results, Warthin's tumours, pleomorphic adenomas and lymphoepithelial lesions were the most common histological types. Accurate histological classification of the tumour was reported in 79.5% of cases (86% for benign tumours and 44% for malignant tumours).Conclusion FNAC is a reliable examination providing important information to the surgeon in the preoperative diagnostic assessment.

International audience; ABSTRACT To evaluate the role of 18-fluoro-deoxy-d-glucose (FDG)-positron emission tomography (PET) scan for detecting distant metastases in the preoperative assessment of patients with recurrent head and neck squamous cell carcinoma (HNSCC). Retrospective study. University Teaching Hospital. Thirty-seven consecutive patients who presented, between April 2008 and April 2010, a local and/or regional recurrence of head and neck squamous cell carcinoma after treatment with radio-chemotherapy were studied. The work-up included thoraco-abdominal computed tomography (CT), fluoro-deoxy-D-glucose-positron emission tomography scan and abdominal ultrasound. The imaging results, when positive, were compared to histology or cytology (conducted during targeted examinations, for example, fiberoptic oesophago-gastro-duodenal search, colonoscopy, bronchoscopy, liver biopsy) or targeted imaging examination (Abdominal MRI, sonography) combined with clinical follow-up. All patients were followed-up for at least 6 months. Positron emission tomography performances were then analysed and compared with those of conventional imaging for detecting distant metastases. Among the 37 patients, 9 (24%) had visceral metastases. The sensitivity, specificity, positive predictive value and negative predictive value for detecting metastasis or second primary were, respectively, 100%, 94%, 86% and 100% for CT and 92%, 87%, 74% and 97% for positron emission tomography. Computed tomography and positron emission tomography were strictly concordant in 32/37 (86%) of cases. No false-negative result was found for CT, while we found one case of false-negative positron emission tomography. The number of false-positive results was two for CT and four for positron emission tomography. From our study, positron emission tomography does not appear to offer a first-choice technique for the detection of metastases before salvage surgery as CT detected all lesions visible on positron emission tomography.

The RH system is one of the most polymorphic blood group systems due to the proximity and opposite orientation of RHD and RHCE genes. Numerous alleles are described and can affect Rh protein expression. This complexity is especially evident in populations of African origin. We performed RHD and RHCE genotyping of the Noir Marron population in French Guiana. This population belongs to the Maroon community who are direct descendants of African slaves, who escaped from Dutch plantations, in the current day Suriname, during the 17th century. They represent an original ethnic group with highly blended culture.A total of 89 DNA samples were collected from four different ethnic groups of the Noir Marron population of French Guiana. RHD and RHCE genotyping was performed using DNA microarray and/or sequencing.Significant allelic diversity was shown, with 45% of individuals presenting an RHD gene variant (most common: RHD*DAU, RHD*DIVa, and RHD*DIIIa allele) and 9.4% with a partial D phenotype. Likewise, 85% presenting an RHCE gene variant and 9% a partial RH2 antigen. One original allele was identified in two D+ Noir Marron individuals: a hybrid RHD*DIIIa-CE(9)-D allele, encoding probably a partial D antigen and associated with an RHCE*ce(48C,733G,1006T) allele. The African diversity of RHD and RHCE genes is found in this population with preserved genetic but mixed cultural backgrounds. These data allow us to describe the characteristics of the RH system antigen and highlights a significant number of partial antigens with a risk of alloimmunization.

BackgroundPreterm infants are at risk of neurodevelopmental impairments. At present, proton magnetic resonance spectroscopy (1H-MRS) is currently used to evaluate brain metabolites in asphyxiated term infants. The purpose of this study was to identify in the preterm EPIRMEX cohort any correlations between (1H-MRS) metabolites ratio at term equivalent age (TEA) and neurodevelopmental outcomes at 2 years.MethodsOur study included EPIRMEX eligible patients who were very preterm infants (gestational age at birth ≤32 weeks) and who underwent a brain MRI at TEA and 1H-MRS using a monovoxel technique. The volumes of interest (VOI) were periventricular white matter posterior area and basal ganglia. The ratio of N Acetyl Aspartate (NAA) to Cho (Choline), NAA to Cr (creatine), Cho to Cr, and Lac (Lactate) to Cr were measured. Neurodevelopment was assessed at 24 months TEA with ASQ (Ages and Stages Questionnaire).ResultsA total of 69 very preterm infants had a 1H-MRS at TEA. In white matter there was a significant correlation between a reduction in the NAA/Cho ratio and a total ASQ and/or abnormal communication score, and an increase in the Lac/Cr ratio and an abnormality of fine motor skills. In the gray nuclei there was a trend correlation between the reduction in the NAA/Cho ratio and sociability disorders; and the increase in the Lac/Cr ratio and an anomaly in problem-solving.ConclusionsUsing NAA as a biomarker, the vulnerability of immature oligodendrocytes in preterm children at TEA was correlated to neurodevelopment at 2 years. Similarly, the presence of lactate at TEA was associated with abnormal neurodevelopment at 2 years in the preterm brain.

International audience; Spondylolysis is a bony defect of the pars interarticularis mainly arising from repeated traumas. Other factors such as spina bifida occulta (SBO) are mentioned in clinical and palaeopathology but have been little studied. The aim of our study is to provide information on the prevalence of lumbar spondylolysis and its association with SBO in three samples: 1) a sample from the general contemporary French population (CT scans from the Nord hospital in Marseille); 2) a large mediaeval and modern osteoarchaeological collection from the cemetery of Notre-Dame-du-Bourg cathedral (NDB) (Digne-les-Bains, Alpes-de-Haute-Provence); and finally, 3) a corpus of prevalence data taken from the literature. Our results show that for the contemporary population, 26 out of 496 adult patients aged 18 to 69 years displayed at least one lumbar spondylolysis (5.2%). Its prevalence was significantly higher among patients with SBO (Odds ratio 3.8). In the NDB series, 9 individuals out of 455 had spondylolysis (2.0%). The prevalence was higher in the mediaeval period (3.5%) than in the modern period (0.5%). The same trend was found in our analysis of all European data in the literature, and the prevalence of spondylolysis was not associated with the presence of SBO.; La spondylolyse est une solution de continuité de la pars interarticularis principalement secondaire à des traumatismes répétés. D’autres facteurs, comme la spina bifida occulta (SBO) sont évoqués mais peu étudiés en clinique et en paléopathologie. Notre étude a pour objectif de renseigner la prévalence de la spondylolyse lombaire et son association avec la SBO au sein de trois corpus : 1) un échantillon issu de la population générale française contemporaine (tomodensitométrie, centre hospitalo-universitaire Nord de Marseille) ; 2) une large collection ostéoarchéologique médiévale et moderne provenant du cimetière de la cathédrale de Notre-Dame-du-Bourg (NDB) (Digneles-Bains, Alpes-de-Haute-Provence) et enfin 3) un corpus de données issues de la littérature paléopathologique. Les résultats révèlent que, pour la population contemporaine, 26 patients adultes âgés de 18 à 69 ans parmi 496 montrent au moins une spondylolyse lombaire (5,2 %). Sa prévalence est significativement plus élevée chez les patients avec une SBO (odds ratio 3,8). Au sein de la série de NDB, 9 individus parmi 455 présentent une spondylolyse (2,0 %). La prévalence est plus élevée à la période médiévale (3,5 %) qu’à la période moderne (0,5 %). La même tendance a été retrouvée lors de l’analyse de l’ensemble des données européennes de la littérature et la prévalence de la spondylolyse n’était pas associée à la présence d’une SBO.

Summary Contemporary medical reports from Britain and Germany on patients suffering from a pandemic infection between 1889 and 1891, which was historically referred to as the Russian flu, share a number of characteristics with COVID‐19. Most notable are aspects of multisystem affections comprising respiratory, gastrointestinal and neurological symptoms including loss of taste and smell perception; a protracted recovery resembling long covid and pathology observations of thrombosis in multiple organs, inflammation and rheumatic affections. As in COVID‐19 and unlike in influenza, mortality was seen in elderly subjects while children were only weakly affected. Contemporary reports noted trans‐species infection between pet animals or horses and humans, which would concur with a cross‐infection by a broad host range bovine coronavirus dated by molecular clock arguments to an about 1890 cross‐species infection event., Contemporary clinical reports on patients of the Russian flu pandemic of 1889 to 1891 show striking similarity to COVID‐19 patients, demonstrating *multiorgan affections of the respiratory, intestinal and neural system, *loss of smell and taste, *and long disease symptom duration.

The COVID-19 vaccination campaign began in December 2020, in France, and primarily targeted the oldest people. Our study aimed to determine the level of acceptance of vaccination in a population of older patients with cancer.From January 2021, we offered vaccination with the BNT162b2 COVID-19 vaccine to all patients 70 years and older referred to our geriatric oncology center in Marseille University Hospital (AP-HM) for geriatric assessment before initiation of an oncological treatment. Objectives were to evaluate acceptance rate of COVID-19 vaccination and to assess vaccine safety, reactogenicity, and efficacy two months after the first dose.Between January 18, 2021 and May 7, 2021, 150 older patients with cancer were offered vaccination after a geriatric assessment. The majority were men (61.3%), with a mean age of 81 years. The two most frequent primary tumors were digestive (29.4%) and thoracic (18%). The vaccine acceptance rate was 82.6% and the complete vaccination rate (2 doses) reached 75.3%. Among the vaccinated patients, 15.9% reported mild side effects after the first dose and 23.4% after the second dose, mostly arm pain and fatigue. COVID-19 cases were observed in 5.1% of vaccinated patients compared with 16.7% in unvaccinated patients. Of the 22 vaccinated patients who agreed to have their serum tested, 15 had antibodies against the spike protein at day 21 after the first dose.Our study showed a high acceptance rate of COVID-19 vaccination, with good tolerance in this frail population. These results highlight the benefits of organizing vaccination campaigns at the very beginning of oncological management in older patients.This study was registered May 23, 2019 in ClinicalTrials.gov (NCT03960593).

International audience; Systemic Sclerosis (SSc) is a chronic autoimmune disease with high morbidity and mortality. Autologous Hematopoietic Stem Cell Transplantation (AHSCT) is the best therapeutic option for rapidly progressive SSc, allowing increased survival with regression of skin and lung fibrosis. The immune determinants of the clinical response after AHSCT have yet to be well characterized. In particular, the pivotal role of the Human Leukocyte Antigen (HLA) system is not well understood, including the role of non-classical immuno-modulatory HLA-E and HLA-G molecules in developing tolerance and the role of Natural Killer cells (NK) in the immunomodulation processes. We retrospectively tested whether the genetic and/or circulating expression of the non-classical HLA-E and HLA-G loci, as well as the imputed classical HLA determinants of HLA-E expression, influence the observed clinical response to AHSCT at 12- and 24-month follow-up. In a phenotypically well-defined sample of 46 SSc patients classified as clinical responders or non-responders, we performed HLA genotyping using next-generation sequencing and circulating levels of HLA-G and quantified HLA-E soluble isoforms by ELISA. The -21HLA-B leader peptide dimorphism and the differential expression level of HLA-A and HLA-C alleles were imputed. We observed a strong trend towards better clinical response in HLA-E*01:03 or HLA-G 14bp Del allele carriers, which are known to be associated with high expression of the corresponding molecules. At 12-month post-AHSCT follow-up, higher circulating levels of soluble HLA-E were associated with higher values of modified Rodnan Skin Score (mRSS) (p = 0.0275), a proxy of disease severity. In the non-responder group, the majority of patients carried a double dose of the HLA-B Threonine leader peptide, suggesting a non-efficient inhibitory effect of the HLA-E molecules. We did not find any correlation between the soluble HLA-G levels and the observed clinical response after AHSCT. High imputed expression levels of HLA-C alleles, reflecting more efficient NK cell inhibition, correlated with low values of the mRSS 3 months after AHSCT (p = 0.0087). This first pilot analysis of HLA-E and HLA-G immuno-modulatory molecules suggests that efficient inhibition of NK cells contributes to clinical response after AHSCT for SSc. Further studies are warranted in larger patient cohorts to confirm our results.

139. New models to estimate fetal and young infant age with the pars basilaris biometry

The pars basilaris of the occipital bone is often found intact when fetal or infant bones are discovered. In the literature, several methods use measurements of this bone for juvenile's age-at-death estimation, but it has already been demonstrated that they can come along methodological and statistical biases. In this study, new age prediction formulas were created from a learning sample of 208 CT scans of fetuses and infants from Marseilles (France) with normal development. The two statistically validated models, with a prediction interval of 95 % of reliability, were tested on a separate sample of 68 individuals, for which we ensured that they did not show any developmental pathology. Models show an average of 97.06 % of correct age estimates, compared to less than 26.6 % when using previously published which tend to overestimate age. The new age estimation formulas make it possible to estimate fetuses and infants age with very good results up to 2 postnatal months. Results are both reliable and precise enough for forensic practice, and we provide an automated tool (a simple excel spreadsheet) to ensure an easy and quick access to the application of this method.

International audience; In Western countries, blood supply agencies encounter impediments in providing blood groups defined as rare or of interest, notably for sub-Saharan African ancestry (SSAA) recipients. To establish warning levels and anticipate future blood needs, an estimate of the current carriers of rare blood groups, both potential patients or donors, is crucial but complex. Indeed, if the strict needs can be estimated in medical terms, the modalities of blood product collection must be considered from an interdisciplinary perspective, at the interface of biological data and social norms. Here, we aim to understand how legal choices and a set of representations of otherness may influence the supply of rare blood for SSAA populations. After examining these issues, considering different norms and limits that govern French society, we compare this data with those of four Western countries facing the same difficulties (United States, United Kingdom, Italy and the Netherlands). This work began as part as the reflections of Social Lab, an institutional programme created by the French Blood Establishment (EFS). How can we effectively improve the qualitative blood coverage for SSAA populations? There is no unique solution, but there are many more or less effective answers. Comparison across countries reveals a strong influence of the socio-political histories and ethical choices before technical and medical considerations. We consider that an institutional policy is required to resolve recruitment issues of SSAA donors sustainably. Lastly, we introduce a working group called the EFS Social Lab, which aims to set up a monitoring mechanism for donors and societal trends to make blood donation effective.

International audience; Objectives: To explore cancer patients' experience of pain and coping strategies as well as beliefs and representations associated with pain, pain management and treatments.Method and participants: A qualitative study based on semi-structured interviews was conducted among 16 patients with cancer. The inductive research strategy adopted derived from the Grounded Theory approach and the data transcribed verbatim were gradually analyzed involving researcher triangulation.Findings: The experience of pain and the coping strategies developed by patients to deal with it are shaped by the experience and representations of cancer associated with death and suffering. Pain acts both as an indication to the patients that they are still alive and as an indicator of the progression of the disease. Cancer also models patients' relationships to analgesic treatments and health care providers, since pain is an area in which patients can take back control of what is happening to them. Patients' expression of reluctance to accept analgesic treatments is also influenced by the significance of opioid treatments in this context, which are perceived as an indication that the end of life is close.Conclusions: The contextualization of pain through the particularities of cancer is critical in order to understand cancer patients' experience of pain.Implications for psychosocial providers: Not only patients' knowledge but also the meaning-making of pain should be incorporated in interventions targeting pain management.

Background Midface skeletal changes through aging have been conceptualized as a clockwise rotation relative to the cranial base. This finding is mainly supported by cross-sectional population studies based on comparisons between groups of individuals in different age ranges. Few papers have evaluated the intra-individual evolution of the facial skeleton, and those that have included only a limited number of patients. Objectives The authors sought to analyze the intra-individual evolution of the facial skeleton. Methods We conducted a retrospective study including patients who underwent 2 separate facial computed tomography scans (T1 and T2) with a minimum gap of 7 years. Employing 3-dimensional reconstructions, we measured and compared 7 facial angles (glabellar, orbital, piriform, maxillary, subdental, chin, gonial) and 6 facial distances (lacrimal crest, orbital floor, piriform, zygoma, labiomental, gnathion) on both computed tomography scans. Results A total of 56 patients (29 males and 27 females) were included. Mean ages at T1 and T2 were 50 and 59 y, respectively. We found a significant decrease between T1 and T2 in facial angles at the midface (glabella, orbital, piriform, and maxillary angles, all P < 0.0001). We found a significant increase in all facial distances between T1 and T2 for the overall population and for men (all P < 0.05). For women, all distances increased, but only piriform and zygoma distances showed significant changes (P < 0.001 and P < 0.04, respectively). Bone remodeling appeared earlier in women. Conclusions Our study confirms the clockwise rotation of the midface during aging. This finding could lead to a better understanding of aesthetic medicine practices.

International audience; Killer-cell immunoglobulin-like receptors (KIRs) expressed by natural killer cells are cell surface molecules able to recognize groups of HLA class I alleles. The number and distribution of KIR genes vary among individuals and populations. The aim of this study is to analyse the KIR gene content in a Comorian population in order to investigate genetic relationships with other populations and to reconstruct past migration events. The Comorian population consisted of 54 unrelated immigrants living in France and a control population consisted of 38 individuals from Southeast France. We investigated the presence or absence of 15 KIR genes, two pseudogenes expressed and non-expressed forms of KIR2DL5 and the two major subtype full-length and deleted forms of KIR2DS4. All individuals were typed positive for the framework genes, i.e. KIR2DL4, KIR3DL2 and KIR3DL3, and the two pseudogenes KIR3DP1 and KIR2DP1. The frequencies of full-length KIR2DS4 (*00101/00102/002) were lower in the French population (F = 29%) than in the Comorian population (F = 72%) (P(c) < 0.05). No significant differences were found for other KIR genes. A total of 11 genotypes were identified in the Southeast French population and 22 genotypes in the Comorian population. The most common genotype (2DL1, 2DL3, 2DL4, 3DL1, 3DL2, 3DL3 and 2DS4) accounted for 41% in the Comorian population and 34% in the Southeast French population. Principal component analysis using KIR gene data from 20 populations was performed to determine genetic differences and relations between populations. The Comorian population exhibited closest kinship with Africans and Asians. As KIR gene content is heterogeneous among ethnic groups, it can probably be used to assess the genetic relationships among populations from different geographic areas.

International audience; Dental caries management requires individualized follow-up and prophylaxis based on patients’ caries risk (PCR). In large pre-doctoral clinics, the use of institutional quality measures (QMs) is essential to control the quality of patient follow-up and to evaluate the need for improvement measures. The aim of this retrospective study was to evaluate the suitability of reusing student activity data for the development of QMs of caries risk. Two approaches for predicting PCR using student activity data were evaluated and compared. The first approach used the procedure codes recommended by the Dental Quality Alliance and the second used these same codes along with three educational codes. The sensitivity, specificity, overall accuracy of the two approaches were evaluated. A Receiver Operating Characteristic (ROC) curve analysis was carried out, and the areas under the ROC curve of the two approaches were compared using Delong’s test. A two-tailed P value ≤0.05 was considered statistically significant. While the two approaches were able to correctly predict PCR, the approach using both procedure and educational codes showed better predictive performance. The reuse of student activity data is an easy and robust method for the development of QMs of caries risk that can help improve monitoring and quality of patient care.
