An 83-year-old woman with a long history of glaucoma and optic disc drusen (ODD) was referred for neuro-ophthalmological second opinion. The patient had been treated for decades with bilateral intraocular pressure (IOP)-lowering eye drops, laser trabeculoplasty and trabeculectomy and had severe, bilateral loss of visual fields and retinal nerve fibre layer (RNFL) thinning on optical coherence tomography (OCT) despite IOP that never exceeded 24 mm Hg. On ophthalmoscopy, only a single ODD was visible in the left eye and no optic disc cupping was apparent in either eye. Enhanced depth imaging OCT (EDI-OCT) of the optic nerve head revealed bilateral multiple, large, deep ODD, which in itself could easily explain the visual field loss and RNFL thinning of this patient. Optic nerve head examination using EDI-OCT is highly recommended for patients with a history of glaucoma but without optic nerve head cupping to avoid a potential misdiagnosis with consequent unnecessary treatment.

The majority of tubular joints commonly found in offshore jacket structures are multi-planar. Investigating the effect of loaded out-of-plane braces on the values of the stress concentration factor (SCF) in offshore tubular joints has been the objective of numerous research works. However, due to the diversity of joint types and loading conditions, a number of quite important cases still exist that have not been studied thoroughly. Among them are internally ring-stiffened multi-planar XT-joints subjected to axial loading. In the present research, data extracted from the stress analysis of 81 finite element (FE) models, verified using experimental test results, was used to study the effects of geometrical parameters on the chord-side SCFs in multi-planar tubular XT-joints reinforced with internal ring stiffeners subjected to two types of axial loading. Parametric FE study was followed by a set of the nonlinear regression analyses to develop four new SCF parametric equations for the fatigue analysis and design of axially-loaded multi-planar XT-joints reinforced with internal ring stiffeners.

Although tubular X-joints are quite common in offshore structural design and despite the crucial role of stress concentration factors (SCFs) in evaluating the fatigue performance of tubular joints, the SCF distribution in X-joints reinforced with doubler plates has not been investigated so far and no design equation is currently available to predict the distribution of chord-side SCFs along the weld toe of brace-to-chord intersection in this type of joint. Parametric FE study was followed by a set of nonlinear regression analyses to develop a new SCF parametric equation for the fatigue analysis and design of axially loaded tubular X-joints reinforced with doubler plates.

The degree of bending (DoB) has a profound effect on the fatigue behavior of tubular joints commonly found in offshore jacket structures. Probability density functions (PDFs) of the involved random variables are necessary for the fatigue reliability analysis of jacket structures. The objective of present research was the derivation of PDF for the DoB in tubular KT-joints commonly found in jacket-type platforms. A total of 243 finite element (FE) analyses were carried out on 81 FE models of KT-joints subjected to three types of out-of-plane bending (OPB) moment loading. Generated FE models were validated using experimental data, previous FE results, and available parametric equations. Based on the results of parametric FE study, a sample database was prepared for the DoB values. Thirteen theoretical PDFs were fitted to the developed histograms and the maximum likelihood (ML) method was applied to evaluate the parameters of fitted PDFs. In each case, the Kolmogorov-Smirnov test was used to evaluate the goodness of fit. After substituting the values of estimated parameters, six fully defined PDFs were presented for the DoB at the saddle positions of the central and outer braces in tubular KT-joints subjected to three types of OPB moment loading.

Increasing evidence indicates that changes in the redox system may contribute to the pathogenesis of multiple optic neuropathies. Optic neuropathies are characterized by the neurodegeneration of the inner-most retinal neurons, the retinal ganglion cells (RGCs), and their axons, which form the optic nerve. Often, optic neuropathies are asymptomatic until advanced stages, when visual impairment or blindness is unavoidable despite existing treatments. In this review, we describe systemic and, whenever possible, ocular redox dysregulations observed in patients with glaucoma, ischemic optic neuropathy, optic neuritis, hereditary optic neuropathies (i.e., Leber’s hereditary optic neuropathy and autosomal dominant optic atrophy), nutritional and toxic optic neuropathies, and optic disc drusen. We discuss aspects related to anti/oxidative stress biomarkers that need further investigation and features related to study design that should be optimized to generate more valuable and comparable results. Understanding the role of oxidative stress in optic neuropathies can serve to develop therapeutic strategies directed at the redox system to arrest the neurodegenerative processes in the retina and RGCs and ultimately prevent vision loss.

Probability density functions of the involved random variables are essential for the reliability-based design of offshore structures. A total of 162 finite element (FE) analyses were carried out on 81 FE models of DK-joints subjected to two types of axial loading. Generated FE models were validated using available experimental data, FE results, and design formulas. Based on the results of parametric FE study, a sample database was prepared for the fLJF values and density histograms were generated for respective samples based on the Freedman-Diaconis rule. Nine theoretical PDFs were fitted to the developed histograms and the maximum likelihood (ML) method was applied to evaluate the parameters of fitted PDFs. In each case, the Kolmogorov-Smirnov and chi-squared tests were used to evaluate the goodness of fit. Finally, the Inverse Gaussian model was proposed as the governing probability distribution function for the fLJF. After substituting the values of estimated parameters, two fully defined PDFs were presented for the fLJF in tubular DK-joints subjected to two types of axial loading.

Purpose: The intrinsically photosensitive retinal ganglion cells (ipRGCs) and sleep quality are impaired in patients with primary open-angle glaucoma (POAG). In this study, we investigated whether ipRGCs and sleep quality were also impaired in patients with normal tension glaucoma (NTG). Methods: We performed pupillometry and sleep quality assessment in 15 patients with NTG and 17 healthy age-matched controls. Pupillometry protocol consisted of monocular stimulation with high illuminance (100 lux) red (633 nm, 300 cd/m 2 or 15.23 log quanta/cm 2 /s) and blue light (463 nm, 332 cd/m 2 or 15.27 log quanta/cm 2 /s) and binocular pupil measurements. Prior to light stimulation, patients were dark-adapted for 5 min. The late postillumination pupillary response (PIPR L ate ) to blue light was used as marker of ipRGC activity. Sleep quality was assessed by Pittsburgh Sleep Quality Index (PSQI) questionnaire. Results: The PIPR L ate to blue light was significantly reduced in patients with NTG compared to healthy subjects (p < 0.001), indicating impairment of the melanopsin-mediated pupillary pathway. There was no significant difference in the response elicited by red light (p = 0.6). Baseline pupil diameter and pupillary constriction amplitude to both red and blue light were reduced in patients with NTG (p < 0.05). The global score in PSQI was not significantly different between healthy controls and patients with NTG, indicating normal sleep quality (p = 0.6). Furthermore, we found no correlation between sleep parameters and pupillary light reflex parameters. Conclusion: Patients with NTG exhibited reduced ipRGC activity compared to healthy subjects, while no differences were observed in sleep quality.
