115. Aspectos diagnósticos e terapêuticos das disfunções sexuais femininas

Abstract

This article discusses the development of the concepts of normal and pathological in sexuality from the perspective of the population studies by Kinsey, as well as the development of the concept of sexual response cycle, from Masters and Johnson to Basson. The article deals with the classification of sexual dysfunctions, based on the sexual response cycle. Aspects of the diagnosis of female sexual dysfunctions reveal the importance of a detailed clinical observation, emphasizing that the diagnosis should take into account the length of the evolution period, the circumstances of the partner, and features of sexual stimulation (regarding focus, duration and intensity). Moreover, the distinction between primary or secondary and generalized or occasional dysfunction, as well as the age of the female patient and her sexual experience are parameters for a diagnosis. Therapeutic aspects indicate the importance of a multidisciplinary team, capable of offering psychotherapeutic and medicine-oriented treatment (antidepressants, anxiolytics and hormones, among others), as well as psycho-educational support. The authors stress the importance of a case by case evaluation in order to make a therapeutic decision. Although female sexual dysfunctions are already well known, the available therapeutic resources are limited. New research shall contribute to change this reality, so that the treatment of female sexual dysfunctions keeps up with the advances in the treatment of male sexual dysfunctions. Este artigo apresenta o desenvolvimento do conceito de normal e patológico em sexualidade, com base nos estudos populacionais de Kinsey, bem como a evolução do conceito de ciclo de resposta sexual, desde Masters e Johnson até Basson. Trata das classificações das disfunções sexuais, as quais têm como base o ciclo de resposta sexual. Os aspectos diagnósticos das disfunções sexuais femininas ressaltam a primazia da observação clínica minuciosa, enfatizando que o diagnóstico deve considerar o tempo de evolução do quadro, as condições do(a) parceiro(a) e as características do estímulo sexual (quanto ao foco, à duração e à intensidade). Além disso, a distinção entre disfunção primária ou secundária, generalizada ou situacional, bem como idade e experiência sexual da mulher, são parâmetros diagnósticos. Os aspectos terapêuticos referem a importância de uma equipe multidisciplinar, capaz de oferecer à mulher acompanhamento psicoterápico e medicamentoso (com antidepressivos, ansiolíticos, hormônios, entre outros), além de suporte psicoeducacional. Ressalta-se a necessidade de se avaliar caso a caso para a instituição de terapêutica individualizada. Embora os quadros de disfunções sexuais da mulher já sejam bem conhecidos, os recursos disponíveis para esse tratamento ainda são restritos. Novas pesquisas deverão contribuir para mudar essa realidade e fazer frente aos progressos terapêuticos relativos às disfunções sexuais masculinas.

149. Unconventional sexual behaviors and their associations with physical, mental and sexual health parameters: a study in 18 large Brazilian cities.

Abstract

Objective: There have been many studies investigating paraphilias and sexual compulsion, but thus far little data about prevalence of unusual sexual practices that are subthreshold for these diagnoses. The associations between unconventional sexual behavior and sociodemographic and health parameters were investigated. Method: A cross-sectional study of 7,022 individuals (45.4% of women) was carried out using a self-administered questionnaire that compared individuals carrying at least one reference of unconventional sexual behavior (group 1) with individuals without such reference (group 2). Results: Women's mean age was 35.0 vs. 35.9 years (p < 0.05) and men's mean age was 36.5 vs. 37.8 years (p < 0.05) being lower in group 1 than in group 2, respectively. More men (52.3%) than women (30.4%) (p < 0.001) presented unconventional sexual behavior. Fetishism (13.4%) and voyeuristic behavior (13.0%) were more frequent. Unconventional sexual behavior was associated with male gender, single or separated marital status, black or mulatto race, elementary and high school educational level, history of posttraumatic stress disorder treatment, alcohol dependence, emergency contraception, difficulty at the beginning of sexual life, sexual violence, bisexuality, and performance of anal or oral intercourse. Conclusion: Unconventional sexual behaviors are important because they are associated with poorer health status and lower educational levels. [ABSTRACT FROM AUTHOR]
