Abstract

This paper proposes a real-time, modular and compact architecture for the FPGA-based voltage control based on random PWM (Pulse Width Modulation) of a buck DC/DC converter by means of the Random Center Displacement (RCD) technique. Here a RCD controller algorithm is proposed, integrated into a PID-driven digital feedback control system and tested using a commercial FPGA. Several details of the implementation are discussed thoroughly and the experimental set-up is described. Finally, waveforms and power spectra are commented in order to assess the validity of the implementation

104. A primary squamous cell carcinoma of the trachea: Case report and review of the literature

Abstract

SUMMARY Primary windpipe tumour is extremely rare. Squamous cell carcinoma is the most common malignant form, especially in smokers. The radiological appearance of these neoplasms can be classified as intra-luminal, wall-thickening, exophytic form; the majority of the lesions are obstructive in nature and then tend to extra-luminal invasion. This tumour is usually diagnosed late on account of delayed specific symptoms: haemoptysis, dyspnoea, coughing, hoarseness, stridor. Surgery, followed by adjuvant radiotherapy, is the treatment of choice; primary radiotherapy, in inoperable cases, can represent a curative management option. A case of primary tracheal cancer is described and a review of the literature is presented.

Abstract

Advances researches in the diagnosis and treatment of childhood, adolescent and adult cancer have greatly increased the life expectancy of premenopausal women with cancer. However, one of the serious side effects of these treatments is the risk of damage to fertility. The ovaries are very sensitive to cytotoxic and radiotherapeutic treatment. The only established method of fertility preservation is embryo cryopreservation according to the Ethics Committee of the American Society for Reproductive Medicine (2005), but this option requires the patient to be of pubertal age, have a partner or use donor sperm, and be able to undergo a cycle of ovarian stimulation, which is not possible when the radiotherapy has to be initiated immediately or when stimulation is contraindicated according to the type of cancer. For patients who need immediate radiotherapy, cryopreservation of ovarian tissue is the only possible alternative. This manuscript reports the different techniques of cryopreservation and the results of transplantation of cryopreserved ovarian tissue. The current techniques allow cryopreservation of human ovarian fragments for a long time with good follicular survival rate after thawing. Numerous studies ultimately in this field have demonstrated to improve the survival rate of the oocytes and cryopreserved follicles. Moreover this manuscript includes a case of a 17-year-old girl who had to undergo pelvic irradiation for non-Hodgkin's lymphoma and the laparoscopic treatment to preserve the fertility (Fig. 2, Ref. 47).

110. Two unusual presentations of cervicofacial actinomycosis and review of the literature

Abstract

Cervicofacial actinomycosis is the most common clinical form of actinomycosis. This bacterial infection is rare. Diagnosis is difficult, often it is definitive only after surgical excision of the cervicofacial mass. Personal experience is reported concerning two cases of cervicoactinomycosis. Diagnosis, in both cases, has been based on histological findings, not on clinical symptoms. A review of the literature shows that mainly males are affected by this condition and, in fact, both patients described here are males. Symptoms of acute infection were absent. Both patients denied any history of oromaxillofacial trauma or recent dental extraction or oral manipulations. Imaging techniques – ultrasonography and computerized tomography – were not effective in making the diagnosis, in either of these patients. Furthermore, fine needle aspiration cytology did not provide a definitive diagnosis. Both patients underwent surgical excision of the mass. Penicillin was the drug of choice in post-operative long-term treatment (one month). In one of the two men, intravenous steroids were administered. As in several reports in the literature, the definitive diagnosis was histological and not clinical.

112. Mixed laryngocele: a case report and review of the literature

Abstract

Laryngocele is a rare, benign dilatation of the laryngeal saccule that may extend internally into the airway or externally through the thyrohyoid membrane. Many laryngoceles are asymptomatic; sometimes they may cause a cough, hoarseness, stridor, sore throat and may present as a swelling on one or both sides of the neck. Laryngocele may be associated with supraglottic squamous cell carcinoma. Computed tomography scan is the most effective imaging method for diagnosis. Surgery is the treatment of choice. A case of large mixed laryngocele in a 75-year-old male is described together with surgical management and follow-up. A review of the literature is also presented.

Abstract

In this paper, we present our results for the anisotropic refractive index measurements of commonly used liquid crystal (LC) materials (E7 and 5CB) in the whole visible-near infrared range. In order to achieve a high accuracy in the obtained values, we have employed a combination of two techniques, namely, Mueller matrix spectroscopic ellipsometry in transmission and half-leaky-guided-mode (HLGM). Measurements with the HLGM technique are usually performed at a single wavelength. In order to obtain a spectroscopic measurement based on the HLGM technique in a wide wavelength range, we modified our commercial VASE((R)) ellipsometer. In particular, we designed a sample holder by means of which measurements in guiding structures are also possible. Thus, we take advantage of the wide-spectrum light, emitted from a standard Xe lamp of the ellipsometer, also in the HLGM setup. The complementary and in-one-setup utilization of both techniques has allowed us to overcome most of the problems previously encountered in applying spectroscopic ellipsometry to the determination of the optical parameters of LC materials and eventually to achieve an accuracy in the obtained results of the order of 10(-4). We discuss how individual elements of the Mueller matrix can be used to this aim. Finally, we compare the dispersion curves for the refractive indices and the birefringence of E7 and 5CB with the data already existing in the literature. (c) 2007 American Institute of Physics.

Abstract

We present results on optical, structural and electrical properties of a-Si1-xCx:H films deposited by plasma enhanced chemical vapor deposition in the low power regime, with C fraction from 0 to 0.28. The absorption coefficient has been obtained in the region 0.73-4.5 eV by means of ellipsometry, reflectance-transmittance and photothermal deflection spectroscopy. The addition of carbon in the alloy increases the disorder and the density of defects: samples deposited with high carbon content have poorer optoelectronic properties. Two conduction regimes are observed: extended state conduction and hopping conduction in the conduction band tail. A visible PL peak that widens and shifts to higher energies as the carbon content increases has been observed. (c) 2006 Elsevier B.V. All rights reserved.

Since the large multicenter DIG trial has shown no effects of digitalis on the all-cause mortality of patients with chronic heart failure (HF), the broad prescription of this drug in patients with HF appears to be at the very least, questionable. The aims of this study were: to analyze prescription patterns of digitalis, from 1995 to 2000, in a large group of outpatients with HF; to analyze the independent predictors of digitalis prescription and to evaluate the impact of the results of the DIG trial on the prescription rate of this drug.From 1995 to 2000, 11 070 HF outpatients (mean age 64 +/- 12 years, ejection fraction 35 +/- 12%) were enrolled in a large Italian database.Out of 11 070 patients, 7198 (65%) were treated with digitalis. At multivariate analysis, the following variables were independently associated with digitalis prescription; atrial fibrillation (odds ratio [OR] 3.3, 95% confidence interval [CI] 2.9-3.8), ejection fraction30% (OR 1.7, 95% CI 1.5-1.9), NYHA class III-IV vs II-III (OR 1.3, 95% CI 1.2-1.5), admission for HF during the previous year (OR 1.4, 95% CI 1.2-1.5). After the publication of the DIG trial, there was a significant reduction in the rate of digitalis prescription: the percentage of patients taking digitalis fell from 68% in 1996-1997 to 61% in 1998-1999 (p0.001).Over 60% of Italian outpatients with HF were treated with digitalis; as expected, patients with a low ejection fraction, atrial fibrillation and in a more advanced stage of HF are more likely to receive this drug. Finally, after the publication of the DIG trial, the rate of digitalis prescription significantly decreased.

Abstract

Two integrated electro-optic switches based on smectic C * ferroelectric liquid crystal (FLC) planar waveguides oriented in the bookshelf geometry have been designed and realized. In order to test the first switch, a particular waveguide structure consisting of a three-stage device, having a thin FLC film middle stage and two glass waveguides as other stages, has been designed and realized. The second one is realized by a planar waveguide with a smectic C * ferroelectric liquid crystal overlayer. In this article the design, the realization, and the experimental characterization of electro-optical switches are reported. The electro-optical behavior and the response time of both devices have been studied for different configurations for both TE and TM polarization. Our main aim was to demonstrate the feasibility of an integrated electro-optic device based on a FLC waveguide, and our experimental results provide a number of interesting indications about device optimization and practical applications.

Abstract

Upper maxillary cysts are a chapter in otorhinolaryngological pathology which have been relatively neglected by the Literature. The reason for this most likely lies in the difficulty in producing a nosographic picture of these pathologies which border on other surgical fields (dentistry, maxillofacial surgery), and because they show significant clinical and etiopathogenic polymorphism. The elements that characterize upper maxillary cysts as a separate clinical entity are basically their cystic nature and origin within the upper maxillary bone, although they can expand widely within the medio-facial region (nasal vestibule, oral vestibule, nasolabial region, palate, maxillary sinus). After having reviewed the various classification schemes proposed over the years, and briefly examining the main clinical and etiopathogenic characteristics and principles for surgical treatment, the present work offers a surgical case study, together with the related iconography. Moreover this work does not neglect embryogenic considerations which are indispensable for the study of some of these pathologies. In this manner the results for 35 surgical procedures on upper maxillary cysts performed from 1989 to 1996 are presented and classified following the Cudennec classification module (1991). This study shows the variety of possible clinical manifestations for these pathologies. Such a variety makes correct diagnosis imperative--today facilitated by modern imaging techniques--and requires diversifying the surgical approach, conditioned not only by the site, extension and nature of the specific lesion, but also by the related symptoms. The significant progress in surgical techniques has made increasingly functional surgery possible and led to the abandonment of such conventional radical techniques as the Caldwell-Luc procedure. Moreover, CT and NMR have provided good image definition, specifying precisely the limits and extensions and, in most cases, facilitating diagnosis of the nature of the disorder with direct and indirect signs of the cystic nature of the lesions whenever the clinical data proves inadequate.
