Cardiovascular disease (CVD) represents the largest burden of disease globally and despite the availability of strong evidence supporting cost-effective treatments for people with CVD, the implementation of these treatments remains low, especially in low-income settings. Shortages in workforce have led to focus on how to increase clinical capacity. However, a simplistic focus on training clinicians will not fill the gaps in research, policy and implementation, which also need to be addressed at the same time. There are multiple efforts to develop early career capacity across diverse areas at national and international level to address these gaps. To-date, there have been limited efforts to compare or evaluate such programmes, and there are no efforts to harmonise such programmes to take advantage of synergies. We now compare three international programmes on global cardiovascular research to train individuals in their early- and mid-career by aims, experience and outputs.

BackgroundSurgery is the main modality of cure for solid cancers and was prioritised to continue during COVID-19 outbreaks. This study aimed to identify immediate areas for system strengthening by comparing the delivery of elective cancer surgery during the COVID-19 pandemic in periods of lockdown versus light restriction.MethodsThis international, prospective, cohort study enrolled 20 006 adult (≥18 years) patients from 466 hospitals in 61 countries with 15 cancer types, who had a decision for curative surgery during the COVID-19 pandemic and were followed up until the point of surgery or cessation of follow-up (Aug 31, 2020). Average national Oxford COVID-19 Stringency Index scores were calculated to define the government response to COVID-19 for each patient for the period they awaited surgery, and classified into light restrictions (index 60). The primary outcome was the non-operation rate (defined as the proportion of patients who did not undergo planned surgery). Cox proportional-hazards regression models were used to explore the associations between lockdowns and non-operation. Intervals from diagnosis to surgery were compared across COVID-19 government response index groups. This study was registered at ClinicalTrials.gov, NCT04384926.FindingsOf eligible patients awaiting surgery, 2003 (10·0%) of 20 006 did not receive surgery after a median follow-up of 23 weeks (IQR 16-30), all of whom had a COVID-19-related reason given for non-operation. Composite of reduced graphene oxide (rGO) and polyaniline (PANI) have gained much attention in industrial applications especially for the fabrication of supercapacitor and flexible sensors. Nevertheless, existing studies mainly focus on the application efficiency and are lacking information on how morphology of the nanoparticles can affect the electrode properties. Herein, the effects of shape of PANI on the electrode resistivity were systematically investigated. PANI of different shapes was synthesized using different reagents, namely [H.sub.2]S[O.sub.4], C[H.sub.3]COOH, [H.sub.2]O, and N[H.sub.3] * [H.sub.2]O, which induced formation of globule, tube, plate, and flake-like PANI. rGO-PANI composite was then produced using ex situ method and cast on a PVC substrate to form a flexible electrode. The rGO-PANI globules displayed superior electrical conductivity owing to its high protonation level. The resistivity of this sample was found to be 0.015 [+ or -] 0.002 [OMEGA]cm, which is lowest as compared to the counterparts. Interestingly, electrode made of rGO-PANI plates had a slightly lower electrical resistivity than rGO-PANI tubes despite its low protonation level. It is speculate that the aspect ratio characteristic of the PANI might have influence the electron mobility. rGO-PANI electrode was found to exhibit a lower specific resistivity than pure rGO and pure PANI, evincing the presence of synergism. Customer churn is defined as the tendency of customers to cease doing business with a company in a given period. ABC Multistate Bank faces the challenges to hold clients. The purpose of this study is to apply machine learning algorithms to develop the most effective model for predicting bank customer churn. In this study, six supervised machine learning methods, K-Nearest Neighbors, Support Vector Machine, Naïve Bayes, Decision Tree, Random Forest, and Extreme Gradient Boosting (XGBoost), are applied to the churn prediction model using Bank Customer Data of ABC Multistate Bank obtained from Kaggle. The results showed that XGBoost outperformed the other six classifiers, with an accuracy rate of 84.76%, an F1 score of 56.95%, and a ROC curve graph of 71.64%. The bank may use XGBoost model to accurately identify customers who are at risk of leaving, concentrate their efforts on them, and possibly make a profit. Future research should focus on various machine learning approaches for determining the most accurate models for bank customer churn datasets.

Travel insurance serves as a crucial financial safeguard, offering coverage against unforeseen expenses and losses incurred during travel. With the advent of the proliferation of insurance types and the amplified demand for Covid-related coverage, insurance companies face the imperative task of accurately predicting customers’ likelihood to purchase insurance. This can assist the insurance providers in focusing on the most lucrative clients and boosting sales. By employing advanced machine learning techniques, this study aims to forecast the consumer segments most inclined to acquire travel insurance, allowing targeted strategies to be developed. A comprehensive analysis was carried out on a Kaggle dataset comprising prior clients of a travel insurance firm utilizing the K-Nearest Neighbors (KNN), Decision Tree Classifier (DT), Support Vector Machines (SVM), Naïve Bayes (NB), Logistic Regression (LR), and Random Forest (RF) models. Extensive data cleaning was done before model building. Performance evaluation was then based on accuracy, F1 score, and the Area Under Curve (AUC) with Receiver Operating Characteristics (ROC) curve. Inexplicably, KNN outperformed other models, achieving an accuracy of 0.81, precision of 0.82, recall of 0.82, F1 score of 0.80, and an AUC of 0.78. The findings of this study are a valuable guide for deploying machine learning algorithms in predicting travel insurance purchases, thus empowering insurance companies to target the most lucrative clientele and bolster revenue generation.

The heart is a very crucial organ of the body. Concerted efforts are constantly put forward to provide adequate monitoring of the heart. A heart disorder is reported to cause a lot of hidden ailments resulting in numerous deaths. Early heart monitoring using an electrocardiogram (ECG) through the advancement of computer-aided diagnostic (CAD) systems is widely used. Meanwhile, the use of human reading of ECG results are faced with many challenges of inaccurate and unreliable interpretations. Over two decades, studies provided artificial intelligence (AI) technique using machine learning (ML) algorithms as a fast and reliable technique for ECG heartbeat classification. Moreover, in recent times, deep learning (DL) techniques have been focused on providing automatic feature extraction and better classification performance. On the other hand, the challenge with the ECG data is its imbalance nature. Therefore, this paper proposes a cost-based dual convolutional attention transfer DL model for ECG classification. The proposed model uses PhysionNet-MIT-BIH and Physikalisch-Technische Bundesanstalt (PTB) Diagnostics datasets. The first part uses the MIT-BIH for ECG categorization, while representations learned from the first classifier are used for PTB analysis through transfer learning (TL). The proposed model is evaluated and compared with well-performing conventional ML models based on their F1-score and accuracy scores. Our experimental finding show that the proposed model outperformed the well-performing ML models as well as competitive with past studies for both the classification and TL part, having obtained 98.45% for both F1-score and accuracy. The proposed model is applicable to real-life trials and experiments for ECG heartbeat and other similar domains.

Gas-forming pyogenic liver abscess (GFLPA) carries a high mortality rate. Early identification of the source of infection in sepsis results in better survival. Bedside point of care ultrasound (POCUS) can be used to help localize a source of infection. A 59-year-old man presented with systemic inflammatory response syndrome (SIRS) and was diagnosed with GFLPA on the initial encounter via clinical assessment and POCUS examination. After commencing antibiotics, optimal glucose control, adequate fluid resuscitation, and early infective source control, he achieved full recovery and was followed up in outpatient medical and surgical clinics. This case illustrates the role of POCUS as a diagnostic tool in sepsis and raises awareness among clinicians to recognize the features of GFLPA on POCUS.

The relationship between microalgae and bacteria in a microenvironment, the phycosphere, has a significant role in enhancing the quality and quantity of microalgal production, which would in turn affect consumers' growth and nutritional quality, such as the zooplankton, which are important live feeds in aquaculture. Thus, selecting and characterising suitable microalgal growth-promoting bacteria (MGPB) for enhancing microalgal production is an important process since not all bacteria promote high growth. In this study, physcosphere bacteria associated with chlorophytes and chrysophytes were isolated, screened for their microalgae-promoting attributes (phosphorus solubilisation, indole-3-acetic acid (IAA) production, and nitrogen fixation) and re-inoculated into microalgal cultures. A total of seven bacterial isolates were recorded to have multiple growth promoting traits, with three strains (CY-2, CY-4, CY-5) showing the greatest (P < 0.05) link for those traits. These seven potential MGPB were molecularly characterised using 16S rRNA approach. The phylogenetic tree of the isolated bacteria demonstrated the dominant bacteria associated with the chlorophytes were in the class bacteroidetes, while the chrysophytes appeared to be associated with Firmicutes bacteria suggesting that the compositions were strictly species-specific to the microalgae host. Enhanced Cyclotella meneghiniana growth by the seven isolated bacterial strains was highly dependent on the growth-promoting traits; especially those demonstrated by Pseudomonas hibiscicola and Ochrobactrum haematophilum. These two bacteria showed the potential to enhance the quality of microalgae, and they could be bioencapsulated and used to improve the quality of zooplankton as one of the main live feeds in the aquaculture industry.

Introduction The empty pelvis syndrome is a significant source of morbidity following pelvic exenteration surgery. It remains poorly defined with research in this field being heterogeneous and of low quality. Furthermore, there has been minimal engagement with patient representatives following pelvic exenteration with respect to the empty pelvic syndrome. ‘PelvEx—Beating the empty pelvis syndrome’ aims to engage both patient representatives and healthcare professionals to achieve an international consensus on a core outcome set, pathophysiology and mitigation of the empty pelvis syndrome.Methods and analysis A modified-Delphi approach will be followed with a three-stage study design. First, statements will be longlisted using a recent systematic review, healthcare professional event, patient and public engagement, and Delphi piloting. Second, statements will be shortlisted using up to three rounds of online modified Delphi. Third, statements will be confirmed and instruments for measurable statements selected using a virtual patient-representative consensus meeting, and finally a face-to-face healthcare professional consensus meeting.Ethics and dissemination The University of Southampton Faculty of Medicine ethics committee has approved this protocol, which is registered as a study with the Core Outcome Measures in Effectiveness Trials Initiative. Publication of this study will increase the potential for comparative research to further understanding and prevent the empty pelvis syndrome.Trial registration number NCT05683795.

Summary: Background: Tumour-promoting inflammation is a “hallmark” of cancer and conventional epidemiological studies have reported links between various inflammatory markers and cancer risk. The causal nature of these relationships and, thus, the suitability of these markers as intervention targets for cancer prevention is unclear. Methods: We meta-analysed 6 genome-wide association studies of circulating inflammatory markers comprising 59,969 participants of European ancestry. We then used combined cis-Mendelian randomization and colocalisation analysis to evaluate the causal role of 66 circulating inflammatory markers in risk of 30 adult cancers in 338,294 cancer cases and up to 1,238,345 controls. Genetic instruments for inflammatory markers were constructed using genome-wide significant (P 70% was employed to indicate support for shared causal variants across inflammatory markers and cancer outcomes. Findings were replicated in the FinnGen study and then pooled using meta-analysis. Findings: We found strong evidence to support an association of genetically-proxied circulating pro-adrenomedullin concentrations with increased breast cancer risk (OR: 1.19, 95% CI: 1.10–1.29, q-value = 0.033, PPH4 = 84.3%) and suggestive evidence to support associations of interleukin-23 receptor concentrations with increased pancreatic cancer risk (OR: 1.42, 95% CI: 1.20–1.69, q-value = 0.055, PPH4 = 73.9%), prothrombin concentrations with decreased basal cell carcinoma risk (OR: 0.66, 95% CI: 0.53–0.81, q-value = 0.067, PPH4 = 81.8%), and interleukin-1 receptor-like 1 concentrations with decreased triple-negative breast cancer risk (OR: 0.92, 95% CI: 0.88–0.97, q-value = 0.15, PPH4 = 85.6%). These findings were replicated in pooled analyses with the FinnGen study. Though suggestive evidence was found to support an association of macrophage migration inhibitory factor concentrations with increased bladder cancer risk (OR: 2.46, 95% CI: 1.48–4.10, q-value = 0.072, PPH4 = 76.1%), this finding was not replicated when pooled with the FinnGen study. For 22 of 30 cancer outcomes examined, there was little evidence (q-value ≥0.20) that any of the 66 circulating inflammatory markers examined were associated with cancer risk. Interpretation: Our comprehensive joint Mendelian randomization and colocalisation analysis of the role of circulating inflammatory markers in cancer risk identified potential roles for 4 circulating inflammatory markers in risk of 4 site-specific cancers. Contrary to reports from some prior conventional epidemiological studies, we found little evidence of association of circulating inflammatory markers with the majority of site-specific cancers evaluated. Abstract Background In 2020, cervical cancer ranked fourth in terms of both frequency of diagnosis and the leading cause of cancer-related deaths among women globally. Among Malaysian women, it was the third most prevalent form of cancer. Published data on nationally representative cervical cancer screening in Malaysia have been limited. Therefore, this study aimed to determine the prevalence of receiving a Pap smear test in the past three years, its relationship with socio-demographic factors and physical activity. Methods Using a subset of survey data from the National Health and Morbidity Survey (NHMS) 2019, a secondary data analysis was performed. Trained research assistants collected data through face-to-face method using a mobile tablet questionnaire system application. Logistic regression analysis was performed to examine the relationship between sociodemographic factors, physical activity, and cervical cancer screening. The analyses were conducted using STATA version 14 (Stata Corp, College Station, Texas, USA), accounting for sample weighs and complex sampling design. Results The analysis included 5,650 female respondents, representing an estimated 10.3 million Malaysian female adults aged 18 and above. Overall, 35.2% (95%CI 33.2, 37.4) respondents had a Pap smear test within the past three years. Respondents who were physically active were 1.41 times more likely to have a Pap smear test. Similarly, respondents aged 35–59 (OR 1.84; 95%CI 1.46, 2.34) and those living in rural localities (OR 1.38; 95%CI 1.13, 1.70) had higher odds of receiving a Pap smear test. Compared to married respondents, single respondents (OR 0.04; 95%CI 0.02, 0.07) and widowed/divorcee respondents (OR 0.72; 95%CI 0.56, 0.82) were less likely to receive a Pap smear test. Educated respondents were more likely to have had a Pap smear test. Conclusions The overall prevalence of cervical cancer screening in Malaysia remains low (35.2%). Efforts should be made to strengthen health promotion programs and policies in increasing awareness on the significance of cervical cancer screening. These initiatives should specifically target younger women, single women, and widowed/divorced individuals. The higher cervical screening uptake among rural women should be studied further, and the enabling factors in the rural setup should be emulated in urban areas whenever possible.

Abstract Background In Malaysia, the previous mortality burden has been a significant concern, particularly due to the high prevalence of noncommunicable diseases (NCDs) as the leading cause of death. Estimates of mortality are key indicators for monitoring population health and determining priorities in health policies and health planning. The aim of this study was to estimate the disease burden attributed to 113 major diseases and injuries in Malaysia in 2018 using years of life lost (YLL) method. Methods This study included all deaths that occurred in Malaysia in 2018. The YLL was derived by adding the number of deaths from 113 specific diseases and multiplying it by the remaining life expectancy for that age and sex group. Data on life expectancy and mortality were collected from the Department of Statistics Malaysia. Results In 2018, there were 3.5 million YLL in Malaysia. Group II (NCDs) caused 72.2% of total YLL. Ischaemic heart disease was the leading cause of premature mortality among Malaysians (17.7%), followed by lower respiratory infections (9.7%), road traffic injuries (8.7%), cerebrovascular disease (stroke) (8.0%), and diabetes mellitus (3.9%). Conclusions NCDs are a significant health concern in Malaysia and are the primary contributor to the overall burden of disease. These results are important in guiding the national health systems on how to design and implement effective interventions for NCDs, as well as how to prioritise and allocate healthcare resources. Key strategies to consider include implementing health promotion campaigns, adopting integrated care models, and implementing policy and regulatory measures. These approaches aim to enhance health outcomes and the managements of NCDs in Malaysia.

Abstract Background No previous studies have examined the associations between changes in objectively-measured physical behaviours with follow-up QoL in older adults. Based on cross-sectional evidence, it is biologically plausible that such associations exist. If so, this bolsters the case for the commissioning of activity interventions and for including QoL as an outcome in trials of such interventions. Methods We assessed physical behaviours (total physical activity, moderate-to-vigorous physical activity (MVPA), light physical activity, total sedentary time and prolonged sedentary bout time) for 7 days using hip-worn accelerometers at baseline (2006–2011) and follow-up (2012–2016) and health-related quality-of-life (QoL) using EQ-5D questionnaires at follow-up in 1433 participants (≥ 60 years) of the EPIC (European Prospective Investigation into Cancer)-Norfolk study. The EQ-5D summary score was used, with 0 as the worst to 1 as best perceived quality-of-life. We evaluated the prospective associations of baseline physical behaviours with follow-up QoL, and of changes in behaviours with follow-up QoL using multi-level regression. Results On average, MVPA decreased by 4.0 min/day/year (SD 8.3) for men and 4.0 min/day/year for women (SD 12.0) between baseline and follow-up. Total sedentary time increased by an average 5.5 min/day/yr (SD 16.0) for men and 6.4 min/day/yr (SD 15.0) for women between baseline and follow-up. Mean (SD) follow-up time was 5.8 (1.8) years. We found that higher baseline MVPA and lower sedentary time was associated with higher subsequent QoL (e.g. 1 h/day greater baseline MVPA was associated with 0.02 higher EQ-5D score, 95% CI 0.06, 0.36). More pronounced declines in activity were associated with worse Hr-QoL (0.005 (95% CI 0.003, 0.008) lower EQ-5D per min/day/yr decrease in MVPA). Increases in sedentary behaviours were also associated with poorer QoL (0.002 lower EQ-5D, 95% CI -0.003, -0.0007 per hour/day/yr increase in total sedentary time). Conclusions Promotion of physical activity and limiting sedentary time among older adults may improve quality-of-life, and therefore this relationship ought to be included in future cost effectiveness analyses so that greater commissioning of activity interventions can be considered.

AimThis study aimed to describe the change in surgical practice and the impact of SARS-CoV-2 on mortality after surgical resection of colorectal cancer during the initial phases of the SARS-CoV-2 pandemic.MethodThis was an international cohort study of patients undergoing elective resection of colon or rectal cancer without preoperative suspicion of SARS-CoV-2. Centres entered data from their first recorded case of COVID-19 until 19 April 2020. The primary outcome was 30-day mortality. Secondary outcomes included anastomotic leak, postoperative SARS-CoV-2 and a comparison with prepandemic European Society of Coloproctology cohort data.ResultsFrom 2073 patients in 40 countries, 1.3% (27/2073) had a defunctioning stoma and 3.0% (63/2073) had an end stoma instead of an anastomosis only. Thirty-day mortality was 1.8% (38/2073), the incidence of postoperative SARS-CoV-2 was 3.8% (78/2073) and the anastomotic leak rate was 4.9% (86/1738). Mortality was lowest in patients without a leak or SARS-CoV-2 (14/1601, 0.9%) and highest in patients with both a leak and SARS-CoV-2 (5/13, 38.5%). Mortality was independently associated with anastomotic leak (adjusted odds ratio 6.01, 95% confidence interval 2.58-14.06), postoperative SARS-CoV-2 (16.90, 7.86-36.38), male sex (2.46, 1.01-5.93), age >70 years (2.87, 1.32-6.20) and advanced cancer stage (3.43, 1.16-10.21). Compared with prepandemic data, there were fewer anastomotic leaks (4.9% versus 7.7%) and an overall shorter length of stay (6 versus 7 days) but higher mortality (1.7% versus 1.1%).ConclusionSurgeons need to further mitigate against both SARS-CoV-2 and anastomotic leak when offering surgery during current and future COVID-19 waves based on patient, operative and organizational risks.

Primary open-angle glaucoma (POAG), is a heritable common cause of blindness world-wide. To identify risk loci, we conduct a large multi-ethnic meta-analysis of genome-wide association studies on a total of 34,179 cases and 349,321 controls, identifying 44 previously unreported risk loci and confirming 83 loci that were previously known. The majority of loci have broadly consistent effects across European, Asian and African ancestries. Cross-ancestry data improve fine-mapping of causal variants for several loci. Integration of multiple lines of genetic evidence support the functional relevance of the identified POAG risk loci and highlight potential contributions of several genes to POAG pathogenesis, including SVEP1, RERE, VCAM1, ZNF638, CLIC5, SLC2A12, YAP1, MXRA5, and SMAD6. Several drug compounds targeting POAG risk genes may be potential glaucoma therapeutic candidates.

Genetic models for cancer have been evaluated using almost exclusively European data, which could exacerbate health disparities. A polygenic hazard score (PHS1) is associated with age at prostate cancer diagnosis and improves screening accuracy in Europeans. Here, we evaluate performance of PHS2 (PHS1, adapted for OncoArray) in a multi-ethnic dataset of 80,491 men (49,916 cases, 30,575 controls). PHS2 is associated with age at diagnosis of any and aggressive (Gleason score ≥ 7, stage T3-T4, PSA ≥ 10 ng/mL, or nodal/distant metastasis) cancer and prostate-cancer-specific death.
